Hi Dave, I just watched the video. I, like you just had my prostate removed due to cancer and the pathology report came back clear. I had the catheter taken out yesterday, and couldn't do any kegel exercises because of a UTI thanks to the cath. Anyway, the amount of incontinence is surprising. I'm sure these videos will help out, thank you

I had my surgery 4 weeks ago. Doing pelvic floor exercises 7 days a week for 2 weeks. I have had limited success perhaps 10-15% improvements. Patience and self discipline are key

Thanks Eric. I had a TURP surgery in 2019. I was never told to do kegel exercise. This year only my urologist talked about kegel excercises. He said to watch videos to learn. I have recently had prostate surgery and I have struggled with incontinence, I mean really struggled !!! I’ve spoken with several different “therapists” and needless to say I am very confused on what I’ve been told. I never knew if I was doing the exercises correctly!!!just from watching this video….. what you have shown makes more sense than anything I’ve ever been told . Hi Eric, I am 69 and had a radical prostatectomy 7 months ago. I had to have my bladder neck stretched a week ago because my urine flow was very weak and my bladder was so irritated. The procedure was a success and of course the leakage has increased a bit. This the best explanation I have seen so far and makes absolute sense. I have tried and have given up because I had no success. I am sure this will work. Thanks for a great video.

This is excellent, thanks. The most helpful part is about how long to hold the contraction when starting out. As an older guy who has never done this before so this muscle is probably very weak, holding for three seconds or more, as I read on a few websites, was not possible without clenching my abdominals. So instead, I started holding it for a half second--like you recommended. It's good to have confirmation that short contractions like this are helpful when beginning.
Thank you, Dr. Eric! At 61 I benefit from this. I have little sensitivity and feeling in the fingers you demonstrate due to Carpal Tunnel but can feel the difference in release. Thats what i take away from this video,as well as the contraction process.
Learned a lot from your video. How many times a day? Seems like some guys say do it 10 times a day. But others disagree, saying overworking the pelvic floor can cause even more problems.

Doc, I tried this & pushing any pressure on my pereneum 8 weeks Following radical prostatectomy is still painful. That is a very sensitive area right now. So maybe that Isn't the best idea for me at this point. Is there another way to tell that you are definitely working the right muscle when doing Kegels? I've been following the nurse's advice since Surgery, but only going by muscle memory from when she had me hooked up to a readout machine. Today is the first time I actually probed and found my pelvic floor muscle with 2 fingers.
A good saying I heard that helps you to locate your pelvic muscle for beginners is " nuts to your guts." This means trying to lift your nuts (testicles) upward toward your gut (stomach). This may help a lot. It's very easy to visualize and helps you avoid using stomach muscles by mistake.
Thank you for this video. I am about to start prostate cancer radiotherapy, and urine leakage can often be a possible problem after so. I am going to start to strengthen the muscles before the procedure and hopefully avoid issues.

I have ED with 3/10 blood flow, very bad. Your kegel exercises are far better than any explanation I have watched or been told, thanks. I am soon starting my third treatment round of Shockwave therapy. Blood flow into is greatly increasing, however I cannot get near to being firm. And any compression very easily squeezes all blood out of my Johnson, so the valves in my veins are not properly closing at all. In my treatment I take many supplements, including sildenafil and tadalafil daily. This has greatly helped. But I am still nowhere close to being firm. And penetration is still impossible. Though I spent 37+ years being a parental Caregiver, working in my scientific field, but unable to date. With little personal time as well, and being so busy I would be awake 48-60 hours once to 3 times every week for 15-18 years. I am 67, and have recently been found by another for romantic and emotional dating. First. How do I get the vein valves to properly close? Second. Why are the two roots of the cavernosum, where the blood flows in, NOT treated by Shockwave therapy? This seems a failure in treatment protocol. If an electrical item is broken, you do not fix part of it and ignore the major area. Say where electrical input comes into the device. That is lazy, inept work. You repair the. Entire part or device. I can find no written information nor can any health professional explain this reason for not treating such an important area and input blood flow, including doctors, nor treatment to close the vein valves. ED doctors, professional treatment technicians, nor anyone in this industry. How can I get both of these treated and addressed? Everyone sloughs this off to "that is the treatment protocol" and discussion is dropped flat. PM me, as I have little free time to check everything. If you have the answers, you will have my extreme gratiude. Any reply is appreciated greatly, and thank you!
I’m here because I was actually researching proper breath support for singing. I found a video where the instructor said core engagement is improper because of excess thoracic pressure and that the proper way is by engaging your pelvic floor during exhales. It seems like there is a lot of truth in practicing that, but the part that gets confusing is that sometimes exhaling for singing can be upwards of 15-30 seconds. Which seems unrealistic or dangerous for a Kegel. Have you encountered a question like this before?
thank you Dr eric after having RALP at the start of lockdown i was sent home with a exercise sheet which i stuck to with little effect , i then had a month of radiotherapy in 23 as i had secondary, i continued to exercise but nothing was happening, i was sent to a physio and given more exercises but i still was not making any progress, i was getting down about it thinking that was it but came across your video and the way you explained it i thought give it a go not expecting much but i am 6/7 weeks in and i have pretty much dried up bar the odd leak twisting or bending which i am confident now i can stop continuing with your plan, thank you again doctor you will not know how much it means.

The tip of contracting the muscle after breathing out was precious: I could feel it extremely better that something is really going on down there. Now, I just wonder how many repetitions should I do and how will I know the muscle reached a good physical condition? Note that I don't have leakage problems and I intent to perform the exercises preventively.

I recently started a Rx for tamsulosin to treat an enlarged prostate, which actually does NOT cause me very many problems, and this Rx causes retrograde ejaculation. I'm older but fit and healthy and exercise regularly including weight training. Would your pelvic floor strengthening and stretching restore ejaculation through my urethra rather than into my bladder? Thx
Hey there Todd, it's difficult to say. I would try start with pelvic floor stretching first for the reason being, you already have the prostate surrounding the urethra impacting your ejaculation. The pelvic floor is also prominent in this region and if you have pelvic floor hypertonicity, this may be contributing to your problem. As always, discuss this with your doctor first but give pelvic floor stretches a try. I'm glad you found the video helpful. No, I'm a fan of not over complicating things. Focus on doing quality reps and I personally do not concern myself with the different types of muscle tissue. You can't even see the muscle so why worry about the types of muscle tissue?

Yes they are essential exercises to learn and master prior to your operation. Also after your operation you will have a catheter for a period of time and in that time you must not carry on with your normal activities such as golf, driving or excessive exercise. You will stretch the internals and contribute to incontinence. Stay home until the day the catheter has to come out. Relax.

Thanks for the good information doc. I have worse elections (once in a blue moon ) ,I can feel knots on either side behind the cap and the nerves are stressed ( I can feel them ) is it any exercise or any remedies ? , ejaculation is almost no or blocked , even my testosterone level is ok according to the doctor.
With every release , urine comes out as if a contra muscle is pushing the urine out. Dribling is there after every urination. Had prostatectomy 5 months ago

Hello Thank you for an easy to follow no nonsens guide. 44 year old ED man here. Do you have a guide for the infamous reverse kegel or stretch, do we guys need that? I know that you need to focus on both the contraction but also the strech in muscles to keep them limber. I'm from Denmark so hope you can deduse the meaning from my rather weird question.
So for someone looking to re-strengthen their pelvic floor - can you make a video with a "workout routine" and progressions with sets, reps and then eventually do kegel holds with sets and reps? Or if you can, what do you think of this routine below: Workout 3 times a week and start with: Monday, Wednesday, Friday for example 3 sets x 6 reps normal kegels for 2 weeks. 3sets x 8 reps normal kegels for another 2 weeks. 3sets x 10 reps normal kegels for another 2 weeks. So less than a month of normal kegels. Once your doing 3x10 normal reps of contracting and releasing, proceed to kegel holds workout. Same as above, workout 3 time a week: Monday, Wednesday, Friday for example 3 sets x 6 reps of 2 seconds hold for 2 weeks. 3 sets x 6 reps of 4 seconds hold for 2 weeks. 3 sets x 6 reps of 6 seconds hold for 2 weeks. Then, after this 2 month workout your pelvic floor should be pretty strong and balanced. What do you think Dr. Eric?
I think you hit the nail on the head here. I like your proposed routine, a couple minor modifications. In weeks 3 and 4 of your normal kegels phase, change up the positions ex: one set do it lying down, the next set do it sitting in a chair. In weeks 4 and 5 add in a standing position for you final set. DO the same progression with the holds. Let me know how you go and thank you for supporting the channel :)

Hey there Mark. I'm sorry to hear that. Yes, start the kegels as soon as possible and I would even get you to follow my kegels routine every day that progresses you through the positions and types of contractions. Doing this will reduce your leakage after the prostatectomy. Also, sorry for the bluntness of this comment but try to have 2-3 erections per week leading up to the surgery as erectile function takes the longest to return. Doing pelvic floor exercises 7 days a week for 2 weeks. I have had limited success perhaps 10-15% improvements.

Thanks Eric. I had a TURP surgery in 2019. I was never told to do kegel exercise. This year only my urologist talked about kegel excercises. He said to watch videos to learn. I was not doing it right and gave up. I really learnt how to find out the right muscles today. I will try the way you taught to work the right muscles. Thanks again for the wonderful video.

I have recently had prostate surgery and I have struggled with incontinence, I mean really struggled !!! I’ve spoken with several different “therapists” and needless to say I am very confused on what I’ve been told. I never knew if I was doing the exercises correctly!!!just from watching this video….. what you have shown makes more sense than anything I’ve ever been told .

Hi Eric, I am 69 and had a radical prostatectomy 7 months ago. I had to have my bladder neck stretched a week ago because my urine flow was very weak and my bladder was so irritated. The procedure was a success and of course the leakage has increased a bit. This the best explanation I have seen so far and makes absolute sense. I have tried and have given up because I had no success. I am sure this will work. Thanks for a great video.

This is excellent, thanks. The most helpful part is about how long to hold the contraction when starting out. As an older guy who has never done this before so this muscle is probably very weak, holding for three seconds or more, as I read on a few websites, was not possible without clenching my abdominals. So instead, I started holding it for a half second--like you recommended. It's good to have confirmation that short contractions like this are helpful when beginning.
Thank you, Dr. Eric! At 61 I benefit from this. I have little sensitivity and feeling in the fingers you demonstrate due to Carpal Tunnel but can feel the difference in release. Thats what i take away from this video,as well as the contraction process.

Doc, I tried this & pushing any pressure on my pereneum 8 weeks Following radical prostatectomy is still painful. That is a very sensitive area right now. So maybe that Isn't the best idea for me at this point. Is there another way to tell that you are definitely working the right muscle when doing Kegels? I've been following the nurse's advice since Surgery, but only going by muscle memory from when she had me hooked up to a readout machine. Today is the first time I actually probed and found my pelvic floor muscle with 2 fingers.
A good saying I heard that helps you to locate your pelvic muscle for beginners is " nuts to your guts." This means trying to lift your nuts (testicles) upward toward your gut (stomach). This may help a lot. It's very easy to visualize and helps you avoid using stomach muscles by mistake.
THis is actually my favourite "cue" to contract the pelvic floor too! Thanks for the comment Andy

I have ED with 3/10 blood flow, very bad. Your kegel exercises are far better than any explanation I have watched or been told, thanks. I am soon starting my third treatment round of Shockwave therapy. Blood flow into is greatly increasing, however I cannot get near to being firm. And any compression very easily squeezes all blood out of my Johnson, so the valves in my veins are not properly closing at all. In my treatment I take many supplements, including sildenafil and tadalafil daily. This has greatly helped. But I am still nowhere close to being firm. And penetration is still impossible. Though I spent 37+ years being a parental Caregiver, working in my scientific field, but unable to date. With little personal time as well, and being so busy I would be awake 48-60 hours once to 3 times every week for 15-18 years. I am 67, and have recently been found by another for romantic and emotional dating. First. How do I get the vein valves to properly close? Second. Why are the two roots of the cavernosum, where the blood flows in, NOT treated by Shockwave therapy? This seems a failure in treatment protocol. If an electrical item is broken, you do not fix part of it and ignore the major area. Say where electrical input comes into the device. That is lazy, inept work. You repair the. Entire part or device. I can find no written information nor can any health professional explain this reason for not treating such an important area and input blood flow, including doctors, nor treatment to close the vein valves. ED doctors, professional treatment technicians, nor anyone in this industry. How can I get both of these treated and addressed? Everyone sloughs this off to "that is the treatment protocol" and discussion is dropped flat. PM me, as I have little free time to check everything. If you have the answers, you will have my extreme gratiude. Any reply is appreciated greatly, and thank you!

I’m here because I was actually researching proper breath support for singing. I found a video where the instructor said core engagement is improper because of excess thoracic pressure and that the proper way is by engaging your pelvic floor during exhales. It seems like there is a lot of truth in practicing that, but the part that gets confusing is that sometimes exhaling for singing can be upwards of 15-30 seconds. Which seems unrealistic or dangerous for a Kegel. Have you encountered a question like this before?

thank you Dr eric after having RALP at the start of lockdown i was sent home with a exercise sheet which i stuck to with little effect , i then had a month of radiotherapy in 23 as i had secondary, i continued to exercise but nothing was happening, i was sent to a physio and given more exercises but i still was not making any progress, i was getting down about it thinking that was it but came across your video and the way you explained it i thought give it a go not expecting much but i am 6/7 weeks in and i have pretty much dried up bar the odd leak twisting or bending which i am confident now i can stop continuing with your plan, thank you again doctor you will not know how much it means.

The tip of contracting the muscle after breathing out was precious: I could feel it extremely better that something is really going on down there. Now, I just wonder how many repetitions should I do and how will I know the muscle reached a good physical condition? Note that I don't have leakage problems and I intent to perform the exercises preventively.

I recently started a Rx for tamsulosin to treat an enlarged prostate, which actually does NOT cause me very many problems, and this Rx causes retrograde ejaculation. I'm older but fit and healthy and exercise regularly including weight training. Would your pelvic floor strengthening and stretching restore ejaculation through my urethra rather than into my bladder? Thx
Hey there Todd, it's difficult to say. I would try start with pelvic floor stretching first for the reason being, you already have the prostate surrounding the urethra impacting your ejaculation. The pelvic floor is also prominent in this region and if you have pelvic floor hypertonicity, this may be contributing to your problem. I'm glad you found the video helpful. No, I'm a fan of not over complicating things. Focus on doing quality reps and I personally do not concern myself with the different types of muscle tissue. You can't even see the muscle so why worry about the types of muscle tissue?

Yes they are essential exercises to learn and master prior to your operation. Also after your operation you will have a catheter for a period of time and in that time you must not carry on with your normal activities such as golf, driving or excessive exercise. You will stretch the internals and contribute to incontinence. Stay home until the day the catheter has to come out. Relax.

I have worse elections (once in a blue moon ) ,I can feel knots on either side behind the cap and the nerves are stressed ( I can feel them ) is it any exercise or any remedies ? , ejaculation is almost no or blocked , even my testosterone level is ok according to the doctor.

I know that you need to focus on both the contraction but also the strech in muscles to keep them limber.

So for someone looking to re-strengthen their pelvic floor - can you make a video with a "workout routine" and progressions with sets, reps and then eventually do kegel holds with sets and reps?
Or if you can, what do you think of this routine below:
Workout 3 times a week and start with: Monday, Wednesday, Friday for example
3 sets x 6 reps normal kegels for 2 weeks.
3sets x 8 reps normal kegels for another 2 weeks.
3sets x 10 reps normal kegels for another 2 weeks.
So less than a month of normal kegels.
Once your doing 3x10 normal reps of contracting and releasing, proceed to kegel holds workout.
Same as above, workout 3 time a week: Monday, Wednesday, Friday for example
3 sets x 6 reps of 2 seconds hold for 2 weeks.
3 sets x 6 reps of 4 seconds hold for 2 weeks.
3 sets x 6 reps of 6 seconds hold for 2 weeks.
Then, after this 2 month workout your pelvic floor should be pretty strong and balanced.

I think you hit the nail on the head here. I like your proposed routine, a couple minor modifications. In weeks 3 and 4 of your normal kegels phase, change up the positions ex: one set do it lying down, the next set do it sitting in a chair. In weeks 4 and 5 add in a standing position for you final set. DO the same progression with the holds. Let me know how you go and thank you for supporting the channel :)

Hi Frank, it really depends on your situation. If you’re recovering from a prostate surgery, doing them a few times per day will help greatly.

I was just diagnosed with aggressive prostate cancer with a Gleason 9. I'll have my pet-scan done next Wed. Is this something I should start doing now? I'm leaning towards a prostatectomy due to the seriousness of my situation. Thanks.
Hey there Mark. I'm sorry to hear that. Yes, start the kegels as soon as possible and I would even get you to follow my kegels routine every day that progresses you through the positions and types of contractions. Doing this will reduce your leakage after the prostatectomy. Also, sorry for the bluntness of this comment but try to have 2-3 erections per week leading up to the surgery as erectile function takes the longest to return. Thanks for your video! Is it right that when I try do squezz the muscle down there that it „comes out“ a little bit. And when i let it go and relax it it just goes away? Is this the contraction that I need to feel?
