    My Dr put me on Ortho Molecular Motility Pro which seems to have the same ingredients as Motility Activator. I started with am only but then read to take it to tolerance and take one now in the pm and it’s made a difference. Thanks for your videos- I’ve learned so much from you!

    The best motility for dyspepsia/heartburn symptoms is iberogast from my experience. For constipation, motegrity (prucalopride) helps bowel movements.

      @@boop5287 indeed, and it also contains licorice, which is extremely bad for people with arrhythmia (Atrial Fibrillation), as it will cause an episode or two post ingestion, which is why I have never been able to tolerate more than a couple of days on Iberogast....

    I really appreciate iberogast as a post meal treatment- when I eat something and I just leaves me feeling bad. It hasn't worked as a prokinetic for me. But following meals is a life saver.

    I noticed that GI motility complex has a higher amount of the ginger artichoke blend. Cant wait to try! PHGG also helps me a lot- half a scoop in the morning and at night

    Are these recommended for IBS-D, or are prokinetics not appropriate for that condition? My doctor has started me out on Viberzi, but I don't like it, so I'm trying to understand what else might be good?

    I've tried Sibo MMC and I find it hard to sleep after taking it at night. I've developed a serious sensitives to any b vitamins since sibo. I've also tried LDN which gives me insomnia. Any suggestions on a prokinetic that doesn't interfere with sleep?

    What about Enzymedica's product ? I noticed it has ACV in and I worry that would increase the bacteria that cause SIBO. I'm new to this and I don't understand whats wrong with just plain artichoke and ginger extract you can get from the big rainforest site.

    Dr D! Great video! i have heard that artichoke can really get the gallbladder going and if you have gallstones you could get an unwelcomed surprised/blockage. I dont know that i have GB issues but i suspect some sluggishness. have you had any issues with that?

    How long does it take for a prokinetic to work? My doc prescribed Motility Activator by IT and I only take it after breakfast and before dinner. I still am bloated first thing in the morning which I am surprised by. I have noticed after breakfast, I actually feel hungrier for lunch, but I still have bloating. Can you take 1 with each meal and are these supposed to provide instant relief? Thank you!

    Have you had any of your patients try Triphala? It’s been working for me. I haven’t tried the others, so I can’t say if it works better or worse.

    Would this list be more for Sibo D? What about IMO or Sibo-C? Was on prucalopride for a while, it works for C but so $$. Is any of those prokinetic had more success for Sibo-C? Thank you!

      Hi Genevieve! They can work for C, D or mixed! True prokinetics (as opposed to laxatives) don't "stimulate" as much as they modulate or balance and normalize the system. So, for people who are constipated prokinetics facilitate elimination and for people with diarrhea they slow down elimination. Best of luck!

    Hi, im half way through my 8 week course of antimicrobials to treat methane sibo. (Oregano and Allicin) i'm not seeing any difference to my main symtom of bloating. (No pain). Would it be okay to start prokinetics now or should i wait till the oregano and allicin are finished?

    Hello, thank you for the video. I checked all the products you talk about. All of them contain ginger, which I can not use due to acid reflex. I have tried motility activator which only has 20 mg, and mu acid reflux got worse.
    The Acetyl CH is the only one that doesn’t have ginger? This is not a clear supplement. What it really does? What other ginger free motility activators would you recommend? Thank you.

    After doing my own research I ordered Motility Activator (it's been shipped as of now) before I watched any of your videos. My questions are: when should I take the prokinetic for the most benefit? I assume 4 hours after my last meal at night. Also, would taking a prokinetic around a meal time increase transit/stimulate peristalsis in a way that also strengthens the same muscles that are used in the MMC or is it not a good idea in refernce to treating SIBO symptoms? The reason why I ask is I don't like waiting until late at night to take the prokinetic and feel it work, I really want relieve during the day, especially when I get bloated mid morning on an empty stomach. I do my own thing with ginger and artichoke leaf extract while I'm waiting for the supplement and like taking it in the morning before my first meal or between lunch and dinner in order to pass trapped gas that legit just won't get out.

      Update: I saw a NP/MD who specializes in treating SIBO and am officially SIBO free. I take prucalopride as a prokinetic. The one in the video didn't work as well as prucalopride does. And artichoke and ginger made me very ill. So I no longer take any supplements not under guidance of a doctor.

      Yes to ginger but look for artichoke extract at similar level to product. Search online.

    For me, gut motility by Enzimedica did the trick. IMO (Methane Sibo). Stomach ache and brain fog were me main symptoms, and now there is a segnificant improvement, thank god.

    For those who don’t wanna spend an insane amount on a bottle. Motility Activator is just artichoke leaf extract and ginger root extract. Which you could also buy seperately for like 10-15 per bottle, also having way more capsules than 60 for like 60 bucks. It’s pretty much a scam in my opinion, 60 bucks for some simple artichoke and ginger extract.

    I was using the dry extract of ginger + artichoke and the symptoms simply disappeared, however, after two months of use, the prokinetics stopped working, I have been researching other prokinetics, but the natural ones are very difficult to find, do you use any another route of administration when the body is saturated with ginger+artichoke?

    The motility activator has artichoke in it but artichoke is on the high Fodmap.. do you still recommend taking? I’m confused on what I can and can’t have

      Without food, ideally. All of the supplement companies recommend dosing between lunch and dinner and again some time after dinner.

    Thanks so much for the information! It was very helpful! I just ordered a bottle from Amazon. In the past, I've tried 60 drops of iberogast with 1000mg of ginger at bedtime. I went about three months before relapsing. I hope this works better.

      Great question! I'd just get the two ingredients of the "winning" formula (Motility Activator) and take them separately :) that seems to work well! It's just slightly less convenient bc you'll need to take two pills per dose instead of one.

    Thank you for providing such great content! Would the artichoke leaf extract in the Integrative Therapeutics - Motility Activator aggravate Hydrogen Sulfide SIBO? As I understand it, artichokes themselves are high sulfur foods. Perhaps the leaf extract is different. I read that ginger itself can aggravate H2S SIBO. Also, I've been struggling to find a biofilm disruptor for H2S SIBO. Mostly all the formulas contain NAC or Alpha Lipoic Acid. Should those who suspect or have H2S SIBO approach these supplements differently?

      Good questions! You don't have to be nearly so strict avoiding Sulfur, even if you have raging H2S overgrowth (I've worked with many). The plant foods that are "high" in Sulfur pale in comparison compared to red meat and eggs, so 95% of the time I only recommend limiting those foods. Moreover, you only need to restrict red meat and eggs for a couple of weeks max, then you should be able to add them back in.

      Now, the Sulfur based supplements like NAC, glutathione, and ALA are enough to trigger symptoms so I agree with restricting those for a similar amount of time.

      I use a lot of ginger and artichike based prokinetics with my H2S SIBO folk and it has yet to be an issue for anyone 👌

Also, it's heartening to know that there's a possibility of adding things like eggs and red meat back into my diet. Woo hoo. Should I eat those things during antimicrobial treatment and abstain for two weeks after antimicrobial treatment? Thank you!

      did you ever have neuropathy or muscle wasting with your H2S sibo? i've read from places the gas is neurotoxic and shuts down mitochondria, but the information and case studies are thin and vague.

    Silver Fern brand “Upper G.I.” has three times as much artichoke leaf extract and 50% more ginger extract than motility activator. Is it ok to take more than the recommendation of the bottle if it doesn’t seem to be working? Or just move on? What is the best time of day or timing around meals for taking something like this?

      @@BT_life2 yeah, FMT is more for gut dysbiosis rather than something like SIBO. It’s basically a more effective form of probiotic I think. Before I try it though, I found an ultra high potency probiotic that is prescribed for gut health issues called Visbiome. It has CFU counts as high as the 900 billion range depending what you order from them

      Basically, prokinetics regulate motility. Poor motility is the cornerstone of IBS and SIBO, so they are often damn miraculous for both conditions :)

      I'm not a prescriber, so I've never prescribed it personally. I've had some patients use it for motility though (prescribed by someone else). A few people swear by it! I've had many more people like Motegrity or the herbals, though.

    Thank you for this video! Your videos are very helpful, I love the analytical comparisons of treatment options and supplements. Your sharing data of what has worked & not worked for many of your patients is invaluable to those of us struggling on our own, trying to dial back our sibo, leaky gut, motility problems and auto-immune disease. One suggestion: Please consider saying “I’ve included the info in a link below” instead of “in the doobly-doo down below”. This is is so distracting in the middle of the video and frankly sounds nonsensical.

      Ideally between meals (between lunch and dinner and again around bedtime). If you forget doses or find it hard to do you can try a double dose at bedtime or try it with meals though!

    Thank you so much for your honesty with this information - you are a breath of fresh air!! Quick question... I am just coming off the ED and am looking for a prokinetic. In looking into Motility Activator specifically, it contains artichoke leaf extract. I am extremely fructose intolerant, do you think I could still take this?

      Hi Karen! I just checked my Monash U FODMAP app and it looks like artichoke does not contain excess fructose, just fructan. Then if you go down to a small enough serving (they list 15 g which is WAY more than those puney capsules can hold) it is also low in fructan. Best of luck!

      Well, the numbers don't lie! It is very helpful for many of my patients. That being said, not everybody loves it, which is why I will continue to do the "prokinetic speed dating" with my patients. Just today a patient told me that MA made her MORE constipated! This is very atypical, though.

    Would it be safe to try any of these remedies for SIBO if you haven’t been tested for SIBO? but have all the symptoms. I do have a form of IBS and have been suffering for years with stomach problems .

      Very meh. Some people like it, some say it does nothing for them. Amongst people with methane it seems like a 50/50 crap-shoot.

    My breath test showed IMO, so that's the context I'm coming from:
    1. Motility Activator: It has artichokes which are high-FODMAP... Won't this cause nasty symptoms with every dose?
    2. Prokine: Chamomile is also high FODMAP; I can't drink chamomile tea, so I question if high-fodmap-sufferers like me could take this supplement without experiencing the same consequences as the tea?
    Cinnamon: This is very acidic. Acid feeds IMO considerably. (I was overloaded with internal toxins/archaea byproducts until I started Omeprazole and had my stomach acid reduced. Before the Omeprazole, I was a mess - liver was overloaded making me yellow/jaundiced, incredibly severe fatigue, brain fog, dermatological issues, and on and on. The Omeprazole was a savior; not the solution, but it's getting me through until I can convince the gut-oblivious powers-that-be to give me the proper treatment.) The only time cinnamon is not acidic, and starts to approach neutral, is when it's boiled like crazy.
    Peppermint: Not good for heartburn and GERD (which develops because of the IMO).
    3. AcetylCH: This is called "Acetyl-L-Carnitine" in your online shop?
    I think it's worth mentioning the pharmaceutical prokinetics too.
    In my experience, herbals have only done me dirty. I tried oregano oil - made my symptoms worse. Probiotics - made symptoms worse. On and on. So I am feeling done with herbals.

      Agree on herbals and pharma drugs (often made by similar cos btw). Many can’t afford access to an MD and or prescription (esp in US.) In Canada only one real motility drug and specialist doesn’t like potential neg effects. Diff situations and microbiomes. Do what works best and right for you. Per Monash University artichoke hearts low at 1/2 cup. Also older study found artichoke leaf extract daily for 6 wks helped eased IBS symptoms. Also 96% in study rated extract as effective as - or better than - antidiarrheals and laxatives. For any who do try extract can do 1/2 dose or 1 pill. Also just artichoke extract at very low dose and if ok try motility product advised in video.

      You may be experiencing herxheimer reaction (the "die-off" of nasty bugs in your gut). Long term use of Omeprazole made my symptoms worse, not better unfortunately....

      @@jesusmerchanreina5069 ​In my experience, so many people make their symptoms worse with random herbal supplements (I did too) and then attribute it to a herx reaction (also guilty, when I was in month 1 of all of this - I'm on year 2 now). Personally, I think it's rarely a herx reaction. There is a very different feeling between symptoms worsening and a herx reaction. And herx reaction only happens if you kill off the bugs with something super potent and they die off extremely quickly. That's rare with SIBO and IMO - it's so hard to get rid of these bugs. It takes antibiotics (RX or otherwise), and often multiple rounds. And even still, each round is 14-days and reduced the overgrowth by I think 20ppm (on breath test) per round, so it's not fast. (Don't quote me on the 20 number, I tried to look it up again right now, but I learned this in one of Dr. Mark Pimentel's research report videos on TH-cam, so it's not easy to find again, and not in my notes. But he said that each round of antiboiticals typically reduced gas levels by ## ppm per 14-day round. And that's why some people, with very high gas levels to begin with, need several rounds.)

Some thing like NAC can really screw up with the lining of your intestines (happened to me over 4 years of taking it) but Herxheimer reactions happen more times than people realise and they think it is a supplement causing it when it is actually the die off. Different bugs have different die off rates, some die easily, others don't. If you take allicin for a few days, you WILL kill off a ton of bugs (especially candida ones). But yeah, this would all be so much easier if western medicine took SIBO and its natural treatments a bit more seriously... my two cents. Thanks again for your clarification Eliz, much appreciated!

      I have used it, yes! It is neither a probiotic or a prokinetic though. It is a short chain fatty acid supplement. Of course, SCFAs are wonderfully anti-inflammatory, so that may result in a motility promoting/normalizing effect for some 😉
