    Hi,
    1. With regards to mesotherapy apart from the active ingredient dutasteride/ finasteride what should the inactive ingredients of the solution be?
    2. Is there a risk of scaring? Apparently too much solution volume and incorrect inactive vehicle can cause scaring?
    3. With regards to regular topical application of dutasteride/ finasteride what is the best vehicle? I have heard that it needs to be lipophilic to be properly absorbed eg liposomal or trichosol whilst alcohol penetration is very poor. Also some companies add growth factors like trichoxidil so thoughts on this would be good. Many thanks

    When you use topical finasteride, even if you would use it on a balding area wouldn’t it penetrate the skin and ultimately reduce the DHT levels and thus work on thinning areas also? In other words, it’s simply a different delivery system to drop DHT levels? Therefore, why does it matter where it is applied on the head?

      The idea of topical finasteride is that it works directly on the 5AR (enzymes that block DHT formation) found on the scalp. Ideally, you wouldn't want to affect your systemic 5AR (in the blood, in other organs as well), because that is where you could potentially get side effects from. But concentration matters for topical finasteride. If you use a concentrated one, it would significantly affect both of the 5AR on the scalp AND other parts of the body, which is the same as taking oral finasteride.

    Please, please, please, talk more about mesotherapy. I think my doctor here in South America has never done a treatment based only in the use of mesotherapy, without any oral alpha reductase blocker. She said I will be doing mesotherapy each six months, but you guys said 2 months and maybe it is because two months is the ideal frame when mesotherapy is the only technique being used as a treatment. Please, talk about this. I may suggest my doctor that I will be doing it each two months. She will agree.

    Great episode!
    Here in the UK I was introduced to GFC (Growth Factor Concentrate). It seems like it's a variation of PRP but some sort of growth factor activating proteins are added to the solution before injection. I was curious as to what this really is. Is it something that you doctors are familiar with but under a different name or something else. Naturally, I was curious about the effectiveness of it as well.
    I've heard it had better performance than regular PRP, but it might be a bit self-induced thinking. I would love to hear your thoughts on this. Thanks

      @@SHAMSHERA- Hey. I've been on the medication for the past 9 years. One thing I've learned is that, nearly all of the side effects are reversible. So, if you (God forbid) had an unwanted side effect. You can either reduce the dosage, or reduce the frequency, or stop the medication. Everything will then go back to normal. Its a win win situation. Answering your question. Yes, you are over-reacting bud. but it's normal. Good luck!

    Guys. There are now 3 “Minoxidil Boosters” on the market that supposedly up-regulate SULT1A1 in the ORS to improve performance for those that don’t respond. Only one of these has good science IMHO and this is now on sale in Asia (not the USA where it was developed). Also in Brazil you can buy Minoxidil Sulphate topicals directly. Btw. It’s the same enzyme in the liver, just a lot more of it. Lastly, Minox directly impacts the Androgen Receptors and effectively blockades the DHT.

    i was on finasteride for 4 years with good maintenance but wanted more growth. i started dut 8 months ago and have lost a lot of density at the temples and my scalp is hurting. what should i do?

      Since dutasteride raises testosterone, maybe that’s what’s making you lose hair. But it could be maybe a really long bad shed. I had that with fin where I shedded 100-200 hairs shampooing for like 4 months and I remember wanting to quit so baldly. Now I lose around 5 hairs. So maybe wait it out.

      sounds like you're going through a shed. The hair will grow back, just stick with 0.5 dutasteride. at 0.5 dht levels will be reduced near 98%, no need to go up to 2.5.

      @beace4436 Stay on dut for a year. If at that point you’ve only noticed loss of density and AGA progression, go back to Finasteride. You might have the rarer gene that alters genetic response to dutasteride and where you will respond better on fin

    I think that changing the diet is a major factor in the use of minoxidil in as efficacy many people eat a western diet which is full of chemicals and manipulations that are initially just bad for the human body and health so may be changing the diet to a more clean and natural source for nutrition could be key in the utilization of things like minoxidil. Frankly I do notSuffer boulders but I have many friends with you and I noticed that they eat a very impure and putrid diet and they use things like minoxidil with minimal results, anyway it’s just a thought on my part I think proper nutrition things like fasting which also encourages the body to regenerates cells and mitochondria is another way to go

      Finasteride was working very well.. however last year I noticed that my scalp was showing a little again.. so he said that probably finasteride had done whatever it could for me and switched me to bicalutamide.. I have no clue whether it is the right thing to do..

    Good day dr. Will finasteride affect athletes performance? I see many high level athletes such as rafael nadal, wayne rooney, lebron got multiple hair transplants but continued to thin presumably of not taking finasteride. I play tennis and soccer at an amateur level and don't want finasteride to affect my performance. ( I am 21 years old with diffuse thinning on my crown and mid scalp)

      It won’t affect strength/muscle size per say but can decrease your aggression to or “drive” which,particularly for a very physical contact sport I can see it affecting it. Now weather it does that to you and if so how much is dependent on the individual.

      @@alexkreyn315 You can do it anytime really. If you don’t have sensitive skin then you can put tretinoin on before bed, wait 15 minutes, and then apply the minoxidil. Both of these can irritate your skin so if you have sensitivity then you may want to put on tretinoin at night (as it is photosensitive) and then minoxidil in the morning.

    I have no interest in localized topical solutions. Anything that soaks my head/hair for 2 hours twice a day, prevents me from sweating for 8 hours a day, and makes my hair look nasty unless washed, is terrible. I was so excited to be put on oral, I couldn't care less if I grow back hair.

    Hello ! The first time I used Minoxidil my hair grew and my scalp was not dry. I took a break and returned to Minoxidil 2% for the second time. Please advise me, I have been using Minoxidil 2% for about 10 months, at first the hair loss decreased, scalp became dry, short hair falls out. Now the hair loss has increased. Is there any sense to change to Minoxidil foam 2% or not? Maybe 2 % does not work anymore, that's why hair is falling out and we should switch to 5 % and how to get rid of dry scalp? Thanks !

      reddit does have lots of fakes trying to advertise. posting before and afters that should be impossible without a hair transplant, probably promoting hims/keeps/happy head etc.

      Tried myself topical latanoprost, very expensive and almost no visible effect after 4 months, I switched to topical minoxi + dutasteride and had way better results at a fifth of the latanoprost price.

      I would use tadala morning...and oral minox at night.
      But I'm quite sure If you take both in the morning wont be a problem. 2.5 of minox is a small dose. For blood pressure the usual dosage (minimum) is 20mg. Make a test.

    Could you please talk about DUPA, retrograde alopecia, and alopecia areata incognito? I'm currently thinning on the sides and on the back and I wonder if you could offer any advice on rarer forms of alopecia? Also, you kind of alluded to this in your video, but could you discuss more about why some medications have no response on the user. Like when you were discussing that minoxidil can only work on certain types of people who have a corresponding enzyme that allows the medication to produce results.

      I‘m also suffering from something like DUPA or retrogade alopecia as i‘m thinning agressively on the whole head (also sides) despite being on dutasteride the most POTENT medication. I started with Dut 9 years ago but yeah.. norwood reaper still destroying my hair. About to give up.

      @@simonecusumano3170 Yeah. I'm thinking it might not even be related to DHT. It might just be inflammation of the scalp triggered by an autoimmune disease or something. It's great for other people out there who have min/fin/dutasteride to fall back on but for these other types of alopecia, there needs to be more research on how these problems are triggered in the first place. It's really frustrating when you don't even know the reason why your hair is falling out and no one can really tell you. I'm not even sure if a scalp biopsy would help in these circumstances. I've looked into other stuff like RU58841 and jak inhibitors, but really not sure if I want to go that far with treatment personally.

      Hey so I have dupa as well. Maybe not dupa but like a scarring type of hairloss in the lower back. What helped me was the following 1) minoxidil sublingual 1mg a day. 2) minoxidil foam. 3) topical finasteride + 0.003% lantanaprost + some steroid from happy head. That literally eliminated any itchiness I was having in my head. 4) 0.1% topical lantanaprost from strut 5) topical dutasteride 0.025% mixed with steroid and 7% minoxidil. Good luck

    I have been taking topical minoxidil daily for around 7 to 8 years but now the greasiness of scalp and on some hair coupled with a few pimples/spots sometimes on scalp is enough. So want to switch to oral minoxidil. I have never suffered any sides on minoxidil regarding fatigue, cardiac or breathing problems. Ive had numerous fbc blood tests over the years always come back in the good to normal ranges as told to me by my gp but my gp refuses to prescribe me oral minoxidil and advises me to keep applying topical minoxidil but the greasiness and a few pimples are irritatting now so I would like to know what amount should i start oral minoxidil on? 1.25mg, 2.5mg or just like the topical minoxidil of 5% once daily? Fearing that if I start too low on 1.25mg I might start losing hairs instead so would it be wiser to start on 2.5mg for say 2 weeks and see if I get any sides and if not then jump onto the same 5mg x once daily tablet? Pls reply and your advice, experience and opinions are most welcome. TIA

      Get a pill cutter, cut 1mg pills in half and take 2 or 3 days a week, check the side effects for a month. If non sides, up the dose. I experienced side effects so dropped it for now

    Good day dr.. I was using minoxidil topical application and finasteride 1 mg for past 2 yrs under the guidance of an dermatologist. Dr told to switch minoxidil topical to oral minoxidil as he didnt find good result. And the oral minoxidil was continued around a year.
    For the past 4 months i stopped the oral minoxidil and was continuing with only finasteride as i felt palpitation whn i do works.
    But now my hairs r shedding which worries me a lot. Can my hairs be back to normal since i m still using finasteride?
