    I just stumbled on your site happenstance as you so eloquently described it.
    I had a crippling bout of sciatica back in October that prostrated me for five weeks. MUCH worse than childbirth, that’s for sure. I presented at the E.D. In the nearby town, and after leaving me for several hours I finally saw a doctor who had no sympathy or advice and sent me home in the care of my 88 year old husband. Three of my toes were numb. I woke one morning to zero pain. Oh the relief, and better still, it never returned. I have since seen an orthopaedic specialist who has written a referral to E.D. With instructions for treatment if the pain comes back. I wish I had stumbled on your site when I was in the greatest pain of my life.

    Not sure about these tests. I have severe lumbar stenosis, principally on the right hand side, and I passed all 3 of his tests easily!

    Hello Dr.G. I’m 65 male. Had laminectomy discectomy 1.7 years ago. Walked out if hospital, pain gone,was in heaven. Now sciatica is back. Started 2 months ago, primary ordered a new mri with contrast for Dr. ling in Spokane. Brilliant original surgeon. Well recommended. Best neuro in Spokane. Anyway.i have to sit in bed for 20 min, for pain to go away.before I can walk a bit. And leaning over feels better. Used to walk 2 miles a day, now no walking.using 2 canes if I have to walk far to lean over on.
    I was un aware that reverse back stretching was bad. I was doing a ton, everyday. Started in June.
    I think I hurt myself. Strength test on toe and heel and calf seem ok.did them before sitting, so terrible pain.
    I’ve learned so much. Makes me feel more in control knowing what’s going on.
    Thank you for your videos. I was a bit confused about the lecture to not wait past 4 months for successful surgery. And that study out 10 years. The surgery patients and non end up at the same place.
    The non surgery patient must suffer for 3 to 5 years. I believe that is unreasonable. I must be a real wimp because there is no way I could last 3 to 5 years with my sciatica. My wife would leave me, my life would be over. I believe what a patient wants to know is am I doing permanent damage by waiting.
    And this back shots at 1000 dollars apiece, insurance doesn’t pay. Did nothing for the pain.
    I had a nerve test before surgery for ling. The guy stuck me 30 times and said I can’t find any problem.
    Then he stuck me and said there it is, I didn’t know what he was talking about till I watched your video.

    Thanks Doc....it has helped me too understand my situation. It's a shame I am in the part of the world that people with little knowledge suffer big due luck of knowledge.     Interesting. I was having sciatica for years off and on, finally got really bad after a few minutes of bending over pulling weeds. I went to my primary who got an MRI ordered, from there a neurosurgeon. He did the first big toe test and immediately told me I needed surgery. I was scheduled for a few days later. I had been having trouble emptying my bladder (could still but it took several tries to fully feel empty) and was sliding my right foot along the floor rather than really picking it up. Having intermittent feelings of what I thought was neuropathy tingling bottom of feet but not a diabetic so that was odd. Now I know what levels were involved. I had surgery 9 days ago now and except for back soreness from the incision, feeling better. I need to see if I can start walking but I really want to go back to my swimming routine and that has to wait another 3 weeks. Thank you very much for the detailed explanation. These kind of videos are the kind of education everyone needs and no one gets. Shown more in school, more people would go into these medical fields.

      Not so fast… You can still have a disc herniation or disc tear which is "referring" pain down your lower extremity. And you can also have real nerve compression from disc herniation that is only affecting the sensory and pain nerves and not the motor nerves - so these tests would be normal.

    Are there exercises to help/fix these, I have a bit of all 3. Severe degenerative disc disease (MD xray diagnosed). Have been to PT several times & continue daily to do some. Be specific please.

    I wish I'd found your generous information before the VA botched my back surgery, leaving me what probably is permanent nerve damage. The quality of care for veterans is horrible and a national shame. Yet, because we don't pay cash for services (some us paid more dearly), we can't complain when we wake from back surgery with feet swollen, numb and dropped (both), symptoms that had nothing to do with the lower back sciatica that brought me to the "surgeon". I really wish I'd learned more and trusted less. I've recommended your channel to the Vietnam Veterans of America to send out in their newsletters.

      Just wonderful, I been tryin to find out about "which disc causes sciatica" for a while now, and I think this has helped. Ever heard of - Sonictoria Stupefying Smasher - (just google it )?
      It is a good one of a kind product for discovering how to stopping your sciatica minus the hard work. Ive heard some interesting things about it and my m8 got great results with it.

    I cant lay flat on my belly, its excruciating.. the muscle in my right glute has been locked up like the worst charley horse imaginable for over a week.. cant sleep, or stand up straight.. sharp stabbing burning pain that wont quit..and gets exponentially worse of a night... I would be beyond grateful for any advice.... thank you :'(

    This is very helpful, I am very glad I saw this video. Thanks for posting this. Now I know my doctor has to believe me, because I will show him the proof. I knew I was having nerve problems for a long time undiagnosed. And I definitely can say now that my L5 nerve is damaged. The first exercise check was key. The last exercise when standing up and flexing up with both feet, I can definitely tell my left leg is struggling, because it keeps shaking as I flex upward and downward and now I know where one of the problems is. I was standing up one day and I was shaking in the legs, and could not feel which or what was causing the shaking. I could not tell if my hips were the problem causing the shaking or the leg or legs were the problem. I was more like tremoring down somewhere at the time. My brain was confusing and I was puzzled. I’ve been trying to tell my doctor that I have been having nerve problems and he does not seem to believe me fully. I suspected MS, because of all the other extra MS symptoms I had together. So at least now I can address this problem with him, when I see him next when my spine MRI result comes in. Funny thing is, my reflex on my right arm and right knee don’t show reflex signs when hit in the right spot where the nerve is. I get no reflex or pain, I also feel less pain on right parts of body if I slap or pinch myself even, things like that. I have more sensitivity to hot and cold feeling and can’t always tell if I really am feeling things or am I just feeling the hot and cold from my hands and fingers. Yet I can move everything. My gait walk changes from time to time as well. And this is also why I need a referral to a neurologist.

      I have chronic pain down my left leg with a herniated nerve root against my spine, I have no “jelly” to cushion my discs it’s all dark. I started PT over 6wks ago.
      I take prednisone for inflammatory pain relief but, chronically. As when I go off prednisone the pain increases 10 fold, my toes are numb from pinky toe to toe next to my great toe, it feels like my muscles are atrophied. Despite PT
      I have been in PT for 7 weeks. But, pain continues and IS NOW an intense burning electrical pain that goes down the nerve around my foot then up the medial side of my leg to my labia. I had urinary incontinence with a severe episode of spasms that had me crying and sobbing uncontrollably. I usually just say ouch when they hit. I don’t usually cry and sob. My husband had to lift me into a sitting position as when I tried to sit up it caused the spasms to hit.
      I have to finish 12 PT visits before my insurance will pay for my injections. Dr feels 2 injections should help. 😢
      I’m on Tramadol 100mg TID, Gabapentin 200mg bid and then 300mg at HS, Diclofenac 75 bid, prednisone 10mg QD, baclofen 20 tid. I still have breakthrough pain waking me especially at HS. I sleep about 3 hours then spasms start and I am awake for the night until I become exhausted and then as I relax the spasms increase.
      It feels hopeless.
      Im so tired and just want to get better at this point my distal foot to under my arch and my toes are numb, my calf feels like it has a constant Charlie horse, my buttocks are sore, my coccyx hurts and sometimes burns.
      I started going to a pain clinic as my PCP does not give more than 3rx for pain.

    I had major back rebuild with screws plates disc fusions l4 l5 s1 in Feb this year. Fine until last week I was going on a hike I was on my way to the hike I was in the truck with my husband and we arrived at the park and I realized as I tried to get out of the truck and I could hardly move I did manage to get my legs out but my my spine felt like it was on fire and now my leg hurts just to move it in any direction I was watching your video about raising your foot up and down and so forth and all right foot will not raise up off the ground I have an appointment Monday to get a CAT scan and then I see the doctor Friday but this does not look good I mean on the way to the park I could feel something happening in my back it didn't feel right and when I got to the parking lot it felt like something fell apart doesn't make sense I just wanted to thank you for your video though it showed a couple of simple tests that anyone could do

    Thank you Doctor , your video helped me to understand my problems,although much too late after multiple FBS (Failed Back Surg. ) by few incompetent ortho- docs .
    When a person is in bad shape & suffering with intractable pain / her/ his family members MUST help by doing background work on specific doctors to assure that their surgery is done by competent physician.
    Sadly ,the Effect is for life

    GOSH! I've just had a good sob! I tore my achilles back in 2017 and well after healing I kept saying to an renowned ortho and various pt's that I can't get my foot off the ground and was poo poo'ed and yet here you have just explained my issue. I have been having progressive issues with IT Band and knee etc. Thank You this was a huge breakthrough.

    I have a central disc herniation on l4 l 5 ( 7mm ) with compression on the dural sac. The doctor said I have to go at the operation but I passed all your test.

      Note: this reply is for educational purposes only: I'm sorry to hear about your central disc herniation (which are more difficult to deal with than paracentral herniations). There are only three absolute indications for disc herniation surgery: #1) you have cauda equina syndrome and are having trouble controlling your bowel/bladder; #2) you have severe back and/or leg pain and are probably in and out of the emergency room; and #3) (And this one can be a little tricky) you have progressive motor deficit (muscle weakness). In almost any other case, surgery is optional. So it really all depends on how you're doing. If you answered yes to any of the three emergency surgical indications, then most likely you do need discectomy sooner rather than later. If not, you probably have time to let mother nature do her thing. Remember, a "successful" microdiscectomy is deemed when the patient scores and 18% and Oswestry Disability Index. Most people don't get 100% better after discectomy - on average, they end up about 18% disabled (Permanently) which is typically a lot better than they were before the surgery. I could go on and on - if you haven't checked out my website (ChiroGeek.com) there's a ton of information about the subject. Good luck!

      Your health is only as good as the medical care you receive. That's s what I've learned. The medical community where I live is of the lowest quality and unfortunately, I paid the ultimate price. Like you, the PCP wrote I constantly complained and she was right. Unfortunately, my complaining was ignored. By the time a new Dr. arrived and ordered an MIR my hip had completely degenerated. Not long after, without choice, I had a complete hip replacement surgery. Of course, I went to a medical facility seven hours away vowing never to seek health care in this hell hole. My suggestion for you is to request all of your medical records, I was shocked to see what was documented, such as I was a drug seeker! I think the MIR proved otherwise. Don't hesitate to seek medical care out of town if your medical community is subpar as is the case in this town. Good luck.

      I've been dealing with foot weakness in both of my feet for over 10 years now. I have lower back problems and doctors act like I'm making it up. They say it's not so bad. I have to use a cane to walk though and I have trouble walking more than a city block. I'm afraid to have any surgery on my back.

      @@pepela5739 well I was only 25 when I had surgery on my back I was in so much pain with it but when I had the surgery I ended up with a drop foot I had to use crutches for 16 weeks then walking stick for 9 weeks I still get lot of pain in my left leg not so much in my back but when I do it's really painful struggle to walk at times I think if doctors think your fracking it there are out of order I know what you are going through hope you get it sorted in the future

    Thanks doctor for showing how to test ourselves if there is a nerve damage. I'm 59 years old. Aprox 6 months ago I got a severe sciatic spasm, the doctor who I visited told me I had one hernia in L4 and another in L5. I could hardly walk.
    Since that time, I have been visiting a chiropractic and doing exercises.
    I did the tests you showed in the video and I'm pretty good but still my left calf and foot is a bit asleep.

      I'm glad you're getting treatment for the condition, but have you consulted an orthopedic spine surgeon? If severe nerve root compression is the problem, most surgeons like to remove that compression within four months all of its start. Chances of regaining motor power start to go down after that time point. Research demonstrates chances significantly go down after 12 months but in my practice I found that four month rule works pretty good. Good luck!

    I have undergone microdisctomy surgery year ago in L4 L5 for left foot drop due to disc slip...this helped me to recover from imbalance posture but I still feel numbness in lateral side of my lower leg upto toe and I am not able to lift up my toe

    I been feeling numbness on my little toe with weak ankle flexion, i got my mri from my lumbar spine and this was the result:
    IMPRESSION:
    1. Small posterior disc protrusion L5-S1 with mass effect upon the right greater than left S1 nerve root and right greater than left lateral recess narrowing and foraminal narrowing.
    2. Mild degenerative disc disease and diffuse facet joint degeneration is otherwise appreciated throughout the lumbar spine.

    Ty great video. I have seen 4 neurosurgeon so far. I have to bulging disc .l4-l5 ,l5-s1. Even though I have weakness and constant pain on my back , my right leg and calf. All doctors doing is giving anti inflammatory pain killer and telling me be careful when lifting heavy). Telling me no need for surgery(I don’t want it either) . But no solution either. I will see another specialist for emg. The problem is, I am paying doctors for appointments, test. Solving my problem (if it is not a surgery) won’t make them make more money. Price/effort ratio. So I can easily say they are not interested.

    After seeing numerous orthopedic surgeons, I finally got sent to a neurosurgeon who laid me down on the table and pulled on my big toe and I couldn’t resist at all. Told me I had a herniated disc, and immediately sent me for an mri to confirm it, mine was a workmen’s comp injury and was a nightmare getting them to pay for the surgery. I was amazed at his little test and wondered why it took so long to for it to be found. Workmans comp sucks.

      Keep repeating the tests, for it can take weeks or even months for weakness to manifest. And often times, sciatica only involves the sensory nerves and skips the motor nerves - just as painful.

    Thank you for sharing this kind of video. This is really helpful. After following all the test from this video, I have confirmed that my S1 problem is not fix, and I guess it's beyond repair, although the pain at the right side of my back disappeared through physical therapy. As time goes by I have learned to adjust and adapt on it.

      In Language Urdu we pay special respect to any one by saying " Mohtarram Douugles Sahab" i.e Very Respectfully & Dearest Douglas, I am a kidney doctor working last 30 years in Karachi, Pakistan & recently developed acute left Sciatica, on leaves struggling with oral pills & self physio via TH-cam, didn't done MRI as I was pretty sure this is L5/
      S1, & after seeing your video this conformed. Every field of medicine & surgery becoming much much refined in your well developed uncorrupted welfare countries but here all is opposite, the details of politics are un- endable.     Oh, and the nerves in your legs, muscles, toes, etc will light up sometimes and cause a lot of spasms. I can sit there and record them to show people how it is. On both calves, near each bone on each leg, if I am in elevated pain from sciatica, you can barely touch my calves near the bones... the pain is horrendous behind the calves and down along the back of my lower leg to my heels. I try to massage them but some days it is very painful to even try to do that. I usually use a roll on with lidocaine and coat those areas the best I can. I also do this with the toes on each foot, especially the tops of the toes as they will be very painful as well.

    Thanks for the video. I had an L4/5 fusion done 3 months ago as I had Spondylolsthesis of l4 over 5. I am doing PT 3 times a week and it is taking time to get better. I had hoped it would heal quicker but I am 65 and it just takes longer to heal when you are older..I had a triple laminectomy 14 years ago and have stenosis as well..I have been working on the foot drop issue and really have to focus on walking right. It bothers me that this is still an issue after surgery but I am told it could take up to a year to recover. So anything I can research and learn about the anatomy of the spine is worth it to me..Thank you

    Hey doc..I'm not one of your patients..but I did your test and it appears I have damaged my L5 nerve root..I can barely get my toes up ..and my calf starts with the pain....thanks for posting this..it gives me at least an idea of whats going on

      Remember, these tests are for muscle power and not pain. If pain prevents you from doing them, then the tests are not accurate. We are looking for muscle weakness during these tests and sometimes they just can't be done because it's too painful. Good luck!

    Douglas Gillard,BS,DC,Spine Researcher * This was a great video an I'm glad I stopped by.My son and I both have this problem and it's funny that not one Dr over the past 45 years have never done this test on me and not on my son.I find here in Nova Scotia Canada the Drs are only in it for the money and they give you less then 15 min s per visit and your only aloud one appointment a month.No wonderall the great Doctors have left and we are in a Doctor's shortage.
    Im going to try this on myself and my son ,Will the resolt be alot different even th9w I have nerve damage in both feet from being a diabetic?😇

    Since 18 I've had back issues due to Arthritis and sciatica. My L3, L4 & L5 get the radio frequency ablation / Deviation also hip injections. To keep me walking because 3 Fails on PT and 2 different Tends unit failed and also a chiropractor did more worse than good. My worst is my left but 6 nerves get burned on both sides due to they are both bad. I am now looking at a wheelchair for when I go paralyzed from the waste down and can't walk. I hope my doctor approves otherwise I'm being carried around.

    Dr.G, my pain started out two months ago as a seemingly routine disc hermiation flare up with pain, but this last one was by far the most excruciating flare up I've ever experienced. My back went out at a friend's and I was stuck there for 8 days, could barely move without extreme pain in my lower lumbar region, I could only think a disc ruptured or slipped far into the spinal column, or just degenerated. I also usually recover from these flare ups after about 3 weeks. This time I nearly reached full recovery after 6 weeks where I could finally walk with minimal pain lasting close to a week. Then I woke up with hip pain on the far left side and peroneal innervation. By the end of the day it was full blown pain frequently on and off in rhe sciatica, adductors, gluteus, peroneal, tibulis anterior. However, the fibularis longus and down to my foot was fairly constant and sometimes felt like it might explode out the side of my leg. I can lay on my back, stand and walk but I cannot sit. At the start of this my lumbar pain was passed already. This is something totally new for me , I have never felt this much pain before so constantly(radiculopathy). Also numbness on shin and top of the foot, foot mobility improves after some walking. I'm waiting for an urgent MRI, but I already suspect nerve root compression. What are some red flags that maybe I should watch out for in case I should get to a hospital?

      Watch out for signs of cauda equina syndrome (trouble starting or stopping urination, numbness/tingling/burning in the perineal region (crotch); severe muscle weakness (e.g. foot drop); or severe in-the-hospital level pain. All of those are considered medical emergencies, especially cauda equina syndrome, and you should go to the emergency room immediately if they occur for an emergency MRI.

      @@Jedi_Mindtricked Hey, sorry you're going through this. Sounds like you have it a lot worse than I do, It took me roughly 2 months to "fully" recover. I had my urgent CT scan and it showed all the vertebrae in my lumbar region are either calcifying(right to my spinal column) or are slightly degenerating, luckily none pressing deep into my inner spinal column, so I don't need surgery. Yet...
      I still have some innervation down my left leg to my top foot and along to the big toe, I have a sensation like theres still drop foot maybe weakness but there 's no actual drop foot left, there's sometimes just tingling, sometimes numbness. I haven't had any pain or flare ups since then though.. which is odd to me...
      Now, while I was bedridden, I did everything I could to either reduce inflammation- while improving blood flow to the region(tough combo to execute balanced simultaneously) using ice and heat therapy and the occasional Cyclobenziprine, I also worked on improving my range of motion, my flexibility and strength, I didn't have a whole lot else to do so I stuck to that pretty consistently and I believe it was likely the biggest contributing factor to my tension release in the area. So I would recommend doing anything you can to achieve the same result.

    I have recurrent sciatica pain . I first got it when I was 15 . I am 21 now . I still get it . For some months I am free from this pain completely then suddenly I get sciatica and this usually lasts for atleast 2 months . It's never on one leg . It keeps shifting sometimes on left leg sometimes on right . Somedays become so unbearable that I am afraid to sleep on bed . On somedays it is manageable. This time when I got it in my right leg I overused my left leg as a result got unbearable foot pain in left side . My pain was mainly on top of foot (L5) but is now on entire foot mainly on sides connected with L5 and S1. I can walk on my heels but I face huge difficulty on walking on my toes . I also can't do ups and down with my left foot when I put my right leg behind my calf .

      Although I don't know what your imaging looks like, I can say when radicular pain starts shifting from the right and then to the left and then back, that's a sign of neuropathic pain, which means your brain might not be correctly processing the pain. You might want to research words like central sensitization and mirror pain.

Apart from shifting Sciatica I have have pain at ankle , on top of foot and on both sides of foot (i.e from biog toe to ankle end and from pinky toe to ankle end ) . It is less from Day 1 but not completely cured . When I place too much pressure on it , Pain radiates on entire leg . What to do sir ? I also get pain when rotating my feets in anticlock or clockwise direction . I also have some points on feet where on pressing with finger I feel pain . Can you suggest some exercises . Pain is maximum when I get out from bed after sleep . How much time will it take to get cured ? I can do toe stretch without much pain but when I stretch my index toe I feel sharp pain on side of foot (where arch is located on side of big toe ) . I had pain at base of 2nd and 3rd fooot finger but it's gone . What to do now sir ? Currently I also have pain in anterior ankle joint .

    Left toe slightly gives. My problem is the big back of thigh muscle weakness that prevents me standing up without arm use if I fall over. I physically have done heavy farm work for most of my life. I passed all your tests with ease with only a slight give in left toe

    Thank You Dr 2 years I had my MRI for back pain & bad sciatic it. showed I have 2 other bulging disc up higher but my worst ones are …. a central herniation of L4-L5
    With moderate mass effect on the the ventral surface of thecal sac which abuts and mildly displaces the L5 nerve root as well as the S1 nerve root! My pain in August 2020 was back pain sciatica I couldn’t mop sweep vacuum sit very long, Burning in right buttock down my leg mostly to the knee. I did have times of relief until a bend once or twice! The Neurosurgeon said let the pain center doctor inject it with Steroids! I suffered tried not to let it heal! The pain the severe burning down the buttock my foot. Now I have been tripping & falling losing my balance can not stand 30 minutes & can not stand up straight. I can not pick up a gallon of milk & by the time I shower I have to sit stool to dry my hair! I am in terrible shape! I was scheduling for injection & saw one TH-cam Video today & yours awhile ago. MY BIG TOE CAN NOT resist me pushing down on it and I can not come up on the ball of foot. My thigh muscle had been having pain and it’s coming to my groin SUNDAY I was trying to dust a little furniture the balls of both my feet went numb ! I feel I walk to one side to! So I will call my PCP tomorrow see if we need another MRI as I try to get a NEW Appt. With new NEURO-Surgeon one that does the non invasive procedure to get me some help this is a living nightmare.       You need an MRI ASAP to see what's going on. If it's a disc herniation compressing a nerve and it's not getting better, then surgery sooner than later may be needed.

    I have sciatica and awaiting epidural injection, L5 nerve block, glad to say my big toe resistance is still strong, no sign of foot drop so hopefully I will respond to the injection well, next month, great video, so informative 👍

      Do you mind if I ask how the epidural went? I've backed out twice now, too fearful, mot of the procedure, but the one performing the procedure. You're only as healthy as the one providing your care.

      @@CynthiaPulver it went fine and I did feel some relief, but 3 weeks later I felt it herniate again when on a cycle, and the pain came back bad as ever, I was then placed on list for back surgery for discectomy, but the sciatica slowly disappeared again so ended up cancelling surgery. Surgery is only to address sciatica and not back pain, so I am left now with just slight back pain and just take small amount of daily painkiller

      @@shughy1 Thank you for taking the time to reply, however. I have a couple of additional questions. The most confusing aspect of my diagnosis of lumbar stenosis (and cervical stenosis) is how the pain can be intense for many weeks then subside without specific treatment, only to be followed by pain or symptoms in another area. My back pain, as you described yours, slowly decreased to manageable. Yet, my pinky toe developed two small bumps on the top (one at the front and the other at the end of the toe). They are very painful if touched. I had no issues of pain with my toes, although shocks are common in both feet.
      The podiatrist who looked at my toe said the bumps are bone spurs caused by arthritis. Seriously, I was confused because I had no arthritic symptoms in my toes before the appearance of these two bumps. The podiatrist requested I follow up in six months and, oh, get an x-ray before I leave. WTH?
      A week later, the big toe on the opposite foot became numb, and every time I get down on my knees to do something, I will feel an intense, prolonged shock in the toe. I have no idea what is causing these new issues. I am not even sure what type of doctor I should consult. Could the severe stenosis at C4-5 5-6 in my neck be a contributor?
      I would give anything for a competent doctor. I am through with these unlikely diagnoses from subpar doctors who graduated at the bottom of their class. These are the only type of professionals with a medical degree who would accept employment in an economically depressed rural community. They go unchallenged by the unmotivated and uneducated citizens. I may travel the seven hours to my hometown to receive a competent diagnosis and the quality treatment that everyone, by all accounts, deserves. I hope you continue to be relieved of the intense pain that plagues your daily existence. G-d's Speed.

    Hey there I have a question...
    My left ankle/lower leg has been tingly and numb for almost 5 years. I cannot spread my toes at all on that foot compared to right foot, I also have lower left back pain if I bend forward wrong that lasts a week and It hurts so bad I cant even change clothes etc..But I do not have recurring daily pain in back or leg, my lower left back just feels stiff/vulnerable if that makes sense.
    Is this sciatica? It's literally tingly 100% of the day Never goes away.
    Would I benefit from some sort of spinal decompression or am I at risk for further damaging whatever I have? Do I need a chiropractor? or nerve doctor? What is my first step as far as fixing it, an M.R.I? Thank you in advance

      Hello, you should see a spine care specialist (e.g., the physiatrist) and get a complete workup to see what's going on. I would guess its a disc herniation irritating one of the lower nerve roots; However, there are many other things that could be causing nerve root irritation: spine related, ankle related, or knee related. They will order an MRI and maybe an EMG/NCV to further figure this out. Good luck!

    Trying to recover from third flare of sciatica since early May 2021. This time it's not getting better, am lying on back and have burning in right buttock and anterior thigh, deep ache along shin bone and numbness at distal calf/anterior ankle where it bends upward. If I rub this deep ache, it hurts more and longer (unlike rubbing a sore muscle). I have absent knee reflexes bilaterally, not just on sciatica leg on right side, if that matters. Saw neurologist who didn't seem concerned. I asked him for x-rays or MRI, and he wouldn't order anything. History of congenital L5-S1 spondylolisthesis, grade 1. Tailbone removed after a difficult childbirth in 1974, with burning while sitting since that time. This burning has spread to include entire sitting area and perineal/vulvar area only when lying down. Ice packs between upper thighs helps a bit. Any thoughts, doctor? Thank you.

    When I did the S1 nerve test, on my bad foot, I had some difficulty raising up. I mean I could do it, but it was painful, and I knew if I continued to do it - even actually, if I just did it just once in full flex mode, I would have a very sore pad (that area that’s on the ground -whatever that area is called? actually I don’t know??), and so it would be throbbing all day because of that pressure I placed on it in this exercise. (Plus my knee fills up for some strange reason and I’m unable to bend it properly).
    So it was diagnosed as plantar fasciitis, but as far as I can tell it’s kind of like the opposite of the typical plantar fasciitis, because I have absolutely no pain in my heel whatsoever, it’s just that pad area that I have pain, all the time (for 5yrs). However, I was very relieved to discover that I don’t have nerve damage with your other two tests, as I had sciatica 10 - 15 years ago. I would gladly pay you for a consult. It would be nice to talk to someone who knows what they’re doing, but since I live in Australia, that’s not going to happen.

    I had MRI of my back, came back normal, no pinched nerve, bulging disk, arthritis.. nothing. But I have severe muscle atrophy in left calf and its the same symptom you present in this video. Unable to do calf raise or stand on toes on left foot. Could it be piriformis syndrome? I pray to have my mobility back… I have young kids and other than this I’m very healthy… I need help and all my doctors seems to be grasping at straws…

      Hello, without knowing your orthopedic and neurologic status, that's really hard (in fact impossible) to direct you with regard to treatment. If the lumbar MRI didn't show anything and you're having severe symptoms, you might consider getting a pelvic MRI or even better something called MR neurography. These things can show problems that cause radicular pain that a standard lumbar MRI cannot.

    I had an L-5/S1 laminectomy 23 years ago that manifested with sciatic pain in the right leg and I still to this day cannot raise up on the ball of my right foot. The good news is that I've had no sciatic pain in nearly 23 years now. I do have peripheral neuropathy with numbness in the toes of both feet, worse in the right foot and after sleeping lying down all night, occasional foot drop with the left foot, but no pain.

    Doctor can u recommend excercise for maintaining the nerves which control the feet.It’s most important at every age. The back lower limbs need to be strengthened . The test suggests the big toe and other part reflects the L1-L4.Can we be guided to do excercises for strengthening the back from mid age onwards . Thanks

    Both my toes are strong my problem is feet soal burning i if you advise me i am clean from diabetes. Your video advise was great and thank you....

    L5 nerve root injury...to right toe, weaker. dorsal area to the top, gives rise to pain, sharp needle like pains and muscle pains..L4, 5, S1 damage from heavy lifting as a Young Nurse.. 1977-82. Felt pinch in my lower back recently, then the pain occurred in my right foot. Cant weight bear at all 😵‍💫

      If you have sciatica and have failed any of these tests, you should immediately get an MRI of the lumbar spine to see what the problem is. You should also be evaluated by a specialist, like a physiatrist, who can more thoroughly check your neurological situation and then make a referral to an orthopedic spine surgeon, if the MRI shows nerve root compression which is not responding to conservative care.

    I definitely have foot drop and I have muscle atrophy. I live in Canada and it’s been a nightmare waiting to see a specialist about my back.

    Wowwwwwwwwwww you hit the nail on the head for me , I have s2 issues that started if with 2 numb small toes and now half of my foot is experiencing cronic sharp pains.

    My doctor suspects something is wrong with my L5-S1 and genetically that problem runs in the family, so it's okay to suspect that. However I also have hEDS and my big toe goes back really far without much discomfort or strain still, which surely would mean no L5 injury. Would this mean my L5 is not the problem and it's more likely the top of my pelvis?

    L5 seems weak. However, I do have either arthritis in my left big toe joint where it meets the foot or I have pain issues from bunion in that toe.
    So, that could be preventing the full Toe lift due to sharp pain in the joint.

    Greeting.Doc I'm glad to see dis video cos I hav a similar situation."I was gunshot on my right knee on the inner part n fracture on upper foot same leg.Doctor said I hv damage nerves.my knee is getting better so I hv a drop foot.i can pull up the back of my foot but im failing to do d the same on my toes.but my toes are moving right.n am still undergoing Physio.thank u

    Thank you for this. I am 26 and it started out as knee pain. And has increased to thigh weakness. I tested my big toe and it would not stay straight up.
