    It's too bad that DHT only affects hair loss in follicles on the most important part of the body. If only DHT caused hair loss on my ass and/or back and NOT on my head! 😎

    I watched so many of your videos but THIS ONE seems to have an answer that I HAVE BEEN SEEKING FOR YEARS! I saw many dermatologists/physicians but no one could ever tell me why at some point my androgenetic alopecia started to have a mild inflamed look. Got a biopsy, some dermatologists said the biopsy was interesting, some said dermatitis, some said Lichen Planopilaris. No clear answer, but it’s interesting how the inflammation is following exactly the genetic and identical male balding pattern of my uncles. I’ve been having it for the past 10 years but it doesn’t seem to be destroying my transplanted hairs. Had a hair transplant 5 years ago.

      Hey, ive tried them both, stemoxydine is cool, it works, but has a very mild effecto on growth. Alfatradiol im not sure, ive only been using it for about a month, and im not sure, but i think it started me to cause shedding, wich might be a positive i guess (like a sign that it is working), i mix it with finasteride (0.25%), so if my shedding doesnt stop ill up my finasteride dosing. In my opinion both are good addons but not very good on their own, they likely wont stop hairloss alone.

    If patient has both DHT and inflammation problem he can use fin for dht and change his diet as well as use steroid cream for his scalp inflammation problems. Diet should not include sugar , processed foods, high carbs, spicy food, fried food, etc..Also stress needs to be managed cuz that has an effect on scalp health as well. Basically, one needs to tackle all causes of hair loss from all possible angles. It’s not just DHT that causes hairloss; it could be the major cause but inflammation and allergic reactions also should be taken seriously. You can use also keto 2% shampoo for inflammation because if it didnt help block dht in scalp it can at least treat inflammation and dandruff which will lead to less shedding and a thicker healthier hair. Before, alot of my hair especially at anterior was thin and whispy and you could see the scalp. After even just a few months I noticed how much thicker the hair strands were getting and it began to fill in. My hair prior to finasteride was like a light brown and now Its VERY dark brown due to the hair shaft thickening and people were asking me if ive dyed my hair darker lol. These doctors say to start at 3mg a week and if thats effective no need to do the recommended 7 mg a week, as it only gives a higher chance of side effects. I’m 23. Would love to see you explain the stages in order you should try different treatments. A dermatologist told me i have a bit of MPB at the front but nothing severe (yet). The shedding is likely tel effluvium. After research, I think going straight onto minoxidil might accelerate the balding in the long term, seeing as min has a plateau after about 5 years.
    Is it better to get on min/fin early or wait until I’m showing more severe signs of it progressing? My concern is that I do not want to get on these drugs too early seeing as they will lose their effect before I’m 30.

      Best to monitor your own hair loss with pictures every 3 months and judge what you are happy with. There is no point waiting till your 30 to start if you have little hair left.
      Work on the assumption that min, fin and lllt will allow you to keep most of what you have and then slowly continue to thin 10 years later. With hair transplants as an option to fill any unacceptable areas. That is atleast what I am doing.

    1. What should you do if Finasteride is not working and should I take dutasteride or higher finasteride dose instead?
    For context: I've been on finasteride for around 8 months and have lost significant amount of hair however, it probably slowed down the rate of loss a little bit.
    You stated that it takes 6 months to see results and stability. In my case, there is no regrowth of hair and no stability.
    Furthermore, both sides of my family only have some thinning hair which is to be expected for men who are 50+ but no one is bald or have excessive thinning.
    I have diffuse thinning on the top and sides of my head but not the back. The hairline is nw1. I'm 21 and have been losing hair for around 1.5 - 2 years
    I consulted with a trichologist and he told me that there isn't much dht build up in the scalp and I should get bloodwork done. I plan on consulting with someone else as well.
    I'll probably add minoxidil and possibly laser.
    2. So, what other causes of hairloss could it be if it isn't genetic (for someone in my case) and what should I do about it?
    Thank you very much

      If monotherapy is not effective maybe you need to add more things to your regimen and if you still continue to lose ground then throw the kitchen sink at it. Finasteride is not getting rid of all androgens in your system and it also causes your testosterone levels to increase so you need a topical anti-androgen to take care of the remaining androgens in your scalp like CB-03-01 or RU58841. That is all you can't do as far as prevention goes. As for stimulation you can also add to your regimen minoxidil, dermarolling and a low level laser hat.

    • @kylekeenan3485
      @kylekeenan3485 4 ปีที่แล้ว +2

      The lowering of the dose is ideal for reducing the side effects and attempting to have as little fin in your body whilst maintaining its affect on reducing your hairloss which is different for every person.
      If your not really noticing any side effects then continue as you are to watch the results. If you do notice side effects, then you should consider reducing your amount, but remember that could reduce its effect. The only way to know is to monitor the benefits you are getting at your current dose.
      If you looking thin then add LLLT and Minoxidil too, they complimemt fin and can make the hair recovery even better. However its not necessary right now if you are getting great results from fin alone. Just have to be patient. Proper results are measured in months not weeks.
      For measuring results and hairloss, I picked a light in my house and stand under it at night time (so lighting will be consistant) and take pictures every 3 months. Try to pick a consistant hair style for the shots too like a middle parting and maybe after showering but when the hair is dry, that way to won't have different amounts of greese/oil on your head changing the picture comparison.

    Hi Dr. Knudsen and Dr. Jayaprikash! Thanks for the informative videos. My question for you involves shock loss:
    I have a 1x2 inch hairless birthmark on the crown of my scalp which I intend to get surgically removed in the near future. I’ve been on Finasteride and Minoxidil for close to a year and have recovered about 70% of my hair from a previous diffuse Norwood 3. Do you think surgery would affect my hair gains in any way from possible shock loss?

      Hello,
      So by the second year mark I realized minoxidil/microneedling was a pain in the ass plus the minoxidil was causing me to have dark undereye circles. I fell off with my minoxidil routine but kept with the finasteride. That being said I lost all my minoxidil gains however it still wasn't as bad as baseline. I got a transplant a year ago, which was around 2.5 years after I started finasteride. It looks way better now than it ever did. The gains you get from a hair transplant are incomparable to anything you can gain with meds.

    I was on Finasteride for around 4.5 years. It worked beautifully for the first 3 years. It even helped me regrow hair in the temples! But after 3.5 year's of taking Finasteride, I noticed my hair thinning again ever so slightly.
    So I made the switch to Dutasteride which I take daily.
    Been on Dutasteride for 3 months so it's still to early to tell.

      Hello Shivam, I’m not a doctor but I’ve also been struggling with hair loss. I think if you are losing hair in all of those places you likely suffer from Alopecia Areata Universalis. Check to see a doctor, especially a dermatologist or a trichologist if you are looking to stop it and possibly try to reverse it.
      They are likely to prescribe you Finasteride, corticosteroids or some other form or treatment.

    Those numbers may be droping because of the Age of the Control Group ? Hairloss does get worse as we age, even though DHT Levels naturally lower themself with age, so there is the general aging effect of the Human Body in play. For my part, I'am 22 years old, eat healthy, do a lot of exercise and do not smoke or take any other drugs, still experience hair loss due to my genetics and the DHT. So, I'am hoping that the dropping in efficiency is due to the age of the Controll Group ! 🙏 Just a thought of mine, just hoping to stabilize my hair loss till I'am 30 ^^ What are your thoughts on that opinion ?

      @Zogixaas09 4 ปีที่แล้ว +1

      pseudo-gyno? or real gyno? Me myself ive beveloped a small but visible pseudogyno. I'm not sure if that's because of the drug but I think it's highely likely. To counter pseudo gyno I tried to stop fen for 2 months but it persisted so I'm now also doing gym and I'm on a low calorie diet to lose fat tissue which seems to be helping so far it's stil there but a lot better than previously. I've checked my estradiol levels and theyre a little over the average so I think that may be the cause. My guess, due to the hormonal imbalance which is being caused by the drug my body may be prducing too much estrogen to balance out with the over the top testosterone levels which in my case are +/- 1000% over the average

    I've been on finasteride for 13 months (1.25 mg every day) with zero regrowth. If anything, my hair has gotten worse during the last 13 months. I added 5% rogaine (minoxidil) foam into my regimen 3 months ago and am hoping the combination will regrow some of my lost hair. I am 42 years and started losing hair (diffuse thinning) at 36 years old. Up until then I had a very thick head of curly hair.

      Same boat. Had hair of a lion until 35. Diffuse thinning then started. Diffuse is the worst cause you basically have to shave it. When there's no strong hair left in the center, it just looks weak all over. Been on fin 9 months, shedded a bunch, no regrowth. Idk, just seems thinner. I just quit looking at it cause its depressing. Really has changed my attitude for the worst.

      Can they take? Yes. Is it safe for women? Nothing says it isn’t. The reason why it’s not approved is because women can conceive and what they take impacts a possible fetus. If the fetus is male, deformities can happen if you are on Finasteride as it can impact his development. Nonetheless you can take it while you are not wanting to conceive and then enter a remission period of a couple of months if you want to start a family.
      My doctor prescribed this to me and I’ve had no side effects. I’d probably say that it’s safer for women than men since there is no prostrate to mess up. In comparison to corticosteroids and the few other drugs females have access to is one of those with the least side effects.
      For efficacy I cannot speak as my treatment is not over yet but 80% of women seem to report improvement after 6-8 months of treatment on 2.5-5 my daily (lower dosages that men take seem to only provide very mild improvement).
