They are rarely held accountable. Why would they care.
It's the career bureaucrat mindset. They think their job isn't to serve the public, but to save money for the organization. The way they see it; you're a filthy peasant, the barbarian hordes, coming in to violate the sanctity of their temple of government, to harass your betters, and they're the last line of defense. They have to do everything possible to get rid of you, to deny you entry.
They don't read the rules to find out how better to help you, they read them to find excuses not to. They don't accept your paperwork to do their job, but to screen it for minute errors that will give them an excuse to deny your request. They don't know the laws that require them to provide a service, they only know the ones that say they can deny it.
I've got a real job, not a government one. Someone drops off paperwork on my desk that's got the wrong date, or spelling errors, I do my best to correct the date, figure out what was actually meant and process it. They will only look at paperwork to find a misplaced comma, so they can refuse to process it.
They act like that at dmv and dhs too.

She willfully ignored the law and then tried to have a person who was simply existing in a place it was legal for them to exist put in torcher cuffs and put in a cage. Then attempted to use their Legal Bar Status as leverage to force her hand. She deserves to be disbarred and jailed herself.
She didn't get her way, so, like a child, she turns to tears to get sympathy. She is seething because she was proven to be wrong.

She was just so used to bullying citizens around and not doing her job. Customer service like this would last about one day in a private company.
Unfortunately, that's not accurate. One of the worst examples are the public companies that offer senior and handicapped housing. There are many that abuse, bully their tenants and seem to rarely, if ever, encounter consequences.
Now, customer service in a business that relies on direct sales, frequent encounters with shoppers, yeah, they will not last long. There are plenty of private business doing a business adjacent to or via contracts with government money.
