    Charlie helped me. I am pain free. I skipped the MRI and cancelled all my orthopedic and neurology appointments. I kept rescheduling, then finally just cancelled them all for good. If you've got pain that keeps coming back for months or years and you think it will never end, it will. Believe me. He is the real deal! Thank you Charlie.

      Thanks I'm currently undergoing diagnostic testing and imaging originally hurt back caused L-5 disk to budget. 23 years later and all kinds of pain and diminished capabilities suffered finally hope. ❤😅😂😂

    I've watched dozens of TH-cam videos trying to get to the bottom of my back and hip pain and this is the single most useful video for back pain sufferers, hands down. Insanely good!     Thanks for not isolating issues like everyone else because I am thinking intuitively that I have more than one of these simultaneously, which is why it’s so frustrating when docs continuously try to focus on one area of concern while diagnosing one condition at a time only.
    Like let’s forget how nuanced and interconnected our physiology really is...

    I find it crazy how little credit physical therapists get. I think they are the MOST receptive to pain complaints. I had an X-ray, mri, exams. And no diag other than having “you have sciatica” finally got orders for PT. Went to the physical therapist and he diagnosed me with a disc injury and started giving me exercises that gave me the first relief I had gotten in almost a month. Still not healed completely but coming from where I WAS, I owe him.. a lot. Worst pain I have ever been in in my life.

      @@annmarie9040 I am terrified of going for an MRI, because I am very clausterphobic and cannot do small enclose areas. I had one done once and passed out. I will do a CT scan over an MRI. I they had other ways to detect your pain and tissues instead of torturing you to go in that machine! Hope you are feeling better.

      I had an mri, I loved it, here in canada its like winning the lottery so no fear I'm even going to see an mri until my injury is years old. And mri should be a yearly exercise, as just the cancer angle alone can save so many lives.
      However, this guy IS a doctor, or I think he said that. Sometimes when I watch these I have a fear that its some guy looking at other youtube videos and jsut making up a name and calling themselves a doctor:) But yeah, so far doctors seem a waste of time. They don't even seem REMOTELY interested in patients usually. Mine is definitely a disc injury, and within a week of starting exercises I'm seeing huge improvements. Got a setback today after driving, I knew I was jinxing it. But I should write a journal to remind myself of the hellish pain I went through for a week not knowing what the hell was going on. Our healthcare industry was useless, I found the best help on youtube. Yeesh. I agree that pain is unbelievable. Painkillers wouldn't even TOUCH it. But exercise and strengthening have me on the way to recovery. But still wish I could get an MRi to at least see what things look like. Heck, many people get saved from cancer from the early detection in an mri.

    Great video. Given me an idea of what is going on. Most of my pain is actually my entire ankle however it started suddenly without an injury. It feels more like aches, no numbness or tingling. No pain with load but more pain with stretching ankle like a permanent sprain. Then after 3 weeks it included pain in my lower leg, shin, Achilles, calve, knee. Slight sciatica in hamstring and glute. Pain in groin and slight pain in outer hip. Working my way up I do have pain in L4 S1 so that’s my likely cause I’m guessing now. Chiro has adjusted it and I get some relief down stream for a couple of days but the pain never ever completely goes away. All started the day after a 8hr flight so I was leaning towards back issue. 2 months now, getting frustrated.

    I REALLY Appreciate the time and effort you have put into understanding the complexities with the human body.     i am a retired General Surgeon. After having treated a lot of patients with back ache, I got it myself. Pain in the right glutae with tingling down the leg similar to what happens when you hit the Ulnar nerve in the elbow. Stretched and stretched with no results. Then I saw your videos and started to reflect. It turned out that my right foot was always in the right outward direction. After correcting this and the pain disappeared. Thanks a lot for your help and very good analytic approach to this problem.

    What a fabulous video! I am a neuroscular therapist and have been practicing for 30 years and This video rocks! My pain has stumped not only myself but my colleagues.     This is the most informative and hopeful vid Iv ever seen. Iv had 4 surgeries last 4 years on hips and back with still no pinpointing my dominators of pain. I will be watching this multiple times and following through because I feel like it will lead to my success of finding issue/issues. I know my body more than surgeons as with experiences considering. I barely was touched with there hands to test anything accept for nerve test with needles. ( pain test ) and it’s like they didint believe or want to understand my cries within. Thank you dear beloved friend for your vid. It made my day by far 🙏🏼

    This video should be part of every doctors training or at least used as a refresher. I now have a better understanding of what is going on with my body and what my doctors have been doing in my exams.     I have severe backpain,left hip,left leg... L4L5 n S1 disc bulge n herniation...but doctors don't believe me that I have extreme pain saying your discs are not compressing your nerves....i have headache in left side... they say you have fibromyalgia n it's psychological....but I can't stand for half an hour...not walk for 15 minutes...if I do any activity my pain increases.. thank you for letting me know I have sciatica... thank you from India 🌹

      Find a bone setter in your region. (not chiropractor).
      They do things a bit different.

      Impressed with your guide. Still in the dark re my problem though. Physio is too. Right leg too painful to walk on after 25 yds. Pain starts in butt. Stop, touch toes, wait a few seconds - gone - for next 25 yds! Only pain source that stays is the excrutiating ball deep inside butt - tennis ball exercise 😱

      Ought to have confessed. I have one hip higher than the other. I reduced the pain by putting multiple insoles into the lower ( pain side) shoe!
      Not cured, but sciatic pain has disappeared!

      Chiropractor says SI inflammation, Ortho says mild hip joint arthritis Celebrex, gave steroid injection helped for about three days, then severe pain, PT tibial band piriformis primary care prednisone pack for inflammation. Ugh.

    This is very helpful, Dr. Johnson! I'm 73, and going through the chain (I know I was diagnosed with sciatica in 2015, had Physical Therapy). It seems to be recurring now. But your flowchart and its many steps helped me reorganize my own history as a female athletic wannabe with ups and downs along the way!
    At 4 AM I was hurting, sleeping only after sleep meditations. Getting across the room tough and painful. I'm not in pain now at 9:41AM. This really helped me focus! ❤However knowing from experience I have woken up to that and eventually gotten up and about. I have had a big change to training habits. Suspect and tests verify L4-L5 thinning visible and starting age 61, spots with osteoarthritis. So, I believe the original diagnosis! It is tempting to go "Oh that's me, that's me, etc. Thanks for advice on narrowing down which is it? Piriformis, sacroiliac pain, trochanteric sensitivity, sciatica? Since I know my disks and osteoarthritis are part of it, this helps me not jump to conclusions. "You shouldn't walk away from this saying 'I have this, this, and this!" Don't apologize for repeating yourself! It's on target! 🎯 Thanks! 👍

      This video is great isn't it? That's my issue too, left SI joint. I dread going to bed because I can't find any position comfortable and if you twist around it just makes the rest of your back out of alignment. I'd like a good night's sleep. Good luck.

      @@aewutube No. Gave up on PT. I recently tried assisted stretching. This seemed to really help the first 2 visits, and I really like it. I’m flexible, but they push you to your limit. Unfortunately, it has not solved my s.i. problems. Shiatsu massage feels great, also, but no cure. I’m pretty much resigned. I would like to try PRP injection, but so far it has not been offered to me. I believe the advanced stretching got blood flow into the area, hence the pain relief.

    This video I would recommend to anyone with back pain.. Im confident that I have a herniated disc or bulging disc, or it may be ruptured. I know it's a disc because I stopped on the video after reviewing all the pain symptoms I've had in the past week and two years agao when i thought it was just a pulled muscle that healed in 6 days with only 3 days of work missed... I looked up exercises for people who has herniated disc. When I preformed the arm hanging, leg and back dangling techniques I felt instant relief. Throughout today on my first day, Im feeling more relieved than I was when I was told to not lift anything over 10 lbs and take steroids and muscle relaxers. I have yet to see tomorrow's results without any medications 💊.. 😊 Thanks! For your video and word of advice Charlie! Ill continue to listen to my body and stear away from pain as I continue to heal. Ill follow up in a few days am confident that my healing process has already began..

    This was an excellent, clear way to help people guide themselves and their health care provider to determine what is causing their pain. I’m a failure analysis engineer and we use ishakawa diagrams to rule in and rule out causes for issues. I absolutely loved this. I recently started having specific left side lower back pain and now I can use this knowledge to better guide a diagnosis. Thank you!

    BRILLIANT, good sir, THANK YOU! Had something such as your video been available to us... Or better yet, had any of the dozens of various specialists I'd visited looked at MY story, I bet my overall quality of life would be drastically different.
    It's now my thought that what may have started as a herniated disk was overlooked due to my age (?) potentially. Over time, it seems as though i've taken a trip down the river, as you say. The herniated disc aggravated the bursitus in my hip, which, of course, swelled, creating an altered gate. Now, with the altered gate together with painful sitting. I now have 2 curves in my spine. Hips are uneven, as well as my shoulders, and both big toes now stay numb. To wake up on my back is basically paralyzing and does take help getting up. It's extremely painful for several hours after, extremely. I can't stand, sit, breathe, nothing really without again paralyzing pain. I mean stop dead in your tracks pain. Literally, scared to move. For me it does seem to hurt the most when taking pressure off of the joints, as opposed to putting pressure on or stretching them. It is just such a morning that I've stumbled across your video. OMG! Where was this 25 years ago!! Your chart/video should be additional MANDATORY curriculum in your field of study. HA! All must adequately show their ability to explain or present this it to their patients (lol my opinion) and should be hung by law in all orthopedic practices.
    There's also another study I have found on Vitamin B12 and Vitamin D2 deficiencies. For me, these 2 things have played a large role in my overall bone health as I am also deficient in both.
    Most of my symptoms started in my teens and is a large factor as to why it took until now, in my 40s to be diagnosed. Don't rank age as a top required symptom for anything. My initial complaint was what I described as bone stiffness in my middle teens. It felt as though someone were separating the muscle from the bone in my thighs. My hips I could only describe as someone having me by my ankles pulling my legs apart, popping the joimt with ice picks. I would wake up at times so stiff. I couldn't make a fist or hold anything in my hand for the first couple of hours of the day. Again, I was 13-14 and now in my 40s and still with minimal explanation as to why I've hurt my entire life.
    My apologies for the lengthy message! I've dealt with a lot of pain in my life because of this, and something such as your video could have prevented much of it many years ago. The prevented unnecessary sometimes painful regimens alone would have been invaluable for me!

    Dr. Charlie. This is a great video to help self diagnose or at least start to understand what the heck is going on in the back, hips, legs, and buttocks. I used it to help understand what could be wrong with my right side. I recently fell about a month ago. I missed the last stair going out of my house and put all the weight onto my right foot, turned the ankle inward and then fell. I had a mild sprain but after that I started to feel so much pain in my lower back, near my s1-s3 area. Thought I broke my back, and was very stiff for a week. That pain went away and then all of a sudden I started to feel excruciating pain down my right side into my calf. Pain was radiating so bad that I could not even put my socks on in the morning. I did some stretching, and I continue to due so, but the pain is still there. Agonizing just to get out of bed. So, here is where I am at now: I have Zero (0) pain in my back, none but my buttock is on fire. I did the leg raise test and nope, couldn't even straighten the leg. I did the Figure Four stretch for groin and that was fine as I don't have any pain there but I tested it anyway to see. Tests results were very symmetrical on both sides, maybe a bit loose on the painful side. So I can put socks on again but not how I used to. Upon reading some more, it looks as though it could be the Piriformis that is causing the problems. Thanks again for this very informative video.

    Thank you SO much! I do not currently have health insurance, so I’m unable to go to the doctor, which was the reason for my research. I am blown away at how detailed your diagram is, and how helpful it is. Based on all of the steps, I believe the cause of my buttock pain, radiating down the back of my thigh and sometimes into my foot, is truly caused by SI Dysfunction (upstream). My symptoms began about 9 months ago, when working as a delivery driver for an HVAC company. Eventually, while driving for even 20-30 minutes, my right leg began hurting so bad, to the point I would have to put my hand under my leg for any relief, and at times, I’d have to pull over and get out and walk around to get relief. When I press the SI on my right side, the pain is excruciating. The left side is painful as well, or at least very tender, so it may even be Bilateral SI. I plan to take some anti inflammatory medication to start, and go from there. I am very grateful for this video, and the fact that the certified information is free. Beyond grateful! Given a series of unfortunate events, I currently do not have WiFi, and the service out here in the country is rather weak and unreliable.       Could you describe your journey? Did you have a lateral shift of upper body vs lower body? What did MRI show? Did you do PT before surgery and was surgery the best thing for relief? Thanks I’m advance

      @@loungydoc No luckily i did not have lateral shift, though i have seen others who did and also benefitted from microdiscectomy. Yes to PT and every other conservative treatment prior to surgery that was available to me - i just couldn't live with the pain i was in, so am hugely sympathetic towards anyone suffering. There is a fantastic subreddit r/Sciatica if you haven't come across it already.

      @@loungydoc Yes, i am 10/11 months post-op now and have my life back. An occasional feeling of nerve pain on my affected side comes up every so often, but much much milder than it was pre-surgery.

      If you have both lower back pain and pain in the SI Joint region- then most likely the lower back (disc or other) as it is UPSTREAM or (in other words) the SI Joint is DOWNSTREAM. In the presence of a back problem the SI Joint is likely to be sensitive and hurt. In the presence of a back issue- the SI Joint tests/story will be FALSE positives. Make sense?
      SI joint will be one sided- no central back pain- in the location shown on the flowchart/in the video- right on the boney bump/dimple area.

      @@drcharliejohnson that’s what I have! Non central pain but seems to be more on the right side especially pointy dimple area. X ray showed hips are uneven and disc near L5 is smaller. So now dealing with SI, bursitis pain, and pinched nerve sensations

    this video saved my life. i am not suffering from back problems, but instead high hamstring tendinopathy. all my PT, doctors said it was herniated disc before MRI. I then googled exercises to for high hamstring tendinopathy and I felt instant relief, 1% pain left!

    Yes!! Just what I was looking for. I will rewatch and do all the tests. Not much luck at Kaiser or either chiropractor. They both did the hip distraction adjustment which I feel made things worse and gave me new sciatica pain. Dealing with pain on and off 2 years after groin pain from rebounder then was told I had a strained patella with some knee clicking. Def hard to put on socks! Magnesium, baking soda bath helped relieve some pain.

    Thank you so much for this free awesome information. My pain sent me to ER and the diagnosis was: Perfect health. Sure yeah, on morphine! I did the entire flow chart, the whole boring thing and all the stretches which most were easy and very painless. I came to the Femoroacetabular stretch and there was my problem. I did the stretches you suggested for this, and boom, pain gone. Yeah, I shared this with many. I would have never been diagnosed by a doctor for this, as they were thinking heart attack, gut issues, kidney failure, and all my organs, blood tests, urine test checked out to perfection. They sent me home with pain meds that nauseate me because they thought it might be, but not sure... muscular.

    Oh are my hero! This flow chart is the bomb! I can follow easily and determined that I must have pulled or injured my high hamstring from all over stretching! Thank you a million times. I still cant put pressure on the leg but at least I'm not freaking out thinking that I blew my disc out!

    Fabulous Charlie! Thank you so much for sharing this information for free. I, like a lot of people on here, have been chasing my tail around (literally!) trying to self-diagnose deep glute pain. I finally had a lower back x-ray taken last week (after my back started to make a sickening clunking sound), which clearly shows a severe L4/L5 stenosis. By the way, your map is spot on! I now know my problem is upstream and to keep concentrating there for a resolution to the pain.
    Unfortunately I've spent the past four months or more (and a stack of money) consulting a wide range of therapists none of whom identified the stenosis, but just drilled in to the deep glutes and piriformis to 'release them'. I now realise that any piriformis pain is either referred from upstream or my poor piriformis is becoming hypertonic due to having to support a dysfunctional spine and/or pelvis or both. I have work to do!
    PS Do you ever come to the UK?

    This was an excellent summary. I have had 2 hip replacements and now also have confirmed spinal stenosis with sciatica (symptoms, X-Ray, CT, MRI and Bone scans). All your clinical indicators are right on with my history. I am a mess & many of these things are truly inter-related. I love your upstream focus. The shopping cart analogy & my posture made me laugh - it was truly like looking in a mirror! I casually (or not so casually) lean on everything I can find for relief … even eating at my counter while leaning! Waiting on a Neurosurgery consult - but in Canada, it may be a long wait yet.

    I have been to the chiropractor, took a somatic movement course, and watched DOZENS of videos trying to figure out my butt and leg pain.
    FINALLY....this video helped to explain it! Now I know NOT to do piriformis stretches but to do contractions instead. I feel like I’m on the road to recovery. THANK YOU, THANK YOU, THANK YOU!!!❤️❤️❤️

    Absolutely stunning explanation. Thank you. I have Epidural lipomatosis due to fat in spinal canal. All other checks ok. MRI for pituitary etc, Never done steroids. Just body fat percentage and genetics. Will not operate to decompress as body fat will repopulate in spine. Just have to lose more weight to resolute. I used to do high intensity boxing etc and soccer. 2 years of pain, taking gabapentin, paracetamol and dihydrocodeine prn. Nightmare. More exercise, more pain so kind of slow process and dietary changes. Extremely frustrating. This explanation is immense in pointing me to the transient claudication/ positional stenosis with random hip pain on mobility. The stopping and crouching every 500m walking to reduce stenosis overwhelming pain is a nightmare but this video is sooooo good I don't feel so isolated anymore. I guess more pain relief before and after exercise gradually increasing exercise alongside dietary changes and extreme patience. Keep the faith people. I was a staff nurse on/off for years I I'm struggling with this condition. It impacts your stress/mental health (which I've studied also) and gabapentin can add to the messing of the mind. It's a marathon. No beer, reduce sugar, sweets, biscuits, reduce your previous high protein intake also. Crazily I never realised that protein meat intake gets stored as fat also. That completely blindsided me so reduce your meat intake if you have Epidural lipomatosis.
    Again, finally, THANK YOU SO MUCH FOR THIS GUIDE.❤❤❤

    Thank you for taking the time to put together such a comprehensive video. I had hearniiatd disc surgery earlier this year and am still suffering very stiff and at times sore lower back, though sciatica is thankfully pretty much gone- though the sciatica leg raise test tells me it is still there a little. This video and your others are helpful and explained clearly. Thaks again.

What took me to searching videos is yesterday during a very easy light run I have appeared to pull my hamstring, but read it could be related to sciatica nerve. It is sore today but more stiff than anything. I can walk okay but definately not run. Will continue to work on strengthening my piriformis as using the test for this both sides had similar flex. Thanks again

      Shoot. Sorry Troy didn’t see this. I do NOT recommend them as they usually just aggravate an already aggravated nerve! If you want to use it- I’d recommend doing it somewhere other than the immediate area of pain.

    This is an amazing video, thank you for taking the time to make it. I developed pain in my left side hip area after a long vigorous bike ride, and was afraid I’d injured a disk as I had zero restrictions prior. By my checkups with a chiro using many of these rests ruled out the back. Based on what I’m seeing here, I hurt my pirifomis muscle as I have front and rear hip pain on that side only, with some light numbness in the kneecap and some light tingling in the shin, but zero back pain. I seem fine during the day (aside from that numbness) but the pain does intensify at night while sleeping, waking me up.

    I'm so glad your video came into my feed today 😊 I've been having back and hip problems for 2 years now, saw the physiotherapist for 10 months but didn't help much, saw a chiro that seemed to make my pain worse. The only relief l got was from medication, tens machine, hot and cold packs, and massage but the pain is much worse now, pain in the butt, and hip, going down the leg, knee pain and sometimes weakness, tingling, and numbness in my legs. I saw a hip specialist and he said it seemed to be a nerve problem in the back, suggested an MRI but I'm still on the waiting list for that and now l can hardly walk. I'm only able to go to work because of the pain meds l have to take everyday but even that is not working so well anymore. I'm at my wits end. What else can l do? Any advice would be appreciated 🙏 Thank you so much Dr Charlie 🙏🙏🙏

      I don't know if this helps with pain. I see ppl addicted to opiates trying to kratom for to get off and manage withdraw. They are always talking about getting it at a smoke shop.

      Have you tried spinal traction? Specifically an inversion table?
      I have a herniated L4-L5 (MRI just confirmed) the thing is that my symptoms started as leg pain just above the knee with no associated back pain (though I do have a history of recurring back pain with year+ symptom free periods). For 6 months it was just leg pain after sitting for a few hours and the only reliable way to get relief was to take a long walk. That would reset me but I would get symptoms again after sitting. Every damn day.
      Doc started treating for piriformis with steroids and in about three days it went to crippling pain in my back. Another injection, then bed rest for a week and it went to excruciating pain in my shin. Finally got the MRI and heavy meds, it was looking bleak.
      What works for me now is inversion table (5min at 45deg), then straight into a lumbar support belt. I can stand or sit for hours, but I do inversion 3-5x per day. In bed putting pillows under my knees is key to sleeping though the night.
      I'm still on hydrocodone and gabapentin but I'm able to work and sleep now.
      Trying to get a surgical consult, and back to physical therapy. Hopefully on my way back towards normalcy.

    How should I go about with healing a herniated disk? I’ve been dealing with it for almost a year and I’ve seem to have hit a plateau. Great video by the way, definitely the best help I’ve had.

      Have you had x rays, mri's or any other tests? If you've been doign the strengthening exercises and are still having issues you should get a good look at it and see if surgical solutions like a microdiscectomy are necessary.

You are so right about wading thru various videos which although helpful, have still left me unsure about what exactly is causing the problem and what is it called! Based on my follow-along while watching your video, it seems likely I have piriformis, poss caused by herniated disc.
    However, I plan to watch it again, following your 'walk-through' step by step just to make sure. I really appreciate the work you put into this video. It is quite unique.

    I have been suffering for a few months now and this video is the only one I have found that explains it perfectly in layman’s terms 👏🏼 Thank You SO Much! I have an MRI scheduled for tomorrow morning as I have gone through physical therapy and it has only gotten more painful. I’m think it is a hip arthritis issues now, I am 54 year old female, highly active in gym 6-7 days a week for an hour minimum each visit. I hope the MRI will show signs of hip arthritis so as to better determine a treatment regimen. Again, thank you so much for that video 👍🏼

      How on earth can you work out for an hour 5-6 days a week with hip arthritis??
      You are actually working out too often also. Every other day is what is recomended because the muscles and such have to have a day of rest. Exercising tears the fibers and the in between rest days the body heals & grows stronger.
      The schedule you are on is what hardcore body builders do, not everyday people, and it 'catches up' with them eventually too.
      . . . No wonder you have arthritis symptoms!

      Yeah, absolutely. This funky symptom pattern can occur with a problem "up stream". You mentioned herniated disc...a disc and structures in the area can cause referred pain to all of these areas.

I developed what I think is sciatica in my left leg and it got so bad that I'd be in great pain during the night and especially when getting up. I've been doing stretches to undo the damage I did for weeks before: twist stretches hand to opposing shoulder to a count of 100 every day. I never figured it could be such a bad idea. Thanks for your offer. I am slowly getting back to driving without too much distracting pain. (Using a lumbar roll behind me now.)

    I have it all! RA at age five, scoliosis at age six. Turns out I was born with EDS with a score of 9/9. I am hyper mobile, especially on my left side, and tend to tear. I have developed dysautonomias such as POTS, and a ton of issues come along with that! It an absolute rabbit hole!! 🐇🕳️

    This video was very enlightening. I do have back pain, central but pain, side hip pain, knee and heel pain. More of all I am very wise! Almost 73. Left side is very loose compared to right. In the past I had an SI ablation and a shot of something in my hip. This video teaches me to look closer at treatment. Thank you.

    Hi Charlie!
    I'm about 25 mins in to this video and have just done the figure of 4 test. After this, I have a burning pain in the glute of the bad side.
    I've been diagnosed with hip impingement (2 years ago) but also have symptoms of an SI joint issue. Hence, I'm so confused. I also have a knee issue (other side) 10 years post surgery but arthritic.
    I get the pinching on the bad side in my groin/inner thigh, when I bend or lean. And sometimes, randomly, I cannot weight bear on this side. It suddenly seizes up and it's excruciating. I also cannot walk around the house bare footed, as this pulls on it. I always need shoes on so that I feel aligned, and not asthough I'm leaning slightly backwards. Does this sound like hip impingement to you?
    Any thoughts??? I'd be eternally, forever gratefully 🙏

    Dr. I am blown away on how well you delivered your video.
    I Did the figure 4 and my rt. Side was maybe 10% tighter than left side. Then I rotated my rt.leg outwards like you did and Ive been having piraformis pain for a few days now with my rt toe going numb, usually after sitting or lying down for a while and immediately my right big toe went numb. Any idea what that is?
    I’m 52.
    Here’s the kicker my SI joint was killing me about a week ago and my doctor pushed on it and it was a bit tender and he said I had SI joint dysfunction. I went to my physical therapist for two days and I believe that made my problem worse and not better because now it seems like the SI joint is fine but everything has shifted to the performance and the outside of my hip. I guess that would be where the end of the Pariformis ends or maybe the glute medius.

    No one has been able to tell me what the problem is, this video finally help me diagnose the hamstring tendonitis! I kept telling everyone my back didn't hurt and the only ROM limited was hamstring, it's painful right where it attaches but everyone kept telling me it was sciatic pain or that I should get imaging of my back done. I almost started to believe them and then this video popped into my feed 🙏

      @@drcharliejohnsonBased on my experience it's a muscle that most of the time doesn't need to be strengthened and if it becomes tight or out of balance it can compress nerves in the lumbar region.
      It can mimic other issues.
      I just don't see enough PT's talking about it....🤷

    Best explanation on the planet! I’ve spent two years trying to diagnose my hip issue and this did it! I’ve seen multiple doctors, specialists and have had injections, plus PT. I’m set for an MRI in a couple of weeks for my hips because the doctor wants it. I hope I can get a solution and no more injections. PT worked well, but I still cannot stand or walk for long periods of time. Thanks for your hard work and care! 👍🏼👍🏼

    Hi. I only have pain in my LEFT butt and hip. It hurts whether I sit, lie, stand, walk. Walking hurts the most. I don't seem to fit in Any of these categories. But, my right leg gets tingling and numbness.

      I have all your symptoms and was confused too! It was only after doing A LOT of hip and glute loosening exercises that my back started to 'clunk'. Not click, not grate, but CLUNK so bad that I got a x-ray which showed a chronic spinal stenosis at L4/L5. Shocking! It's so bad that the vertebrae have touched together on one side (hence the bone clunking) and the disc is bulging out the other side (hence the referred sciatic nerve pain). My point is that I felt NO pain in my lower back; I just have chronic piriformis syndrome type pain, and then an occasionalmonths

    This is very helpful to me. I have 10+ years ddd diagnosis and new bad hip pain. I've seen physical therapists, orthopedists, pain management, etc. without real answers. This flow chart is great. I am also going to use it to help diagnose my youngest grandson (age 11). He was born with severe bilateral clubfoot. He's had casting, and has had several surgeries and has issues with both feet, both legs his hips and his back. I'm going to use this flowchart to run down what problems he has to try and prevent long term skeletal issues from inappropriate treatment. Don't be surprised if I reach out to you when I get stuck.     AMAZING, THANK YOU!! I have had knee pain for 4 years and I was always suspecting it was coming form my lower back, turns out I might have a herniated disc according to my physiotherapist. This is so true! Be careful with misdiagnosis from doctors who dont pay much attention to the spine etc

    I downloaded the flowchart. It is 3:49 am/and I couldn't sleep, so here I am. I have been looking for answers for exactly 4 months today. The one part of your video I had not found anywhere else (based on your pain figures) was the mention of FAI. I couldn't find that you have addressed this specifically in any of your videos. Without getting too windy that is exactly where my problem manifests. It isn't constant. It comes on like a storm out of nowhere ravages my body for a period of time (sometimes short sometimes nearly the entire day). It leaves the same way it arrives wind/hail and water damage everywhere. And then I am 100% again. I can walk. I can do house work. Cook (if you can call what I do cooking) and generally enjoy life.
    I am 75. Seven years ago I decided to loose weight. And for the past 7 and a half years I have been riding a recumbent exercise bike. At first I could only go about do 15 minutes and about 6 miles when this happened I was riding 1.5 hours and 22 miles nearly every single day.
    My husband considered it an addiction and I have worn out 2 bikes and am on my 3rd (diamondback). The past 4 months I have been miserable. Frustrated doesn't even begin to describe how I feel - in addition to the excruciating pain that attacks at no particular time and because of no trigger that I can find.
    Giving up on my health is NOT an option. Still searching. Thank you Dr. Charlie.
