Plantarflexion Taping for Plantar Plate Tears

  • The digital plantarflexion taping technique for plantar plate tears and inflammation involves using cloth adhesive tape to restrict the dorsiflexion of the metatarsophalangeal joint of the affected digit to only 10-15 degrees of dorsiflexion during the propulsive phase of walking or running.
    Three 8-9" pieces of 1/2" cloth adhesive tape are used to prevent the proximal phalanx of the affected digit from dorsiflexing during propulsion which, in turn, prevents excessive tension force on the plantar plate to allow it to become less inflamed and, hopefully, heal over time while the patient continues their normal weightbearing activities.
    Digital plantarflexion taping, along with twice daily plantar icing therapy (i.e. two 20 minute icing sessions per day), custom foot orthoses to accommodate the affected metatarsophalangeal joint, wearing thicker soled cushioned shoes and avoiding barefoot activities greatly enhances the healing of plantar plate pathologies. Cortisone injections and even plantar plate repair surgery may be required if other conservative therapy options fail at rendering the patient asymptomatic.

    Omg this really helped!! I have this plus a terrible bunion going inward and my middle long toe is super crooked .. sorry I don’t know the technical names for all these stuff … and yes I always wore high heels through my 20’s and 30 now in my 40’s I am mad at myself … lol

    Thanks dr for detailed explanation. Taping of phalange 10 degrees dorsiflexion to surface well explained no other taping video explained it. I was doing it wrong by extending 30 degrees plantar flexion.

    Does this taping work on the third instead of 2nd toe? I ran my third toe phalange into a bed post.

    I had my (painful foot) taped like this by a physiotherapist this week, for suspected plantar plate tear. From watching your video it appears she did it correctly with approximate 10 degree angle. But within an hour I had to remove the tape, it was causing me to limp/and pain across the top of my foot ?

      @@denise8216 minimalist shoes. Even the cheap water shoes from Walmart works! Just make sure it's roomy.... And that you aren't wearing tight socks.

    Does this patient have morton's toe and does this problem cause the pain and other foot issues he has? If so, can this taping overcome the problems in a permanent way? Great video, thanks.

    Fabulous Kevin, but Johnson and Johnson have apparently now discontinued the tape. Zinc oxide sports tapes seem to be a reasonable alternative.

    Should one be using a post op shoe while doing this? It seems I have a plantar plate tear after breaking a toe. The bone is healed but I still feel pain. How long for this ti heal? I am taping just like this.

    A bit confused. In my case, it's the 3rd toe that "floats" and the 'V' is between 2nd & 3rd toe. So, do I tape the 3rd toe down? Thank you.

      The taping is generally put on after bathing/showering in the AM, then taken off before bedtime.

    Why around the second toe? Is it unusual to feel more pain in the big toe? I'm a dancer and was diagnosed with a spiral fracture of the first metatarsal. After 8 weeks in a boot, I returned to dancing and still had significant pain. An MRI showed a bone contusion, osteoarthritis, and a partial plantar plate tear. All my pain is in the ball of my foot under the big toe. Would this taping method be helpful?

      Have you tried metatarsal pads? Not the ones sold for dancers, those ones make it worse, but the felt or gel ones usually marketed for regular shoes? They go behind the metatarsals, in the area towards the middle of the foot. By elevating that, it takes pressure off the metatarsals, helping them recover faster.

    Anyone's toe red and irritated on the outside part of the toe from the taping? I've padded the toe with a cap but still extremely uncomfortable. I likely won't be able to tolerate taping 4-6 weeks😐

    What kind of tape it the best for this? I tried with regular sports tape and it keeps getting stuck and bending over on itself?

      Try Johnson and Johnson Zonas tape. It is what athletic trainers use on their professional athletes for strapping.

      Well, it ended up not being a good idea. I found out I had a fracture in the joint of the second toe. Supposedly it's healing, but the only thing that helps to keep the swelling down is buddy taping the second and third toes with coban wrap cut thin. Any other suggestions? I've seen a podiatrist bu he doesn't have much else to offer.

      @@serenity503 I have the same injury and apparently it is forever or you will need a surgery plantar plate injury doesn't get cured without surgery, taping is just to postpone surgery and help with the pain 😭😞

      @@corygirl Hi Corina. I am happy to say, my injury finally has healed but it took months! I ended up doing the best with buddy taping the 2 toes using the cuban tape that I cut really thin. This along with putting an insert in my shoes late gave me a little bit of a lift, but not too much in my arch. I thought it would never go away but it finally did. I had a stress or compression fracture in the joint and it just takes a long time to heal since it's hard to avoid bending the toes. This is why the buddy taping helped. I hope yours gets better soon!

    Help me understand something--What good does taping/immobilizing the proximal interphalengeal joint do when that joint is mobilized the moment he lifts his heal to run (thus flexing the MTP joint)?? This taping procedure seems to only immobilize passive motion and does nothing to inhibit actual biomechanical movement. To truly immobilize a plantar plate injury, it seems you'd have to immobilize the MTP joint...which would require an entirely different and far more elaborate taping procedure. Am I missing something?

      The taping as described limits metatarsophalangeal joint dorsiflexion thus limiting stretching of the injured plantar plate. The PIPJ is not immobilized by the taping.
