I think at this point Ainz Ooal Gown finally recognizes that he has lost his humanity. The deaths don't concern him at all and he is more concerned of humanity becoming a potential threat. Despite having no feelings on culling people he still has his intellect and memories which tell him this is not normal. Normal for a lich but not normal for who he use to be. I believe he is using Nigredo and Pestonya as a form of artificial conscience. If he silences or kills them then there will be no one in Nazarick to tell him when he goes too far as the majority have no issue killing humans which are like insects to them. Without these voices to temper him his goal of uniting the races in equality under his rule would likely never occur. More likely he would simply drive all sentient species to extinction which would eliminate many forms of art and cultural beauty.
I dont think hes realized it at all. Initially he noticed that he didnt feel anything for humans but it seems to be getting worse. And will just continue to get worse without noticing it until he potentially takes it out on one if the npcs like he did that one wolf npc.

Nah, he is looking at this more coldly: he doesn't want humans to industrialize unchecked. He was born on an Earth where you needed an air tank to walk outside, or else you're lungs would fail. He doesn't want that world to end up like what he sees as the natural result of human expansion.
Suzuki desperately wants more Lawful Alignment NPC to keep him as Human minded as long as possible. He knows he's slowly sliding out of his norms and that scares him. I know I would.
I don't think that it scares him. It's hard to explain, but I've experienced something similar. At first, I didn't notice, but then someone scolded me for being insensitive. Then, someone I knew got hurt and I didn't care. I just knew that this was an unusual reaction, as I remember caring for him even in less important matters. I just found it odd. At the time, I was absolutely obsessed with my work, as I did my best to keep my j0b. However, I got ill more and more frequently, to the point that I was tested for auto-immune diseases. Still, I just felt tired, sickly, and somehow empty. Then, I got injured and everyone around me freaked out, whereas I remained calm and applied first aid to myself. It was like I was treating someone else to whom I had no real connection. I still felt the pain though. At that moment, I really noticed, that probably my most pressing issue was my state of mind... Not long thereafter, I was diagnosed with burnout and depression.
I say it is complicated... as in the spin-off or What-if story he still proved to be mostly the same if slightly detached, but otherwise a mostly good friend to those he called friends as he just enjoyed exploring the world. The issue is partly due to his guild NPCs. Who have a bit of a complicated history in following orders even when seen in the first volumes when their plan/goal was to lie low and not cause too many problems. As the joke goes - Ainz - Let's not kill children. NPCs - You heard him! Skin them and then feed them the cooked remains of their parents. Ainz - That is now what I meant.
Then remember he also has to give reasons for the excuses he gives to not be a total evil villain... the amount of good aligned NPCs are in the real minority... and the Good aligned people of the new world... ussualy tend to become the dead variety. Seriously we could have used the former Knight Captain to help give Ainz more good aligned people within the guild and the selfish human chose death without revive. That leaves only the village, the lizardmen, and a maid... as the remaining good, aligned people. Two people from the village might count as possible guild members and the maid... but that is a very tiny amount compared to the evil aligned new world people who joined/got conquered by the guild.
Ainz: don't kill children! NPCs: oooh we should fatten them up and slaughter them for food when they get older. Brilliant! Also "more scarier" isn't a thing, it's just "scarier".
Overlord is probably the only setting where (should you find yourself "lost" in Nazarick) running towards the monster with no face and a giant pair of blood-stained scissors _increases_ your chances of survival .. Pity it took Nigredo so long to appear in the anime, she's a character I like.

If Nigredo didn’t ask him, he would’ve killed all the children without mercy but only did so because he respected her opinion as another of Tabula’s creation. A lot of his acts of kindness so far were calculated such as the carrot-and-the-stick tactic. Remember, his Karma is just as bad as Albedo and Demiurge.
For all Momonga plans and counter arguments , most of them are not well funded with his overwhelming power , life spam and monitoring , it is just paranoia and the few desires to improve have more cons than pros , he is just becoming the monster people think he is , and becoming the ruler his legions always worship , truly a pity , supreme power corrupts supremely .

The actual people in the world or the random character's in the world mean very little beyond what they offer us. Ainz views everyone outside of Nazarick through how they benefit or hinder him and his own. Him being a literal undead that can't feel just amplifies that message. We don't truly care about the random people in the game world and Ainz doesn't truly care about the people of the new world.
