I’m still in pain four months later. I don’t think I’m moving forward to another surgery. Almost 60, recovery is super hard.

Finally, somebody who knows what he is talking about. My podiatrist said to exercise my flexors. Showed me the towel pull which I mistakenly thought was pulling my toes. I got my hammer toes from wearing orthotics for two decades…. Orthotics tip your foot forward, making your toes grip down abnormally strongly. I instantly felt relief and saw improvement when I ditched the orthotics and started bending my toes downward. At the same time I started wearing a huge spacer next to my big toe, which took care of the bunion, which was the reason I started wearing orthotics to begin with. Plus I am now wearing Xero shoes, which have a zero drop. All these things together are making me feel so much better!

Great information! Very helpful in understanding the real causes of hammertoes and how to treat them. Best explanation I've ever seen. It gets to the root of the problem. One question, Dr. Tom, are you familiar with Foot Chair Orthotics? They have an adjustable arch. I have been using them. I sometimes use Powerstep Pinnacle Maxx also. But for some reason, the Powerstep Pinnacle Maxx become very uncomfortable around the lateral part of my ankle, just under the ankle bone, after wearing them for a few hours. For treating hammertoe, which of these two orthotics would be best -- FootChair or Powerstep Pinnacle Maxx? Thanks very much!
You are very impressive. Very logical. Step by step for the whole process. You know what are talking. It is very hard for me to be impressed. But you are the winner. Thanks for your VDO.

I have recently been diagnosed with a hammer toe. It's not too bad yet. But I definitely want to avoid surgery. Your information makes total sense to me. It's not about the TOE as much as it is about the muscles on the bottom of the foot, the ankle, calf, knee and hip. It's why my knee on that leg hurts. (also, I live in flip flops...yikes!) I am 76 male with awful bunion on R/foot. My foot slipped down a rock ledge very hard and within a short time my second toe became clawed, but not underneath, with pain on the ball of my foot. Tried to get an appointment with my podiatrist, but had to wait 2.5 weeks. He took Xrays and put a bottom cast on my foot and gave me a shoe that was more like a straight plank. He said the claw toes do not go back to normal. From watching you, I thought they would.
I've had a normal arch since childhood but had claw toes since, before 10yo. I've got duck feet. Narrow heel and wide toe area. By the time length was good it would slip off my skinny heel. I also had big and hard to fit feet. So not knowing better I scrunched my toes in tight to make them fit the shape and size of shoes. My mom would get exasperated every time when nothing would fit. I learned to fake a shoe fit so I wouldn't go home with no shoes at all. I have been stretching and splinting my claw toes for about a year, off and on. I go barefoot as much as possible. Even wearing socks are a bit of a setback, coaxing my toes to curl up again. For a couple months anything touching the underside of my toes felt so unnatural and sensitive. Now I think they are 80% better. My right foot is a bit bigger and so those small toes have needed more work. I wish I had started this transformation sooner. I feel so much more grounded and....normal!
I beg to differ in regards to what you say that its only the smaller toes that are classified as Hammer Toes and that the big toe is a bunion toe! All of my toes are Hammer/Claw Toes and the biggest toe is the worst without bunion! Your video has just described my condition, i.e, I have bone fusion due to arthritis as my toes, especially the big right toe cannot be straightened at all. I was offered surgery by way of inserting a pin to straiten the toe but declined due to age! Great video, Thanks 👍👏
Thank you for watching. 100% correct, the great toe can hammer in more aggressive calf contracture as well. This is a little bit on the more advanced progression side, but is definitely possible. doc, my last toe joint is only flexinble when i put a lot of pressure both on the joint and on the tip of the toe simultaneously. otherwise even when im walking it stays at a 90° degree angle. does that indicate arthritis or is it the posture of my feet as shown in the beggining of the video that causes this flexion? should i see an orthopedic?
I feel like I’ve tried all these and I still have horrible pain in my feet, and it’s been all but impossible finding any shoes that feel comfortable again.
Excuse me doctor. I want to know the reference that you know. the knowledges are so valuable that I want to read original article. Is there a book explaining mechanism about foot?

Well.. I had surgery , left foot broken calcaneus open wound on the side where the tendons route 3 hammer toes ,pinky and big toe are ok, I can move like you explained, you think there’s hope? 😩 Ps. I appreciate all the details on your videos, nice work 👍🏼
I am 31 and have ankylosing spondolitis and a hammer toe turned into big toe joint issue too it's a mess it hurts really bad. Thanks for this video I'm dying here
I had two bunion surgeries each big toe side, 20 years apart. First surgery no cast just the big surgery shoe and it healed beautifully but painful because I didn’t rest as told. Second surgery different doctor. He broke it to reset it and the ugly scar on top of foot rather than on side hidden like my left foot. Got a cast made for foot and crutches 4 months. Pain was manageable but a shooting pain for a year and toe is slightly longer and foot always puffy. Scar still red after 3 years. I have trouble fitting shoes. The bunion was not worse than the first if anything it was smaller. Why he had to break the bone to reset a bunion? I don’t know… and he retired soon after.
My anxiety started from one rainy day it my projects got flooded wit worms we wercan feel Dem under my foot from dat point curded toes n pigeoned feet especially wen it feels like ppl are watching or talking bout me walk it gets worst

The silicon gel toe straightener worked for me. Just wear it on and off for months. The toenail should look normal color- never look like blood circulation has been cut off by too tight of a toe straightener.
We really appreciate that you came to us for help and we really wish we could help and see you in person. Unfortunately there are rules preventing an answer online, and this could be dangerous to your without having specific information. A problem of this nature really does require a proper history and physical. By getting non-specific advice online, it really can make your problem worse. For a problem this complex and with so many variables, you really would benefit most from seeing a podiatrist in person. Thank you so much for being a fan and we hope to make better and more educational videos in the future!
Not sure how i got it. It doesn't hurt, but im a dancer so my foot cannot point with hammer toe... I can point the foot only when seperating them with silicon separator.
How'd I get a mallet toe? Not sure. I was born with bad hips, so severely pigeon toed that my feet turned inside-backwards. Had shoes on braces that connected them to each other, and they would tighten them more each month (this only worn at night). Didn't walk until I was 3.5 yrs old. (I'm also on the spectrum - if you couldn't tell by now. We have a tendency to walk more on our tip toes.) Anyway, I had to wear my shoes on the opposite feet until 2nd grade - to try to straighten them - as they wanted to curve in. Worked 34 years on concrete. Now, all I wear are sketchers with memory foam. I do have a mallet toe I am trying to deal with. I came here because I am wondering what works best for those? (The dip joint bends down, and I end up walking on the very tippy tip of that second toe bone. Brutal.) Ok, so I'll stretch, calf raises and go back to practicing walking more.
I have mallet toe but the middle phalange is inverted, causing the toe to pad in the middle rather than the end. Never found a web search that has this in it.
easy solution to dress shoes is barefoot style dress shoes they will have toe boxes with more then enough room. but you won't get that pointy look everyone wants which is half the reason we have as many foot issues as we do
That sounds like a very difficult injury. Always check with a doctor in person if you have a complicated or painful injury. These are unfortunately meant to be general advice and cannot give specific advice for individual people.
@@DrTomBiernacki understood. My doc is old school and keeps suggesting surgery but that's not what I want. Having read if I'm flexible and not arthritic and (that i) may be able to correct without surgery is more ideal. I appreciate your quick response and well wishes!!!

I got a hammer toe at 17 It was kinda funny, actually. I came back from a trip, my left pinky toe was a hammer and both of my right shoes had a hole at the pinky
Thanks for the input Michal, in this case I would always start with a supportive insole like the ones found in the links, they should be relatively low cost. The insole would take pressure off of the ball of your foot and prevent overpronation. My right foot and toes have spasms and ovèr a period og time my doctor took any notice one of my toes has become impossible to straighten. My podiatrist thinks thè toe wiĺl die.
Thanks for the video. I would say static stretching muscles doesn't work too well. Shape changes tend to work better. Check out Zac Cupples videos for reasoning/references and exercises. I've had a hammer toe on my right foot since I was a kid. I also got shin splints and plantir facitisis. I think it was from poorly fitting shoes. Why the shoe industry doesn't make damn shoes that are foot shaped is beyond me. It's like a foot binding culture! Thankfully it doesn't cause me pain but recently I started taking up running and would always develop extensor tendonopathy on the top of my foot. As well as addressing my tight calves and center of gravtiy being forward by the exercises mentioned above tranitioning to toe socks and minimalist shoes.
Em what??? The shoe industry dosen't make shoes that are foot shaped? hint: its not 2004 anymore/no flame intended🤷 Altra, Xero, Vivobarefoot, Lems? there are tons of company's out there these days/hit up Amazon plenty of choices;and you won't even need to leave your home to get em. Sounds like your already familiar with minimalist shoes, Altra/and other brands you can get minimalist(natural) shape;wide toebox and high stack height (cushion) if you want/Even New balance a few models, Nike offer a model now. Best of luck, 🙏 -Cheers

I was born with hammertoes on both Feet and my toes hurt 😞 me I went to the. Foot doctor today and he said I need surgery 😪 on them and he wants me to get a xray on both of them I wear medical tape or moleskin and he recommended 🤔 me to get new 👟 new balances My cousin Gina is going to take me to get my x-rays on both Feet and when I go 🚶♀️ back to the podiatrist I have to take my x-rays with me so doctor wiener can look at them 🙃 and see 👀 what treatment I can have or surgery on both feet

doc, my last toe joint is only flexinble when i put a lot of pressure both on the joint and on the tip of the toe simultaneously. otherwise even when im walking it stays at a 90° degree angle. does that indicate arthritis or is it the posture of my feet as shown in the beggining of the video that causes this flexion? should i see an orthopedic?
I have hammertoe on both feet, but a bunion on the right foot as well. Will toe separators help at all? Also, I work 12 hours in steel toed boots!!
I feel like I’ve tried all these and I still have horrible pain in my feet, and it’s been all but impossible finding any shoes that feel comfortable again.
Excellent info, hilarious presentation, great help
Glad it was helpful!
Excuse me doctor. I want to know the reference that you know. the knowledges are so valuable that I want to read original article. Is there a book explaining mechanism about foot?
I wish you had a video of JUST walking us through the massages so I could make sure I was doing this right. ...
I have to use steel caps for work it's really hard to work when I get blisters on my feet from the shoes
You should be highly appreciated Docter! !**

What is the opposite of a hammer toe? Where the 2nd toe hyper-extends backwards? Do you have any information on that???

Well.. I had surgery , left foot broken calcaneus open wound on the side where the tendons route 3 hammer toes ,pinky and big toe are ok, I can move like you explained, you think there’s hope? 😩
Ps.
First surgery no
cast just the big surgery shoe and it healed beautifully but painful because I didn’t rest as told.
I lost my big toe, so now my second toe starting to hammer. It does not move if i try to wiggle it. It was fractured, now healed. What do I ask my Dr
Thank you very much.
My anxiety started from one rainy day it my projects got flooded wit worms we wercan feel Dem under my foot from dat point curded toes n pigeoned feet especially wen it feels like ppl are watching or talking bout me walk it gets worst
Thanks so much for the info

What about the pinky toe buñion bone hurts
The silicon gel toe straightener worked for me. Just wear it on and off for months. The toenail should look normal color- never look like blood circulation has been cut off by too tight of a toe straightener.
Excellent!
What kind of treatment do you recommend for mallet toes? What kind of splint?
We really appreciate that you came to us for help and we really wish we could help and see you in person. Unfortunately there are rules preventing an answer online, and this could be dangerous to your without having specific information.
A problem of this nature really does require a proper history and physical. By getting non-specific advice online, it really can make your problem worse.
For a problem this complex and with so many variables, you really would benefit most from seeing a podiatrist in person.
Thank you so much for being a fan and we hope to make better and more educational videos in the future!
I get hammertoes after my tibia and fibula surgery and its two years now how can i fix it? thank you
Not sure how i got it. It doesn't hurt, but im a dancer so my foot cannot point with hammer toe... I can point the foot only when seperating them with silicon separator.
What about if you have high arch feet and it causes hammer toes and bunions.
Never had any problems with my foot/ toes until I fell at work and broke my elbow. Now my toes curl under when I walk or excert myself.
I have mallet toe but the middle phalange is inverted, causing the toe to pad in the middle rather than the end. Never found a web search that has this in it.
Great!
My toe is just straight and hurt when i try to bend it.I am 13 year old. At that time I was playing soccer.
Please help me.
easy solution to dress shoes is barefoot style dress shoes they will have toe boxes with more then enough room. but you won't get that pointy look everyone wants which is half the reason we have as many foot issues as we do
Absolutely, I love wide toe boxes!
Can you show proof that your suggestions actually work?
I have flexible claw toes from an accident. A degloving from the heal to be exact, 7 mo. ago. What can I do for this?
That sounds like a very difficult injury. Always check with a doctor in person if you have a complicated or painful injury. These are unfortunately meant to be general advice and cannot give specific advice for individual people.
We wish you a great 2021 and hope you feel better soon!
@@DrTomBiernacki understood. My doc is old school and keeps suggesting surgery but that's not what I want. Having read if I'm flexible and not arthritic and (that i) may be able to correct without surgery is more ideal. I appreciate your quick response and well wishes!!!
Sometimes surgery may be needed, especially if there is extensive scar tissue. Thanks for commenting!

I am 58 years old my one day it just started hurting When I was working one my feet a lot for my job and I like to hike when I could. I told my wife about it and what I was going to do I was going to hit it with a rubber Mallet problem solved.
I got a hammer toe at 17
It was kinda funny, actually.
If possible, we would appreciate a quick review from you, and these make a HUGE difference for us!

My right foot and toes have spasms and ovèr a period og time my doctor took any notice one of my toes has become impossible to straighten. My podiatrist thinks thè toe wiĺl die.
I got mine from a stroke
I’m so so depressed. Idk how to pick an insert. I need help :(

This video might help!
@@DrTomBiernacki nope. I need to be dealt with directly. Too many options and I’ve bought a ton of inserts that didn’t work
Thanks for the video.
Has anyone said “just call a toe truck!”?
My toe feels like it was run over by a toe truck!
They don’t make small enough gel pads fit my size
Can I still fix This is I been like This since I was 8 years old I'm not 24
I got claw toes after I had a steel frame and pins on my leg and feet for 6+ months. Its still so uncomfortable even a year or two after my surgery.
I hate when younger people say “yea when you’re older who cares” like you no longer matter. 8:48
Well I would say that's taken out of context... Definitely not what was meant there 😊
The point is you make different health care decisions at different points in your life
For example as a 5 year old kid you would not do a reconstructive arthritis surgery, nor would you pursue a dangerous surgery at 100yrs old
I was born with hammertoes on both
Feet and my toes hurt 😞 me I went to the. Foot doctor today and he said I need surgery 😪 on them and he wants me to get a xray on both of them I wear medical tape or moleskin and he recommended 🤔 me to get new 👟 new balances
My cousin Gina is going to take me to get my x-rays on both Feet and when I go 🚶♀️ back to the podiatrist I have to take my x-rays with me so doctor wiener can look at them 🙃 and see 👀 what treatment I can have or surgery on both feet
Skateboarding gave me bunions and hammer toes.
Inserts literally do not fix the problem, the problem is misalignment and weakness.
The goal of orthotics is to help with misalignment, it's not perfect in all situation but if flexible it does help if it is long tendon substitution

No strengthening exercises?! You madman
My hammertoes came after a Achilles tendon reconstruction
Bunion caused hammer toe

Awesome!
I have hammer toe
What’s the worst to injury you have ever had?
Personally ripping knee ligaments and a hamstring and when I played college football, it was tough to run again for a few years!
As a podiatrist unfortunately we see a lot of gangrene and extensive amputations and injuries on a weekly basis, there are some bad injuries out there!
@@DrTomBiernacki oh that sucks You ever broken a bone?
Actually a big toe break haha, those take a while to get better!
