    What is PIH
    Post-inflammatory hyperpigmentation starts to occur after some type of skin injury or inflammation when the skin is left darkened and discolored after a wound has healed. This can be from your good ol typical acne, or sometimes even triggered by dermabrasion or a chemical peel. PIH is a type of hyperpigmentation of the skin that is mainly triggered by the way skin regenerates post inflammation.
    PIH vs PIE
    Its different than post inflammatory erythema which leads to pink and red marks after acne and is seen more in lighter skin tones. And is different than melasma, which you often find in hormonal changes in the body.
    How does PIH happen? What causes post inflammatory hyperpigmentation?
    Post-inflammatory hyperpigmentation can occur in anyone but youll see it more in darker-skinned individuals. Fitzpatrick skin types III-VI to be exact since we got more melanin and produce more melanin.
    What Causes PIH?
    In terms of what causes post inflammatory hyperpigmentation. Most people will fall under 3 of the biggest reasons.
    1: Acne vulgaris
    2: Atopic Dermatitis
    3: Impetigo
    There are of course other reasons as well. Contact and atopic dermatitis, insect bite reactions, psoriasis, burns, chemical peels. If youre thinking why on earth is there an overproduction of melanin that comes with all this, you have to take a look at the pathophysiology: basal keratinocytes, which are your skin cells, when they get damaged, they release large amounts of melanin, more than they should, they then get phagocytosed and deposited, either in the epidermis, which is easier to treat and can go away on its own or the dermis, causing a blue-gray discoloration of the skin.
    Will PIH go away? What does PIH look like? Types of Post-Inflammatory Hyperpigmentation
    1.) lichen planus actinicus, which a lot of times happens on sun exposed areas of the skin where you dont think to put sunscreen, like the back of the neck.
    2.) Violin playing friction, which is also known as a violin hickey, that happens to violin players who have to rest there instrument on the neck. 3.) Irritant dermatitis, which you can see happening to the knuckles
    4.) Inflamed cyst, which can sometimes leave you a long scar that takes a lot of time to heal.
    5.) Oven burn.
    6.) Photodynamic therapy.
    How to Treat PIH. PIH how to get rid. When does pih go away?
    The first thing you want to do is protect against offending agents, so your daily protection against ultraviolet radiation is a good start. Spf 50 or more, wearing a wide brimmed hat, sunglasses that dont have metal, since they absorb heat and disperse it back on the face, and seeking shade especially between the hours of 10 am to 4 pm.
    What to do PIH?
    1.) Tyrosinase inhibitors, which prevent melanin production.
    2.) Sometimes though, depending on the severity, you may need a slightly heavier lifter, like tranexamic acid, which has been seen to diminish laser induced melanogenesis.
    3.) Topical retinoids like tretinoin, adapalene, and tazarotene, might be the next solutions to look at.
    4.) Chemical peels, if youve been consistent with topicals and theyve failed. They basically work by removing the epidermal cells containing the excess melanin. Glycolic and salicylic acid are two big ones there.
    5.) Q switched ruby laser that was done on the indian population and found it to be 70% effective, especially in those with PIH from acne.

      I have post inflammatory erythema since 2 year and i do 4 session pdl
      The redness decrease but i see my colour of face dont same
      I fell i hade hayperbegmentation
      I nedd product to help me but my skin very very sensetive

    I tried Paula's Boost with 10% Azelaic Acid , but it didn't work for me, I'm thinking of doubling down and going into a stronger azelaic version of 15% as a base, and
    than apply something over it if necessary, maybe Niacinamide or AHA, or Kojic Acid for a 2nd layer of treatment,

    Love this video! I have PCOS and a gluten intolerance. I used to have HORRIBLE cystic acne. I started getting chemical peels and it helped with that tremendously. The esthe there told me to go to an allergy specialist.. from there I found out about the gluten allergy. So the foods I used to eat causes me to have bad acne. So I stopped… dairy, too much sugar etc
    Now I don’t get the cystic acne anymore, but I do have pih and work with my dermatologist/esthetician to rid it. Now I just get mild pimples… Paulas choice is my fave to clear my acne. I use cetaphil, and dermalogica also. I have the purple bottle you showed from PC, I’ve used their salicylic acid, and niacinimide by the ordinary faithfully. Derm prescribed azelic acid and tretnoid. I need to try the others by Paulas choice.

      Thanks for your feedback! I’m glad the chemical peels helped you! That is so interesting that it was from a gluten allergy. Food does play a role contrary to some beliefs. Did stopping dairy help at all too? PC has always been my go to recommendation. I like the products they have. Feel free to share a future update if you like. 😃

    Thanks for the information! I did a touch up cosmelan 1 over a week ago and it made my skin horrible with PIH. There is discoloration and hyperpigmentation where the mask was applied but no peeling . I’m so disappointed. The first round was ok and I used cosmelan 2 home care. My esthetician didn’t give me cosmelan 2 for the second round and said it would peel on its own . It did not but caused acne and discoloration . Not sure how to correct

    I got some darkspots after 4 days of completely peeling 😞. Is that normal ? I have too many pigmentation but i never exposed to the sun. Now it is almost 9 days and still persist ( I got superficial chemicalpeeling)

      You are young, your skin will heal quicker over time. Look up tca peels, glycolic, asyilic acid. To lightly peel the top layers off over months. Just don't use too high a percentage to start with. Platinum skincare allows you to buy online and do yourself, for cheaper.
