A Human MRI? Doctor Reacts to WILD Sequence at UFC 246 McGregor vs Cerrone

  • After a brief exam during UFC 246 McGregor - Cerrone, Maycee Barber was told that she had a small, partial ACL tear and was fine to continue fighting. I had many concerns and questions watching this exam and subsequent diagnosis go down during UFC 246, and this video is an explanation why.
    Not everyone can be expected to be experts in these physical exam maneuvers, but hopefully we can learn from this situation so that you can properly assess for an ACL tear if ever in a situation like this.

    We now know she had a full, complete ACL tear. That doesn't change the teaching I'm trying to get across in this video. Exam quality is crucial otherwise you can't make any claims about something. Even if someone has an good quality, abnormal anterior drawer/lachman, you can't say partial/Complete/small/large. All you know is ACL is compromised. Especially without comparing to the normal other side to get an internal control, you have no idea what's abnormal movement on the exam

      @Brian Sutterer MD I guess the doctor came to such a conclusion through his years of clinical experience. He probably would have seen numerous such cases of ACL tear with similar presentation.

    In defense of the ringside doctor, the reason he said to the ref "She has a SMALL PARTIAL acl tear, she's fine" is because the ref asked for the doctor to look at it, which means he was thinking about stopping the fight...the doctor likely knew the knee was injured, but he made it seem as insignificant as possible by using the adjectives "small" and "partial"... if he would have said, "Yea she might have a torn acl, the knee is compromised" then the ref would have likely called the fight. Fighters will fight through an injured knee, whether that's smart or not, but the UFC put a lot of promotion behind Maycee so there was pressure on the doc to basically do damage control, perform some little test make it look like he knows what he's doing, and then say whatever keeps the fight going.... at least that's what I got from analyzing the situation.       The ringside doctor did know that her acl was compromised because he checked it. These type of doctors are trained professionals, they know what ligaments to check for in the knee. Why would the doctor "perform some little test to make it look like he knows what he's doing", when obviously he knew that he was doing lol thats what he gets paid to do

      @@anthonyfranco4232 ​if the patient is not relaxed then the hamstrings can tighten up and prevent the tibia from moving forward which could simulate an end feel which would feel like a ligament but it's just the muscle preventing the tibia from moving forward

      I'm an LCSW in NYS and practiced in the behavioral health field for nearly two decades. Trust me, there are practicing physicians who's lack of competence is alarming. Unfortunately, plenty of these inept doctors are "working their magic" in the world of medicine.
      Over the years, I've reported multiple physicians, not only because of a breaching of the bio-ethical code, but malpractice actions, which in my opinion warranted criminal charges. And I am a loose motherfucker who just chills and keeps to myself, but I swore to uphold an oath in protecting patients from these sanctimonious morons with the mentality of a cheeseburger.
      Most of them are from Arab nations. My guess is that it's both a lack of a high-level graduate experience and culturally related.

    He knew it was an ACL injury; he might've just claimed it was a partial tear so she could continue to fight.... He knows she didn't want the fight to stop

      The facts are:
      doctor suspected an acl tear without any way of knowing with 100% certainty what it was or the severity...yet claimed she was fine.
      Fighter was diagnosed with a complete acl tear.
      Doctor performed a cursory assessment with inaccurate results.
      Even if we pretend you're correct in all your "facts," allowing her to continue and blow the knee completely was a completely irresponsible conclusion.
      **Don't forget, seek help.**

If you hear the word "torn" and were worried about it becoming worse. Don't fight. It's like complaining that you have a small tear in a piece of paper yet continue to ruffle it around. She's a grown up, she knew what she was doing.

      @@enabler2456 I have torn ACL meniscus right and left medial ligaments do for him to figure that out in ten seconds under pressure is pretty good. I unfortunately walked around......hobbled for two months in agony before any op due to my place of work refusing to take responsibility. As soon as went private a 30 min look at knee by a doc literally said exactly what I just mentioned. Very clever. Not sure how you take this in to MMA though. Do they stop fights? I bust my nose three times and last fight it broke in sparring but fine with it. Fk knows but interesting as not seen this before

      I think the question we should be asking is why did he perform the (modified) anterior drawer test if he ultimately was going to let her continue to fight irrespective of its outcome (as we saw).
      I don’t see much of a problem in how he described it to the referee because he was probably trying to simplify his quick assessment to let the fight carry on rather than get caught up in the unnecessary medical jargon

    Yea that test for the ACL isnt always 100 percent accurate doc did mines and said oh looks like no ACL tear. Got the MRI and the my ACL was torn😭😂 sadly.

    I tore my ACL a little over a year ago. Went to the doctor wanting to get a MRI and instead, I got the Lachman test. The doctor told me I had partially torn my meniscus and that I would be good to go after a month of rest.
    6 months later, I went to a different doctor and had a MRI. The ACL was completely torn.
    Anyway the point is, if you can’t actually see the injury and can afford it/have free healthcare, do everything you can to confirm the diagnosis. Because I waited so long to get surgery, part of my meniscus was damaged and had to get taken out. FYI: there is no substitute for a meniscus. At the very least, get a second opinion.

      You got me a little shook, because my doctors did the Lachman test as well and told me I have a meniscus tear. I’m currently waiting on getting an MRI because the pain is sometimes unbearable and seems as if it’s getting worse. How was your recovery process?

      Tommy - It’s good to hear that you’re getting a MRI anyway. I hope it goes well for you. However, if it is a torn ACL and you opt for surgery, I can recommend you an orthopedic surgeon if you are in the PNW. He’s was absolutely awesome.
      My situation was different in that I was never in much pain at all. Just a lot of instability. I think that’s what messed up the diagnosis and why it took me so long to get what I needed. But I’ve healed almost twice as fast as I was expected to. Whether that’s because of my surgeon or genetics (or probably both) I don’t know, but I wish you the same speedy recovery.

      The doctor was absolutely right though, maycee tore her acl. So I guess you can diagnose a ligament tear without an MRI but, it is safer to confirm it with an MRI.

    The doctor had little time to his thing and he did a great job with the exam realizing it was a torn acl , but she wanted to continue .. A+ to the doctor for not taking that much time!

    Her and her father confirmed today that she has a fully torn acl and will need a 9 month recovery...while I know it is not the best way to diagnose you have to understand the conditions it is between rounds they don't want to give her an extended break if she is still capable of fighting so I'm sure it was a rushed test. I would agree he still should have done it properly and should not have spoken matter of factly about the injury but more in terms of "her acl is torn it is hard to tell how bad, obviously it is affecting her, blahs blahs blah"

      She’s doing it smart then. Most of the time nfl, ufc, or any other serious physical athlete will rehab like crazy thinking they can be ready in 4 months minimum. The acl has very little blood supply so its not fully healed and matured until the 9-10 month mark and even then most ortho’s will HIGHLY reccomend waiting a full year before returning to sports.

    The referee is probably the person who requested the doctor to look at knee. Further more, an exam of a knee is almost certain. Her knee was injured regardless of the test and you will find out she did have a small patlrtial tear.

    If she did indeed have an ACL year, is it really advisable for the fight to continue? I get it, these athletes are tough, but is it worth risking an even more severe injury by continuing to fight? She clearly showed pain when the doc yanked on it.

      I'd imagine having a leg go offline severely impedes a person's ability to defend themselves, and thus should definitely be grounds to stop the fight.

Also to address the very top cmmnt NO its not advised to continue the fact he said slight and its okay keep going is a false positive one falsely saying "shes fine" knowing there after if continued nah she wouldnt have been fine positive for the diagnosis being she did in fact tear the acl. But youre right as a boxer myself when youve endured and bad injury its best just to cut it as a loss because if not there could be long term affect which arent worth the moment of glory tbh.

    What was the big deal about a doctor examining a fighter between rounds. Isn't that what they're there for? No, I've never seen a doctor examine a fighter's leg, but don't remember seeing a fight where a fighter lost the ability to put weight on a leg before. I've seen doctors examine cuts up close and with a flash light though. He was just doing his job, but he did a poor job.

      I think that's the point, he did a poor job. If the doctor suspects, he should stop the fight. She clearly wasn't okay and an MRI would have shown that it was more than partial. Even if it started as partial, allowing a fighter to finish the fight and possibly exacerbate the injury is totally irresponsible.

    I tore an ACL in 2000. First doctor did same exam and said it was a sprain. No need to worry, go back to work. Knee was deep purple at the time.
    Second orthopedist braced himself like he was doing lat-pulls and my knee moved so awkwardly that he said, “enjoy your year off”.
    One thing I’ve learned is doctors assume they’re right, and can be very stubborn. Maybe it’s from writing published papers, or 12 years of school, but it sometimes causes suffering and lack of attention. Great video

    Orthopedic Surgery student here...
    A few things of note:
    1) The doctor in the mma video also had his thumb placed firmly into the knee which easily could gauge the condition of the ACL to an experienced physician.
    2) A doctor only would need to compare the other knee having never dealt with this patient before. Considering he is ringside and watching her warm up and fight, he may be familiar with the action in her knee movements.
    3) A diagnosis of full tear is not indicative of her status mid fight. A partial tear can become a full tear in a fraction of a second, especially in a fight.

      You can't feel the ACL on an exam like this. Putting the thumb there is actually not the correct position. Even if the test were done correctly, you still can't for certainty say partial vs full. Nobody commenting here is a physician so of course people see it was a tear and think oh wow his exam was right! But if you actually look at what qualified orthos on social media said, everyone was very critical of this at the moment

      He wasn't trying to feel the ACL. An experienced physician in a specialized field such as combat fighting would be able to discern between a slight and full acl tear. Actually writing a diagnosis in a modernized hospital would of course utilize MRI equipment etc. You are correct that a thumb in that position would not be correct for the procedure you stated, but it appears to me he was using the thumb to both gauge her pain level and feel for movement more precisely.
      To reiterate he was not giving a diagnosis, he was telling the ref she was okay to continue.

    Seeing all these comments grilling the Doctor about calling it partial tear. He most likely called it that so the fight would continue. If he told he referee she possibly tore her ACL there is higher chance he stops the fight

      But that's his job, to protect the fighters ("do no harm"), not help move it along. Assuming the tear was partial to begin with is fact, allowing her to continue and blow her knee completely is egregious.

      @@mediocrates1937 Part of that involves the risks involved in allowing it to continue. If a fighter has the arm broken from an armbar, the fighter could still fight but there's a significant risk to the hampered fighter and is typically stopped
      . Allowing a fighter with an obviously painful ACL tear to continue is crazy. Most fighters who have experienced them often make mention of how they'd rather break a bone than a blown ACL. The pain is worse, the surgery is more complicated, the recovery is three times as long minimum, and it may not heal up properly at all in the end, no to mention the rehab process.
      I can't help feeling it should have been stopped right then and there.

I have no doubt that Sutterer is good at what he does, knows far more than I ever will, but that doesn't mean that a keen eye or feel couldn't catch something that another didn't. Perhaps it was a guess, but none of us know, all we do know are the procedures normally used, and that the ringside dr was correct.

    I actually saw this when it happened, and I did a double take when I heard the doctor say she was "fine" with a small ACL tear. No. No, she was very much NOT fine with an injury like that. That should have been obvious without even needing to watch the rest of the fight and how she couldn't even put weight on that leg. How exactly the doctor was able to diagnose a "small, partial ACL tear" with his hands alone is another layer of baffling, but his boneheaded decision to say that she was okay to continue fighting is the dumbest part of the whole thing.

    I worked with this doctor for several years. Maybe he chose to use an improper description but in the end he was correct. He is a very good doc and I’m glad I worked with him and got to pick his brain from time to time. He is also a big patient advocate and tries to always do the right thing.

      I don't think it's a safety issue. Unless someone has a dislocated unstable knee I think they could keep fighting. I wouldn't let a basketball, soccer, football, etc player back out if I thought they tore the ACL, but mma is different

    Thing is, idk why they stopped the fight in the first place to check. Even if there was substantial noticeable damage or a dislocation of the patella etc. that isn’t a reason to stop the fight.

    What no one is saying is that the commission doc could have already have known she had a partial ACL tear from the pre-fight screening. YES it is possible and within the bounds of the state commission to allow fighters to fight these types of injuries...i.e McGregor at UFC 189...and if the dr felt no distinction from pre fight tests then that's maybe why he phrased it the way he did to the ref...Also if you've got a fighter adamantly telling you they are fine, in addition to the fact you're not checking the fighter on your own volition (the ref was the one who inquired for the dr) then that also could be why he said "...small/partial tear...she's fine" as opposed to just "torn acl"

    In Mexico my doctor checked my knee injury which was a stretched ligament the exact same way among more different angles he also checked my ACL that way , and it cost me 40$(2019) for the initial check up and 10$ for really good medicice and even gave me a full leg brace for 10$ more, in the US it was an estimated cost of 2400$ because i would have to go to the ER get checked up, xrays, and i saw on my cousins previous ankle injury a month prior that using the mri machine was already 1248$ the rest was the medicine and the check up
