His Medallion is wrong even for the forgotten Wolven armor set that you get in and update patch side quest from 2020. And I don't remember this quest either even with both expansion packs blood and wine and heats of stone
Are you running an old version of the game? That cloak mod doesn't work on the current build, unless your the author an are playing with mods youv made yourself
It's in the Bloody and Wine DLC, if I remember correctly Orianna gives us this quest in the final mission sequences, however, you have to choose the right options.

Modding the game to look exponentially worse and play comically faster is a choice for sure.
I mean the faster combat is cool because that's how monsters and witchers would normally fight
