    “The hospital was charged $75,000”? Seventy five thousand dollars for criminal negligence with a defective autoclave where a human being would boil to death if they didn’t think to prop open WITH A PIECE OF WOOD?
    There should be hospital management in prison. For many years.

      The issues with the hospital and the Bacardi plant both stem from the same thing... greed. Prioritizing production or money over the lives of people. It happens constantly. I worked at a tire factory and the disconnect from those in the upper management offices to the floor workers particularly surrounding the issue of production over safety was vomit inducing. So many big businesses cut corners and will do anything to make an extra dollar so they look good for a board meeting or quarter review. Upper management and head of corporations positions are magnets/ breeding grounds for the absolute worst of humanity. Its amazing how many companies are led by people who are actual text book definitions of a psychopath, being very self centered and having little to no boundaries or remorse for their actions. An example is to look at almost any of the largest video game companies. They have some of the worst business practices and cultures in the business world currently and it's all because of upper management not having what is considered basic human morals or standards.

      @@scarlettrose842 Oh sorry! I didn't mean for it to sound that way. I just meant that video game companies are a great example of greed and turning a blind eye. They are a great example right now specifically because there is a lot of knowledge about these game companies and their greed being aired to the public. Serveral of the largest video game companies like Activision Blizzard for example are actively in court battles over things like greedy micro-transactions and sexual abuse charges as well as those sexual abuse issues within the company leading to a suicide on a company trip. I definitely wasn't trying to compare what happens in video game companies to being boiled alive in a steam room of a hospital. I just wanted to point out that many large companies have a serious issue with morality in upper management and game companies are a great current example of that breakdown on display to the public. The horrible things that happen from that breakdown is definitely going to differ from one company to another.

    That hospital should be held accountable for more than $75k. They were aware of a major safety hazard and deliberately neglected to properly fix it. Total negligence on their part. And sadly that negligence cost a life. My heart aches for her loved ones.

    I can't tell what I'm more appaled by. The gross negligence or the fact the young man's life was valued at less that $500k. Bacardi should have been sued millions for his death.

      The family could have sued for more than that, and OSHA SHOULD HAVE fined them WAY higher than they did.
      No proper training, no safety equipment, no ones following protocol... simple safety standards aren't being met that could have 100% prevented his death.

    Dae’s story just broke my heart. A young man who wanted to provide for his family and went to great lengths on his very first day at work to make an appalling impression And the company went to great lengths to prioritize quotas over Dae’s safety. A piss, poor despicable impression on Bacardi’s part. 😡

    The fact that Dae asked repeatedly if that's what they want him to do so he can get clarification and they just ignored him means they should have been fined wayyy more and thrown in jail

      If you're upset about that, you must not be aware of the first degree murder charges Ford slipped after intentionally, while being aware of the issues, still released the Pinto.
      It was deemed cheaper to just kill their customers and pay them out vs pulling the vehicle.
      Not a single charge was laid... First Degree murder!

      I WAS A TEMP & NOBODY ever told me about lockout/tagout procedures. I had several factory jobs where I thought it was “normal” to fix the machine while it was still running, and being a teen, I didn’t know any better
      .

Also, who fails the math portion of the armed services while studying to be a medical assistant? No wonder he didn’t get hired. Math is huge in the medical field. I worked in healthcare for 5 years before moving on to a logistics company and I used more math in my last job in healthcare than I do now

      It's also infuriating! When you work around machinery that can maim or kill someone, you don't just have them watch a 15 minute video and assume they're aware of all the risks. Shouldn't have sent a newbie to clean up that glass!! Very sad.

      This makes me soo sad and soo angry.
      A young black man's life, only worth $250,000? That's it? He still had a family, a young woman who loved him, and his future ahead of him.
      A company that can clearly afford 2pay out a whole lot more for the reckless disregard of this young man's life it's a damn shame.
      More ppl should know about how Bacardi handled this sad situation and how they basically didnt even change anything. I wonder if those disgusting employees even got fired.
      This is bullshit and some ppl dont like race brought into the situation but I know it does I know how these companies treat people of color and it shows when he didnt even make it out alive from his 1st day of work.

      Idk how they thought they could get away w not fixing the autoclave ffs, that’s literally the one piece of machinery that u cannot avoid using?? Like even if it were smth like an X-ray they could divert their clients elsewhere if necessary but there’s no way to get around using an autoclave unless they shut down the clinic.. the negligence & frankly recklessness is mind boggling & idk how they weren’t given an extremely harsh punishment considering their carelessness cost an employee’s life, who they seem to think is worth no more than 75k... I’ve seen injury lawsuits be awarded millions but this is what a human life is degraded to?? It’s infuriating tbh

    You know the third story is so tragic and I feel so sorry for this guy. The fact that he took a picture of himself in the bathroom because he was proud made me seriously brought tears to my eyes. It is so sad and unnecessary considering the fact that he just wanted to make some money to help his family.... sorry bro... u didn't deserve what happened.

    For the last story, I can definitely back up the lack of safety in a lot of warehouses. I had several instances where a supervisor put me in a dangerous position because either they didn't care or didn't care enough to explain something properly, even when repeatedly asked to show or elaborate further what they meant.

      I've had similar issues. That's why I will no longer work in them type of jobs. Seen horrible accidents, and was almost involved in 2. After the second one. I quit that line of work for good.

      @@MrBallen it is bad but there are certain situations where a machine must be on to diagnose the issue. In this case, a professional tech needs to be called in. At the time I was just an operator but they made me climb all over the CNC’s while they were on, whether it was trying to find a sensor or whatever.

    I was a temp once when I was younger. They made me lift heavy things all day knowing I was pregnant so I brought in a doctor's note saying I can't do that so they fired me. Sued and won. They thought I was dumb 🤣

      The problem is that those fines are hard coded in the laws and since these companies pay big bucks for lobbyists to sway the only people that can change the law to actually punish these corporations... nothing ever changes. The only thing that ever hurts these companies is when they do something that causes so much public backlash the it would be political suicide to defend them or the stocks bottom out and the company goes under. But we all know another business will just take its place, doing the same behaviors and practices.

    Being the eldest son from a fatherless household, getting himself education, struggling to get a job for his family.the happiness he felt when he told his mom as if life is gonna get better for them.the future,the possibilities,just gone.

    That poor kiddo! Doing anything and everything he could to help out his mother and younger siblings. Im sure feeling very high on life and so proud of himself when walking threw those Bacardi doors that very first day. ONLY to in just a short time, realize that he's just a second class citizen to that company and his safety (temp employees safety) is second to all others. Such a sad story. Not that what I think matters. But Im super proud of him and all he was attempting to achieve. It's extremely obvious that he had the potential to do ABSOLUTELY AMAZING things with his life!

They always settle for a higher amount with corporations and then OSHA and the like let them off when the dust settles and the public has stopped looking. every time.     I worked as a temp about 3 years ago. They sent me to this dirty shop with OSHA violation written all over it. I ended up falling and getting a concussion. I left only about a month after that happened, but almost a year later I found out that the fall I had caused more problems than I originally thought. The retina had detached in my left eye, and tore the macular hole. Well 4 surgeries and $50,000 later I’m left mostly blind in my left eye.

      Where i live government has just brought in new laws where employers can do jail time for negligence. Also the company you go to work at is responsible for injury costs and such so they have more interest in proper training and such instead of thinking of temp workers as expendable.

      Working in a factory, it's always what happens : safety does not go first, productivity does. Combine that with people who do not care enough wether by nature or because they are not paid enough, and you get a ton of accidents. I've seen and experienced many of them, people in my factory getting sent to hospital for some serious injuries, and heard about one death, all in only 3 years and safety is still lacking. Just today got a small machine explode on me that clearly had problems but no one did anything to fix it, thankfully it was a small one and it didn't burst into flame, but I've seen gas exits that could have sent the whole building blowing up and people getting seriously injured on fingers and arms multiple times, to the point where it's unrecognizable.
      And I'm willing to be this happens in many factories throughout the world, factories just need to be safe on visits and anything goes the rest of the year. It does not seem that dangerous when you work there everyday, but one mistake for one minute and consequences can be disastrous. I see people judging harshly employees for not taking this or that in consideration, not doing this or that, but you have to remember most of them don't want to be here, many work for hours and days with little in between to breath and are subsequently exhausted to the point where mistakes can easily happen, and when machines are new or on the contrary are very old to the point you know them by heart, combined with all that was previously said, mistakes happen very easily if you're not careful, and a surprising amount of people working there tend to forget about it.

(oh well) Because we had a strong union, they couldn't even write me up. We need collective contracts so workers are not afraid to do the right thing!

    I worked for a temp agency, and I can tell anyone who asks, the temp company really does not care if you have an issue with the place they send you to work, in their minds if you have a complaint the first thing they they think of is (NEXT !) . You lose your job and someone else will take your place fast. I worked in a bottling plant for weeks, basic training which lasted only 5 or 10 minutes, no safety film or instructions. After a few days They had me train other new hires to do my job.I did this for a few weeks then I asked can I be hired permanently since the 3 or 4 new hires that I had trained were doing good( I was also) and the new hire were full time employees with benefits.
    I sure could use the benefits as I had a family, but they told me to work for a few more months and then they would think about it. A few weeks latter I saw a worker get burned by melted glue that they use to glue boxes into cases of soft drinks. The hot glue is suppose to come out of a tiny nozzle under some pressure onto sections of the cardboard then its folded into a box shape, but the hose connection to the glue spray nozzle broke in half and hot glue shot out all over this worker face, neck, and hand. I saw him a few months latter and his face looked like it had been branded with stripes.

That is literally a mistake. A miscommunication. Exactly what I already said.
      Mistakes happen. I am a nurse and one of the time periods where the most mistakes happen in hospitals is during change of shift. You have new staff taking over and things can get missed during report. Of course there are safeguards in place to prevent that from happening, but mistakes will still happen, no matter how well trained someone is.
      It's not like the other employees conspired together to kill someone. They likely felt terrible about being a part of a deadly mistake. It's bad enough to have that on your conscience without assholes on the internet calling you a murderer for a mistake. People aren't perfect. Accidents are going to happen, no matter how much training and safety protocols are in place. In fact, it is accidents that help to get many of the safely protocols in place to begin with. You have to identify a problem before you can know there is one to fix.

    I’m a teacher in Jacksonville, FL. I work in a very poor community. I work with the kids who end up in situations like Dae. They deserve so much better. Whatever race or background. Every child deserves equity, and a real chance in life. I can’t watch the news because I’m just constantly confronted with what my students will face when they leave me.

      I both attended (briefly, thankfully) and worked in inner city schools and my heart always hurts for them. They were good kids with all of the cards stacked against them. I went into the social sciences though so I can study the problems and prescribe solutions, as well as law to see them out. The sad part is, most people don't even know or care. Even most of the teachers in the schools stop giving it their all because they get so worn down. I am so glad to read your comment and know that there ARE other people who are caring for these kids in their community. I commend you. I know it isn't always easy, no matter how much you care. It's hard to watch the dysfunction and feel helpless. It's why I left.

      Yeah not even negligence. That was just manslaughter. Although it’s impossible to tell what happened in someone’s mind. With something like drunk driving resulting in man slaughter it’s objective because the driver consciously was sober, decided to drink, and at some point chose to drive whether before or after drinking. They made that decision of their own free will. No matter what. With this, maybe the foreman just blanked that he was there. Definitely agree with you but it could never be because we can’t prove it was manslaughter. It was negligence at least. Definitions are super important

      I agree, 250,000 is a slap in the face. Millions should have come into play as far as I'm concerned.
      Safety should always be the top priority for any industrial production plant. Nothing is more important than making sure the lives of the workers are the #1 priority..... making sure that the people working for you, the people making you money, get home to their loved ones at the end of the day, not only alive but with all their fingers and toes. When a complete failure happens like this and someone is killed the cost to that company should be astronomical. This will incentivise the plants to make sure safety is built in to the mindset of all their employees. These things will only stop happening when these businesses have a safety first mentality. I hate to say it but 250,000 to bacardi is like me or you spending a 20 dollar bill at the gas station. It's a ridiculous settlement

    The last story is heartbreaking. I’ve worked in enough places that value productivity over safety and I’d honestly rather lose my job than risk my life in any way for a company that probably doesn’t even know your name. It’s sad that Day didn’t have that kind of knowledge or experience to protect himself. He was so excited too, ugh so sad.

    That last story about Dae made me feel physically sick. I’ve heard the story before but in a much more clinical fashion. The way you tell the story not only humanises Dae but also built such a connection with him and his family, I will never again be able to see this story as a simple “lock-out/tag-out” cautionary tale.       The fact they blatantly ignored the lock out/tag out procedures is what caused the issue. In good factories cleaning and maintenance are only done if the machine is rendered safe. As soon they told Dae to clean it without doing that I knew what would happen. They deserved far more fines and a larger payout. The operator and manager should have been charged with negligence.

    The aftermath of the autoclave and Bacardi incidents on Tracy and Day left me incredibly pissed off. Not just that they pretty much gave out the implication that both the deceased's lives value only a sum of thousands of dollars, the people that should've been held accountable get to continue working there, in fact the companies were even pardoned all because they promised to improve their safety protocols only after they've taken the lives of two employees as if those tragedies were as mundane as accidentally running a forklift car towards a stockpile of fragile items.

      They knew the payout could be so low bc they were already suffering from poverty and probably couldn’t afford a good / proper lawyer to take it to trial and the company probably knew that since they were already poverty stricken that anything is a lot so that’s most likely why they only gave the poor mans family 250k because to them that might as well be 250 million but you’re right it’s a big ass slap in the face.     My fiancé also works for temp agency. And we watched this one together, and I was saying how upset I would be if he ever got seriously injured or worse at one of his jobs they send him on and he responded with “that wouldn’t happen, I’ve watched enough mr.ballen videos to know when somethings dangerous” hahaha thank god for mr.ballen!

    That’s so tragic, that poor young guy was just trying to make an honest living for him and his family. To be excited and finally getting work and feeling good about himself, he literally gets crushed. As someone that has worked in manufacturing and had basically every member of my family work in manufacturing facilities, it’s so disheartening to hear about this kind of thing. Lock out/tag out is so so important and I truly feel for this man and his family

    Dae's story is why lock out tag out is so important for factory workers, and if you work in a factory remember you always have stop work authority ( the ability to stop work if you feel like you or your life could be in danger) and cannot be fired for using it.

    I worked at this factory up north. They were definitely more serious about safety. My maintenance manager told my team about one guy he had to cut out of an industrial mixer. The way he described it was “like cutting a person out of a tuna can.” The guy had already died by the time he got there. Lockout/tag out is so important and will save your life. Dae’s family was done all kinds of dirty.

      I've worked in a hospital before. The only way to get any problem that could result in injury or death fixed, is to threaten to go public with the info that they are ignoring safety concerns.

      They probably kept this very quiet, too. For me, this is a local hospital, and I've been a nurse for 13 years, and it's the first time I've ever even heard of the incident. I'm super surprised about how low the fine was, too. I've worked for facilities who were fined more for using eating and drinking supplies (cups, etc) made from Styrofoam instead of plastic. $75k for something that resulted in death is just...wow.

    I live in Hawaii, and these types of tunnels are pretty common on the big island mentioned in the Second Story. They're created by the lava that flows from the volcanoes that eventually stops running, and the lava finishes flowing through them, leaving empty Caverns before the lava tumbles into the ocean and creating hard rock that expands the land.
    If you ever end up in any of these tunnels it's pretty widely known that no one's going to come and get you.

    My dad had an experience similar to Dae’s. When he was 23 he was working in a textile factory and because shutdown/lockdown wasn’t done, while he was cleaning the conveyer belt his hand got pulled into the press. He managed to free his hand but one of his fingers was ripped off. My heart goes out to Dae’s family.

      I used to work in a factory at 25 years old and my hand got pulled between rollers and a drive belt. I couldn't reach the emergency shut off switch and luckily another guy on the machine shut the machine off. It was a terrifying experience and I pray that Dae's family is doing well. I pray for that young man and for those 2 co-workers that were supposed to be looking out for him.

      Honestly I thought she’d have died from that. Autoclaves can have super heated steam ( depending on the application) but maybe it was older or something. I talked to a dentist about his when it was broken , so I learned a lil bit from him

I'm shocked she survived, but never in a million years would I think about going into one of those alone, let alone it being broken. They have many warnings on ours that say DO NOT ENTER. Just crazy to think about this one..

    3rd story. My dad died from a somewhat similar accident. Wasn't even supposed to work that day. Something was wrong with a machine and they asked him to come in to help fix it. So he does, and gets into this huge machine. Little did he know, no one put the emergency brake on. 2 thousands pounds crushed him instantly. I was 7. Miss him every single day.

    My name is Jerry . I work at regions hospital from 2003 to 08 in transport. I was in the hallway when that women got burnt that morning. We had to bring a crash cart into the room. When she came out as they were rushing her to the ER. I prayed for her. When I learned what happened in why they didn’t protect her safety. That was one of the worst days. 🙏🏽

      I've worked temp agencies before and it's a toss up what job you get...out of all the jobs I was sent to whether in an office or a factory setting...the common factor is the negligence and rush put into the process of getting you where they need you and back to their business. Consideration should be taken when working with any machinery. It's infuriating that this system is set up in such a way that this man lost his life and his family had to endure the loss plus the battle of loopholes in court.

      Seriously, usually any warehouse or factory work through temp agency, is usually because that place can't keep employees, and there's a reason for that.... So they link with the temp agency. People between jobs who need work immediately are usually the ones who get stuck in these places.

    I didn't know that about temp agencies until my big brother got hurt at a plant and wound up with brain damage, he did get a lawyer and some money, but he actually died before everything was complete. That made me so mad at the world when that happened. He was a father figure to me, he raised me and taught me so much, and now he is gone cause of an accident that happened when he was working for $10 an hour.

    Fun fact: here in Italy we used to have a type of college specialized in teaching jobs, not only they had courses on safety regarding your chosen study course, but made you take temp jobs while making your boss to file reports about both the work and safety procedures taught each day, it was all financed by the ministry of education, but then education minister changed, laws were revised and even started a propaganda to shut down these schools, this happened almost 20 years ago, fast forward to today, we have more people unemployed and more accidents

      Right?! A walk-in autoclave would cost way more than 75K too. That’s the worst part. They saved money with her death...I hope they were also forced to replace it but that’s not enough punishment.

    Born and raised in Jacksonville Florida, my uncle was working at Bacardi when that happened. That poor family. They should have shut that place down forever for their negligence.. It was horrible.

      I was shocked by how little bacardi was fined and how little Day's family received. Not only did they not value his life and safety, but couldn't care less about his family. I suspect those responsible still have their jobs.

    that last one broke me. that poor young man had his entire life ahead of him, only to have it taken away because of lazy, greedy people who don't care for anyone. not enough money in the world can be paid for someone's life.

    That third story was devastating. Of course the fact that he was black played a role in his difficulties finding work as well. What a tragedy. Our country fails people like him all the time. I also felt horrified at the number amounts Bacardi had to pay, I was expecting millions (same with the hospital… WTF!)

      Same. It bothers me severely how little is put into safety and procedure. Even with things like apartment elevators or motor vehicle operators, people are constantly breaking safety rules, regulations etc, and nothing is done and none of these people care until life is lost and they have to pay a small fine. 😔

      I doubt him not being able to find work very easily had anything to do with his skin color. I do think Bacardi should have been sued for millions and those supervisors should be in prison.

      Thanks to affirmative action it's actually easier to get hired now if you are black. Same with getting into college and many other things. You don't even have to work as hard or be as qualified. It's harder if you're white or Asian.

    After giving birth to my son I had to find work ASAP so I went to a temp agency. The sent me to a factory that packages beauty products. On my first day they didnt even explain anything to me. Just gave me gloves and a hairnet and told me to stand in this line and do what everyone else is doing. They kept switching me around through out the day to other stations and would get annoyed if I wasn't working the line fast enough!.
    The machines we were working around moved so fast and all I could think is how fast I can get hurt because they didnt train me or anything!!. After that day I didnt go back!! And a few months later I got a letter and a check in the mail due to them being sued.

    That autoclave one is a perfect example of why hospitals should never ever "repair" machines whose design is faulty.

      I get that the machine's door was faulty, but how was the *design* of the machine itself faulty?

      the door is a part of the design; there should have been provisions to open from inside.. or at least to prevent from self-closing ..

      There's not much info in this video about the machine. The door was broken and fixed but supposedly it was a persistent problem as it kept breaking, which would be a design flaw. Thank you for the reply.

      @@encycl07pedia- it was faulty install, not design. It was not level so the door shut. It was fixable, they were just dumb, not a design flaw.
