    Great presentation thank you, I am 48 experiencing terrible headaches, worse lying down, blurred vision, facial pain, diagnosis migraines, CT Scan say Pituitary Gland is generous. Does this mean large because I am experiencing many of these problems and currently off work for it

    My mother was diagnosed with pituitary microadenoma(prolactinoma) in 2011, she took her medication as prescribed (tab.cabergoline).In 2016, after a routine MRI check up, the neurosurgeon told us that the tumor is fully cured & advised us for regular check up of prolactin level under an endocrinologist. From then, my mother is under a regular prolactin level check up on every 3 months. Her prolactin level is normal but for past few days she is experiencing severe vertigo. Now, I am really worried Sir,is there any chance of recurrent tumor or malignancy??
    I will be really grateful with ur answer.TIA🙂

      Chance of recurrence is there but malignancy is very very very unlikely ...if there is doubt about recurrence better to get MRI brain and confirm

      Yes You need to be under regular follow up and need to undergo Follow up MRI brain to rule out any residual or recurrent tumor. You also need to undergo regular hormone analysis , at least thyroid so that medications are optimized. If there was pre existing visual problem, you better get complete vision check up too.

    I am trying to find information on long term post operative outcomes after the transphenoidal approach. I just underwent the surgery and have a duragen and duraseal combined closure, but there is very little information on possible complications or things to be aware of now that I am living with a skull base defect.     Sir my father recently lost vision from left eye, by MRI diagnosed Macroadenoma(~2.8AP,3.1 TR,4.8CC ) Doctor suggested surgery to remove it. Can it will be safe? And vision will be regained or not ? Macroadenoma compresses optic chiasma…… plz sir reply me

    One month back, I had my prolactin test check and my result was 40 in the reference of 5 to 20 and after one month, I have checked again then it was 256 with the with the same reference and then after one week, I again tested and result was 8 with the reference of 5 to 20 ,on my first test I had 1.5 hours empty stomach after waking up and the second test I had like 2.5 hour after waking up, but I took Pantoprazole before the test then third test was like four hours after waking up empty stomach. so is there any possibility that I might have the pituitary adenoma or last test was normal so I should not be getting any further test? please respond.

    If possible, I would kindly like to ask you for your opinion about this case...
    30 years old woman with microadenoma (3mm) and Rathke's cleft cysts (8mm) and prolacin 1500, was prescribed bromocriptine by her ginecologyst, which has a lot of side effects. What is your opinion about what the person should do? She is about to see endocrinologist soon.

      Any symptoms related to hyperprolactinemia ? If she has, then better to take cabergolin/bromocriptine

    I am a pituitary macroadenoma patient. I have no headache, no eyesight problems, no nausea but have enlargements of finger, toes, feet, limbs and facial parts, face has started darking. Halting speeches, snorting, salvia secretion in sleep. And also bleeding from teeth. Can not pass urine clearly, have sexual problem. And also pain in hand. Size of Macroadenoma is A 1.5*1.4*1.3 cm on 16.09.2023.
    HGH-63.6 ng/mL, IGF-1(Somatomedin)-497 ng/mL(19.10.2023) .
    Please Please sir suggest me.

    Hello Sir, I saw the whole video really informative. My prolactin is 148, I went for an MRI, the MRI says suspicious of microadenoma of size 4.3 mm, will this require surgery? Is it life threatening or can cause any vision issues. I’m not facing any issue other that galactorreha. My gynecologist has suggested to take medication for lowering the prolactin. Pls do let me know

      Thank you. It seems to be a very small adenoma. If all your hormones ( apart from prolactin) are within normal limits, you definitely do not require any surgery now. As rightly suggested by your gynaecologist you need to start taking medications to lower your prolactin levels so that galactorrhea can be stopped. However you need to undergo follow up MRI after a year to see for the growth in the size and take necessary action at that time

    Hello sir. I have a pituitary tumor prolactinoma, which is macroadenoma. I suffered from galactorhea and sever amenorrhea I started using cabergoiline for 3 months. First my levels were greater than 200. But after a month it came down to 127. Again. I used it for two months which I was taking twice a week. But its still the levels are 127 after using for 2months. Why do u think its not getting back to normal. Pls reply

    My father was diagnosed with pituitary adenoma. The Doctor said he needs to remove the tumor and they will put permanent shunt inside his head all thru his stomach coz they have also seen in the CT-scan that he also has a Hydrocephalus.
    But my father said his head is not even aching. He only feels weakness with his half body.
    We are really afraid thinking that the neuro is going to insert permanent shunt inside our father's body. Is it okay if we do not give our consent with the operation and is it okay if my father would only undergo for physical therapy and take oral medicine instead?

      Pls consult an endocrinologist. They will evaluate the cause of high plasma ACTH. Sometimes pituitary may not be the cause and it may just be an incidental finding. Let them establish the source of production of high ACTH. If it's proved to be pituitary , then surgical procedure can be considered ( but it will be tough to localize as the size is
      only 3mm )

    Thanks for such a wonderful and informative video. I am 59 years old and post menopause and I had my MRI done last week and a small nodular-like focus measuring about 3.5 x 3.5x 3.8mm seen at superior aspect of pituitary gland. All my tests fell within normal range but I was having headache, blur and double vision,pain at eyes nerve and numbness at head and hands, irregular speech and heart beats and Trigeminal neuralgia. My specialists only told me that I had cataract and migraine with aura and my prescription was Fludan capsule 5 mg every night..I would like to have your 2nd opinion on the benign. Thanks

      This small tumor is unlikely to cause any symptoms. It most likely is a incidentaloma. You just need regular MRI to make sure it's not growing and if there is significant growth you may require operation at a later date

    Hi there!
    If you have the time, i'd like you to look at my personal case. I have a microadenoma of 7.7 mm and the doctors think that it is not the reason for my hormonal imbalances. I have a chronic shortage of testosteron but bij lh, tsh and fsh levels are normal according to one test i've done 3 months before the adenoma was discovered. I'm a 24 year old male, my testikels decreased in size and i have some sort of heavy feeling in the same area. Not painful but just really uncomfortable. The levels of testosteron differs per test, but never within the normal levels.
    Do you think i need a second opinion? Personally i think this isn't a coincidence, do you think the adenoma is big enough to compress the area? Tests did not assume that the adenoma is functioning, as my prolactine and growth hormone levels aren't that high. It is top of the "normal" levels, but was not to high yet. Again this was the case 3 months before the adenoma was found.

      Problem in the feet is very very unlikely to be related to your pituitary surgery

    Well defined lession in left half of pitutar with fluid -fluid level suggestive of pitutar adenoma with bleed .
    Sella is midly enlarged aprox(8vertical×13AP×12TRANS)mm
    It is mri statement
    What doews it mean Sir ???

      It means there is a pituitary adenoma and it has bled (apoplexy). Pls consult a Neurosurgeon at your place. Hormone analysis and vision testing needs to be done and may require a surgery

      Yes low dose cabergolin can be started and value be repeated after 6 wks . It's usually well tolerated

    Sir at 23 September my prolactin level is 62.5 and also I have a 3.8*2.5mm microadenoma
    Thn I took cabergoline for 3 mnths
    And today when I tested my prolactin level shows 1.20 Which is too low
    How is it possible? I am suffering from severe headache problem

      Did you have any symptoms at a prolactin level of 62.5 ? It's not really too high anyway. You need to undergo follow up MRI after an year to look for any increase in the size of the adenoma . Headache may or may not be related to this

    Hello sir,..I have seen your video..really it is good and help me..I have suffered pituitary gland tumor past one year..I have headache and eye pain so I have checked with doctor and identified..I have tumor in pituitary gland..at the time my prolactin level was 230ml and tumor size was 2mm. Doctor advised to take cabgolin 0.5 tablet..I have taken it weekly once..now my prolactin level is 13ml and before six month it was 8ml. My doctor advice to take tablet regularly.. but I have feel headaches and have some eye pain in left on side. Is there any issue or is it common..

      You have written that the size is 2 mm in which case it is a microadenoma and apart from prolactin if all your other hormones are normal (ie if it's non hormone secreting) then you don't need to get it operated now. Just have regular MRI brain to monitor it's size and also get your baseline visual acuity and Visual field charted .

      It's not prolactinoma. However any other type of pituitary adenoma is still possible depending on your other symptoms / mri findings and other hormonal values

    Sir , Mom had this issue earlier around 20years ago , used primocriptin 2.5mg those days , but from 2019 doctor said to stop that medicine
    But now 2021 she has severe headache, Is that issue comes back after 20years? Now her age is 54+, basically she is a high pressure patient

    Doctor my mom 55 yrs old is diagnosed with pituitary macroadenoma, this was found in mri when she got blurred vision in left eye, in report it's mentioned as sella is expanded large lobulated solid enhancing mass measuring 23*36*23mm is noted in sella with suprasellar component. It is so intense on T1 and multiple petechial bleeds within.. Suprasellar component of the mass is seen compressing and stretching the optic chiasma & left optic nerve, it is seen indenting the 3rdventricle. Small protrusion of the lesion into the left cavernous sinus garde 1 is seen..Please help me that if we should go for surgery or will it get reduced with medicines? What is the success rate for 50 + yrs..Please pray for my mom..

      Definitely requires surgery. 50+ age is not a problem. Get it operated . Need to do the hormonal evaluation but as there are hemorrhages inside (apoplexy) , even if it's a prolactinoma it needs to be operated

    Hello docor,i first suffed from ed and went for a harmonal test.The results were normal.After 1.5 yr again performed tests tsh,testosterone,prolaclin and diabetes .my tsh levels were 5miU/L and had high prolactin level 278 .Is there chance for pituitary adenoma since it is affecting sexually plus tsh and prolactin are high?

    Hello sir, my prolactin level is 88 now my other hormones are normal and I have been diagnosed with microadenoma ( 9*5mm), I'm so afraid that it will grow or that I will have to do surgery on it, is surgery required? Is it life-threatening? Is it treatable? Can the tumor go away for life? or I will have to take medication for the rest of my life?

      Do follow up MRI after 6 months ...if it's growing in size you may require surgery. If it's same you can repeat scan after a year

      Before that pls test prolactin levels. If it's elevated then you definitely need MRI brain to look for pituitary adenoma

      Yes as rightly advised by your doctor, if your hormones are within normal limits you don't need surgical treatment now. However just get your visual field charting done by consulting an ophthalmologist. Headache may or may not be related to this

      Value of 62.5 ka matlab prolactinoma tho nahi ho sakta. Medical management prolactinoma me karte ha. In your case either just follow up with regular MRI scan or operate. Pls get a complete hormone analysis and vision field charting ...consult a Neurosurgeon at your place . He /She will consider all the above things and advise the best mode of treatment for you.

      I think I misread the size...ye tho bht chota ha ...operate nhi kar sakte ..milega hi nahi operation me ...tablet le sakte ho prolactin kam karne keliye if you are having symptoms because of it ..if not just regular follow up MRI bhi kaafi h

    My prolactin level was 150... initially when I checked and was diagnosed with microadenoma last year.
    But now my prolactin levels are normal from almost 10 months
    And I'm not on any medication as well..
    Will the adenoma shrink on its own sir?

      Levels upto 150 are not diagnostic of prolactinoma...this much increase can be because of multiple causes ..kindly also get your other hormone levels evaluated mainly thyroid profile, cortisol and growth hormone . Though I don't expect these to be abnormal now, by testing you will have a clear base line which can be used for reference in the future . Also you need to get your visual field charting done again to have as a baseline for future reference. Yearly follow up MRI should be enough unless you have some drastic changes in your symptoms

      Shrinking on its own is unlikely but it can remain the same without giving you any trouble. Hence just follow up MRI should be sufficient . Headache may or may not be related to this
