    I've never seen a surgeon actually FOCUS on non-binary results. I've never been able to articulate why I might want my results to slightly vary. Listening to the beginning, breaking down the binarism of the chests and the gray area in between... You knew my thoughts beter than me!

      @@genderconfirmationcenter why is non-binary always taking from a position of the people assigned female at birth. Why not the people assigned male at birth? Can they not get masculinization surgery while also getting BTM surgery and vaginoplasty. There are some XXYs out there...

    I cried. I just cried because I thought I would have to lie and say that I'm trans, which I'm not, to have my top surgery and you're here. As a nonbinary borned woman who never wanted to have a developped chest... I thank you. I hope I can find someone like you to operate me soon (I'm European) I'm in agony and everyone thinks I'm crazy.

      @@angelicgacha Have you ever worn a corset?
      No, not the ill-fitting ones worn by actors - a properly fitted one that has been worn for short periods until it can be worn for hours. Like a new pair of shoes.
      As a large cup size I've always found corsets far more comfortable than a bra, and definitely easier on the shoulders. Plus different styles of corset give different chest shapes! :)

      Yeah I never asked for a chest but it came early (thanks to my uncle). I lived with it for 45 years but became increasingly more uncomfortable once menopause hit and I began gaining weight. My body began to look more feminine and worse, I began to look like my mother! I'm having top surgery this year but don't know what to do about my giant hips if insurance doesn't approve contouring/lipo. Plus the older I get, the higher the surgical risks.

    Wow. I am Non binary, this is amazing. I actually super agree with gender infinite as well. It has bothered me for a while why people need to be placed into tiny boxes *AKA categorizations* instead of being free to be themselves. I used to like labels because I found solace in knowing I wasn't alone, but now I just see the world as an infinite rainbow of possibilities. I call myself non-binary sometimes still because it is something people can at least somewhat grasp. In reality, I have no name for who and what I am, I am just a person, and I just want to live a good life and love myself for who I am.

    8:15 Thank goodness, that was EXACTLY what I was worried about!! I was worried that surgeons wouldn't take me seriously because I, born a female, still identify as female, but feared the idea of growing a chest since I was 8 and have bad chest dysphoria now with DD cups. Hearing you say that plenty of people going through the same thing as me getting top surgery is so relieving, especially since I live an hour from San Francisco and am considering going to the Gender Confirmation Center for top surgery.

    I'm non-binary and I've both been wanting top surgery and scared about it because I don't want a masculine chest. This really helped me figure out what kinds of non-binary features I'd like to have with my chest! Thank you, this means a lot. It's difficult finding information on non-binary top surgery and how it can differ from top surgery for trans men

    ok so I am FTN and I really want top surgery. there is not much info out there that focuses on non-binary surgery as this video does. I didn't even know that having a blank chest was a thing but it completely resonated with me. this video was so helpful, I have never really been able to articulate into words what I want but this video explained things so well, like genital nullification. I really hope that you might be my surgeon someday because you just seem so good at what you do

    This guy’s really good at his job. My NB chest surgery turned out really good, but the surgeon definitely was like “here’s how I do it...” not “let’s talk about all the options.” They made a point of “warning” me 3 separate times against not getting nipples and then shrugged and said “ you can always get tattoos” even though I was adamant.

      Dr. Scott Mosser, Transgender Top Surgeon Hey, thank you for answering so quickly! I would enter the plane immediately to go see Dr. Mosser, but I guess the cost of travelling to the US alone is in the way of me being able to get over there in the foreseeable future. But concerning feedback, I actually do have something. It’s more a question, though. I want to ask it in the public space that is TH-cam, because it might be relevant to other folks and has been widely under-addressed. I am trying to collect information on going through physical transition while having a rheumatic or auto-immune-disease, like non-radiographic ankylosing spondylitis controlled by Tnf-blockers in my case. Can you point us to any resources concerning the effects of HRT (T, DHT-blockers) and gender affirming surgeries on (n-r)AS? For example, can T, hysterectomies or invasive surgical procedures cause rheumatic flares, accelerate the progression of AS, worsen osteoporosis or inflammation? So far I could only find one specific paper on this. I would like to know more about the latest state of research and maybe names of practitioners and researchers who specialize in this area. Your help would be greatly appreciated!

    WOW!!! im FTNB and ive never heard my wants for top surgery acknowledged! for years, ive been searching for a way to remove most of my breast tissue as i dont want mammary glands left behind, but i do want some fullness left as my dysphoria comes from the organ, not the fat, and as a plus sized NB person, imo it would look a little weird with an entirely flat chest and no fullness whatsoever.
    i hope more surgeons begin to take on these options and i hope someone near me does too, as i live in the eastern US.

    Thank you so much for this presentation. I still don't know if I want top surgery, since the most important thing to me is nipple sensitivity (I don't want to risk that at all, and I'm okay living life as a boy with boobs instead), but it's so nice to see all the options that are out there. What they actually look like, and how to think about them realistically. Even if I can't/won't have surgery for now, I want to learn as much as I can, since it makes me feel better and more in control of my life. This presentation has been by far the most comprehensive, inclusive, and helpful collection of information I've seen - and the first resource that doesn't assume every person seeking top surgery is a binary trans man who wants a completely flat/male chest at any cost.

    I've commented on your videos before about wanting to get top surgery from you and I've come back again, about a year later. I am so close, I just did a session with a therapist to get my top surgery letter. soon I'll be able to set up a consult and get insurance stuff over. You actively go out of your way to support and include non-binary people, I am very touched. As a non-binary person myself I am determined to get top surgery from a person and a place that is so accepting of me and of course I am pleased with your post op results. I will come back one day when my dreams are achieved.

    As a nonbinary person living in the bay area who is planning on getting top surgery, it's really reassuring to see a surgeon who is very informed about and open to nonbinary gender identities and expressions. I didn't know I had so many options to customize my chest! This video was really informational and helped me a lot in figuring out what I do and don't want. Hopefully this means I'll be able to clearly articulate my needs when I eventually have the opportunity to get it done!

    this is a wonderful presentation. i am a binary trans guy, but heavily considering going with no nipples if i get double incision just because i like the aesthetic and i would rather have no nipples at all than have nipples with limited sensation. i absolutely love how accepting you are of how other people wish to appear, and how much you care about non-binary people and their needs. that is a surprisingly difficult thing to find.

      basically my thoughts right now are- if i can find a surgeon i really love who does buttonhole, i will do that. but if i need to do double incision i will opt out of nipple grafts entirely and have a bare chest :))

    Thank you very much in particular for the information at about 25:20 discussing the angle and overhang issue. This is something that is hard to identify as an aspect of dysphoria but explains very well my own experience. I see lots of focus on size but not so much on these details, movement/weight of breasts and actual embodied experience vs. just appearance.

      Yes! I loathe the embodied experience of overhang, so much that it makes sleeping hard! The experience of having breasts is the thing that I hate really, so finding results that resonate and address that has been really hard but this video gives me hope.

    there is absolutely nothing I want more in this world than a phalloplasty without a vaginectomy. When I asked my doctor and he said "There's almost a 100% chance of urinary complications, with or without urethral lengthening. Maybe in 10 to 15 years they'll figure it out." fml

    God I hope surgeons around the world watch this. The button hole with the curvy scars I'm in love with, and the fish mouth seems like a good option too tbh?? I with I could get rid of overhang, and make my breasts into very gentle ovals, without changing much else - but when I say get rid of overhang, I mean I'm a very long GG so what I want basically would mean taking skin off of my ribs? Wish I could just do it myself with kitchen scissors lol.

    I'm so happy to see someone talk about the options I might want to take as a non binary individual for when I transition in the future. I've had questions answered that I didn't even think about, or didn't know I had, I'm happy to say that I now have a better understanding of the different types of surgery and what happens during these surgeries because not knowing the pros and cons of these surgeries was a big concern of mine. I always see a lot more about ftm and mtf people than I see about non-binary people, and although I'm very happy to be aware of these options they have and I wouldn't even know about these options if I hadn't learned about the options binary trans folks have I'm glad to see something for non-binary people as well. It does make sense that there are less resources since we are a small community but I am glad that you're one of the resources I've found since it answers a lot of questions for me.

      I just found out about fishmouth and im wondering when someone was gonna tell me about it lmaoIt clicked instantly thats what i want. I was just gonna settle with like inverted-T but it didnt click like this.

    I'm a nb afab, and I know I present feminine, but I've always felt masculine at my core. I've always lamented not being able to grow a beard, and I hate my reproductive system, yet I'm not sure between top surgery and breast reduction. thank you for making me feel valid in my identity 💕

      I just finished the video, the picture on the right side (starting from 10:18, and to the end of that picture group) is exactly how I'd like my body to look, whereas my body looks more like the left side, unfortunately. I'm also unfortunately in Scotland and broke 😅🙈
      edit: I do bind, because my dysphoria is too bad otherwise, but I admit I do tend to bind 'wrong'. I'll adjust everything outwards, and under my armpits to get the flattest look, and I do acknowledge the damage I'm doing but I just hate the too big bump otherwise. I'm size 32F and UGH. 😅

    After roughly 1 year on T my female fat deposits totally changed and my body looks male. I thought I was going to have to have body contouring but I definitely don’t need it at all. I became very thin and muscular. I had really hypertrophic scarring and required steroid injections on my scarring so I tattooed over the scar. I’m super happy with my chest surgery results. Everyone thinks I look great.

    I'd be curious to see if you've had any patients who wanted fishmouth-style incisions but no nipples. Do you still have issues with extra tissue left behind? Would you still call it a fishmouth, or would it be a modified double incision? I understand that's probably not a common request, as keeping nipple sensation (and, consequently, the nipples) is half the point of the different procedure, but I'm sure you've had at least one.

      Hi there, these are all great questions. Individuals can choose to have no-nipples for any incision type and doing so is a completely valid choice. If an individual wanted to have a fishmouth procedure with no nipples this would need to be discussed between the patient ant their surgeon, but would not be considered a modified double incision.     I'm in germany and I'm pretty sure I want double incision with no nipple grafts and straight scars that are slightly diagonal. The problem is that surgeons here tend to be a bit... old-fashioned. I've heard of many people who have asked their surgeons to get no nipple grafts and they got told that that was not an option since it didn't look like a traditional male chest. I also have to pretend I'm a binary trans guy or else I won't get anything covered by insurance (and also I've heard that enbyphobia is also widespread among doctors who are treating trans people like for example surgeons). Many use the excuse of wanting to get them tattooed instead of grafted for easier healing but that wouldn't make sense for me since I'm most likely elegible for peri or maybe even keyhole. So I either have to come up with a really good story to explain why I as a totally binary trans man who wants totally binary looking results would want to get double incision and no nipple grafts, or I need to find a surgeon that is cool about those things.
    If y'all know any surgeons in germany that are ok with doing double incision without nipple grafts then I would be very grateful if you told me. Also if you have any negative experiences about that it would also be helpful so that I know which surgeons to avoid.

    Excellent talk, both from the perspective of someone who's going into a research field that involves a fair amount of (animal) surgery and of someone who's in consultations for top. (Hate being east coast...) I consider myself a binary man but it REALLY bugs me that there's so little transparency about what anatomical attributes (direction of nerve growth, blood supply, skin tension, issues with scar pursing in peri, enough opening for tissue removal, etc) limit how top surgery can defacto be performed. I dislike how DI outcomes look (too flat, don't make sense with my body type or male family members) enough that I'd rather have the same amount of scarring using an 'oddball' method like fishmouth, but the option may just not be available to me.

    The surgeon who will be doing my tip surgery at Stanford said that he has never had body contouring approved by insurance. Have you had the same experience with your patients? If so, why is it so hard for them to grasp that we dont want huge hips?

      It's difficult to give a simple answer for this. Unfortunately, an increasing number of insurance companies have been dropping coverage for body contouring, but doing so in a rather diabolical way. They approve liposuction but then will only pay practices a very small amount which is not enough to cover basic costs. We are working to engage with insurance companies to help them understand that some (though not all) liposuction is essential for the gender journey and insurance companies need to support all relevant parts of the gender journey. Right now, insurance coverage for body contouring depends on an individual and their exact variation of an insurance plan at a specific given time. It also depends on how particular an insurance company is. We have been able to cut through red tape with some companies when a patient has a strong HR department in their company who will advocate for them.

    Hello! Around the 40 minute mark you mention that you don't really do the T-anchor incision style anymore, but I wasn't clear... do you still use this incision type for breast reductions and radical reductions?

      Dr. Mosser & The Gender Confirmation Center provide gender affirming top-surgery procedures for the transgender, non-binary & gender expansive community members and those that identify with this experience. Gender identity & sexual orientation are very different from one another. For example, an individual can have a non-binary gender identity and a sexual orientation identity as a lesbian and present both of these identities as valid at the same time. If a non-binary chest appearance in any variation would achieve a sense of euphoria for you is a question that you would need to discuss with a primary medical provider & your chosen top surgeon.       That’s perfectly fine! Many people only use nonbinary and don’t use any more specific labels. At the end of the day, it’s your identity and you get to define it how you want.

      There are some added implications for folks who have had a previous chest reduction; however, top surgery is still a possibility! Generally, the procedure types are more limited for these folks. I am currently going through the transgender process and it feels like they are almost pushing me into a more male category. they have their ways on FTM, but nothing for non binary people.     I can't lie... this felt incredibly focused on AFAB nonbinary people. That disappoints me. It kinda others me out even more as an AMAB nonbinary person. This video covers a lot of masculinization of feminine bodies, to a neutral point, rather than the feminization of masculine bodies to that point.

From the title and such, we completely understand how that would not be clear, and we appreciate your bringing this to our attention. We are currently working on updating the language we use in regards to procedures as well as educational content, to foster more inclusivity beyond the binary, and we do offer MTN top surgery and body contouring.

      @@genderconfirmationcenter Thanks for the reply. I think it just seems to be apart of a wider discussion and examination that may need to be had about why most nonbinary people are AFAB. This is true in media as well, where most nonbinary representation comes from the look of "AFAB+androgynous+wearing mens business casual".
      Generally, I just wish there was better representation everywhere for MTN people. There's lots of surgical options as an MTN I'm considering (BBL, body contouring stuff) to achieve that same "slender but androgynous form". I wish I knew what resources to look for in that regard. I don't even see a lot of results for mtf body contouring, surprisingly.

      Thanks for your question! Any chest is non-binary if someone identifies as such (with or without top surgery). What’s discussed in this hour presentation is the aspects of a pre pubescent chest prior to a pubertal period, breast removal, reduction and also breast augmentation. The way male physique is discussed is used to help frame the conversation but is not the center of the presentation.       @@xbduf4gb one must be recommended via gender therapist in order to get surgery
      A therapist will not reccomend you for surgery if your chest doesnt have some sort of negative impact on your life specifically "gender dysphoryia".
      Therefore if someones who's confused comes in, it's very easy to tell the difference between them and someone actually struggling with the way they are perceived.
      Simple science.

Our goal with this presentation is not to state what Nonbinary is, but rather demonstrate how gender affirming top-surgery procedures are much more dynamic, intimate and expansive than most folks realize and is not a procedure that is limited to affirming binary gender identities.       Dr. Scott Mosser, Transgender Top Surgeon If biological females have breasts removed they will have a post mastectomy chest with contouring. They will still look female to the casual observer unless they are on testosterone. I think it is important to encourage your patients to stay in touch with reality and minimise unrealistic expectations that they are getting a “non-binary” chest.

      @Alx I had this surgery and it was exactly what I was looking for. I don’t look male or female, hence NonBinary. I’m not interested in looking male, so I don’t take testosterone. In clothes I look like I did before puberty. I just look like me with no unwanted sexual features. That’s different from adult females with small chests like ballerinas, gymnasts, yoga instructors, etc. They still have visible A-cups.

      It is exceptionally rare for patients to regret transitioning genders. Although rare, regret after surgery is often due to the patient’s unrealistic expectations, or due to the very rare complications from surgery, or (unfortunately) due to a poor result executed by an inexperienced surgeon. This is why Dr. Mosser specializes in transgender surgery, and emphasizes coordination with the patient’s support team, including physicians, family members, partners, or close friends assisting in the transition.
