I actually just had my top surgery three weeks ago, and tonight while I was little too medicated I started really thinking about my procedure, wondering how the incisions were joined together and how the fat was taken out and whatnot. This was extremely informative and easy to understand (even while high) and it’s made me appreciate the results my surgeon procured even more. I wish I had seen it prior to surgery to discuss more of this with my surgeon, but luckily she did exactly what customizations I would’ve needed without me asking. I’ll definitely be recommending this video to my other pre-op trans masc friends!

Hey Dr. Mosser, I have a couple questions that I don’t think you covered in your videos. 1) Is it possible to be put under with gas before inserting an iv or needle into your patent? Many of us with fears of needles and anxiety are needing to know. I understand it differs from doctor to doctor on whether they are willing to do it or not, but I would like to know if this is possible. 2) Does the patient’s period affect the result? My chest gets tender and achy when it’s shark week for me. I also notice the chest itself has a different texture to it. Does it matter if your chest is sore? Thank you for your time -Luke
Hi Luke, I’m really glad you reached out to us here with these questions and I’ve done my best to answer them as clearly as possible. Please share any more questions that come to mind. In regard to your question about your period, one’s period does not affect the result - the breast tissue is removed regardless. With regards to needles, we let the anesthesiologists decide what is necessary. We find that after a conversation with the anesthesiologist, patients virtually never need additional help, as the insertion of an IV by a competent individual is (the vast majority of the time) both relatively painless and quite simple. I have a question: I’m really wanting to get this surgery because my chest gives me a lot of dysphoria even with a binder but I don’t know when I’ll be able to but I’m also a trombone player so would I still be able to play trombone right after the surgery? Or how many days would I have to wait until I can practice again? Also I live in Canada so I don’t know how different getting on the waiting list is.

I’m have too surgery on December 5th! Very amazing explanation. I had a question. The tips of my nipples are larger and don’t look like most men’s. They are average for a woman but big for a man. Do the tips have to look the same on my nipple grafts or do some surgeons trim them down to a size that looks more masculine?

That's a great suggestion! I'll definitely submit that when we create new versions of these videos. It's important to show a variety of body types. How old must one be to get this surgery? I am turning 14 in April , my cheat dysphoria has been getting really bad. Do you think I could get it in a year or two? Or do I have to be an adult?

Depends on the country, in spain i believe you can get it done at 16 (maybe younger) with a parents permission, but it's usually €5,000-€6,000 depending on where you go
I also have lots of dysphoria with my chest. I bind all the time but it’s still not quite enough. I’m 19 right now and I’m really hoping I can get top surgery soon. I’m also hoping to do hrt eventually but I’m not as in a rush for that but my round chest really gets on my nerves.

Great question! Though the first few days of recovery should be spent resting, you are encouraged to move around. However, you should initially minimize many other physical activities, especially raising your arms or lifting over 5 lbs. Taking it easy will minimize pain, discomfort and other possible complications after surgery. Typically patients resume non-physical activities after 8-10 days. Hopefully you'll be able to get to this but, I'm pretty over weight and one of biggest concerns is my chest looking weird if it were to be completely flat because cis men will have a bit more fat on their chest if they're overweight. Do you suggest I lose weight before looking into surgery or is it something I don't have to worry about? Also, will being overweight affect the result?
This is a great topic to bring into discussion! It's always suggested that folks try to get to their most healthy weight before top surgery. It's true that some FTM/N surgeons have weight or body mass index (BMI) requirements and some do not. Why is there a difference in opinion/practice? Some FTM/N top surgery doctors have a weight or BMI requires primarily because of literature studies that have revealed increased complications with tummy tuck surgery or breast reduction in patients with a higher weight or BMI index. At Dr. Mosser’s practice he hasn’t seen those risks carry over so he does not have a weight or BMI cut off. He has performed top surgery on folks with BMI’s as high as the 40 and 60 range with good results. I hope this provides some encouragement and let us know if you have any other questions!

Dr Mosser wanted me to share this answer: "I have performed DI FNG surgery with someone with a VP shunt. Keyhole would be very difficult to do and might not be possible. We would do this in coordination with the neurosurgeon and have a plan with that person in case anything happened to the shunt during surgery. In the case we performed, the shunt was seen clearly and was unaffected by surgery."
Is there a way to ensure that the scars dont connect? I really don't want that when i get sugery, but i do have a larger chest so im worried that might not really be up to me
That depends on the individual, their anatomy, and the kind of top surgery/results they want. Not everyone has to have one big connected scar. Many can have 2 separate scars. Your best bet would be to schedule a consultation with the surgeon(s) of your choice so that they can give you a specific answer. Different surgeons can also have different opinions about and/or approaches to 1 scar vs 2.

When you get this type of top surgery and you've lost a lot of weight so there's a lot of loose skin, does this affect the end result? And will you still see the stretch marks?

Hello! This is a really late comment so I'm not sure if anyone's going to answer, but How young can someone be to get Top Surgery? Im 15 at the moment and my main issue is my chest, They're Getting big, they're a little bit uneven and I get alot of pain from them. My #1 dream is to get top surgery as soon as I can get it. Also, I know it costs alot, but my family has very hard time with money so Im really nervous about not being able to pay for it, can social workers/Insurance/whatever Pay if they could? Hello doctor! I saw somewhere in the comments that it was possible for a minor to have top surgery if both legal guardians agree to it. I wanted to ask if it was necessary for both legal guardians to agree, can it be one legal guardian? What if it were the case that the second legal guardian isn't uhm I don't know how to explain it... not able to accept. For example, maybe divorce parents.

In a general sense, we typically do not recommend leaving behind breast tissue to mimic the pec because this tissue is firm and rubbery and doesn't resemble muscle when you touch it (although in photos it might appear like a more muscular chest). A person can choose to leave behind more tissue to have a more full result, but we would seek to set expectations that in actuality if someone is looking for a muscular appearing chest, it might not exactly have that effect. A patient can request "no nipples" if they so desire. Keeping one's nipples is not required. Some people choose not to have nipple grafts, but they will get nipples tattooed on later. Others prefer to keep the "blank" look of a chest without nipples.

i have a friend whos dysphoria is so bad i worry he might kill himself. none of us have enough money to help him. what do we do? he is an adult but he still lives with his parents because he is disabled. his parents are transphobic and will not care if he dies.

Hey Dr. Mosser, I have a couple questions that I don’t think you covered in your videos.
1) Is it possible to be put under with gas before inserting an iv or needle into your patent? Many of us with fears of needles and anxiety are needing to know. I understand it differs from doctor to doctor on whether they are willing to do it or not, but I would like to know if this is possible.
2) Does the patient’s period affect the result? My chest gets tender and achy when it’s shark week for me. I also notice the chest itself has a different texture to it. Does it matter if your chest is sore?

I’m really glad you reached out to us here with these questions and I’ve done my best to answer them as clearly as possible. Please share any more questions that come to mind.
In regard to your question about your period, one’s period does not affect the result - the breast tissue is removed regardless.
With regards to needles, we let the anesthesiologists decide what is necessary. We find that after a conversation with the anesthesiologist, patients virtually never need additional help, as the insertion of an IV by a competent individual is (the vast majority of the time) both relatively painless and quite simple.

I’m have too surgery on December 5th! Very amazing explanation. I had a question. The tips of my nipples are larger and don’t look like most men’s. They are average for a woman but big for a man. Do the tips have to look the same on my nipple grafts or do some surgeons trim them down to a size that looks more masculine?

How long would it take to heal after this surgery?
Great question! Though the first few days of recovery should be spent resting, you are encouraged to move around. However, you should initially minimize many other physical activities, especially raising your arms or lifting over 5 lbs. Taking it easy will minimize pain, discomfort and other possible complications after surgery. Typically patients resume non-physical activities after 8-10 days. Hopefully you'll be able to get to this but, I'm pretty over weight and one of biggest concerns is my chest looking weird if it were to be completely flat because cis men will have a bit more fat on their chest if they're overweight. Do you suggest I lose weight before looking into surgery or is it something I don't have to worry about? Also, will being overweight affect the result?
This is a great topic to bring into discussion! It's always suggested that folks try to get to their most healthy weight before top surgery. It's true that some FTM/N surgeons have weight or body mass index (BMI) requirements and some do not. Why is there a difference in opinion/practice? Some FTM/N top surgery doctors have a weight or BMI requires primarily because of literature studies that have revealed increased complications with tummy tuck surgery or breast reduction in patients with a higher weight or BMI index. At Dr. Mosser’s practice he hasn’t seen those risks carry over so he does not have a weight or BMI cut off. He has performed top surgery on folks with BMI’s as high as the 40 and 60 range with good results. I hope this provides some encouragement and let us know if you have any other questions!

Would the person having a VP shunt be a disqualifier?
Dr Mosser wanted me to share this answer: "I have performed DI FNG surgery with someone with a VP shunt. Keyhole would be very difficult to do and might not be possible. We would do this in coordination with the neurosurgeon and have a plan with that person in case anything happened to the shunt during surgery. In the case we performed, the shunt was seen clearly and was unaffected by surgery."

That depends on the individual, their anatomy, and the kind of top surgery/results they want. Not everyone has to have one big connected scar. Many can have 2 separate scars. Your best bet would be to schedule a consultation with the surgeon(s) of your choice so that they can give you a specific answer. Different surgeons can also have different opinions about and/or approaches to 1 scar vs 2.

Hello! This is a really late comment so I'm not sure if anyone's going to answer, but How young can someone be to get Top Surgery? Im 15 at the moment and my main issue is my chest, They're Getting big, they're a little bit uneven and I get alot of pain from them. My #1 dream is to get top surgery as soon as I can get it. Also, I know it costs alot, but my family has very hard time with money so Im really nervous about not being able to pay for it, can social workers/Insurance/whatever Pay if they could? Hi Jayce! Thanks for feeling comfortable to share with us. It is possible for those under the age of 18 to get top surgery. The process is a little more involved than the adult process, though. I wanted to ask if it was necessary for both legal guardians to agree, can it be one legal guardian? In a general sense, we typically do not recommend leaving behind breast tissue to mimic the pec because this tissue is firm and rubbery and doesn't resemble muscle when you touch it (although in photos it might appear like a more muscular chest). A person can choose to leave behind more tissue to have a more full result, but we would seek to set expectations that in actuality if someone is looking for a muscular appearing chest, it might not exactly have that effect. A patient can request "no nipples" if they so desire. Keeping one's nipples is not required. Some people choose not to have nipple grafts, but they will get nipples tattooed on later. Others prefer to keep the "blank" look of a chest without nipples.
