What to Do With High B12 Levels

  • In this article, we will look at what to do with high B-12 levels. For instance we will look at what approaches you can take to find out:
    How you can think about what's going on in your body to create high B-12 levels?
    Whether or not you actually have excess B-12? Should you be checking your white blood cells, kidneys, liver etc. What test should you to be doing?

    2. Cell membrane support: www.amazon.com...
    3. Antioxidant support: www.amazon.com...
    The first thing you want to look at when you have high B-12 levels is to find out why you have high B-12 to begin with. In previous 2 articles;What Does A High B-12 level Mean?
    Causes of High B-12 Without Supplementation
    we discussed this in some detail. Still it is essential to find this out before you go on and start planning what to do about it. One of the problems with finding out you have high B-12 is that the serum B-12 test gives limited information. It is not the best at showing your true functional B-12 status. In other words, sometimes your tissues and cells do not have enough B-12 and it does not match the results of the high serum test.
    This may seem strange that your tissues do not have enough B-12 but the B-12 blood test shows that you are high in B-12. However, you will get a get an appreciation for this as the article progresses. The main point for now is that the serum B-12 test is not the best way to understand your B-12 status. Neither are some of the functional tests like methylmalonic acid and homocysteine.
    If you do have adequate B-12 levels, how do you know that there are not other problems with your white blood cells, with your kidneys, liver, etc? How do you know this is not a signal that's something deeper is going on with your tissues?
    The first thing I recommend you do if you have a high B-12 level, is find out if your cells are getting adequate B-12 levels to begin with. To do this, you want to run two tests called methylmalonic acid and homocysteine. If these two test are normal (and the serum test is high) then you could likely assume that you do have adequate B-12 levels. By normal, for simplicity sake, we can use the lab reference range for normal. So if normal, then you do have adequate B-12 levels. Keep in mind that there can be problems with these tests too, but all in all, if normal there is adequate B-12.
    Another way to look at this is to distinguish if you do or do not have B-12 deficiency symptoms, regardless if you have adequate B-12 level per lab values. If you have symptoms of B-12 deficiency and your methylmalonic acid and homocysteine levels are high, then it's clear you want to treat for B-12 deficiency. However, you still should dig deeper, try to find out why the B-12 level high and make sure that there's nothing more going on. On the other hand, if your homocysteine and methylmalonic are low (or normal) but you still have B12 deficiency symptoms, you may still want to treat for B12 deficiency. There are various ways to go about that. However, in the case that your labs are showing normal homocysteine and methylmalonic acid, then use more caution using B-12 and focus more on finding the cause (see below). The main reason for this distinction is that the symptoms of b-12 deficiency can have many different causes.In the first case you have elevated B-12, high homocysteine, high methylmalonic acid with B-12 symptoms. Here you will be you're treating for B-12 deficiency. In addition you want to make sure your white blood cells, your liver, your kidneys are al normal. This could be done through some simple standard labs called a CBC and Chemistry panel. If it's way outside the range, then there may be a problem there and you need to look a little closer. If it all looks looks fine then probably there's nothing going on with these organs and tissues currently. Regardless if it looks fine now, you should still be following a doctor and still looking at these tests on a regular basis. While the levels could be normal now they may become abnormal later.
    If all tests are normal and you avoid extra B-12 supplements for 6 months, you could start supplementing with B-12. For both these scenarios, there are other test you can do to possibly uncover another layer of information. There is a test called holotranscobalamin.

    Hi! Thank you for caring enough about strangers, to post videos like this! Turns out I have elavated B12 (976) along with elavated S-CRP (11.7) and B-Leukocytes (9.0). I found out after going to the Dr, due to tingling and numbness in my legs. I feared it was MS, because my immune system is bonkers already (alopecia). Symptoms started in a right big toe and then went overnight into all right leg, then left - but lesser degree. Itching, tingling, numbness. I don't have the other typical MS symptoms. Now I'm hoping this is caused by something as silly as having overdosed on B vitamin (took supplements + was following the Keto diet with lots of beef and fish...!) I also have tendonitis in my hand, as of a few months... Trying to sort myself out and I'm in between UK and Swedish healthcare right now... Neurologists have 4-6 months queue to see them - private ones are not taking new patients. The GP said he couldn't help. I should go to the A&E or wait (6 months!) for the neurologist. I pasted the test results from the Dr I saw earlier if you have a sec (it's in Swedish but the words are very similar to English). I just hope my strange symptoms are something relatively harmless - not MS... I would be so grateful for some advice about what I should do with this!

      B12 does not cause these symptoms or cause elevated CRP. Whatever is causing the elevated crp (not easy to say what that is) is the likely cause of your symptoms.

      @@swintegrative Thank you! It's a bit better now, but I'll keep trying to figure out what it is, until I my appointment with a neurologist, in July (finally a private neurologist here in Sweden agreed to see me. Crazy shortage of appointments... I'll try all I can think of to lower the crp & inflammation - in terms of what I eat (anti-inflammatory diet) + I decided to take some diclofenac while I'm waiting.

      @@yourneighbourhoodclownwith8490 Hi! No it's still going on... I've seen a neurologist who didn't know.. and took some more blood tests. Nothing that would be causing this was present. The one blood value that is missing, is B6. It can give neurological symptoms both if it is too low, or too high. I'm getting that tested - I just manage to organise it. The neurologist has set up for me to do a test of the nerves using electrical shocks. I'm doing that on 1 Aug. I'm beginning to realise I'm on my own here! I've done lots of research. The neurologist simply said "these things happen sometimes with no clear reason". The other doctor got hung up on me losing weight and doing Keto. I've had some terrible problems with autoimmune diseases - I just want to be sure that this condition is not progressive, or autoimmune, such as Multiple sclerosis. It flares up sometimes, and sometimes subsides to just the underside of the feet. I'm hoping it's something silly like vitamin shortage/overdose. It's very distracting and is constantly on my mind. My CRP, which SW Int. Medicine commented on, has reduced, but the SR (other inflammatory value) remains at 20 - too high. I plan to take antibiotics and maybe some cortisone for a short period - just to rule that out too.... Bought it online since the Dr won't approve of taking it just as an experiment.. It's a mystery and it just came on overnight - for no reason I can understand. Other than this problem, I'm feeling better than ever! I'm clearly not sick. Let me know how it goes for you! Good luck and God bless!

    thanks for this video, you're about the only youtube dr covering it.
    i wonder if many of the people here were symptom free then had ibs/sibo that led them to high b12 and paradoxical deficiency symptoms.
    my story is 2018 food poisoning, 2019 sibo onset, candida, dybiosis, and one finding of giardia, and then an onset of crepitus, then knee pain, tmj, knee atrophy, tendonitis, ankle pain, then pain and atrophy spreading to all limbs and hands. have a great deal of muscle wasting in distals, and now pins and burning sensations are setting in, after all other testing the one remainder is neuro. i'm still dealing with ch4 sibo, lipopolysacharides likely leeching into the blood for a long time, and other barcterial gases i've read can be neurotoxic. i've had serum b12 creeping up to 1500pgml last count, and bilirubin has also climbed 5 points over ref. it's hard to tell if my issue is overproduction of b12 at the illeum or mitochondrial shut down from the endotoxic stressors, the carriers inbetween or a wild mixture. other bloodwork is normal, homocystein came back within range on standard lab tests, i await mma via organic acid tests. in the meantime i battle to reverse sibo, because it all started 2 years ago.

    Hey doctor ! I have a question for you and I hope you see this… I have been having neurological symptoms since 2018 with no explanation.. I’ve seen a neurologist, hematologist, rheumatologist, etc and all of my testing is normal EXCEPT my b-12 is very elevated… and my bilirubin is elevated as well.. not all the time but sometimes it is elevated along with the b-12. I’ve had liver scans, liver functioning panels and have been told it’s Gilbert’s syndrome and nothing is wrong. Great… the only issue is that my neurological symtoms which are pins and needles in my feet, burning feet and hand, numbness and tingling in the right side of my face, oral sores, etc has not gone away. I am negative for any autoimmune disease at this time.. and my b-12 levels are usually about 1200-1500… I have a mma test done and a Homocysteine test done which was normal, but my b-22 binding capacity test was very very low, so low that it was marked as unreadable… less than the reference range. What do I do??? My family doctor said it could be Transcobalin deficiency causing a functional b-12 deficiency but I don’t know. My hematologist is stumped and said to self inject B-12 and see if my symtoms subside. With mma and homocysteine being normal I am confused why my b-12 binding would be so low. Please comment back if you can. Thank you so much.

      Your symptoms are likely not from B12 deficiency with these results. There are other ting that can cause neuro sx. However , it would not hurt to try the B12. Also your might consider a urinary MMA as it is more accurate. You need MS/LC for MMA urine test.

      Hi Skylar, I know someone with suspected GS ( not tested yet) and his b12 is above range. He has been advised to take molybdenum, b2, thiamine and selenium with iodine ( test those levels before you do) to bring it down. I believe it’s based on the b12 protocol as cofactors are required for b12 assimilation.

    Hello, I am currently undergoing chemotherapy (breast cancer) and my blood values (leukos currently 2.6) are not that good. But Liver and kidney, vit d etc. values are normal. But I'm particularly worried about the vit b12 value. This is increasing more and more (>2000)-without supplements.The holo-tc test showed 26 - an absolute deficiency in the cells. The cause with cancer and chemo lit is also clear. However, I urgently need vitamin b12 for the next treatments (blood formation) and the chemo also affects my nerves. But how do I get vit b12 into the cells? What can I do to mitigate the impact? And do you know if it goes back to normal afterwards? none of the doctors noticed or knew about it. I'm really worried about the consequences. Thank you once again for your answer

    • @swintegrative
      @swintegrative  ปีที่แล้ว +1

      There are a lot of contextual things that I do not know about you. So we his is not advice to you specifically. However a low holo-tc would mean you are lacking or low on the transporter. This could happen for genetic reasons or simple low wbc’s. Either case not sure there is a known solution. A high dose of b12 may diffuse into the cell.

    hi! thank you for these videos. i’ve been watching them.. back in august they found that i had slightly low b12 (around 300) and slightly anemic. i never took any supplements for either issue. yesterday they did another blood test to see how everything was ( i did not eat or take anything 10+ hours before my draw) a doctor has not gotten back to me yet but my b12 has increased to 1,200. and also my lymphocytes have increased above “normal” ( i am not sick, and have no infections). when comparing my results from august to now i noticed that my RBC’s have all SLIGHTLY increased. WBC’s have all slightly decreased ( besides lymphocytes which are above average) and my platelets have increased a little. i haven’t had any symptoms that i’ve noticed besides been napping a little more often, some night sweats & the pst 2 weeks felt very full after a small amount of food.. but i figured this was nothing related. i have no pain or anything though. i’m only 21. so i feel like i’m fine. but my doctor has not gotten back to me and it’s causing me now to stress out.. what do you think about this?

      Hi Keana, there are a lot of variables so it is hard to say from just this information what is going on with you. The main thing is I would not be too concerned about these results but I would not ignore them either. As a start, you should re-check your b12 level and wbc's. WBC's can fluctuate a lot from one test to the next.

      Hi, It's hard to say for sure. You should re-check the levels and make sure you are not taking any extra B12 when you do it again. If still as high, consider seeing a blood specialist, hematologist. Some people are genetically more prone to having, high B12 levels but you should rule out any other serious issues.

      First I assume you don't take extra B12? If the answer is no then yes it is a good idea to do some tests. Some of the same reasons people have a diagnosis of M.E. and CFS can cause high TCN -1 and TCN-3 (B12 carrying proteins). In coordiation with your health care provider you should do a MMA and homocysteine to see if intracellular b12 if low (along with liver, kidney and white blood cell tests).

    Dear doctor,
    How is it possible that an oral multivitamin can affect the levels of b12 ?
    I understand that the injections may have an impact however I don’t understand how the oral way can affect it since the body should excrete any excess.

Is there anything I can do to make them listen or do you have any info I can send them? Thanks so much.

    • @swintegrative
      @swintegrative  3 ปีที่แล้ว

      Hi Laura O'Reilly, I recommend following up with the doctors and doing repeat testing of the B12 levels to show it is high multiple times. You can also ask for urinary methylmalonic acid test and homocysteine tests. The B12 being high does not cause symptoms and there is not definitive treatment for it. The reason B12 levels are high (whatever it is in your case), does cause symptoms. To find this reason, you have to be persistent and patient and find a doctor that is as well. In other words its a clue not a diagnosis.

    Hi There, thank you for this video, which I found very helpful. I am 55 year old male, living in London, UK. ı consistency have slightly elevated B12 (around 850s) and my GP both in UK and in Turkey (home country) are not much concerned about it. I am however a bit concerned and that is why I watched your video. My question is that I have also slightly high iron level around 250 and my white blood cells are also slightly high. Do I need to concern about these markers? What advice would you give me please?

      I wold be a bit concerned but context is king. Check out my videos on high iron. If WBC are consistently high (more than 2 tests and depending on how high) you might see hematologist. There are just too many scenarios to go into what to do. All these things may be related too which is why a hematologist may be helpful.

    Great information but how are you meant to test CNS levels of B12? I'm pretty sure testing the spinal fluid of MMA and Hcys is the only direction. Once the brain is depleted then it has been proven to be extremely difficult to resaturate the CNS. MMA can come back normal in blood as the CNS and body can have completely different levels

      Hi DanHambo, there is no perfect test for B12 deficiency. you may find this video helpful as it discusses neurological manifestations of B12 deficiency th-cam.com/video/hr2FnA_oHLU/w-d-xo.html You are correct there is no way to test for tissue specific B12 deficiency. However the initial blood and urine samples should show some indication of deficiency. It is up to the doctor to determine when the tissues have been saturated and stop the supplementation.

Also I have coldness, tingling, pain and numbing sensations is my feet. I have been tested for possible nerve damage and blood circulation issues, however those tests came back normal. After watching this video Im thinking I should be tested for methylmalonic acid and homocysteine, is that my next step ?

    • @swintegrative

      @@swintegrativeThank you, I will book an appointment and have the blood tests done. So just so I understand, elevated serum levels of vitamin B-12 can be accompanied by signs of deficiency as well. This can be caused by defects in tissue uptake and action of vitamin B-12 at the cellular level. My question then is, if my tests come back as abnormally high meaning I truly have a vitamin B12 deficiency then how can this issue be treated? If vitamin B12 is abundant in my bloodstream but not at my cellular level or the tissues, can supplementing with high doses of vitamin B12 still be beneficial?

    • @swintegrative
      @swintegrative  4 ปีที่แล้ว

      Hi Edward, The actual treatment for this issue is not the same for all. I do think adding more B12 can help. However you have to be under care of doctor to make sure no other serious issues are the cause of high B12. You can for instance have high homocysteine MMA and B12 and still have a liver issue or white blood cell issue. Once these have been ruled out, raising B12 may help drive more into the cells.

      Hi Sorsara78, This is a good question. Either being high suggests B12 deficiency. Methylmalonic acid is more specific to B12 though. Nothing is absolute but a high MMA is almost always B12 deficiency. High homocysteine can be other things. Normal or low MMA does not mean normal B12 but means there is not severe longstanding deficiency. Hope that helps.

You can ask to see a hematologist to rule out these other problems. If they can't find a problem this is good. You can also see a functional medicine doctor that has knowledge in this space.
