The lyrics are based on the 1939 novel Johnny Got His Gun by Dalton Trumbo, which is about World War I. A specific passage that inspired the song is: "How could a man lose as much of himself as I have and still live? They wrote some original music using amplified distorted cellos the best album to hear the progression is their compilation album "Amplified: a decade of reinventing the cello" from 2006... you can hear the cello slowly go from symphonic to metal as the album proceeds
