It is covered at 11:00.. It has to be a US address of any family / friend or a colleague.. considering its a Us passport, assuming you would have some in the US that you know :)
Can you download the ds11 form and then fill it electronically? Make a pdf and get it printed from outside? Or how do you do it if printer is not available at home.
