    I thank you for the information. I have tried all of what you have suggested over the last 4 to 5 years. Except for the very last one that you talked about. nothing has worked so far. Five different kinds of medicine along with all the other plus taking a cocktail. I have interstitial cystitis along with the bladder problem. I do not leak unless I sneeze cough or laugh very hard.. I do go more when I am laying down every half an hour to hour all night long during the day I can hold it for a couple hours while I am upright. I am going to suggest to my Urologist about the inter stem you talked about. Thank you for this information.

    I’ve had OAB for more than 13 years. And it seems I almost cured it just a month ago.
    Was trying different doctors. It took multiple years until they set a proper diagnosis. I tried electrical stimulation, Mirabegron, Solifenacin. Medication helped a bit, but after I stopped taking it, the symptoms got back to what had.
    In November here were my complains:
    * I went to a bathroom about 15 times a day (60ml - 130ml per one time)
    * 2-3 times during night
    * During mastrubation I had to make a break to pee
    * If I wanted to go to a bathroom then I had to do it ASAP.
    So in November another doctor recommended trying a Bladder training. As a main treatment. I adjusted it a bit my way and unbelievably I got my first results in 2 weeks: first time for maybe last 10 years I didn’t go to a bathroom at night. Then I started to go to the bathroom more rarely.
    Here is what I got now:
    * I go to a bathroom 5-7 times a day (200 - 300ml per one time)
    * Haven’t gone to a bathroom at night last 1 month
    * I can easily wait if I need
    So for some reason doctors didn’t recommend trying bladder training and hence I thought it wouldn’t help, so didn’t bother.
    ========
    One important thing to mention. Those recommendations are for Overactive bladder, which is a diagnosis urologists made for my case. It’s not like I just decided that OAB is my case. Before starting OAB treatment I made blood, urine tests as well as Cystoscopy and Urodynamic testing to exclude other reasons.
    ========
    Here is my modification of the bladder training which helped me to get cured from Overactive bladder which I suffered from for about 13 years:
    With the help of pelvic floor physiotherapist learn how to achieve these things:
    How to understand that your pelvic floor muscles are contracted. The doctor might use “biofeedback” device to help you with this
    How to relax pelvic floor muscles
    Keep a bladder diary. Record how many millilitres you urinate every time you go to a bathroom. While at home you can have a plastic bottle at the toilet with the volume marks. While not at home you can use a zip-bag which you also have to mark. Like 50 ml, 100 ml, 150 ml and so on.
    Analyse the reason why you might want to urinate. Be aware that sometimes the reason is caused by some patterns. For example, here are some patterns that I have: when getting up from a chair/bed; when sitting with my legs bent underneath me; when coming back from outside to home; when feeling depressed; when taking shower; when pooping.
    Then every time you want to urinate ask yourself: “Why do I want to urinate? Is it caused by a pattern? If so, what can I change? Can I wait a bit more?“. F.e. if the urge is caused by a body position like when sitting with my legs bent underneath me, then take a position when pelvic floor muscles are not contracted. If the urge is caused by a pattern like when you get up from a chair, then recognize that it’s just a pattern and try to ignore the urge for some time.
    When you are already at the toilet and about to urinate. Do these steps:
    Try to analyse how badly you want to urinate. Is it 10/10 or 5/10?
    Do you feel that sometimes your stomach is a bit stretched when you want to urinate and sometimes the urge is just in the head?
    If the urge is not that intense, can you try to not to urinate and postpone it a little bit? Like 15 minutes?
    After you urinate, write down in the diary how many millilitres you urinated.
    Next time you at the toilet play a game: Based on your feelings and urge evaluation, try to guess how many millilitres you are going to urinate. This was the key technique for me. After some time you’ll learn to understand by your feelings/sensations how many mls you’re going to urinate. Use this prediction skill and when you feel that the volume is going to be below 200 ml, try to relax the pelvic floor muscles and just ignore the urge. Go to the bathroom only when you think it’s going to be above 200 ml.
    The only exception when you go to the bathroom to urinate and you feel it’s going to be less than 200 ml is before going to bed at night.
    During the night try not to go to the bathroom right away. Try to resist as much as you can, but don’t torture yourself. Just make a bit of effort.
    Don’t drink like 3 hours before bed. Try to see if reducing the coffee intake helps.
    Don’t bother with how often you go to urinate. For me if I drink a litre of cola then at the very beginning of the training I can urinate within a 15 minutes interval. That doesn’t matter. What matters is to be above 200 ml.
    The doctor advised me to try the bladder training for 1 month and if I don’t see any changes, then we can add the medication like Mirabegron to help following the training.
    After 2 weeks of the training I didn’t go to urinate at night. Probably that was the first time in the last 10 years. Then later I went to urinate less and less at night. Last month I didn’t urinate at night at all. Wish you all luck!

      @@wtfscene6993 well, sometimes I still want to go to the toilet earlier then I should. But the intensity of urge is not that high. So I learned how to ignore that.
      But in general the quality of life has improved as day and night

    I’ve unfortunately been dealing with this for a good number years. if you’re a guy over 45 you get a surgery that takes a general express your concerns if you have an enlarged prostate. It’s very common to get urinary retention. And that will stretch out your bladder and bladders don’t shoot down so quick then you screwed. As far as when I’ve heard from therapist specialize in this a lot of people don’t do public floor or Keagle exercise correctly. All I can see is they’ve helped but considering the amount of time I’ve put into them. I am going to consult a physical therapist who specializes in this.. thanks for this video

    I went to a urologist, realized I wasn't fully emptying my bladder. He suggested he run a catheter, and I said I wasn't comfortable with that. I figured it was BPH. He disregarded everything I said and squirted some ambesol in my urethra prepping me for the catheter, saying 50 was too young for an enlarged prostate. When he went to his other patient, I got up and left. If he can't listen, he isn't a good Urologist/Physician. - Hot Springs Arkansas

      I’m not a dr by any stretch but not being able to empty you blatter over time could effect your kidneys. You should not mess with this and get to another Dr asap

    Best video I've seen on the subject today. Thanks Dr Crawford, love your relaxed, intelligent attitude ....
    One question though, how come that for decades we were advised to drink at least 2 liters of water a day ? Was it in those days of jogging/exercising like mad and that amount was required to rehydrate ?
    I' ve been taking analgesics for chronic back pain and hands + sciatica which dry me up totally for donkeys years. I drink mainly water, green tea and herb tea, with the odd light beer and glass of wine here and there. During lockdown(s) in the UK, I have put on 2 stones, off course around the waistline which does not help with pressure on back and hips, and am 75 in 3 days which doesn't help either. Have managed to lose 3 kilos lately, perhaps the gorgeous weather and pottering a bit in the garden again, another stone and a half and I'll be happy :) I realise I've to shun everything I like, even if am careful...

      Thank you for the feedback. A good benchmark for water consumption is 60 - 80 oz daily. Two liters is 67 oz so you are right on target there.

    Would like to see a video on irritable bladder, I've had pain in my bladder but no positive test that shows infection. What diet will stop the pain? What supplement will help?

    I'm 24 and I've been dealing with OAB. It has never really been a problem until I started not sleeping because I'll have to pee every 30 minutes. There have been nights where I don't get any sleep. Every night I have mental breakdowns because I don't know if I'll be able to sleep. It's exhausting and I'm mentally beat up by it.

      Hi. First it is important to rule out bladder problems like incomplete emptying, infection, and cancer, prostate problems, and diabetes. See your doctor for this. Once the diagnosis of OAB is made consider avoiding coffee, tea, alcohol, and soda. Start PfilAtes exercises.     I realized that coffee is one of the worst drinks for this problem, i stopped it fir a month then i tried one morning the next days were terrible. I have still a little more weaknesses for my almond Matcha drink every other morning.😢 but is noticed a difference while stopping sodas, coffees and lemonades.

    Thank you Dr. Is there medicine that reduce the number of time o go to the toilets especially in the night . I can’t count number of time I wake up to pee. Even during the day I think I urinate every hour or 30 minutes sometimes

    Thank you for the update, the issue is that when the bladder is full you go to release it, then suddenly it disappears then as you wait for it then it will release bit by bit and then after 10-15 minutes you have to go back for more release on and off till the bladder is completely emptied, this happened mostly only in the night. What best thing should I need?

      Hmmm not sure. Sounds like urinary hesitancy. Could be related to medication, anxiety, letting bladder get too full during the night. If it persists please see a urologist

    Thank you so much. I had a bladder lift 13 years ago it helped so much, the dr says I have OAB
    its botox or interstim is the recommendation. Vesicare mouth was dry I am willing to try healthy options. Really I just wanted a pill. I've had a kidney stone removed , many bladder infections. I was given Methenam Hip 1 GM to change the acidity no more bladder infections ,way more bathroom trips., or not making there.

    My father has urge of frequent urination at night. He is 63. He had coronary artery bypass surgery and taking cardiac medicine. Is he patient of OAB?

    I thought i had a prostate issues a year or two before the pandemic. Saw urologists got all kinds of different diagnosis and till rested on CPPS and I started pelvic physical therapy. It worked better than not doing it but I kept have flare ups though. I was hospitalized for bad intestinal issues, diverticulitis in my colon and a fistula formed from the diseased colon to my bladder . I had surgery they took out a foot of colon that was bad and repaired the bladder that fistula had wreaked havoc on. A year and half later I’m starting to get the flare ups again and my urologist said it could be trauma from the surgery or scar tissue. He said it sounds like symptoms of overactive bladder. I don’t have problems leaking but I just have consistent urgency and frequency, and sometimes flow is weak and not strong. Sometimes I’ll get a flare up after sex with my wife . I know the usuals coffee and alcohol and carbonated drinks. And I have really minimalized my intake. I’m just tired of this , I’d like to go travel and not have to think abt urgency and pain and where is the closes bathroom

      Armando- Suggest you research a urological condition called interstitial cystitis and a procedure called bladder instillation which is intended to calm the bladder lining. I currently take a OTC supplement called Bladder Rest which has been very effective. I know these issues can cause significant anxiety and stress and play havoc on one’s sleep hygiene. Good luck.

    I went urologist 2 to 3 doc. Overactive bladder problem. Doc told do pelvic floor exercise and kegal exercise and behavior al changes and also stress.. These all types told just 2 week s I done now today fever came.. Can u plse help in that video what dr told. I didn't understand.. Give me suggestions.

    I've done so many Kegels that I must have the strongest pelvic floor muscles in the world, but Kegels won't stop my bladder spasms. I avoid all the drinks you mention, except for one cup of coffee in the AM. I'm still spending way too much time in the bathroom. My anticholinergic gives me the driest mouth that it's hard to eat & swallow. This has been a nightmare!

      I’ve done too many kegels I got hypertonic pelvic floor or too much tension. So I’ve been working on relaxing -still strengthening-but relaxing pelvic floor and abdominal muscles. I have muscular dystrophy so I tend to tighten my abs or core for stability a lot especially while exercising. So now I can’t do any kegels. Trying to work on other things first. 😊

      Hi. It is unusual for med to get a UTI or to have incontinence or OAB at such a young age. See your doctor, get screening for STDs, consider asking for a Urology referral

      Again very unlikely for a 17 year old male to get UTT. Sexually transmitted diseases are much more likely

    Does Botox treatment PERMANENTLY change the condition of the bladder? In particular could someone who has previously not suffered urinary retention start to have a problem emptying the bladder that is enduring?

      Usually Botox treatment needs to be repeated once or twice a year. Yes urinary retention is a risk for anyone

    I'm not diabetic but after I eat I pee every 30min for like 2hrs until don't have to pee as often anymore. Is it my insulin goes up after I eat that I pee so much after each meal.

      Hi Cha, No it is not your insulin. Could be what your are eating and drinking. Spicy foods, coffee, tea, alcohol, and soda can cause this.

      1. Make sure it is not a prostate problem. 2. Make sure it is not diabetes. In other words you need to see a doctor.

    Demo was perfect a few misplaced words but i knew what u meant. U said zioper on each uterine tendon near the vagina opening. But i got it i got a full uterine cystocele rectocele at a 4 and been controlling it for 5 years and over philates excercised and went kinda rigid on the inside and i believ it all started.from the dr that delivered..my 3rd chikd and ripped the placenta away from my uterus it hadnt detached yet they dosed me with more plutosin and disnt wait for it to contract away and seperate n just ripped and she cleaned me out during a contraction because i was impactes from laying in a hospital bed to prevent having my baby too early and she put her hand in there and cleaned me out like a cow! It was so painful but.i got.over it cuz i was in labor!

    Constant urge to urinate even after I go. Pressure down below. Bloating, peeing 13x a day. Started with uti pos leuk neg nitries cutlure ecoli ...now I have leukocytes and blood ..culture showed no bacteria. I'm so sick of it.. docs did blood my wbc count was a lil higher they checked sugar levels they were fine but didn't actually have a1c in morning. Cmp bladder scan. 6 uti test/cultures.. went to er last week for these tests all they seen was I was dehydrated. Gave fluids bc I'm peeing more than I'm drinking... I want to give up this just started suddenly with uti. Even during treatment I felt no relief 😭

      Seen urology he did a urine drink to voiding 3 days. And wants to do a CT scan end of Feb and a cystoscopy in middle march... I feel I'm already getting dehydrated... I just don't know what to do... I can't wait this long

      Wow. Deep breath. Consider seeing a Urogynecologist. Consider eliminating coffee, tea, alcohol, soda. Keep follow-up with urologist, ask urogyn if they think you have a hypertonic pelvic floor,

    Is overactive bladder known to give tje added bonus of ED? I've noticed now with the 2nd flare up that my whole entire reproductive track feels shut down entirely. Still have desires but my body just wont..react

      Hi. Both can be the consequence of the autonomic nervous system not working properly. Consider seeing a urologist after talking to your PCP. Diabetes, vascular problems, and neurological problems need to be ruled out.     Im 66 retired & a rat leg amputee in a wheelchair … I have to go to the bathroom a lot and it seams like nighttime I don’t sleep much due to the bladder deal I take a high blood pressure pill/water pill

    I have a hypotonic bladder, that spasms painfully. I also have PFD. I'm getting PT to relax my pelvic floor muscles, but I always feel that I have to urinate. Taking Oxybutynin and Myrbetriq made me unable to go and I had to be cathed. So now I am self-cathing when I feel urgency, only to find there is nothing in there yet. This all began after a UTI in September. I follow a clean diet with minimal caffeine. Any advice for the urgency sensation? No OTC meds get rid of it.

      Hi. Urinating urgency is very common among women with hypertonic pelvic floors. Pelvic floor massage can help if you can find a well trained Women’s hHealth PT. Meditation may help as well

    Hi Dr C Thank you so much for this video, it is the most information and comprehensive that I have come across in years!
    I have suffered with overactive bladder/urge incontinence for decades..it's the feeling of having acid in my bladder most of the time:( I've tried a no of different meds which haven't worked and have the side effects you describe.
    I've also got a KegelElectronicPelvicFloorToner, which can overstimulate the bladder so isn't always helpful!
    Is the pfilates available to me if I am UK based?

      I agree. I bought two early on just to keep my vagina tight. I work out, so I always thought that our private parts should be strong as well. I ended up lifting a weight instead with a wooded small egg and rocks attached to it. I lift them just fine. No weak pelvic floor here; or vagina. For me it's the Turkish coffee, I believe.

    Do you have a solution for Weak Urine flow for a male 55 years but not due to Prostrate but Bladder Neck Obstruction is the cause as per Urologist.

      A weak urine stream may be due to a weak bladder wall muscle or loss of nerve supply to the bladder wall. Urodynamic testing can look at this issue and also look at the coordination between the muscles of the urethra and the bladder muscle during urination.

    I cut coffee out to no avail.
    I pretty much travel with a wide-mouth plastic cup.
    And not drink anything for a 4 hours.

      Loosing weight can help some forms of incontinence. For example, women who leak urine with physical activity. For men with too frequent urination or leakage with urgency weight loss may be less helpful unfortunately

      I see. If the pelvic floor muscles are unable to relax it can make it difficult to empty the bladder. It can also make intercourse painful. You can help these muscles to relax by inserting one finger or thumb into the vagina and apply pressure downward to the right for 20 seconds then downward to the left for 20 seconds. Repeat for ten minutes twice daily. If the pelvic floor muscles are too tight this may be a little painful at first. Good luck

    Sometimes overactive is cause from pfd, interstitial cystis, Prudential neuralgia. These r very bad interference with ur daily life and self esteem 😢

    Pls help.. im only 31 yrs old and i have over active bladder. Like the interval is seriously often. Sometimes i even pee after a min or two. I consulted 6-7 doctors already. Was prescribed with vesicare but that's not helping. And i dont have that money to really maintain it. Plss plss help. It's really depressing me so bad since it's making me seriously frustrated. 😭😭😭

      @@mikec5391 ... Yes I had retention with the Botox insert. The retention result from Botox was so good, that I almost forgot I had bladder issue. There was no catheter. No surgical scare. They just insert a Botox ballon near the bladder. They put me to sleep. Procedure takes about 10 minutes. The only soreness I had was slight rawness in the area where I urinate from which lasted for 24 hrs. Not sure where they go in to implant this Botox ballon for a man. My only issue is that I got to go back every 12-18 months to get it redone, and I got to find somebody to drive me back home after the procedure.

      @@thepelvicfloorsolution well I read that citrus fruits (citrus acid) can all irritate the bladder being all acidic like they are..I'm sure there's plenty of that in lemonade which I've been drinking heavily for roughly 4 years. Also tomato based foods and spicy foods which I eat often. So I may have answered my own problems

    I'm 38 and out of the blue woke up 3 weeks ago urinating frequently. Sometimes once every 20 mins or once an hour it varies. Went to the doc and currently been on antibiotics for prostatitus. It's been over a week on the meds and it hasn't stopped yet. Should I be worried?

      @underated17 All my urine test came out normal. Prostate is fine. Urologist said it may be from my urine being too concentrated, but I've always drank about 3 liters of water a day for years lol. Didn't understand how that could be possible. I still have my days where I'm urinating quite often but it has let up a bit thank God! Hang in there.

    Urine passing at nigh for 54 years woman is shameful.
    That i know. But since I am taking medicines like tegretal, gardinal, lamitor are my medicine which cannot be stopped .is there any way that urine can be stopped at night. Because if I dont drink water in morning I can have Constipation.

    I just turned 20 and I have been dealing with OAB for maybe half the year now, I went to doctors and they all treated me like I’m stupid because I was too young to have really bad problems like this maybe. This has effected my life in a very negative way no matter how much I tried to make it positive. I’m glad I found the exact name for it because first thing tmrw I’m fixing this shit. Hopefully all the sex and alcohol didn’t permanently ruin my dick while this was taking place. Wish me luck if u see this ig haha

      Vaginal estrogen cream can help overactive bladder but rarely resolves the problem. Pelvic floor exercise is essential

      So so true , many of my girlfreinds as well as myself in peri menopause and menopause urinate frequently and in U.K. they prescribe vaginal oestrogen and it does the trick perfectly . Also for elderly ladies that Have UTI problems This has been found to help .

      My OAB started with onset of menopause, but hrt, and vaginal oestrogen haven’t stopped the problem.
      I’ve used bladder meds which help a little, and I had Botox 3 months ago, it helped reduced the spasms, but now I think it’s worn off as the spasm are back and painful.
      I saw a womens health physio two weeks ago, and trying some exercises and bladder trying, but it’s so hard. I’m exhausted by this awful condition it’s ruining my life 😢

      That is too often. I suggest seeing your primary care doctor as there are several issues other than overactive bladder that can cause this problem.
