Honestly, completely up to you! The more coffee you use, the less room for milk it'll have, so just consider that in terms of taste. When I make it using my 3-cup moka pot, I certainly use all of the coffee because it's not as strong as espresso. Hope this helps!
Oh my goodness! This happened to me once! So... what I noticed is that this happens when the temperature of the melted sugar is too high! Make sure you use the heat as low as you can, remove the pot from the heat as soon as the sugar is melted and mix immediately after pouring the half and half to avoid crystallisation.
