    I think you made a mistake on staging HL. Both side of diaphragm in stage 3 is above and below, not left and right based on spine.

    I was diagnosed with HL last week. I’ve watched this 3 times, and it is so well done! I have a really good handle on what’s happening in my body thanks to this. Really educational stuff

      I was also diagnosed with this type of cancer during pandemic. Thankfully, I've fully recovered. Hope for your fast recovery!

      @@Miyanazuki so far so good. 1 cycle of chemo so far with one to go. My neck lymph nodes are completely shrank down to normal looking at this point. I’m feeling good

      @@junefreeman7955 so essentially it was entirely chemo since radiotherapy was too dangerous long term due to the locations of my cancer, i initially had 2 courses of ABVD with no unexpected side effects, a pet scan after those courses showed that only a small amount of cancer remained and in turn i had 2 courses of BEACOPP with a PICC insertion, thankfully after that i was cancer free, fortunately i was just young enough to be in a children’s hospital (the food is immaculate)

    Thought very necessary to add that the Ann Arbor staging got muddled along the way viz:
    * ' On either side of the diaphragm ' refers to grom chest upwards and abdomen below as opposed to on either side of the chest.
    "A single extra nodal involvement is still stage 1 in the absence of a lymphadenopathy .

    Great video. The description of stage III CHL was stated correctly, but I think the drawing for stage III is slightly incorrect. While it includes both sides of the mid-line (which is shown in the diagram), it also includes both sides of the diaphragm, which was clearly stated, but not draw on the diagram.

    This is probably the most advanced lecture i saw about Hodgkings. I really love the way you lecture,and with all the stuff writen and showned. I just wanted to tell you that you should add on this video about 5 minutes about radiation therapy in treating Hodgins. I am medical physicist and we radiate lot of patient with Hodgins, cervical , clavicular, axilary, mediastinum, and nodes bellow diaphragm. Keep doing i really loved your work

    Dear zach please rectify ann harbor staging , if H.L is present on both sides of diaphragm above and below not side by side it is stage 3 and if on same side it will be either stage one or two

      That's not quite right. Just the neck (one side or both) is stage 1. Neck and chest is stage 2. Neck, chest and abdominal area is stage 3. Neck, chest, abdomin and groin is stage 4. Speaking as a stage 3 HL survivour

    I don't ever drink, and the main reason is that for a few years I experienced pain in the back of my head everytime I drank alcohol. Thanks to this video I now know why. Very common in people with HL

    I am very grateful for all your amazing videos. They are of a great help.
    But Regarding the Staging of Hodgkins lymphoma, it is determined according to the presence of lymphadenopathy above or below the diaphragm. not the right or the left sides of the body
    Stage I: Either of the following means that the HL is stage I:
    HL is found in only 1 lymph node area or lymphoid organ such as the thymus (I).
    The cancer is found only in 1 part of 1 organ outside the lymph system (IE).
    Stage II: Either of the following means that the HL is stage II:
    HL is found in 2 or more lymph node areas on the same side of (above or below) the diaphragm, which is the thin muscle beneath the lungs that separates the chest and abdomen (II).
    The cancer extends locally from one lymph node area into a nearby organ (IIE).
    Stage III: Either of the following means that the HL is stage III:
    HL is found in lymph node areas on both sides of (above and below) the diaphragm (III).
    HL is in lymph nodes above the diaphragm and in the spleen.
    Stage IV: HL has spread widely into at least one organ outside of the lymph system, such as the liver, bone marrow, or lungs.

    Great video, I was diagnosed with classical hodgkin lymphoma stage 2b.
    Had 2x cycles of abvd but moved onto escalated beacopp-dac.
    Had 4 cycles and got me into remission, but now having radiotherapy to the mediastinal.
    Currently 6 sessions done and 9 left. Thanks for the video 😊

      @rociolahere6637 yes thanks i have completed all the treatments, and now I will be seen every 3 months for checkups and a blood test. It's been a hard 9 months, that's for sure.

    My cousin has just been diagnosed with T cell rich B cell lymphoma
    She originally went to her Dr 15 months ago with Autoimmune hemolytic anemia that was treated with Rituxinab and Prednizone.
    After getting her Hemoglobin up within a normal range they attempted a sample from the bone marrow with no success and decided to wait and see how her response was.
    About two months later anemia was back so they added a drug used in transplant patients along with more Rituxan infusions. They made note her spleen was enlarged but thought possibly due to Covid. ( she did contract Covid two weeks prior to this second bout with anemia)
    She just continued to improve a bit only to have her hemoglobin dip, seemingly faster each time.
    About 6 weeks ago she collapsed, they put her in the hospital and decided her spleen has to go. It was 5-7 times the normal size loaded with cancer.
    She became very confused. Very anemic and on top of that they found hypercalcenia.
    Her result of Bone marrow sample said 95% cancer. Not sure what they mean by that.
    They’ve recently said she has T-cell rich Bcell lymphoma.
    Help!! I’m guessing this is really bad!

    50:44 i thought same side meant lymph nodes were grouped together superiorly vs inferiorly (as in the cutoff is the horizontal line that demarcates above vs below diaphragm) not left vs right (hemidiaphragm) can somone confirm?

      yes, the classification shown in the video is wrong. If you do a google search of ann arbor classification (Lugano's revision) you'll find the right one.

    In 10:31, shouldn't the B-cells be the ones that express the antigen on their MHC class 2 molecules, not the T-cell? And that the B-cell activate T-cells, and let them produce specific cytokines, such as IL-4 and IL-5, which will induce the class switching?

      Correct. Good lecture, but I think the Ann Arbor staging was mistaken here. It’s based on whether the LNs or above/below the diaphragm, not based on which side of the body (left vs right).

    hey great video as always, i am a huge fan, depending on you in my medical years, but in the ABVD treatment the V stands for Vinblastine not Vincristine, the latter is better with leukemias
