    I’m living a dental nightmare and cannot find a good dentist. Long story short I received no dental care as a child and first visit to dentist was at age 16. I had many large silver fillings then braces. Despite my diligent cleaning and care of the years, one by one things are failing. Tooth breaks, cavity under filling, etc. I have two crowns on opposing sides and can barely chew. Facing oral surgery soon and then I really won’t be able to eat much. I’ve lost weight and my health is suffering. I don’t know what direction to go in and whether to get implants or what. Seems every few years I lose a tooth and Weill have none left. It’s depressing.

      Hi Jayden. I am so sorry to hear about your issues. Your situation is a complex one. I have encountered patients with a similar problem before. It sounds to me like you may have to place a handful of crowns on your teeth. Large fillings are vulnerable to breaking over time. If you have a handful of large fillings, you may require a bunch of crowns to protect these teeth as they start to break over time. Unfortunately, this treatment is extensive and also very expensive. Plus, you need to get the work done within a relatively short amount of time. If you want to just do one or two crowns a year, you will never get anywhere. Also, there are no guarantees that your crowns will last forever. You could end up needing a few root canals, or even end up requiring a dental implant in the process. So what should you do? I recommend that you find an expert dentist who can fix all of your teeth within a course of several months. You can probably get a better price this way as well. I would prefer this treatment to removing a whole bunch of teeth and getting partial dentures or dental implants. It would help for me to see your X-rays. Good luck with your treatment and keep me posted.

    Hi Dr. Jazayeri, very clear and informative video!. Question: I have one upper central incisor missing and the other central incisor is broken to the gum level. I was advised to get the broken tooth and the incisor (with root canal) next to the missing tooth, both extracted. The plan was to get implants on the extraction sites to act as "anchors" and place a tooth in the gap in the middle. My concern is losing those two teeth that are still "alive" as I don't have many teeth left. What's your advice? Many thanks

      Hi Julia. Thanks for the comment. I would say, implant sounds like a great option for you. I know people don’t like the idea of losing teeth. But if you’re teeth are not strong enough to support crowns and bridges, which sounds like the case here, then an implant (I’m guessing you’re getting an implant bridge) is a great option. You’re implant bridge will have much better prognosis than a bridge on your own compromised teeth. Again, I’m assuming your teeth have poor long term prognosis. Implants don’t get cavities and they don’t rot, so don’t feel bad about removing your weak, fragile teeth and replacing them with hefty dental implants!

    I have been trying to get full mouth restoration or all on four without spending a million dollars, maybe leaving the US and ultimately whether or not to save or remove the couple teeth I have left. I literally cannot say enough good thing about these videos by this dentist! He seems so knowledgeable and I finally have figured out Iam just going to get full dentures and no longer trying to pens my life savings for implants! Thank u so much Dr!

      Hi Rochelle. Glad you found my video helpful. I tell this to a lot of my patient who have no more teeth and are hesitant to go for implant treatments: Try the dentures and see how it goes. If it goes well, great, you save tens-of-thousands of dollars. If you hate the dentures, then we can discuss your implant option. Afterall, you need a denture as a temporary or permanent solution, so why not try it out and see how it works out?     I wore a metal upper partial (3 teeth) for years and it had a metal bar going across the roof of my mouth. Miserable! So, I mainly wore it for cosmetic purposes. Just lost an anchor tooth. After watching this video, tomorrow, I will opt for the Valplast. Thank you so much for this great information!!

    Im happy I found your informative video. One week ago I recieved a metal cast partial that im learning to adjust to. I was never told about Valplast and would rather have that one.

      Hi Sabirah. I'm sorry that your dentist did not discuss this option with you. At a minimum, I think every patient should at least be given the option to choose between a metal and flexible denture.     Hi Doc, this was really helpful to me. I have 2 actual teeth in my upper arch and a few weeks ago my dentist built a 4-tooth bridge between them that I really like (now if I need to remove my partial for medical tests, etc. the bridge stays in so I don't look like a vampire!). On the other hand, now I have a temporary acrylic partial with 3 teeth on each side that I hate. My main complaint is the thickness and sharpness of the back of the denture. It's causing some gum soreness and I don't like the way it feels on my tongue. From your great video I'm thinking the flexible partial might be the way to go when my gums heal up (I had one extraction and I suppose I had to have some gum work done when the bridge was installed). Right now the temporary connects into the bridge without any clasps so it looks very natural, unlike my old partial that had metal clasps that were very noticable. Do you have any thoughts for me on flexible vs metal? I considered implants but my sinuses are so horrible I was concerned with the recovery protocol because the implants would include bone grafts. And I'm 74 so it's not like I need to worry about the denture lasting 20 years (I think...). Thanks so much!

    Hi, thanks for all the info. Im considering just a single pre molar partial denture. In terms of the disadvantages youve highlighted, would just a single tooth partial be less problematic against two or more teeth?

      Hi Mark. I usually prefer dental implants, or even Maryland bridges, over single tooth dentures. If you're going to go with a partial denture, I'd go with a Nesbit partial which is more comfortable and more esthetic.

      Hi Matt. Glad you found some useful info. I would say yes, this is a great candidate for Valplast partials. You can put clasps on 5, 6, 11 and 13 so the partial is stable. And the clasps will not be very visible since they will be gum tissue colored. Also, the pink clasps are gentler on bicuspid teeth which are more likely to break with metal clasps. Good decision, and best of luck with your new teeth!

    Hey Thanx Doc, stumbled across your video, I as a matter of fact currently, (I have dentist & have been with him since about 2015 or 16),bottom teeth we got dialed in👍 the UPPERS now the way it worked out : two chewing teeth bridges together per side ankored with a single root canal per the double molars within a week of each other they broke off/out, so I've been having to chew all of my food w/ my incisor teeth, I have lower morals but no upper. Don't take me the wrong way, he's a good doctor & stood behind his work & kept word when he had too, but he basically rebuilt my whole mouth (I still have original front lower teeth, every thing else replaced; he's let me like pre-pay as we went along) but I've given him thousands & thousands & more thousands & I am just afraid of what this is gonna cost me... Lord have mercy ! we've like sorta talked about some of this & implants he'd probably have to do bone grafts & then implants; the partial I need : upper, a pair of molars each side so I can eat normal again, hey sorry Doc... how I sorta unloaded on you... I want you to know that this information really helped (like no hocus pocus) me the stupid "flipper" will not help as it's cosmetic ! looks like I am headed towards the metal ones still on a count of $$$ peace Doctor I am in Riverside County, State of Confusion (I mean) California👍☺🖖V3

    I only have 5 teeth uppers both canines in really good shape, two front teeth worn do to bruxism and I have one molar but my bottom teeth are only missing 2. Do partial uppers have that palate cover or can they anchor without all that roof material? Approximately what would it cost for good flexible dentures average not exact price? Btw I'm a veteran but the VA doesn't cover dental unless it's service connected and I think my teeth are causing my diabetes to get worse 66 yo army veteran thx.i I don't know where you are located I'm in the Midwest and I will travel for a good dentist like you. I just got back from living in Mexico for 6 months 👍

      Hi Philip. Thank you for serving our country. Here are the answers to your questions:
      1. Do partial dentures have to cover the palate (roof of the mouth)? Yes, upper partials always cover a portion of your palate. However, since you have 5 anchoring teeth, which is a great number, you can request that your dentist thins out the palate region as much as possible. This means you will have only about 1/3 of your palate covered instead of the usual 50% or more.
      2. What is the cost of flexible partial dentures? There is a huge variation. However, without insurance, I believe they cost as little as $1,200 (cash not charge!) up to $3,500 (at "dental spas"). Most dental insurances cover a portion of your partial denture cost, but dentists usually charge a bit extra to make it in higher-quality material, like the flexible which you had mentioned.
      3. Could my missing teeth make my diabetes worse?
      Absolutely! I've had a handful of patients referred to me for dentures and dental implants by their physician or gastroenterologists. The concern was they were not chewing their food properly due to multiple missing back teeth. Without sufficient chewing teeth, you can't break your food into small enough particles, which may affect your digestion. Fix your teeth, and you can see an improvement in your overall health within a few months (the time it takes to get used to your partial dentures).
      There are plenty of good dentists I'm sure where you live. FYI, if you were in Mexico for 6 months, you should've gotten your dentures done down there Philip! Maybe next trip...

    I am missing one front teeth. I am using acrylic plastic denture. I am finding it very uncomfortable and therefore I cannot wear them. I am thinking about the metal partial denture- will this be better and more comfortable than the acrylic one to wear? If i lose bother teeth, could you add another denture to the metal? let me know.       Hi Sean. Sorry to hear that your poor experience with acrylic partials. You may find that a metal partial is more comfortable, but don't forget, there will be several hooks and clasps on several adjacent teeth. Also, you CAN easily add more teeth to metal partials should the need arise. Did you consider getting a flexible plastic denture? Those may be more comfortable, although it's difficult to add more teeth to flexible dentures. One of my favorite types of dentures for replacing a single front teeth is a nesbit denture. These are very small dentures which only hook onto one neighboring tooth on each side. You might also want to look into a "Maryland bridge" or "dental implants" if your budget allows it.       Hi Joe. This type of partial denture is particularly loose, since there is no back tooth to stabilize the false teeth. As far as dental implants, it depends on how much jawbone you have. The sinuses are in this region, and often times they interfere with implant placement. You may need a surgery, such as a sinus lift or lateral graft, to add bone before implant placement. So yes, you can place implants in this area, but it might being difficult and quite expensive. How is your partial denture doing so far?

    I have a dental bridge on teeth 9 10 and 11. Also a missing tooth on 12. Under the bridge on 11 there is decay and will need to be extracted. I believe that is the abutment for the bridge. Would a partial work? I don't know where the partial would hook onto. If I did implants how many would I need. I believe the dentist would section the bridge. I am a senior age 74 and don't have the money for all the implants. What would be your suggestion?

      Hi Ed. If you would like a second opinion from me, please send me your X-rays to TPMyMouth@gmail.com and I'd be glad to guide you in the right direction. Nevertheless, let's go over the different scenarios which you might come across here:
      1. Small cavity under an older bridge: If you have a small cavity, one that isn't infected or causing your teeth to move, then you always have the option of "watching it". In other words, leave it alone! Of course, I wouldn't tell this to a 24-year-old patient. However, if you're 74 years old, on a limited income, and your bridge isn't causing any problems, then you can always keep the bridge until it fails and then proceed to the next step.
      2. Medium-sized cavity: This is when the cavity is slightly larger, but not a full-blown infection. One option is to see if they can do a root canal through your bridge and then seal up the tooth without having to redo the entire bridge. In order to do this, you might have to see a root canal specialist (an endodontist). The dentist will drill a hole through the bridge, perform the root canal, and then seal the tooth right back up. This is an affordable way to preserve your bridge for a few more years without breaking the bank!
      3. Large-decay or infection into the jawbone: If your cavity is causing the bridge to move, or if there is an infection, then you should treat this right away. No more waiting to see what happens or shortcuts. If you can afford it, then obviously dental implants are your best option. The next best option is to remove the canine tooth and extend the bridge to the neighboring bicuspid tooth. This means that the bicuspid will support a bridge that is one tooth longer. Now I'm not crazy about bridges that span the canine tooth due to their curvature, but this might last you for several additional years. Your last option should be partial dentures. Transitioning to partial dentures at an older age can be difficult. Unless your insurance covers it, partial dentures still cost a few thousand dollars and only give you false teeth that go in and out. You may do well with them or you may not. I recommend that you get a temporary partial denture, which we call a flipper, and see how you fare with them during the recovery period after you remove your canine tooth. This will help you make a more informed decision regarding wearing a permanent partial denture.
      Sorry for making things complicated! Hope this answered your question, and good luck with your treatment.

      Hi Cliff. You have a great point. Why would a plastic denture cost more than a metal cast denture? After all, metal cast dentures have tons of expensive metals, such as nickel, chromium and cobalt, which is worth much more than plastic. Still, that's how dentists charge for these things. Generally, you have to pay a premium to exclude metals from your dental prosthesis, despite the fact that it costs more to make a metal partial denture as compared to a plastic one. The same goes for crowns. Dentists charge more for ceramic crowns as compared to porcelain-fused-to-metal crowns, known as PFM. However, ceramic crown material costs less than PFM crowns, which contain valuable precious metals. Thanks for commenting, and good luck with your treatment!

      Hi Michelle. Your partial denture will have 3 clasps, two on the canines (eye tooth) and one on the back molar. I would consider this as a good partial denture. Having 3 claps makes your partials relatively stable and the molar clasp will help significantly to stabilize your false teeth. Be sure to take excellent care of your remaining molar to keep it for as long as possible!

    I asked my dentist if they damage the remaining teeth. She told me that they did not. I read that they do and I saw the damage that happened to my sister's teeth. This was years ago that my sister had hers, so I tried to give my dentist the benefit of having better techniques and materials. This is why I am so nervous about my new partial.

      There are a few reasons partial dentures damage your remaining teeth. The obvious one is that the denture clips that hook onto the remaining teeth cause damage. The other factor is the added stress that is exerted on the remaining teeth. Partial denture teeth are not as effective as handling bite forces, so the pressure on natural teeth goes up significantly when wearing partial dentures. Be sure to remove your dentures at night. Brush the remaining teeth really well. Keep your regular cleaning appointments so that your dentist can monitor the condition of your remaining teeth. I usually have my partial denture patients come every 3 to 4 months for checkups and cleaning to help them retain their remaining teeth. Hope this helps. Good luck!

    Hello Dr.:
    What might you suggest or recommend for someone who recently (3 weeks ago) had their lower first molar (#30) extracted due to a failed root canal? Upset because otherwise teeth are very nice, which have been fairly straight; never needed braces. Received compliments over the years; being in this position is rather upsetting. Nervous because the gap left behind looks so much deeper than I expected. Trying supplements to help bone... VitD3/K2, Vitamin C, magnesium (think I need a better formula of it, though).

      Hi Nancy. I would recommend placing a dental implant. It is your best option, bar none. Partial dentures don't work well for a single tooth. Bridges require damaging two neighboring teeth. Leaving a gap is just uncomfortable, especially for someone with otherwise perfect teeth. Get the dental implant and you'll have the closest option to having your natural tooth back.
      Taking bone supplements is great and I don't discourage it. The healthier you are, the better your success rate. However, bone loss after tooth removal is more of a localized issue, not systemic. If there is not enough bone, you would need a bone graft procedure. How do we know if there is enough bone? That depends on where your nerve is located. Your dentist will take a CT scan to see if you have enough jawbone or if a graft is required. So even if you lose a lot of bone, if your nerve is located in an unusually low position, then you don't need a bone graft. Hope this helps and best of luck to you!

      @@johnjazayeri Thank you so much. Now I hope my nerve is in a good place! Cannot believe how sunken gums have become where the tooth once was... almost expecting the teeth on either side to collapse/become loose very soon.
      The second molar (behind missing tooth) has a dark area on lower portion of tooth. Dentist had told me this on last xray. Never could floss between that crown well; would always shred or get stuck. Hoping a root canal will not be in store for that tooth!

      @@rosemarywaldie4958 Not yet, but I have had 2 consults about it with 2 different periodontists ($200-300 for each consult 🥴). I may get a 3rd opinion... To my surprise, both said I do not need a graft. Another practice I've only corresponded with via email so far told me that a lot people are unnecessarily recommended grafts.
      It's going to cost close to $7000 when all is said & done (for one molar)! That's hard to think about. But then I have signicant loss of chewing ability on that side (it's a second molar). If I live Another 30 or 40 years, it could make eating/proper nutrition difficult.

    Im missing four molars in the upper jaw and Im thinking of using a metal partial denture but I have many doubts and fears.
    Can I chew and eat well with it?
    Do the clasps damage the healthy teeth where they are attached?

      Hi Emilio. A partial denture, specifically a metal cast one, provides for decent function. It also improves your chewing efficiency. You will be able to eat much better with them as compared to with no dentures. It's true, the clasps can damage your healthy teeth. How to avoid this? Be sure to brush and floss the anchoring teeth very well. Remove your partials every night so that they don't overstress your teeth. Do these things, plus see your dentist for regular checkups and cleanings, and your anchor teeth will last for many years to come. Partial dentures are a decent option. They are not as good as dental implants, but they are much simpler and much more affordable. Good luck with your treatment!

    Dr john it seems valplast is the best. Am I right? My insurance does not cover partial. I have 4 missing teeth all between the canines.

      Hi Mike. I would say yes, valplast is a good option for replacing the missing lower front 4 teeth. It has pink clasps, so it's less visible, and it grips firmly so it doesn't move.

    I have to get premolar biscuspid extraction bc I don’t want to do a root canal Im thinking of some type of partial but I’m also like … can’t I get a grill to hold the teeth in place from shifting and wear it sometimes??

      Hi Logan. Good luck with your tooth extraction. A grill, I’m guessing your referring to removable teeth made by a Jewler, can replace your missing tooth. However, it can also damage the teeth which it sits on. Wouldn’t recommend it. The best option to keep your tooth from shifting is a partial denture or an orthodontic retainer. A bridge or a dental implant is your best long term solution.

      Hi Chandra. Great question. Generally speaking, partial dentures and dental implants are mutually exclusive treatments. Basically, people either choose to get dental implants to replace their missing teeth or go for partial dentures instead. After all, a single dental implant costs as much as an entire partial denture, so it doesn't make a whole lot of sense combining these two treatments. Now, what if you already have a dental implant and want to get a partial denture to replace several more teeth. Could this be done? The answer is yes. You can use a dental implant in conjunction with natural teeth to stabilize the partial. The important thing here is to make sure that the implant does not have an active (tight) clasp. Dental implants are immobile object, whereas dentures move. This means that your dentist needs to be careful to place a passive clasp on the dental implant. The other option is adding a dental implant with an attachment to help with partial denture retention. Hope this answers your question and good luck with your treatment.

      Hi Africo. Sorry to hear about your issues. If you can't chew properly with your partials then you may require a better, sturdier type of partial denture. Cast metal partials are best for chewing and I recommend that you go with them. The other reason you may be struggling with chewing is poor teeth relationship. Your denture teeth need to contact your opposing teeth correctly. If there is a gap, or the teeth are not set correctly, this will affect your chewing ability. Hopefully, by getting a nice denture, with proper teeth relationship, you will find yourself able to chew again.

    Can you tell me the feasibility of a 3 bridge implant crown over 31 & 29 as well as 18 & 20 with a partial denture spanning the rest against an existing upper denture to increase bite force as well as general comfort with the mandible?

      Hi Tom. Very interesting question. Your proposed treatment can be done theoretically, however, it is rarely performed in real life. Let me tell you why... Making a pair of implant bridges is an extensive process, and also quite expensive. You'd have to place four implants and make two sets of bridges. To do this, plus make a partial denture to fill in the remaining gap, does not make much sense to me. If you're going to go through that much hassle, why not connect these two bridges and convert it into a full lower bridge (non-removable)? The front 6 teeth can easily be supported by the four implants in the back. This way, you pay for the 6 additional pontics (the gap teeth on the bridge) and receive a full set of teeth instead of two bridges combined with a partial denture (FYI, the implant positions would have to move up 1 to 2 teeth for this treatment). If on the other hand, this ends up being too costly, then just go with snap-on dentures instead. It's a much simpler treatment that gives you dentures that don't move as much for a fraction of the price. I always think patients should either spend the big bucks and go for permanent teeth, or spend a little bit to secure their dentures using snap-on implants. Combining the two treatments doesn't make much sense. I hope this helps answer your question. Feel free to post your X-rays if you'd like a more detailed answer. Best of luck!

      @@johnjazayeri Really appreciate your feed back John. This idea came out of frustration with acrylic dentures and the absolutely pathetic bite force with them. The theory was to have 6 decent molars to at least enjoy basic realistic mastication. I just assumed snap in dentures wouldn't afford any more bite force than acrylic dentures and would be just as absurdly bulky as acrylics. The idea of a clasping cobalt or titanium denture filling in the rest was because of their comfortable light framework over the gum tissue which is so easily bruised from acrylics. I sincerely appreciate your frank and honest response to this dilemma most people with basic dentures are facing. If I didn't live so far away, I'd already be at your office door :-)

    Had the most basic flipper for a single front tooth going on 10 years and from a Well experienced flipper patient I can COULD NOT DISAGREE more with what he is saying. basic flippers last years not a month also a
    WARNING TO NEW FLIPPER PATIENTS less is best! they always made my flipper way to big it doesnt not need to wrap around all of your teeth to stay in place its made correctly. Also Make sure you feel the flipper touching evenly throughout your mouth and speak up if it doesnt. unrelated but this guy talk like everyone who has a flipper is just constantly loosing teeth.

      You are right, some people do very well with their partial dentures. Glad you're one of them! Some patients do well with just a basic set and don't feel the need to upgrade. Typically, the labs make dentures that cover the whole mouth. If you don't like it, speak up, like you said. Ask your dentist to cut the denture and make it smaller. Now, as far as the fact that everyone loses or breaks their dentures, that is obviously not true. But it does happen. The more you care for your partial dentures, and your remaining teeth, the longer it will last you. I've had patients with a single tooth flipper that's lasted them 30 years. No redos, no problems. They might look a bit old and stained, but that can be fixed by getting them professionally cleaned. You can always opt to go for a basic flipper and see how you fare with them. If it works, great. If not, consider upgrading your partials, or go for a bridge or dental implant instead. Thanks again for sharing your experience.

      Have had my one tooth flipper for around ten years also..soon to get a four tooth partial on top .goodby one tooth flipper..and a full denture on bottom hopefully can keep my molars to make it more stable and better chewing.

    Hello Dr Jay, I have a unique situation. Five years ago, the dentist I went to was suppose to give me partials on the top and bottom. So at the time the teeth on the bottom front (6) were left in and she pulled all the rest. On the top, instead of making a partial, she pulled all my teeth on the top. Making me have to get a full denture for the top. I was devastated. Since that time, I have never ever gotten used to the top. She pulled 18 teeth that day, causing me immense pain, not to mention my pride. Can anchors be put in so that I can get a partial on the top. The full denture is awful, I was unaware she was going to pull all my teeth on the bottom, that was not the plan. I can not afford implants. But a partial would be covered by ins. Can anchors be put in to hold the partial in place . Thank you Pamela Betterley

      Hi Pamela. Sorry to hear about your difficulties. There's no point in discussing whether or not you could have saved more teeth, since the teeth have been removed. There is no option other than a full denture for the top. You can add a few implants to make them tighter, but you'll have to pay for the treatment. It's usually a few thousand dollars. I would save up and shop around to find a dentist who specializes in implants and offers a good price. You may be able to use your existing denture, or you might need a new one. It all depends.       Hi Dan. I think a Valplast partial denture is a pretty good option for this case. You will have pink clasps which aren’t as visible. The partial will look good, be tight, and hopefully you can chew well with them. As long as your remaining teeth are stable and strong, this is probably the second best option, after dental implants of course. Best of luck!

      @@johnjazayeri I got used to my Valplast now. You can eat anything with it. Really amazing. But it Takes about 5 Till you chew with Maximum force. Now I will not get implants. IT would mean 1 year of suffering due to augmentation etc. And the Risks I am Not willing to Take about implants. Valplast feels Like Natural teeth.

      @@timcarr6401 weeks. That's normal. You also need to have to wait abot 6 months after getting your implant. Most people stop using Valplast because it hurts at the beginning. But I promise. After a few weeks you can chew anything like bones, chips whatever. You don't need to hold back at all. Having it for almost 2 years now.

      Hi there. The palate is the portion that covers the roof of the mouth (when discussing upper teeth of course). Yes, partial dentures cover the palate. You can ask your dentist to remove some of the material to make it more comfortable. However, without this portion, the denture will be too loose and fragile.

    Hi Doctor..great video. I had an accident, and the two teeth next to the two front teeth were knocked out. They y are crowns and are in a jar . I want a Maryland Bridge using these two crowns which will fit perfectly. I know they may only last a couple of years, but I could save for implants or just reinstall them. My dentist does not like the idea. I will not even use them for serious eating/chewing. And can I just have a little porcelain added to the top and rounded out to fit nicely. And will that keep the real tooth material in the crown OK. I would really appreciate your comments.

      Hi Mary. Sorry to hear about your accident! Unfortunately, I would have to side with your dentist here. First off, when crowns break off from your teeth like that, you can't really reuse them. Also, you can't convert existing crowns into a bridge. Separate crowns can not simply be glued to one another to create a bridge. They have to be made as one piece from the beginning. Of course, if you like the way the crowns look, you can always send them to the lab to use as a reference for shape and color. However, the lab can't covert separate crowns into a bridge or a Maryland bridge.
      This brings me to your next idea, which is a temporary Maryland bridge for a few years until you get dental implants. Again, I don't think this is a good idea. First off, Maryland bridges work fine when you have one missing upper tooth or 1, 2 or 3 missing lower front teeth. I'm guessing you are missing two upper front teeth (since those are the ones that usually get knocked out during accidents). To make a Maryland bridge, you'd have to make two crowns connected to a wing on each side. However, this usually is not enough to hold the front two teeth in place and you really need to add a second wing on each side to be able to glue them in. So basically you will need two wings, two crowns, and then another two more wings on the other side. The problem becomes the cost. This thing will cost you a fortune, almost as much as one implant! And the whole point of a "temporary" is to be simple and cost-effective, and this is neither.
      I think you need to do a bit more research and choose one treatment and just go with it. Here are my two suggestions for you:
      Option 1: Bite the bullet and come up with a way to do the dental implants. You may have to shop around and find another dentist who is more versed in dental implants.
      Option 2: Remove the teeth, bone graft the region, and get your partial denture to replace these missing teeth. Then, in 3, 5, 10 years, etc. whenever you are ready, do the dental implants.
      I frankly see the Maryland bridge as a bit of a waste of time and money. Especially if your dentist does not feel that it is strong enough to work over the long term. The last thing you want to happen is to spend a few thousand and then keep going to your dentist over and over to re-glue the bridge back in your mouth! I hope this helps shed some light on your situation. Feel free to post your X-rays (Panoramic preferred) in the comments so that I can take a closer look at your situation and see if there are any other solutions. Oh, and a final little tip for you: There is one way to use your existing crowns as a temporary. You would have to place them in their original position (using wax or glue), take a putty impression, make a set of clear aligners (like the Invisalign ones) and then stick the crowns back in there and use them as temporaries. This is not common practice, but it is something that works as a fairly simple and effective temporary option for a year or two. Good luck with your treatment and let me know how it goes!

    Should a flexible partial denture strongly affect speech because of the palate and how would a dentist fix that? Would the dentist cut it back or thin it out? I’m afraid if the dentist were to cut it back he will leave it jagged and rough like he did the clasp. Even after me explain to him that it was jagged and rough, I was still sent home with a jagged and rough clasp. What should I do? I can’t afford another partial.

      Hi Kayleen. Sorry about your headaches! It is very difficult to adjust a flexible denture once it has been processed. Having said that, the palate is much easier to trim as compared to the claps. I would ask your dentist to adjust the palate and see if he or she can smooth it out afterward. Most dentists have a wheel and pumice which they use to polish dentures. You can also ask your dentist to send your denture back to the lab to get it polished. Most labs do this complimentary for a new denture. If your dentist refuses, you could probably find some DIY methods to smooth it out yourself. Hopefully, that won't be the case. Good luck!

      @@johnjazayeri yea, no. The dentist offered to take my partial, throw it away and give me a refund, not in a caring manner either. I thought maybe the dentist would mark the problem areas, write a verbal statement of what needed to be done to the partial and send it to the lab to have it adjusted or remade to fit properly and look better. I don’t think this guy likes being a dentist to much because he doesn’t seem to have his heart fully in his work.

      Hi TenRen. You can use clear aligners (Invisalign/ Smile Direct Club) to close gaps between the front teeth. However, closing gaps between the front teeth without addressing the missing back teeth is not a good idea. Your front teeth will shift back into these gaps within a few months or years after finishing your treatment. I suggested you make a plan to replace your back teeth as soon as you straighten your front teeth. Either wear a partial denture, or better yet, place dental implants to permanently close the gap.       Hi Pauline. Thanks for sharing your story. Tori, which are large bone pieces that some people have in their mouth, could interfere with denture design. For full (complete) dentures, the tori usually needs to be removed. For partial dentures, your dentist may have to work the denture design around the tori. In your case, your dentist recommended a metal partial to design the false teeth to have less contact with the tori. Glad that there are dentists out there that pay attention! How is the partial working out for you?

      Hi Myiesha. You certainly can get permanent dentures (we don't call these dentures anymore, we call them permanent teeth). There are two options:

      Both of these options give you permanent teeth that are connected to dental implants and stay in your mouth 24/7. Keep in mind, these are very expensive treatments, and take a long time from start to finish. Let me know if you have any other questions.

      Hi there. Ok, so you have ten upper teeth in your top jaw and want a partial denture to chew with. Honestly, I'm not a fan of this option. First off, your partial denture WILL cover some or most of the palate. Also, you only get 1 or 2 additional teeth on each side on the partial. And, since there is no back teeth, the partial will be very loose. I would recommend either getting dental implants or doing nothing at all. I mean 10 upper teeth translates to 2 posterior teeth on each side. You will have nearly the same chewing efficiency with these 10 natural teeth as you would with ten teeth plus a partial denture which has a few molars on it.

    Hello, will the flexible denture restore any face shape? I've got a missing premolar and the two molars on the top right arch that have been missing a while.

      Hi Jack,
      Dentures can restore some facial features, but only to a limited extent. The most important question is whether or not you have a terminal tooth to anchor your partial denture. If you do have a tooth in the back, then the partial denture can anchor on to it. As a result, your partial denture will be quite stable and it can restore some of your facial features. However, if you are missing your last few teeth in the back, then the partial denture will be quite loose as it will not be able to anchor onto anything to stabilize it. As a result, this type of partial denture serves very little function retoring your facial form. Ultimately, partial dentures are better than nothing at restoring your facial features, however, they are nowhere as good as dental implants are. Hope that helps answer your question.     hmmm interesting video. some of us can't afford implants. I know I can't. I don't have 50 grand for my teeth. I already have 4 bridges and I just can't start paying for implants. I am looking to replace 4 teeth with partial denture. this was a lot of good information. I will have to make it work.

    I have to have partials and I'm terrified. My dentist told me "I don't want you to be overly excited about getting partials but it will help keep your mouth from shifting" my self esteem has been crap my whole life. I've had teeth actually come in "de a d" I have awful gray looking front top teeth but they won't be getting touched. I have one thats just a stump. A tooth has broken (#18) and whats left is also loose. I had a criwn when i was a kid and i lost it decades ago. So it started crumbling and now theres a hole and a stump bc most of the tooth is now gone. I have free ins through my state so they arent going to do anything to make you look or feel better and they are 100% in favor or pulling teeth vs filling or root canals.

      Hi Jorge. It’s quite rare to be allergic to dentures. Most people who can’t handle their dentures are more so intolerant of their false teeth as opposed to truly allergic. Still, there is a small percentage that is actually allergic to the metal found in partials. There is always the option to use plastic material, like flexible dentures, for these situations.

    I’m absolutely panicking. I’m picking up my partial tomorrow and never knew they would damage the teeth I have left. I hate myself for getting them now. Does it always damaged the remaining teeth? Surely they wouldn’t do it if it was that bad! Do I just have to make sure I clean them more thoroughly? I thought having the partial would take off some of the pressure my other teeth are experiencing? I am always chewing on one side and front teeth and they were wearing out. I wasn’t able to get implants.

      Hi Paige. Please don't panic, the point of the video is to teach patients about the pros and cons of different dental treatments. You should know what can go wrong, so you take steps to prevent it. Let me address your concerns one at a time:
      Do partial dentures always damage your remaining teeth? It depends on how you take care of your teeth. The teeth which are used to clasp the partials are the most vulnerable. You should remove your partials overnight to give these teeth time to rest. You should also brush and floss these anchoring teeth very thoroughly. Finally, be sure to go for your regular checkups (3, 4, 6 months, etc.) so your dentist can catch any potential cavities before they get too big. Follow my instructions and you have very little to worry about. I have had patients who have maintained their partial dentues without any issues for decades. It all comes down to how you take care of your remaining teeth and your partial denture.
      Do partials take some of the bite pressure off of your remaining teeth? Sure they do. As long as you have a proper bite, some bite forces are transferred to your partial denture. This only happens if the bite is calibrated and you wear your partial denture consistently throughout the day.
      Try to avoid chewing on one side if you can. It's always best to chew evenly on both sides. This protects your partials as well as your natural remaining teeth. Don't forget, getting used to your first partial is like riding a bicycle and takes some time and practice. Give yourself a month or two to get accustomed to your new teeth. Good luck with your treatment and let me know how it goes.

    The dentist office told me my insurance refused to cover a partial because the root is still in the gum. I don't want the root removed as it is not infected (verified) and not causing any problems. But, they wouldn't cover it (supposedly).

    I need 8-10 crowns all my back teeth… I wonder if it’d be better to get implants supported dentures long term. I’m worried I’ll spend a fortune on crowns that end Up failing. I’m only 37.

      Hi Millie. Sorry to hear about your dilemma. I'd love to see your X-rays, but let me try to guide you in the right direction:
      Should you spend a small fortune on 8 to 10 crowns? Or should you remove these teeth and wear a partial denture?
      The chewing difference between natural teeth (with or without crowns) and partial dentures is night and day. Dentures are bulky, they cut your gums, the get loose, they only chew at 40% chewing efficiency, etc. Do not even consider removing so many teeth and getting dentures. Especially if your are only 37. I'm not even discussing the impact of bone loss losing so many teeth at a young age, facial collapse, teeth shifting, etc. Get the crowns! Just make sure they are perfect crowns, seal the teeth well, and fall in line with your current bite relationship (aka, go to an excellent dentist)
      Will placing 8 to 10 crowns cost more down the line? Of course. Nothing is guaranteed. Let me tell you what will happen IF you go to a great dentist. you will get 8 to 10 crowns. One of them may break or have to be redone within the first year. Fine, they should redo it at no cost. Over the next 10 years one of the crowns may fail. You may need a root canal or a dental implant. But you will get the enjoyment of chewing comfortably for years to come! Now, let me tell you what happens if you get shoddy work... 5 to 6 crowns will fail. The failing crowns will damage neighboring teeth. Eventually, most of the teeth will fail and you will need partial dentures.
      So what should you do? Find the best dentist in your neighborhood, not the cheapest, and get your teeth fixed (doesn't have to be a fancy office, just a skilled and experienced and compassionate dentist). It will cost you less in the long run and you will get the best long term results. Good luck!

I do have a good dentist I trust (just wanted another opinion), I got one crown back in 2020 thats been working great for me and my dentist is keeping an eye on all the large fillings and not pressuring me to get work done before I need it.       @@johnjazayeri That's rather sobering information about chewing efficiency, the cutting of gums etc. I will have a premolar extraction in Feb, and want to get an Zirlux Acetal resin-based Nesbit. It isn't metal but acts like it supposedly. Are you familiar with them?

      Hi Reena. It's always best to remove your dentures at nights. This allows your remaining teeth and gums to get some rest. It also extends the longevity of your dentures. Patients who never remove their dentures have very inflamed gum tissue and damage their dentures at an accelerated rate. Dentures are always designed to go in and out. If you want something that doesn't come in and out, then you need to look into dental implants instead. Hope this helps and best of luck!

    Brother has lost 5 lower front teeth, he hates implants feels that they can damage later, he asks does if using partial denture without metal for 2 hrs a day be good for longer run, i mean he doesnt need to put for more than 2 hrs a day so he asks , without metal is coz you also said that the teeth can get destroyed right ? how to ask dentist to make the denture if not metal used to keep it in place

      Hi. I think your brother needs to do a bit more research. First off, implants are not going to damage anything. Secondly, why does he want a partial denture for just 2 hours a day? The point of a partial denture is to use them daily to chew, speak, and hold your remaining teeth in place. Using them for 2 hours a day seems a bit unusual. As such, I suggest that he gets a flipper, an acrylic partial denture in other words. These are the easiest to make, and the least expensive as well. In the future, he can decide to upgrade his partial denture or get implants once he is ready for them.

      @@johnjazayeri thanks for reply, dr said clearly that implant can get bone loss again if not maintained , to do this implant he has to do graft it seems, as bone loss is high in his lower front side, 2nd he wanted to know how are implants removed later on if they get damaged , again drill has to be done and again some more holes in mouth on the bone sir ? he seems worried on the failure part. as he had gum disease solved with lanap for now, can i mail you the pics of his current state and you could pls guide on that , thanks

      Hi Jorge. Allergies to dental materials are quite rare. Most people who are allergic are actually reacting to some sort of metal. Luckily, there are plenty of metal substitutes offered by most dentists. For instance, you can get white composite fillings instead of metal based silver fillings. Instead of a traditional metal fused to porcelain, you could get a ceramic crown. There is even an alternative to traditional dental implants, which are made from titanium, and you can get them from zirconium.

      @@johnjazayeri I got a zirconia onlay and they use glass ionomer for the attachment. Instantly I got a serious irritation. I waited week to take the decision to remove it. Now the molar is so mutilated that it may need a root canal. I’m afraid of dental materials and it’s chemical side effects. I’m thinking to pull it and get a partial denture.

      @@jorgevillacis8127 I’m sorry to hear about that. Dental materials aren’t the most organic, and most permanent cements are acidic based which can irritate the tooth. Still, I wouldn’t rush pulling the tooth this quickly. Have you tried putting in a temporary filling material, like IRM, to see if pain goes away? Also, you can try using a temporary cement to see if the tooth is less reactive. I just don’t want you rushing to remove your tooth, as you could regret it later on.

    But is not as simple as that it all adds up extracting, treatment of gums, bone drafts it can add to thousands, maybe the partial is not a lot but all the other things needed

    I just got my permanent partials and my top partial is scraping the back in the front part of my tongue and makes talking drinking and eating so uncomfortable

      Sorry to hear that. Either you need more time to get used to your new partials, or you need to have it adjusted. I suggest you practice using your partials for a week or so, and then schedule a follow-up with your dentist, if you don't already have one. The more specific you are with your complaints, the better your dentist can resolve your problem. Good luck!

      My husband and myself go to low income dental clinic. Google low income dental care. The town you live in. There should be dental clinics to call. If that doesn't work Google your county health Department see if they have dental clinic. Both of those options charge based on income. If those don't work Google see if there are dental colleges in your area. They don't charge much online I found one several hours away from me they charge $25 for cleaning. Or you can be like me. Have really advanced tooth decay. Infection that has already spread to the in your jaw. I am telling through experience you do not want to be sitting where I am. I sat here for 16 years knowing I needed major dental work. Made no effort to find anyone to help me. Now it is way too late. I am looking at lots of tooth loss. Please find someone to help you.

      Hi there. Partial dentures can damage your teeth, especially the anchor teeth. However, if you take care of your teeth and your partials, your teeth may not get damaged. I should say that wearing partials increases the likelihood of your teeth getting damage. It all depends on your hygiene, quality of partials, whether you take them out at nights or not, etc.
