Rotator cuff probably... when pushing in this position, we always have to press down into the floor actively... otherwise the scapula can loosen and sag into a cow back (instead of pushing into a cat back)... when your scapula form loosens too much, shoulders take over to hold the load... can put too much stress on a weak/strained rotator cuff (especially if your left shoulder slouches forward protracted much of the day).
