Here’s the link to the deep dive!: How to Use Topical Estrogen Cream For Aging Skin | Menopause + Skin | Dr. Sam Ellis th-cam.com/video/7sqtzYw-vcQ/w-d-xo.html
progestine is not natural, so progesterone should be taken; as estrodiol is the strongest of 3 possible forms of estrogen, it might be better to take estriol instead
Dr. Ellis can I apply Retinol after applying Topical estrogen cream on my face. How long does estrogen take to absorb in the Skin? What are your recommendations regarding using retinol and topical estrogen cream as a night time routine?
Thank you for this important video! I am almost 74. I was post menopause by age 42. I was on HRT for probably 15 years. I decided I had been on it long enough as I was reading all the misinformation and health scares at the time and was worried about my overall health. After I went off of it is when I ended up with osteoporosis, Afib, a debilitating rotator cuff injury, arthritis and fainting spells. And of course vaginal issues. I really wanted to go back on HRT but my GYN told me that because I had gone off of it, because of my age I couldn't go back on it. I am very interested in starting on topical estrogen for my face and hands and have high hopes that my GYN will OK a prescription for me. I just thought someone would be interested in my HRT journey. It was a blessing and kept me young and I shouldn't have gone off of it.
I’m 76 and was recently re-started on HRT by a functional medicine GYN. I was off of it for 4-5 years after losing access to a practitioner, then the pandemic. I’m now on compounded bio identical estrogen, oral progesterone and several nutritional supports (for things like low B-12). This type of HRT has been recognized as safe and beneficial to protect cardiac function, muscle mass and bone integrity, as well as restore better cognitive function, sleep and more. I had to find, and pay, this person privately and will be looking in the future for someone who accepts insurance to continue this protocol. Wishing you good health - the traditional alternatives are miserable… 😊
I was on HRT from 45-52 and got off due to my sister getting breast cancer and her telling me I shouldn't be on it! I felt like I was aging too fast, arthritis, joint pain, depression, weight gain, just overall felt bad. So I just started again at 62. The research on breast cancer was flawed, although I think there was a slight risk with oral estrogen (I use bioidentical cream now and a progestrerone pill). To me it's better for my overall health, and I want to try and avoid the dementia my mom has. Your brain needs estrogen or it shrinks! I go to Dr. Dian Ginsburg - she is a virtual Ob/gyn who only does menopause medicine now. You should check her out. Luckily there are many ob/gyn's switching to this practice so we can get better information. Many I follow say you should be on estrogen for life and I believe them! I will continue to have my yearly mamogram and check for breast changes of course. Dr. Ginsberg said put a little of the cream under your eyes and on your crows feet, forehead wrinkles and hairline. Then half is vaginal and half is on alternating thighs.
I am 84yrs of age I had breast cancer aged 34. I was on HRT for 20 years during menopause and after until my Doctor refused to prescribe it in my 70's since then things went downhill fast mood changes, irritability, dry skin. My belief is women should be able to have HRT for as long as they need it.
As a gynaecologist, I am so appreciative of the information you provided to your viewers! It’s accurate and up-to-date and I am so impressed! Thank you for your dedication in creating these opportunities for medical knowledge deep dives!
You are not. You can’t even spell it. Additionally if you were, you would know by now that estrogen replacement doesn’t cause breast cancer, it is protective
I am so glad mature women's health is finally being addressed. It's IMPORTANT and there should be no mystery to our bodies as we go through life. It will make people less scared overall and enrich lives. Its amazing to be 20 years old, but it's a stage of life and all stages are worthy!
I"m 65 and use topical estrogen. I've been using tretinoin since I have been 42 and had a deep C02 done when I was 59, and that really helped a lot. When I turned 64, I didn't recognize the person in the mirror anymore. Topical estrogen has helped. I would recommend it. Thanks for address this. Very few dermatologists address this problem. We all get there some day. My mom passed away at 93 and she was on hormone therapy and looked very good in her senior years.
How do you use it? because I have tried mixing my Retin A & Estradiol cream for my face & neck to firm and where I may see some moisturization I have not seen any firming! How do you apply it?
I started topical estrogen about 2 months ago and have felt like im back to being more like myself! Its amazing and i had no idea how much better I'd feel! Im 67 and this stuff is wonderful!!!
You are the most professional and trustworthy dermatologist in the internet. Informing people about what really works in dermatology instead of just selling products for profit. Thanks !
I’m a 45-year old, medium fair-skinned Asian woman. For the last three months, I’ve used Musely’s Aging Repair Cream for my face in the morning ( 0.3% estriol and 1% vitamin C) and the Estrogen Boost in the evening (0.4% estriol, 0.1% estradiol, 20% progesterone; note the progesterone) and had fantastic outcomes so far. Overall, my face and body looked de-aged 10 years. The most dramatic change I’ve ever seen tbh, and I already had fairly good skin (like most of us here, I have a regular routine of 1% retinol, vitamin C, sunscreen, BHA/AHA exfoliant). The loss of elasticity, plumpness, and “bounce” of my skin was something I wanted to keep (or at least have high standards regarding) and was looking for something to address that. IMHO, it’s not wrinkles per se that age us but skin texture and tone changes (AKA crepey-ness and age spots). Because the creams are a hormonal treatment and I’m already in my mid-forties heading into perimenopause, I had the extra benefits of having more energy and sex drive than I had since my early 30s. Wowowow muscle tone improvement overall. The brain fog was real and much improved too. I sleep so much better. All cherries on top! I definitely say: give topical estrogen a shot. The dosing is low and I experienced very mild side effects (initial site irritation and mild breast tenderness that eventually dissipated; so far, no pigmentation issues either). Thank you truly to Dr. Ellis for keeping this important conversation going. We don't have to suffer these mid-life changes!
@genevievedeguzman9293 This is concerning! You’re only applying the hormonal cream to your face? and yet you’re seeing improvements in your body muscle tone, a greater sex drive, sleeping better and less brain fog(?). This means ‘it is systemic’ and not just local. Have you discussed all the other benefits with your prescriber? And if so, what did they say?
@@Tica5012it’s the estrogen boost cream with the estrodial and progesterone that’s giving her all those additional benefits. She likely uses that one on the arm or belly (maybe her face) and it does have systemic effects because of the higher blend of hormones. The aging repair cream that she uses on her face in the morning (I use this one as well) is really localized because it’s a much lower amount of estriol and doesn’t have the other hormones. Just my take on her comment being a long time musely customer.
I’m a topical estruses for both vaginally and on my face. It has helped on both areas with no negative side effects. I’m 70 and plan to use it forever.
I had mentioned to my Optometrist that I heard estrogen maybe good to use on the face… she did not seem impressed… I have since researched it more and it may actually help with eye issues caused by hormone drops. I hope more research goes into this one!!
Just wanted to add that post menopause you can actually have low testosterone, too! As women we actually need testosterone (just much less than men). My mom suffered with debilitating anxiety and irritability for 2 years (which we all certainly noticed, just didn’t know the cause) before she went to a hormone specialist who told her she had low testosterone and she began supplementing. Within weeks she noticed a huge difference in her emotional state
Interesting. I would think that lack of progesterone or maybe estrogen would make someone irritable but I associate testosterone more with motivation, but who knows! Glad she is feeling better. It's incredible how much our hormones affect our moods and "personalities".
@@AvsFan32TOO MUCH! That’s a simple fix-you need less…but don’t throw out the baby with the bath water! It’s 100% VITAL for bone health, motivation, maintaining muscle which is one of the most important components of aging.
@@AvsFan32You were using too much. I’ve been on it for 5 years and my doctor had to find the best amount for me by checking my blood. I love it…keeps you strong and active
I’m in HRT for last 2 years. I’m 47! It has totally been a game changer for me! I had debilitating side effects of estrogen loss & my patch & progesterone took them away! I feel almost normal now.
@@Kne2450 weight gain of 20 lbs in a couple months, when before I’d always maintained my weight. Depression, not sleeping or waking at 3 am and not able to go back to sleep, migraines that lasted a week, itchy dry skin, feelings of bugs crawling all over my skin, crying all the time for anything, feeling terrible with flu like symptoms most of the month. No hot flashes but I had cold flashes so bad that my skin would itch and I would scratch and make myself bleed over nothing, joint pain, no energy, no desire to workout (which I’ve always done) I was a completely different person and hated myself and even was contemplating suicide. My Dr put me on an anti depressant which didn’t help my menopause symptoms. I started watching videos on TH-cam: Dr Mary Claire Haver, others, and educated myself on what was essentially happening to my body. It was peri menopause and I’d started having symptoms around 42. My periods which had been very light before that were non existent now, and I was having awful Vaginal dryness that it actually hurt to walk. Everyone’s side effects can be different so I had to just explain everything I was going through to an Endocrinologist who knew based on my age 44 that I was going through estrogen withdrawal basically. I got the hormone replacement I needed and they help a lot. I feel 80% like myself.
I’m 55 and have been on HRT for about 2 years now and it helped me tremendously. I started using topical estradiol on my face 3 months ago and already have noticed a difference in my skin. Thank you for coving this important topic, Dr Ellis.
I have been on Alloy’s .3% estriol face cream for 6 months now and what a difference it has made. My skin is so supple and looks alive again! I also use your Multi-quench serum as well as daily collagen and tretinoin! I am on estrogen and testosterone therapy as well. The total combination has made a difference in my skin and my physical/mental health as well.
I’ve been using facial topical estrogen for a month now. I’ve noticed some improvement in my skin elasticity and firmness. It’s not a huge improvement but enough that I want to continue. The improvement is most obvious is photos, especially around the jowls. I’ve not experienced any negative effects
I had breast cancer back in 2008. I used to use estrogen vaginally for atrophy. Good to know that at 65 I might have a chance to improve my skin. Thank you!
I was put on estradiol gel tabs in May. By July, I was off of them. I continued with testosterone and progesterone but nothing compares to estradiol. I had to research for myself why my lymph nodes under both arms ached 24 hrs a day. Why I had breast pain. I have a hormone specialist and I’m not sure if she understands that taking estradiol orally converts to estrone which is inflammatory or she didn’t think to tell me at one of my many visits to find out what was wrong with me -that cost hundreds of dollars every appt about this detail. It’s drs like you on TH-cam that I am learning from because i HAVE to. I asked for the patch after 2 months of estradiol replacement and WOW. I am watching my skin rejuvenate again. No lymph node pain. No breast pain. Just the benefits.
I did not know this oral conversion to.estrone Must be due to.having to go thru the liver. Been on estradiol for 15 years. last year, due to the crazy increased cost of estradiol.patch, I started oral estradiol. Does not work as well as the patch so I was happy recently to discover a more reasonable price for the patch which I just started again today.
I don't believe estrogen is necessary for women after menopause. If using cream vaginally, yes , it is beneficial. I have not tried it on my face. I'm curious to try it. I do know that if your testosterone is low , it is important to supplement that at a very low dose. The trokees work well .
Yes, and Yes. Decrease in sag, wrinkles and bags. What a difference. if you can, go for it. All the skin care which I have been on for years did not produce the changes I got in a few weeks. So ladies... go for it. I'm happier for it. cause I was considering a lower face lift. If I can get these results in a few weeks I'll keep doing it and use my face lift fund for something more fun... like travel or a sports car. Don't get me wrong. I haven't turned the clock back to 20 but at 57 I look like a good 8 years maybe on some days even younger. I'm just glad the sag around my month and jaw area is gone for now.
I have no menopause issues by doing this. Also I take the max so I have no bone, heart or brain issues for the future. That is mostly why Im on HRT I did not have hot flashes I take it as a preventative for heart, bone and brain.
I really want to see the medical community significantly increase research concerning menopause and treatments. As I am entering this new phase of life, it's shocking how little literature exists currently. Thank you for your video. I'm open to using a topical estrogen, but would probably opt for HRT for a more systemic result.
HRT is ideally started during menopause and is studied safe used for 5 years a test 4 genetic mutations that make women way more susceptible to breast cancer should be done first & yearly mammograms should not be missed I’m on bio identical progesterone it’s amazing
Thank you for discussing menopause at all. It’s difficult for us old gals out here to get proper care or an OB that even knows or cares what they are talking about for this stage of life. Appreciate the honest info!
Just remember: Every 'BODY is different! Not all things work the same for everyone. Find that practitioner who works for and with YOU. Its a journey well traveled, yet worth it. ❤
I've used topical estrogen for a year without any issues, and I have seen an improvement in my fine lines. I always use SPF and I am very happy with the results.
Thank you for this! Even though I am not in that period of my life, I think this normalization of speaking of menopause and perimenopause empowers us millennials and younger generations to face it without fear and knowing our options. 😊
Don't worry you will be there in s blink of an eye. Most are going in younger now especially your age group. I guarantee that will happen. Premature ovarian insufficiency will be rife.
Fight for normalizing our needs and insurance coverage . Men. are covered by insurance for Testosterone and have 21 prescriptions available for impotence. How many do woman have covered ? ZERO!
I think us GenX woman are really pushing to create a lot of awareness. My Mums generation silently suffered through. I speak to anyone in my circle (work/personal) about it, create awareness, there’s loads of great resources online. Dr Marie Clare Haver has had some great podcasts on the topic, Davina McCall has a great book on it and is working hard in the U.K. to create more awareness. Look them up. The sooner you know the better you can prepare. It will massively increase the quality of your life during peri/menopause.
I’ve been on the hrt patch and oral progesterone. I got CO2 laser. Just life changing. Melasma spots are gone. I sleep like a baby. Hot flashes are gone and I started using weights 2x a week with a trainer via FaceTime. I feel 22 at 60, and I’m glad I did it.
Thank you for this information Dr. Ellis, you are a gem on this sea of misinformation. Menopause only happens to women, that is why we still learning & fighting to be able to get HRT
Be aware if you have pets. We had recently gotten a puppy when I started using my topical estrogen (bioidentical) cream on my inner thigh at night. The puppy did not sleep with us but would come in and cuddle in the morning. Several months later, our vet noticed the puppy had an enlarged vulva and larger than normal teats. She the asked me if I was taking topical estrogen. Apparently, she knew the topical estrogen can transfer to pets. I immediately replaced the topical estrogen to troches and the dog returned to normal. It's amazing to me how powerful the estrogen transfer must be after a night of sleep and the dog having no direct contact with my thighs. There is plenty of research on this if you check, but I had no idea prior to my experience.
This is so helpful. I don't usually pick up my dog until after my estradiol and testosterone dries but I will be more careful. I think I may have to switch to a patch. Tried the troche, made me nauseated and bloated as I was probably swallowing some of the estradiol and testosterone.
This was excellent, thank you. Very well presented and so helpful. I have topical estrogen for applying to the opening of the urethra to help with stress incontinence, I didn’t like the pessaries. I’m also on HRT but thrilled that I can use the cream on my hands, face and neck. My hands have hardly any fat and are by far the oldest looking part of my body! My lower pelvic physio at the hospital told me during an exam that she can tell I’m on HRT. A slightly awkward compliment but good to know!
I was hoping that someone would address incontinence. Thank you. I have been on HRT for 27 years now (the patch). I also have a bladder condition called interstitial cystitis which causes feelings of urgency as well as urine leakage. About a year ago, I started on additional topical estrogen applied vaginally and around the urethra. It has made a huge difference. Sometimes systemic estrogen is just not enough. I would encourage any woman with bladder issues to speak to her gynecologist about topical estrogen, as well as personal benefits and risks. For me, it's been a quality of life saver.
I am 63 and have been on HRT for almost 3 years. I have been using Estradiol for several years but it wasn’t enough for my symptoms….foggy brain, lack of energy, hot flashes, weight gain, none existent sex drive. Now I am on Biodenticals….”Progesterone, Pregnenolone, DHEA and a second Estradiol cream ( inside wrist 2x day). Blood test shows I don’t need to be on Testosterone because I work out intensely with heavy weights 3/4 times a week. If you cannot find a doctor who will listen and take care of you find a nurse practitioner. I found mine in a weight loss/hormone replacement clinic. Life is GOOD!
You need Nanderlone & Stanzinol - Dr Edward Lichten , Textbook of bio identical hormones. 💯 nothing will ever work until you get your hormones, balanced . I’m 62 years old at 80% muscle mass size 3 🎉
@@jenniferjohnson5683you still need to never skip mammograms and should be tested for the genetic mutations that often cause breast cancer I’m on bioindentical progesterone there’s a little risk but with breast cancer but it protects against other cancers
I once started BCPs just before going on a cruise. I had very clear, even skin and no history of melasma. I went out on deck one day, came back, looked in the mirror and had brown patches on my cheeks and upper lips and have had it ever since. Literally in one afternoon. Something to consider before any hormonal therapy. If only my doctor had informed me of the possibility. Thank you for this in depth video!
My mom have had a total hysterectomy (severe endometriosis) when she was 28 (now 56). She was prescribed since 28 a estrogen gel to apply on her lower arms. With this she avoided all the menopose symptoms and she is still on it. Her skin is indeed very good looking too. Barely have wrinkles and she never been into creams or any kind of skincare routine and she never stepped into a beauty clinic. Just showering, using a washcloth and soap for face and body. On the other hand, my grandma and my mom always wore sunscreens since the 90s for outside doors activities. So I imagine that helped a lot for their skin preservation.
I'm a BC survivor of 8 years and started estradiol a couple months ago on my face and neck. Dr. Ellis just reminded to do our hands as well. It hasn't hurt my complexion, so I'll continue it to see what the future holds. I also just had my first round of Sculptra, so can't wait to see what that does too. I'm getting two more treatments in the coming months.
since I carrie the APOE4 gene for alzheimers I went on HRT before menopause. Best decision I have ever done. I went on HRt at 54. menopause at 57. take .75 dose
Topical estradiol cream (Ovestin) absolutely works mixed into your usual moisturizer. I’m 61 I am not a candidate for systemic HRT unfortunately. But the topical cream is a total game changer. I’m so happy .
I am in my 30s and take HRT for premature ovarian failure. I also insert Vagifem daily, but after seeing this video will be talking to my endocrinologist about topical estrogen at my next check in. Thank you!
I am on a low doe of HRT because I had polyps in my uterus which I was told was caused by too much estrogen. I have been using vagafem for years with no problems. I just started using estrogen on my face so I haven’t noticed improvement yet but I am hopeful. I am thrilled that us older women are finally being taken care of as it has been discovered what an important part all these hormones play in our bodies! Thank you for this explanation!
I’m 57, started using HRT in April. After 3 months, I noticed my face had increased volume, and I look less tired, when I’m not tired. I can imagine topical estrogen would be great for places where women have extra concern.
I am 54, post-menopausal now for about 2 years. I am on the estradiol patch, prometrium, testosterone and vaginal estradiol . I also use the vaginal estradiol on my face and neck as well. I use the vaginal estradiol cream on my face in the morning since I use Tretinoin at night. It takes a good 6 months before you start seeing some changes. It is somewhat drying and not cosmetically elegant but it’s cheaper than the compounded estriol creams and I put a moisturizing cream on top and then sunscreen. One small study comparing the estradiol cream to the compounded estriol cream showed the same skin improvement results in both groups. And, I did check with my OBGYN and she approved. By the way, LOVE your Prequel Vitamin C! New holy grail status!
You and I are on pretty much the same regime only I'm a bit older (58). Is there a reason not to use the estrogen with the tret? I usually do my tret and serums and then later, after it's had time to soak in, I go in with the estrogen cream (usually when I brush my teeth right before bed). I definitely have noticed a difference since starting the estrogen on face and neck.
@@Cherylebreaux Hi, from what I understand you can use Tretinoin and estrogen at night. The recommendation from Alloy website for their M4 estriol cream is estrogen first then tret. I would assume it’s the same for the estradiol cream. I separate them AM/PM because they both tend to be a little drying on my skin. If you are not experiencing any irritation and you are seeing results using them both at night, I think your routine is great.
@@reginarizer5797 Yes, I checked with my OBGYN. It’s not an elegant formula and can be drying but I put a moisturizer on top and it’s fine. The vaginal estradiol cream is stronger than the estriol cream that is made for the face so keep in mind if you are prone to melasma it can worsen it. I have not noticed any issues with melasma on my skin. Just make sure to use sunscreen everyday. If you are prone to melasma the compounded estriol cream might be a better fit.
@@reginarizer5797 I do and my OBGYN approved it. It’s stronger than the compounded estriol cream so if you are prone to melasma it can make it worse. I have not had any issues with it.
Thank you so much for this video! I'm in perimenopause and have done quite a bit of research. I get my hormones tested every 3-4 months (on my own through Private MD Labs) to make sure they're balanced and my last test showed I have no hormones left. The skin sagging is real :/ I've learned the key to avoiding cancers is balancing estrogen and progesterone. I use a natural progesterone cream just so I can sleep. Without it my peri-caused insomnia is awful. I also take 5 mg of DHEA and even though that is a low dose, its helped tremendously with my energy and strength. My doctor and the articles I've read said that DHEA raises testosterone and in women that raises estrogen. However, with all of that I still have vaginal atrophy and after this video will be talking to my doctor about estrogen cream. I just had polyps removed from my uterus and cervix-benign thankfully-and wanted to wait on adding estrogen until they were removed. Ladies, get your hormones tested please! Regardless of what your doctor says. I was told my symptoms of erratic periods was normal because I'm in peri. Before my last pap appointment I tested my hormones, which is how my doctor realized something was wrong. I had no hormones but had a cycle, which was caused by uterine and cervical polyps. A lot of docs don't even order hormone testing. They just tell you you're in perimenopause. Benign polyps can become malignant so please do your due diligence with your health :)
DHEA is the precursor of our hormones. And I'm not happy that insurance does not cover the cost of lab work and prescriptions for hormones as it does for our male counterparts. Humph
Such a great topic. I'm 61, on HRT and incorporated using estradiol on my face 4 months ago. My skin is responding positively and looks and feels healthier. I discussed this with my Gyn prior to starting and got her 👍🏼. Thanks for another video with great information
@Tracimakingwaves Did you use estradiol on your arm, then ask about applying to your face instead? Wondering if you apply nightly or alternate locations? Thanks for any more information you can share!
@Marie-oy1bd I apply about 1/2 pea size to my face each morning. Sometimes, I blend with my moisturizer and sometimes I apply it as a single layer, alone
I'm about to start using 0.3% estriol and would like some instruction on how to incorporate it with my nightly skincare routine application of 0.1% tretinoin on my face, neck, chest and hands. I also apply niacinamide and Hyaluronic serums followed by La Mer Creme moisteriser. Which goes on first? Can you mix them together, etc. I might also give it a try on my "old lady" arms. I have been using Omnilux red light therapy every morning for several years followed by Vit C Ferulic, peptides, CoqQ 10 & Hyaluronic serums and La Mer Creme with good results. My skin looks better at 69 than it did at 50! I have been on HRT patches for many years which also helps. I can't wait to try topical estrogen-- the clinical trials I've read sound amazing.
Thank you for making this video easy to understand. I have watched other videos on the subject and more than often they are not doctors and unable to convince me that their opinions are valid and worth following. A study and baseline has been long overdue on the benefits and risks (melasma and vascularity). Melasma loves to make a surprise visit during pregnancy and creeps up in menopause, the uninvited guest at a party...returns. Having 3 pregnancies in my life, me and hyperpigmentation have had a long-term unfriendly relationship. My derm gave me script for a compound combination of topical estrogen, hyaluronic acid, DMAE, glycerin, oleic acid, vitamin E and Tretonoin. I found that topical estrogen is very drying to my face and hands and didn't want to risk the hyperpigmentation returning. Derm is closely monitering any changes in my skin. Its too early to give an opinion on the compound, I think 3-6 months is a fair time to see any changes and to find out if this will be a lifetime daily routine.
Wow! Thank you for this information. I am mid 40’s and have been taking estradiol & progesterone for the last couple of years simply due to intense peri-menopause symptoms. I have been using the estradiol gel over the front of my body nightly as instructed but never knew that it could be used on my face and hands! How interesting. I will give this a try.
Anyone honestly worried about breast cancer who is still drinking alcohol, smokes and is overweight/diabetic, is totally missing the boat wrt estrogen. The studies do not show a statistically significant increase in breast cancer. Yet smoking, being overweight, and alcohol all increase the risk in statistical ways.
@@healershealth5818 I had breast cancer and I'm on HRT. I had to fight with my oncologist, but I convinced her. I was having serious mental health problems due to the decline of estrogen.
@@healershealth5818 if you have been diagnosed with hormone positive breast cancer you cannot take hormone replacements. You are placed on a hormone blocker. My oncologist did tell me last month that the topical estrogen cream would be ok but I think I'll pass for now.
Yes I started estrogen cream on face about a month ago. I’m 56 and have been on HRT for about 5 years. The estrogen cream has made a noticeable difference in my skin clarity and suppleness. And it was noticeable within 2 weeks if not earlier. I love it. I haven’t had side effects from estrogen cream at this point but I did up the tretinoin use at night which did irritate my skin. Thx for a great video!!
I have been on bioidentical hormone replacement therapy since age 46. I blasted through perimenopause and it was awful and compounded by the fact that it was a challenge finding a doctor to acknowledge and address it. I am 71 and found my people! All this to say thank you for putting this out for today’s audience. ❤️.
Thank you so much!! I would absolutely use topical estrogen. I’m on HRT as prescribed by my gynecologist, and I’m visiting my dermatologist this week to discuss topical estrogen for aging skin.
I’m currently doing an ivf cycle where I was temporarily put into menopause to treat endometriosis. Let me tell ya! Estrogen does so much. My skin was dull and I developed wrinkles. A week back on estrogen I felt and looked like a new person.
Thank you for the great information. Can topical estrogen be used on the face and neck or only on the face? Would bringing it down the neck cover too much area and make it systemic? Also, how can topical estrogen be incorporated into a skincare routine? What skincare ingredients can it be used with or layered with or shouldn’t be used with? How often should it be used to be effective? These are the answers I was really hoping for.
I'm wondering that too - I'm thinking any estrogen on your skin becomes systemic so I hope people are taking this into account when they start using it on your face. My menopause doc prescribed the amount she tested my body to need, I use a tiny bit under eyes, a little vaginally and the rest on my thigh. I'm wondering if people may be using too much if they use a lot for their face and neck?
Thank you for bringing out, very diplomatically (15:45), that women are more than just baby making machines and that just because a woman cannot reproduce anymore does not mean they are worthless and that we deserve the best medical attention through all phases of our life.
Thank you for this educational content and attention to a plaguing and relatively mystified topic… great tools as a 43 y/o woman navigating perimenopause. Any recommendations for Bay Area OBGYN/Derm specialists would be greatly appreciated!
I’m in the Bay Area. You could look on The Menopause Society’s website where they list menopause practitioners in your area when you enter your zip code. I’ve had a gynecologist who was helpful with HRT to a point, but told me I needed to come off at a certain age, which is not true per the menopause society HRT guidelines. I decided to go with telehealth. There are good options now. Some take insurance. They are staffed with MDs, NPs or DOs trained in menopause care. Some names of companies are Midi health, Alloy, Evernow, and Dr Heather Hirsch’s “The Collaborative”. Good luck!
Great informative video. Yes, i have been using topical Estradiol cream prescribed by my doctor on my face for the better of six months. I haven't had any issues with side effects. I have seen good results, most particularly in areas where the skin is thinner, around the eyes, and neck areas. I've also been using it on the upper inside of my arms with great improvement in thickness as well. I do use it along with Retin A and other skincare. I'm quite happy with the results. Thank you 😊
I was put on hrt at 55. Game changer. My skin is finally getting back to what is was. Not a cure all, still hard to maintain moisture, but the acne finally stopped. So yea estrogen depletion is real. By replacing it, I can attest, game changer
I'm 46 and on the estrogen patch and yes, I noticed my cystic acne has all but vanished, knock on wood. I still get small breakouts, and I do think I am getting very small brown patches due to the estrogen, but I'll take it! I had horrible inflamed acne in my 30s and 40s that mauled my face, and it's so frustrating to know now that I should have at least investigated hormone treatment options, e.g. the pill. I feel like for a long time there was so much scaremongering, especially with HRT, so I suppose I should feel lucky that at least now there seems to be a more enlightened, evidence-based approach, at least where I am in the UK.
I’m 61 & on oral estradiol & progesterone and noticed my facial skin went from combo to EXTREMELY oily and having acne like a teen. Wondering what I’m doing wrong 😑
Yea that only goes so far. And you need to it nightly for probably 4 months. Then go back again. I do agree it's great for inflammation and joints etc. But it doesn't get down to the crux of the matter. Which is the fiberblsst cells. What feeds them????
Great topic. I'm 65, but at 42, I was diagnosed with Premature Ovarian Failure. No symptoms, my periods just stopped. I was put on the Estradiol patch and progesterone. At 48, I was taken off of the hormones. I went on Estrace Vaginal Cream, and have been on it twice a week, since then, and have had no negative side effects. I'm definitely going to ask my gyno if I can apply to my face. I must admit, that I've tried it a few times, but wasn't sure if I should be doing so. I have no risk factors such as family history of breast cancer. I've also been on topical Tretinoin since my 20s. Wow, what a great combination the two would make. Thank you, Dr. Sam for such valuable information. Very much appreciated. 😊
Your gyno won't know. Look at the % as the vaginal cream is normally 1%. You want 0.01 %. I use the 1% but just a smidge of cream, about the size of a grain of rice.
I meant to say dermatologist. And that's the exact strength that I have. I did use a pea size amount last night. For now, I'll use it twice weekly, on the nights that I use it for dryness. Thanks.@@Archiebean
I wish they knew more back then. I would have fought to stay on in. But, via a heart monitor, it was discovered that I went into non sustained v-tach, and threw me off of the transdermal estrogen patch. Ugh.@ammonitelight
@@reneklein6049 With any hormone, it is better to use daily, so that it is not the case of one day there is a hormone and the next day there isn't. I have been researching this subject for 18 years. It would be way better to use a smaller amount every day. 😁
100%. There is a neglect that seems to happen. Not one doctor gave me information or told me what was happening. I suffered from severe menopause and was never told it could be mitigated. Most doctors don’t deserve our trust. Also, most doctors are prescribers, not healers.
I had a female OBGYN about 15 year younger than me tell me this. She said to get fan for the round the clock hot flashes and would not talk about HRT and promptly dismissed me. I was disheartened. I asked for another doctor in the practice. They assigned me to a very young male doctor. I was prepared to read him the riot act if he tried to tell me to suck it up. I gave him the benefit of the doubt and after I shared all my issues, he sympathized and immediately provided an rx for HRT. Maybe the younger ones are better educated about this, or maybe his momma told him he better not let women suffer...!
Every woman should read the book Estrogen Matters by Avrum Bluming & Carol Tavris to make informed decisions. It’s pretty much changed how I look at my health. Spoiler estrogen doesn’t cause cancer.
I watched a video a few weeks ago, from a dermatologist, who stated that you can apply both. You should apply the estrogen cream first, wait a few minutes, and then apply the tretinoin.
I am.on my 2nd round of Alloy .3 estriol.cream. i am 70, live in FL so lots.of sun. I see a difference with this.cream. I also have been putting oil on my face for over a decade, Coconut, Almond, Grape seed every night. Recently also added castor oil , which I put on top of the estriol cream.
I was listening to this podcast just to get more information on topical estrogen cream on the face neck and hands. My mother had breast cancer however I never did. I’m 66 years old and my mother passed at 49 years old. I’m fairly healthy woman and I’m just trying to mitigate any aging process if I can.I would try the estrogen cream if a physician said it was OK.
Great video! In Europe we have estriol cream which has less systemic absorption than estradiol cream. I would be wary of systemic effects of estradiol cream, seeing as it is used in both patch and gel form as a systemic HRT. I use the patches myself.
Thank you very much for the information. What I miss is the differentiation between östriol and östradiol and the differentiation between bioidentical hormones versus synthetic hormones.
this explains it! i have to take hormone blockers to prevent my metastatic stage 4 breast cancer cells from eating their favorite thing that keeps them thriving and that is my hormone ESTROGEN! This explains why i looked far younger a year ago i was 47 when diagnosed but looked 37 until AFTER nearly a year of taking letrozole and zoladex injections that one is a hormone blocker that prevents estrogen absorption in the body and the other shuts down my ovaries to prevent further production of estrogen. It was within less than a year that i now look older than my age. People were asking me what is it that is making me look years younger than was i really was prior to my treatments, now people are asking me why do i look so old. No that is not them being mean, they were worried about my overall health as they didn't know about me having stage 4 cancer. Even with stage 4 cancer i had for well over a year and did not know it, i still looked way younger in years so it is not the cancer that is causing my aging in this case, though it made me feel old as dirt and dead on my feet. I have the slow growing kind of cancer that is why it didn't kill me right away at stage 4 but the fact that my body is being starved from producing and absorbing estrogen is causing me to age rapidly facially.
My heart goes out to you and others for dealing with the double whammy of cancer plus having to take estrogen blockers. Having experienced significant symptoms associated with menopause and finally getting treated with HRT and feeling normal again, I can’t imagine dealing with what you had to experience….So thank you for sharing, I’m glad you are ok, and I’m sorry you have had to deal with all that!
I had radical hysterectomy early in life. NOBODY warned me what to expect 😢 I’ve hated my body ever since. My skin on EVERY part of my body changed, my stomach ballooned and my back aches from all my internals moving into the space that was created.
Love this presentation. HRT is the bomb and at 65 I have been using it since my early 50's. I added Testosterone a year ago. Game Changer! We need our hormones to be our best and we have estrogen receptors throughout all systems in our bodies for a reason. Unfortunately, the focus has only been on reproduction for so long. Too many millions of women have suffered unnecessarily for too long. So glad to see things are changing and it is being talked about studied, and prescribed. So important! Lots to learn on this topic, and so much fear and misinformation, it is great to have another sound voice on this topic. Find providers who are educated about the benefits and understand all the nuances. It is complicated, but the bottom line is we need our hormones and they benefit your health your entire lifespan.
Thank you so much for this! As someone who is going through perimenopause, the changes to my skin and hair are upsetting and it's hard to find anyone who truly knows anything about this topic. Would love a deeper dive into this topic and suggestions you might have for skin and hair products if you choose not to go the estrogen cream/HRT route. My hair goes from feeling like straw for a few days to shedding like crazy to back to normal all within a few weeks. My skin is combination but now to the extreme resulting in crazy hormonal acne and eczema around my eyes. None of my makeup or skincare is working for me the way it used to. 😭Thank you!!!
I’ve been using Empelle night cream and serum for over a year now. It’s for estrogen deficient skin. I use other products in the morning. I’m now 67 and fell this is a great product. It’s creamy and feels nice. Hope this helps someone. It was recommended by a plastic surgeon.
Perhaps it would be useful to speak about the gut-skin axis. I.e. the impact of poor digestive health, impaired gut barrier or imbalanced in gut microbiome and the effects on the skin
Just came across your site. So I certainly learned alot today! You are very clear and easy to understand all this medical information. I will be following you for sure. I will reach out to my endocrinologist to discuss by testosterone and estrogen levels. Can you also do a segment on testosterone for woman
I am already on vaginal estrogen , after hysterectomy. So, I will definitely consider using it topically. YAnother aspect for me is how topical estrogen effects rosacea? after hysterectomy my rosacea worsened significantly. Thank you, our video comes in great timing, considering that I am 45.
Excellent information and thoughtfully presented, as always. Though I must say, reading the comments about topical estrogen users experiencing a reduction in perimenopause symptoms certainly challenges the notion that there is minimal systemic absorption… 🤔
Such an informative video as always. I am 56 and menopausal since 50. I have been on HRT almost from the beginning. I have been using Alloy cream topically on my face for about 2 weeks so the jury is still out on efficacy. I haven’t experienced any side effects thus far…fingers crossed. Alloy instructions are to apply it before tretenoin but that just seems wrong to me so I use the estrogen cream in the AM and tretenoin in PM
This information is so valuable! Thank you so much for making a deep dive into this topic. As a post menopausal woman that has suffered from the symptoms you mentioned, I am super interested but still haven't made my mind around it. Personally, at the moment I am not convinced about using this cream topically on my face for aesthetic purposes only. After almost 5 years of starting this process, I discovered that this new stage of our lives (at least in my case) required more than 'just' taking HRT, which has proven to be so beneficial for me, but also lifestyle changes, from exercise to nutrition, skin care, hair care and so on... It takes some work (and years) in getting to know this new body, but if you learn how to give it what it needs you can get great results out of those changes, from the outside and inside as well. What I mean is that this cream could be an option but it is not the only answer. At least that is the case for me now.
Im 44 yo. Definitely perimenopausal. I started tretinoin in 2020 and feel my skin no longer benefits from tretinoin. It’s just crazy. It could be estrogen wreaking havoc on my skin. Rather the lack there of. Today I bought 1mg estriol cream and I’ll start using it on my face. I’ll combine estriol And Tretinoin and hope to have the good results. My skin looked so good a year ago. It’s disheartening:( Thank you doctor for the overview.
Thank you so much for covering this topic in absolutely the best and most understandable way. I'm almost 41, and started perimenopous about a year ago (confirmed by my obgyn). The night sweats are brutal. But I haven't ever heard the explanation of hormonal changes the way you've presented it, and I feel like I've learned SO much from this that explains the changes happening to my body. Already hit "Save" on this video!
I heard through various sources as well as the great Dr. Lee that progestin is not good for women to use and that progesterone as well as it being Bioidentical is much better and safer for women. Can you clarify what you mean by progestin? Thank you.
I was anorexic for 2 years of my life from 34-36. My skin looked horrible as I was protein and estrogen deficient. I was 93 pounds and skeletal thin. Now at 36 1/2 and at 135 pounds with a regular menstrual cycle - my skin is desperately trying to catch up.
Another great informative video, love the honesty you bring to the table. Thank you especially for this video I started using Estrogen (two months) but have noticed Angiomas! Did not put the dots together! Can you have Angiomas & thin broken blood vessels safely removed? I tried having blood vessels - very very thin veins removed last year but the treatment made it worse not better - any help on this wonderful new aging symtom would be greatly appreciated - I am on tretinion not sure if this has any impact on the treatment.
Thank you for all your videos, Dr. Ellis. I have followed you since the beginning, and you are always very organized, thoughtful, clear and informative in each of your presentations. This information is extremely helpful as we navigate collectively through our various stages of life.
Thank you for doing a video on this. I'm curious what your thoughts are on OTC estriol that's made from wild yams. Is it effective or a waste of money?
I’m def interested in trying it. I’m 62 and just went on hrt patch and it’s been great but I could def use the cream. Thanks for this, I had no idea this was out there.

@@karenthomas9506 Exactly! I've been to the gynae and derm they won't prescribe it here.
progestine is not natural, so progesterone should be taken; as estrodiol is the strongest of 3 possible forms of estrogen, it might be better to take estriol instead
Barbara Hoffman at Better Health Naturally is a good source of information to help you
I was on HRT from 45-52 and got off due to my sister getting breast cancer and her telling me I shouldn't be on it!
Ugh! Not okay that we're so misinformed😢
I think you can definitely go back on hrt
I am 84yrs of age I had breast cancer aged 34. I was on HRT for 20 years during menopause and after until my Doctor refused to prescribe it in my 70's since then things went downhill fast mood changes, irritability, dry skin. My belief is women should be able to have HRT for as long as they need it.
I’m so sorry you had this experience. I agree 100%. There’s no reason to stop HRT. It’s not like our bodies will start producing hormones again
U mean u had breastf CA at age 35 ....but still u used HRT for 20years ....is It True ???

When I turned 64, I didn't recognize the person in the mirror anymore. Topical estrogen has helped. I would recommend it. Thanks for address this. Very few dermatologists address this problem.
@genevievedeguzman9293
This is concerning!
I saw this brand recently! I would like to try the one with progesterone 🤩 . Thanks for sharing your experience.
@@Tica5012it’s the estrogen boost cream with the estrodial and progesterone that’s giving her all those additional benefits. She likely uses that one on the arm or belly (maybe her face) and it does have systemic effects because of the higher blend of hormones.

Thank you for discussing menopause at all. It’s difficult for us old gals out here to get proper care or an OB that even knows or cares what they are talking about for this stage of life. Appreciate the honest info!

As a 67-year-old, I must say I know nothing about any of this. I'm learning, finally.

My
Mums generation silently suffered through.
I have been using topical and systemic forms of HRT. I love the way the topical on my face has made a noticeable effect. I am 57.
Which are you using? I am considering Musley
Mmmm and what about your husband/partner
@@vanessameintjes9896 Didn't notice any unusual symptoms with him. It is easy to check on the information yourself, which I recommend.

This is so helpful. I don't usually pick up my dog until after my estradiol and testosterone dries but I will be more careful. I think I may have to switch to a patch. Tried the troche, made me nauseated and bloated as I was probably swallowing some of the estradiol and testosterone.
@@carolwong9279what’s a troche please? My dog does have enlarged teats! And had a pyrometra, I hope I didn’t cause that….

@@constancehall1231 I’m glad to hear that you’ve seen improvement with topical estrogen! That gives me hope, thank you for sharing!
That's nice to know :) I'm just turning 30 and I had my ovaries out at 28 due to Endometriosis. I'm also on HRT and had worried about my skin aging 😅
Maybe it was great genetics too.

I'm 57 and yes, I use topical estrogen , 2 x week in vagina plus occasionally a bit on face. Plus on low dose systemic HRT.

Oh, that's interesting! What ratios do you use?
Yes you are a candidate. Who told you that?? You have many many options now. Do the research. Don't go into fear. Fear blinds everyone.

Why don't you go on micronised Progesterone?
You might want to consider Nascent Iodine for your polyps. Lack of iodine is often what is behind lumps in breast and reproductives .
I’m 57, started using HRT in April. After 3 months, I noticed my face had increased volume, and I look less tired, when I’m not tired. I can imagine topical estrogen would be great for places where women have extra concern.
I love that you are informative without resorting to drama!! Great info. Thank you.
So, I can use the vaginal cream on my face?
Humph

Yes I started estrogen cream on face about a month ago. I’m 56 and have been on HRT for about 5 years. The estrogen cream has made a noticeable difference in my skin clarity and suppleness.

She answers some of these questions in the video
I'm wondering that too - I'm thinking any estrogen on your skin becomes systemic so I hope people are taking this into account when they start using it on your face. My menopause doc prescribed the amount she tested my body to need, I use a tiny bit under eyes, a little vaginally and the rest on my thigh. I'm wondering if people may be using too much if they use a lot for their face and neck?
I would love to hear about all the different HRT's; benefits and risks for Perimenopausal women!!!
Thank you 😊
Hi - Do you put retinols under or over your estradiol?
@@Lulu-kt6gr hi, I use the estradiol cream first than retinol.
Try castor oil and talllow for moisture of the skin 😉
Get off the oral Estradiol. Transdermal cream or gel.
Why did they discontinue your HRT? It can be continuously used and really should be because of the protective effects!

That's true and so sad. That's exactly what they said to me and now in my 60's and grrrr.
100%. There is a neglect that seems to happen. Not one doctor gave me information or told me what was happening. I suffered from severe menopause and was never told it could be mitigated. Most doctors don’t deserve our trust. Also, most doctors are prescribers, not healers.
Well said!
Please show us the results to using topical facial Estrogen.
Dr Ellis thank you. The timing here is perfect. Can the estrogen cream and Tretinoin be applied together at night?
I watched a video a few weeks ago, from a dermatologist, who stated that you can apply both. You should apply the estrogen cream first, wait a few minutes, and then apply the tretinoin.
I am.on my 2nd round of Alloy .3 estriol.cream. i am 70, live in FL so lots.of sun. I see a difference with this.cream. I also have been putting oil on my face for over a decade, Coconut, Almond, Grape seed every night. Recently also added castor oil , which I put on top of the estriol cream.

Which Patches do you use? Same Problem here living in europe
I wish you the best ❤
So sorry. Stay strong. Perhaps a hysterectomy could help? so the estrogen is not there to feed the cancer? 🙏🏼
I wish you all the best❤
I am 70 and I never used hormone therapy for menopause symptoms. Would topical estrogen be beneficial for me at this age?
I had radical hysterectomy early in life. NOBODY warned me what to expect 😢 I’ve hated my body ever since. My skin on EVERY part of my body changed, my stomach ballooned and my back aches from all my internals moving into the space that was created.
Find a provider, it I not too late to improve your wellbeing!
Ditto.
Wouldn't a prescription be more economically as oppose to Alloy?
@@tnt01 Technically, Alloy is a prescription.
You are so articulate and clear in your videos, I love them!! Thank you for taking the time to talk about this issue. ❤
I'm so happy to see this topic coming up more and more. I would definitely have availed of a prescription oestrogen cream formulated for the face.
I really appreciate your clear concise and balanced explanation. Thanks!
I’ll combine estriol
And Tretinoin and hope to have the good results. My skin looked so good a year ago. It’s disheartening:(
Thank you doctor for the overview.
I’m on micronized progesterone for perimenopause and a topical an over the counter topical estrogen half my cycle I love it
I heard through various sources as well as the great Dr.
Congratulations on your recovery ❤
So happy for you that you are able to be a healthy weight now! It’s not easy. ❤
I just wonder how this would work on male skin? What's your say about that, hypothetically, Dr. Ellis?
This was thorough, and appropriately cautious. Thank you for this!
Informative in relatable language and very useful video without the selling...thank you.
Thank you for doing a video on this. I'm curious what your thoughts are on OTC estriol that's made from wild yams. Is it effective or a waste of money?
I would like to know also
I’m def interested in trying it. I’m 62 and just went on hrt patch and it’s been great but I could def use the cream. Thanks for this, I had no idea this was out there.
