I just did this, but used auto fill, then printed out the two pages. Its a much better look than hand writing.

On number #10 where it asks for your name "as printed" on your most recent US passport book: passports have the Surname first and then the first and middle initial if included so should it be Doe, Jacob Marley or Jacob Marley Doe?

Hello, for question #11 if by marriage in another country is the name change location the overseas city and state (as shown on marriage certificate) or the US city and state where the name change took place? If the US city/state, what date would be used? The day SSN and/or driver's license changed? The question is confusing to me based on my situation.

Hello, I have a question about the hair color section. If my natural hair color is black but there's some grey due to aging, do I still put black for the hair color?
If I previously had both book and card but now only want a book (trying to get this thing very quickly and figured a card would just slow it down). Should I enter my expired Card info on the form?

I'm having trouble with question 9. I recently made a small change to my name: I changed one letter in my first name, middle name, and last name. Could you please clarify how I should enter this information? Should I put my first and middle names on line A and my last name on line B?
those lines, A and B, are for separate former names, so you would write your previous name on A. And because it's likely that your name will require more space than what's provided, simply attach an additional sheet of paper to the application that includes the info

Question! Do I need to send my passport card with the form to get my passport book? My passport card isn't expired, but will I need to like trade in my passport card for the passport book?

Other than the travel area at the end, you did not explain if a space should say N/A, be blank or always put NONE if that question doesn't pertain to the applicant. I am referring to a name change when I have none. Can you elaborate? Should all blank spaces say NONE. or just leave blank. or say N/A. ?? Thanks.

I have a question. So I’m using the form to change my name on my passport. My maiden name is on my old passport. I’m changing it to my married name. So my married name is in section one and my maiden name is in section 10. Do I still write my maiden last name in section 9?

If you don’t have a passport book or card yet, you should apply with form DS-11. This form here, DS-82, is for renewals.

And if you didn’t get an official name change, how do you fill out section 9 for the name change? Just leave it blank?
I printed my application but I noticed that I left some boxes blank but I do not have an answer to that are optional but one of the questions is do I have travel plans and I do now so am I able to manually write that in in Black Ink before I send my application in and can I leave any other boxes blank or do I have to put in a manually
How do you know the fees when renewing? For example I want the card and the passport and get it expedited. Can I go to the post office and ask them ?
Good video but there are several contingencies you leave out....what if no middle name? Leave it blank? Or what if the word exceeds available boxes, i.e. city, country is longer than boxes provided?
You can leave out any info that doesn't apply. And for any info that doesn't fit, you can attach an additional sheet of paper and include that info there

In pade 3 or middle page on the top right , there's a box with small squares with the letters D,O,S and NFR. Is that for the use of the issuing officer? Thanks

Hi! I'm looking to renew my passport but I'm now using my married name. On box 1, do I write my name exactly as it was on my old passport (unmarried) or do I write my married name? Then on box 9, maiden name or married name? Thanks!
Hey! I had the same question, I think you can write your married name as long as you’re sending in your marriage certificate. That’s what I’m doing! Would love for him to appt and confirm this is correct :)

student would be okay, unemployed would be fine...I dont think that one matters too much because it's not info they include on your passport

Does it have to be all caps? I did it like normal. I was afraid they would require caps..
For field 10, it says 'as appears' which on passports is last, first, middle. When filling this out however you enter first middle last?
"as appears" refers only to the spelling of the corresponding item
Uh oh, the clerk at the office told us to do last first middle!
I'm sure it won't hurt if you included commas, they'll understand how it's supposed to be written. But definitely double check your data when you get passport back to make sure it's all accurate 🙌
What if your not employed or go to school? Or you have a remote part time job in the u.s but none in country of residence

include as much information as possible. anything that doesn't fit can be added on an additional sheet of paper

Same place it would belong in your name, I.e. after your surname
Can you fill out the forms online and then print it out and sign it instead of hand writing it.

What if my first name has 26 letters and it does not fit in the boxes? Surprisingly my expired passport can fit my whole first name, but this application does not have enough spaces. What do you advise that I do? Someone please help me.

What if for hair color, your hair is greying out, so it’s sort of a combination of black and grey. Do you write black? Or grey or both?

If you are a Jr and it’s included in your passport do you include it in the middle name or what are the appropriate sections? Jr is in the given names section of my passport.

Regular stapler is fine. I made a video about passport photos where I believe I showed the way to do it

Her name on her passport has always included my last name.

An x or check mark will suffice. As long as the information is clear, it’s usually okay.
What about the box that you left 'blank' on the top right corner on page one. The box has End.# and Exp. Is that box to be left empty?

What if your name is TOO long for Question #10?

I have a different question. My printer didn't print the shades of gray, basically everything that's meant to be blue but on your form is gray, it only printed what isn't blue but what is black.. does that matter? Because it's just shaded background
The colors don’t matter too much as long as everything is clear and legible. What your write should be only in black, however.

My passport has 1 month leave before the expiration date. Can I still renew my passport? I will choose to expedited option. In some questions if I don't answer do I need to leave it blank or use N/A? Another question is travel plan. I have plan to travel at the end of this year 2024 but not very sure on the date do I need to write it on?
You can renew. It’s okay to leave some answers blank. You don’t have to include your travel plans.

Which date do I use regarding my marriage certificate? There is the date issued, the date it was received from the officiant, our marriage date, and the date I picked up the certified copies!

Good video! I was wondering what to do with the top right box that has 4 small boxes underneath labeled D,O,S,NFR.. I see you left them blank, no need to fill. And section next to picture name as it appears on current passport... your presentation shows First name first, middle then last, thank you, because my passport shows last name first....
The top right box is office-use, so no need to do anything with that. For your name just use the template that I described here because my last name appears on top of my given name on my passport as well

The photo do I attach it with staples or paper clip? I only attach (1) or (2) photo's?
Edit: State Department got back to me. They want (4) staples close to the edge vertical (2) on the left side and (2) on the right side. The forums shows where the staple go. If you use paper clip or some other sort of other methods. The photo will get lost and there going to be delay's.
