- www.footankle.... The 2nd toe is the toe most prone to pain and injury. The reason for this is that it has the most potential to have abnormal pressure applied to it. The big first metatarsal bone should take up most of the weight of the ball of the foot but in a number of situations it is pushed out of the way and the second metatarsal bone has to take that extra pressure. This can lead to pain under the second toe and in the second toe.
Often the 2nd toe is also the longest toe so it can get injured by the shoe more than the other toes. Just some of the problems associated with the 2nd toe include capsulitis of the second metatarsal joint, fracture of the second toe, thick 2nd toenail and tears of the plantar plate of the 2nd toe.
