  • Zion Williamson was supposed to be one of the biggest stories and stars of the 2019-2020 NBA season for the New Orleans Pelicans. But he still hasn't made his season debut after having surgery to remove some damaged meniscus tissue in his knee, and there are additional concerns around NBA communities regarding his current weight and how it could affect his career longevity. In this video I'll explain the medical evidence behind why I think this is a legitimate discussion to have regarding Zion Williamson's NBA career.

    BMI in NBA players? Check out this study of NBA players showing there was a significant association with mensicus tears and BMI > 25. So yes, I do think it's still relevant.

      BMI is literally the worst indicator of anything. Association is merely a correlation, doesn't mean it caused anything. BMI doesnt account for differences in bone structure and pretends we are all built the same. It's a trash stat that is almost worthless. He isn't obese. That's just ridiculous. But according to BMI he is? Just shows you how dumb of an indicator it is...

      You can cite any article you want, this is complete garbage science and as an "MD" you know it!
      BMI has long been a useless indicator, because (as I'm sure you know) there is no account for lean mass percentage. This also negates any article which attributes meniscus tears to BMI. For starters, how do you measure the fact that someone with a high BMI might have tons of muscle, developed through lifting weights, which has during the process strengthened their tendons and joints too. You can't measure that if you use BMI!
      This is a garbage video.

      He only played 50 games in a 4 year career. He played for several teams that year including Miami. He was 6' 11" and weighed over 300 pounds.

    Thank you for posting the references! I think another thing to mention would be the quality of rehab he’s gonna be getting in terms of the average obese person and their physical rehabilitation that occurs immediately after and weeks/months following the surgery. From research I’ve read, long term outcomes (1+years) are generally the same for general vs specific rehabs but for a high level athlete the few weeks right after do matter a lot. Just a thought, keep up the great content Dr. B!

    Personally, I think BMI is a horrible measurement in general, especially for athletes. Guys like JJ Watt (who is 6’5 285) are considered obese even though they are in the best shape of their lives. That being said, football is completely different game than basketball because there is not as much force coming down on the athletes knees. It would not hurt Zion to drop some weight. He obviously won’t lose that much power considering he’ll still be one of the biggest guys on the court and his longevity will be boosted.

      Yeah but in relation to how much force somehow generates/puts on their body it's actually pretty useful. Especially in a case where Zion has clear mechanical issues. Those issues are exacerbated when you're 270 and landing from a 45 inch vertical.

It’s gotten to a point where most NBA media members don’t recklessly opine or speculate about injuries, and often note, “I’m not a doctor, but...”, yet it has always seemed to me that actually having a MD to speak -in general terms- about specific injuries would improve the understanding of fans and members of the press.     It isn't just weight with him. Body mechanics are going to potentially go a long way with him. He may have to learn how to fall, how to protect himself when jumping, and potentially different ways to make the cuts and quick directional changes he makes without blowing shoes and popping knees.

      Mike Clayton he will not lose 50lbs. He is fairly lean all weight the he loses will mostly be muscle which will hurt his game maybe he can lose 10-20. But I agree knee alignment need to be a big focus of his.

    He just needs to learn to land with minimal knee impact, roll to ground, don't depend on his knees to slow his momentum to much when landing, this alone would save 80% of the wear and tear and his career.

      WorkBreakLounge Bo was just too strong and fast and he basically pulled his hip out on that play. Others would of just gone down without that happening. It was a freak accident though. He probably could of lasted a long time. Gronk was too strong. He would get injured as people tried to tackle him. He had too much strenght and his body couldn't handle it.

      Dre Chillin that’s a really healthy weight but he’ll have to realize mentally he can’t play bully ball as much as he’d like when attacking the rim protectors. I believe he can do it . But I’m afraid mentally his motor is too high and he’ll be going for the highlights over jus wisely euro stepping or using a floater . Most young guys go for the highlights lol they almost have to in order to send a message

    It seems to make sense, as he's just starting his NBA career, and like you said , removing any part of meniscus increases the risk of repeated injury..

      While that’s true. MJ didn’t stay at 195. He stayed around 200 until he got his ass physically kicked by the bad boy Pistons, and got his weight up to the 225 range to finish better through contact and it helped tremendously.

    Given the situation at hand, in terms of having his weight reach ‘normal’ levels , would not be beneficial for his recovery as well. Most likely change in style of play and his role in the system of the team may be best bet to maintain longevity for his career 👍

    I always thought he would have serious knee problems but not so soon. Hes to bulky for his body to withstand the impact of playing basketball.i mean i put osme weight up & went to play basketball & the impact was too big on my knees now I know whi i cant run the same as when i was 170 lbs Im now 180 at 5'8 so my knees are resenting every time. Superb Channel man!!! Its amazing how science explains a lot of things in our favourite games!!

    Dr. Sutterer- Really like your channel. Would love to see a video of Pistol Pete Maravich, an idol of mine, died 1988 age 40 due to very rare undiagnosed congenital heart defect, found during autopsy. Born without a left carotid artery. By that time his heart was massively enlarged and scarred. I am curious: 1) how would this defect typically affect athletic performance, limit endurance, etc? Any warning signs/symptoms? 2) should/could a physical exam (ie: listening to the heart) detect at least an abnormality (so can order followup tests), or not necessarily? And lastly 3) have exams (technology, team physician knowledge etc) of professional athletes evolved to the point since the 1970’s/80’s where this would have more likely been detected? From what I’ve read on pediatric cardiology sites, this likely could have been corrected by a “routine” bypass surgery. Is it possible younger patients who have that procedure could go on to play pro sports?
    I want to say Reggie Lewis (another Great player), Celtics also died on court in practice stemming from heart defect?

    the stress on knees is insane how much it improves when you lose weight my knees and shins where in so much pain at 240 and running 1 mile was killing me and was taking almost 12 min ive lost 36 pounds but it only took the first 10 to make it an 8 minute mile now I don't feel complete without running 6 miles. knees feel so much better now also

    Administering cutting phase (weight) may surely help to help ease the load in the upcoming seasons, but would you think aside from anatomical, and anthropometric factors, a change in Zion’z style of play and load managemnet (minutes) are essential tenets as well to consider for the longevity of his career? :)

    Hey Brian, the main reason I think BMI is a good index for this is that joint size is heavily correlated to height, therefore when you're examining peak stresses on an athletes joint BMI is near perfect for telling you how much force is seen through the key joints that are the main mechanism of failure, or source of catastrophic injury. I've done kinematic studies on knees for a long time and it was one of the controlling factors we took a look at over a varied specimen population. Love the channel tho.

      Zion's BMI assumes that he has much more fat mass than skeletal muscle. There are many studies that suggest that skeletal muscle mass and fat mass have a much different impact on stress to the joints. So I would say that BMI is far from a perfect index

      @@peazy99 I understand the implication, but I have yet to see a source that says that composition influences geometric stress concentrations in joints, which is what I would be talking about. I.e your ankle doesn't care if its fat or muscle, 480 ftlb of force is still 480 ftlb. It's in no way perfect, but for this gross overview i think it works fairly well. I've taken a lot of knees apart, and we never accounted for body composition, but did look at weight at death.

      @@luke53285 I dont think it's the actual composition that makes the difference but how fat vs muscle help reduce the peak load on the joint especially during explosive dynamic movements. Using BMI for Zion suggests that the joint takes on a much greater degree of peak load, which doesn't make much sense for a guy who is probably in the top .1% of lower body muscle mass percentage.

      @@peazy99 what you're implying is that extra muscles to generate more force would reduce forces seen at a given articular surface, the resultant forces may be more balanced in better athletes, but they see far greater forces than non athletic people. I can't think of a way to test or prove your hypothesis, because if you were to do an observational study (the only way I could think to try this) you'd see decreased joint injury with increased fat composition (heuristically) because you're looking at a more sedentary population. What we learn in studying joints is basically that form dictates function, and what I mean by that is the shape is the ultimate determining factor. The shape and smoothness of your joints are what determine the factors that magnify force in out joints and increase risk of injury. That being said, your knee, ankle wrist, etc have no concept of how much muscle or fat you have, they see a load and a bearing surface. To your point about added muscles reducing the risk, to answer that better, yes to an extent. They do in the end put more stress on the joint, but they help reduce (center not decrease in magnitude) resultant force into normal movement and help protect against avulsive and evulsive stresses. But these are of diminishing returns, you could have the strongest muscles in your leg humanely possible, and it would increase chances of catastrophic injury because now your resultant vectors are so great in magnitude that a small force of a given impulse may cause your muscles to tear the joint apart. We see this with patellar dislocations sometimes. Really nasty. So in summation, yes prehab as pt people call it has benefits, but bmi is quite litterally mass/ht^2, a direct correlation between articular surface dependencies (ht^2) and forces those faces see (mass). Its a crude way of doing it but for a baseline, I cant think of a better measurement without taking external joint circumference and extrapolating. I hope you do read all of this and see my point.

      paolylo it’s easier on the body... there’s plenty of players who don’t load manage and play as much as the old school players. Kids today start playing intense sports early and start intense training earlier than ever. I think that’s why we potentially see more injuries than before. We are stressing our joints, tendons, and bones at a young age while they are growing at a rapid rate and are constantly changing. I think this increased load at such a young age doesn’t help.

    I pay attention to the way players land and the frequency of injuries and my initial observations are that the players that get a lot of injuries on there lower extremities tend to land on one leg or with stiff bodies frequently. The thing I am wondering is 1: do you know if there is research behind this that shows that players who land a specific way are more or less likely to get injured and 2: do you think that there is merit to the idea that frequently landing in a sub-optimal position might make a player more susceptible to major injury?
    While I do see that Zion lands funny a lot as did a young Derrick Rose, and a Younger Joel, I am aware that this could be a nice combination of selection and confirmation bias. While I don't know if this unfounded or if there is something that we could really point to I do think that it is something worth having someone research to see if there are things young players should be taught so that they can protect the longevity of their carriers.
    Bottom line, does landing matter?
