Is there hope for me? Here's my MRI Report. No significant adhesive capsulitis is seen. There is marked fluid distention of the subacromial/subdeltoid bursa which may be from the rotator cuff tendon tear and/or bursitis. IMPRESSION: 1. Massive full-thickness, full-width tear of the supraspinatus and infraspinatus tendons with medial tendon retraction. 2. Diffuse intramuscular edema of the infraspinatus muscle compatible with concurrent muscle strain injury (novel lesion of the infraspinatus). 3. A complete tear of the inferior fibers of the subscapularis tendon. 4. Subluxation of the longhead biceps tendon which is perched onto the lesser tuberosity. 5. Acromioclavicular joint arthropathy.

Rubbing alcohol removes all
I had rotator cuff surgery 4 years ago and have a bit of muscle atrophy. Anything I can do to reverse it ? I work out daily. Thanks
Have your ever heard of Bemer therapy, its a medical device that uses pulse electron magnetic field to stimulate specifically micro circulation in the whole body but also in helping post surgery recovery with really good results.
Rubbing alcohol will take it off and not damage the board
Hi Kristine, Thanks for the tip! Are you having shoulder surgery soon?
Hi. My surgery hasn't been scheduled yet. Waiting for that. 😢
I recently had a posterior labral repair with 3 anchors (5 30 to 1 o clock). I was having mild pain but when I sleep on my shoulder and woke up I feel mild clicking during the rotation, then it become alright once I start my regular activities like walking or doing regular rehab. But never had any dislocation, it it something normal or something I have to be worried about? My ROM is pretty good in all directions but feeling mild soreness sometimes
Hi @halfman.halfamazing3113, Posterior labral tears/repairs are tricky. Typically it does not result in a dislocation. Make sure you get with your doc so they can evaluate you. Good Luck!
@@rotatorcuffexpert I have dislocation it 2 times before getting the surgery, it was due to lifting heavy during dumbbell inclined bench press.
Posterior dislocation or anterior?
Im on day 4. Heading to therapy this afternoon and smelling like a month old water buffalo corpse. Ill tackle the shower today with a little confidence. I had the right one done twice, 11 years ago. This is rough. Thanks so much for the great help.

@@rotatorcuffexpert I'm in Australia 🇦🇺
@carol, Nice! I will be in New Zealand this december, unfortunately I will not be able to get to Australia on this trip
@rotatorcuffexpert Australia is hot in November December some states have dry heat some have humid heat.
I was diagnosed with full thickness tears on supraspinatus and subscapularis back in 2010. I didn't want to have surgery, so I treated it conservatively for 13 years. Last year my symptoms returned after some gardening work. I had a new MRI and it revealed the tears were now classified as massive. I had successful surgical repair straight away as I was concerned that doing nothing would lead to my condition becoming irreparable. I'm now one year post opp. Doing ok. My lesson is to first educate yourself on how to prevent rotator cuff tears, then put that information into practice. The whole experience has been a 14 year long nightmare. In the end, I felt like surgery was really my only option left on the table. Prevention is better than cure. Tread your own path. Thanks for another great video doc. All the way from Tasmania, Australia. 🇦🇺
@naturevideos.8802, Thanks for the comment, excellent perspective. Great to hear you, especially all the way from Tasmania. I'll be in New Zealand in December, but unfortunately will not have time to get over to Tasmania
Hi Dr, one more question, the surgeon is concerned that the bicep pulled away from the bone after tendonesis ! (Muscle seemed to drop a little) what would be the concern if I’d actually did? Surgeon wasn’t too concerned if it did pull away
Stefano, Well if it pulled away then it would no longer be attached. It could effect the shape of your biceps as well as may make it easier to cramp.
@@rotatorcuffexpert thank you
Good Luck Stefano
I had medium sized tear in my labrum that I had surgery on a week ago. Today when I took my sling off to shower, my shoulder felt unstable for the first time, it hadn’t felt like that since I started taking it off on Monday to shower, is this normal or did I injure myself somehow?
Hey Adam, Great question. It doesnt sound like you had an injury after surgery. It may just be weak because of the surgery. May sure you check with Surgeon.

Does a No retracted full thickness tear mean there is no tear?
Hey Tim, good question. No a full thickness tear means a tear all the way through a portion of your rotator cuff. No retracted means it is torn but not pulled away from the bone. Retracted means torn and pullled away from the bone. Hope this helps. Please consider joining our newsletter. Just email me at dro@mybodyprotector.com and put yes in the subject line and we will get you set up with a few tip sheets as well!
Do you have any opinions on a SLAP tear repair not holding up? It’s the same injury for a second time. Not sure what kind of solution there may be.
Hey Chris, great question. Yes sometimes especially when we get older a SLAP tear may require a biceps tenodesis not necessarily a SLAP repair, I will link to my vidoe on it. Please consider joining our newsletter. Just email me at dro@mybodyprotector.com and put yes in the subject line and we will get you set up with a few tip sheets as well!

@@rotatorcuffexpert I watched the video, thanks for the information. I’m only 38 years old with a very physical job and the first repair lasted about 10 years and this time the rotator cuff is involved as well. Are there any circumstances where a shoulder replacement is considered? That’s my biggest concern at this point.
Chris, Tears in the labrum or rotator cuff typically do not lead to a shoulder replacement. Hope this helps

Can you research your rotor after you go back to work?
Hey victoria, Sorry I am not sure what your question is
Sorry that is supposed to say re-tear your rotor after going back to work after surgery
Ok Victoria, Yes unfortunately you can retear the rotator cuff either at work or at home
@@rotatorcuffexpert Thankyou for your very quick reply.
So for a shoulder partial tear prp 3x shots yes or no ? And will any anabolic steroids work or the quality do the healed tendon might be weak ? Cause as we know so far testerone steroids sort of drys up the tendons
Hey @semaj.18272, It may work. I don't think anaboic steroids are supported
Nail polish remover will remove the sharpie marks 😁
Hey Chris thanks for the tip. Are you having surgery soon?
Yes. It's been a year since my surgery and I still wish they had made better after care plans. That surgery was not a walk in the park. I toughed out 4 shoulder procedures with only tylenol because the night after I had my surgery my O2 dropped to 70 from the pain meds. I literally had no aftercare. Fortunately I survived but still with residual issues from anesthesia and trauma. I finally got an ASV machine for central sleep apnea and am beginning to sleep again. The CPAP was not enough. I wish I had seen all of your videos before I had the surgery. I might have been better prepared. I learned never to allow surgery without having my cpap for the recovery room. I was told it was not necessary.
That is tough for sure pamela! Glad you made it through. Thanks for being part of our community
@@rotatorcuffexpert It is good to be honest beforehand with patients. I was told I'd be fine in a few weeks and able to do most things. I guess they knew I'd refuse if I knew how tough shoulder surgery really is.
Yes but a patient that has been given false information before surgery becomes a frustrated patient after surgery. That is no good for anyone
My surgical team said the ice machine will be delivered to my home on the day of my surgery. Why can't I pick it up at the hospital?
It all depends on the hospital and insurance plans
My doctor only does shoulder surgery and elbow. Read online review. It seems all his surgeries are rotate cuff repair and shoulder replacement. Is he good enough? I was scheduled in September.
Sunflower, it sounds like he does the right stuff!
@@rotatorcuffexpert Thanks a lot. I watched a lot of your videos. I understood all the cons and pros. My doctor let me make the decision. I decided to do it.
Great, good luck! Pls let us know how you are doing.
Thanks to your videos, I didn't make any mistakes. I am 7 months post surgery and all came out well. I couldn't get the nerve block so I used an ice machine to manage pain. They did give me Oxycodone and also Naloxone that rapidly reverses opioid overdose, without telling me what it was for. I used Oxy for 2 nights to help me sleep and it was tylenol after that. If any of you have been wanting to get an adjustable bed, before surgery would be the time to get it. Sleeping in a recliner was not easy for me. They sell adjustable bed platforms now that fit in your current bed frame under your current mattress. I did get one two weeks after surgery and it was a Godsend! I wish I would have bought it before surgery and avoided the almost two weeks in the recliner.
@thenewmexicoman, Thanks for the comments! Glad you are doing well!
Where I had my surgery in North GA, they won’t give you your prescription prior to surgery, so I took the list to my pharmacy and check to make sure it was things they normally had.
Hey Candace, so sorry they won't supply them early. Hopefully your pharmacy will have everything you need. Thanks again for subscribing!
Thank you for your videos. It is amazing how little preop patient education there is for this surgery. I am literally begging for everything you mentioned. And, they will not provide prescriptions prior to the surgery.

You're so so right on all of these points. Must say, I failed miserably on every one of these point forty years ago. The long story short, my operation has been a mess from day one. Now, after a bad fall, my shoulder as been again damage, tears from the hold chop-up operation + apparently, a few new one's... I just can't find the right surgeon who will give me the right procedure to follow, like I told you before, now my shoulder is constantly sub-luxating and I can feel the bones rubbing. I prefer finishing my day's on morphine than going through a shoulder replacement as the surgeon seemed to suggest in last result. I've made peace with all of this and will simply live my faith as it is. Thank's so much doc for your clear and simple explanation.
Oneom8158, I wish you the best! Thank you for your comments

@@Dsuji245 Do you really believe this ? What aspect of medicine, education, politic,... this collective social soupe that humanity has become, do you believe it to better today than 40 years back ? I would love if you could post a video explaining partial-thickness intrasubstance supraspinatus tears. I have a small partial-thickness intrasubstance tear of my rotator cuff in both shoulders. Will the tear always show up on an ultrasound even years later as scar tissue? Thanks so much for all of your videos!
@michaelbasilio2281, I know it is confusing and I will do a video discussing that specifically. In general these types of tears are are within the tendon and so there is nothing to "close". far as Ultrasound, it is not my favorite imaging as MRI is so much better for quantifying tears.(IMO). Hi, I have a left rotator cuff tear that happened two years ago. Pain and ROM is getting worse. Right shoulder bone on bone AC joint. Elbow pain along with that . I have weak , not much muscles in my arms. I probably have some atrophy, I am 48 and beyond scared of surgery . I already backed out of my right shoulder surgery 2 yrs ago, then i injured my left shoulder. I cant take more than one norco for my pain now, so to take more for shoulder surgery, is really considering for me. I have a weak stomach, also suffer with diverticulitis. Is there any other way pain can be managed?? If arms are this weak, how can i bounce back?? If ever?? I have never been this petrified of surgery, also it being my dominant arm, taking care of myself, eating ,writing, working, i am just full of pain agony and worry daily. Thank you for listening and Thank you for any advice or encouragement you can give me.
@stacy1477, So sorry to hear about your shoulder(s). I understand why you are scared. This surgery is tough and unfortunately a long recovery. I think you have a lot of great questions that your surgeon will need to answer prior to surgery. There are a lot of other meds that may work better for you than norco. In total shoulder replacement, do they remove the labrum, capsular ligaments and all other stabilizing tissues besides the rotator cuff tendons? I assume the deuce will take the place of those but cna it really since to me it seems without a labrum and capsular ligaments there will still be some instbaility or more accelerated wear and tear to the shoulder replacement parts.
In a reverse total shoulder replacement the labrum is removed to allow the placement of the glenosphere. There is no attempt to remove the capsular ligaments or the rotator cuff. The joint is a semi-constrained joint where much of the stability comes from the design of the implant. I hope this helps batman
I played volleyball for 30 years and last year I suffered from a full shoulder dislocation. Months later. an MRI showed that I had full tears of my rotator cuff, infra, subscap, and teres minor. The surgeon said when he went in, they were all torn with retraction. I'm 5 months into rehab. I'm turning 46 in September. I hope to play ball again in 2-3 more months. Great video. Good info.

Great video Doc. There are some TH-cam content providers claiming that large full thickness rotator cuff tears can heal naturally without surgery. Thanks for clearing up this confusion. I had surgical repair to Supraspinatus, subscapularis about a year ago. This video helped me to understand the reasons for the necessity of my surgical repair. Better to get it repaired than wait until it's not repairable. As always though, prevention is better than cure. Thanks again. All the way from. Tasmania, Australia. 🇦🇺
I’m 4 days post op for bicep tenodesis and slap tear repair and I needed this video! Thank you!

I just had my second total shoulder reconstruction and rotator cuff repair. Had both of them done in under 8 months. Nicest thing Ive ever done for myself as an adult. I'm a new person it's changed and took 25 years off my life.
That is awesome Todd. So glad you are doing well!
@@rotatorcuffexpert thanks it wasn't a big decision. I was tired of all the cortisone shots sick of not being able to go fishing, found a great doctor we both were on the same page and it's been wonderful. Yes I had some pain and had several weeks of physical therapy but that's minor. I have both arms back they feel great and no more pain or shots anymore. I sleep great my surgery increased my standard of living, and quality of life. Boy medicine certainly has come a long way since we were kids.🌟🌟🌟🌟🌟
I am glad you are doing well.
You use rubbing alcohol. Done.
yes it can heal if its 50% or less 5:55
@mangooooo33, you are right, it can heal. What does 5:55 mean?

@tondog9070, thanks for your tip! Are you having surgery soon?
@@rotatorcuffexpert +12 days post surgery, torn SST, long head of bi & labrum 🤤, was thinking I may go my whole life sans any surgery but oopsie caught an edge snowboarding so guess I'm human after all...the injury didn't feel all that bad but as I began to get ready for surf season my ROM was real limited as I began to practice popping on a practice board in my living room so I knew something was amiss...hoping I'm able to return to the line up and paddle out sans pain in a couple of months...hey there bigger problems in the world but this is my little problemo 😆

Try to draw over the sharpie with the dry erase marker and wipe immediately. That will sometimes get the sharpie off!
Thanks for the tip, are you having shoulder surgery soon?
@@rotatorcuffexpert August 1st, I’ve been trying to research what to expect after and practicing everything one handed as it is my dominant arm getting the surgery.

Great video. Very informative. Thank you Dr. Orcutt.
Thanks Pammers, I hope you are doing well!
Thank you so much for your videos. It has really helped me with my shoulder surgery. All the tips are spot on. Really appreciate
Meow, I am glad we could help. How long has it been since your surgery?
Dec ‘23 I tripped & dislocated my shoulder, at that time I had a 2 cm tear, but last weeks MRI shows it’s 3.3 cm now. I’m scheduled for rotator cuff surgery in 2 weeks. The MRI said nothing looks like it’s died off, so I pray everything goes smoothly & I should be moving my arm around within 24 hours. I’ve been doing PT this whole time so that has helped a lot with my mobility.

What happens if TENS treatment and PRP treatment are taken at the same time?
@aqua22000, as far as i know there is no contraindication to doing them both. Are you having shoulder trouble?
@@rotatorcuffexpert sadly ı got a intrasubstance supraspinatus tendon tear. well i think tens decrease inflamation but prp increase it, so that didn't make sense to me ıdk, what do you think about that?
I think TENS can decrease pain not necessarily inflamation, PRP is supposed to provide healing "elements" to a less than ideal healing environment. This does cause what is supposed to be controlled inflamation.
This is helpful-I have been around the block with a shoulder fracture, osteonecrosis, frozen shoulder, cuff repair, another frozen shoulder, and finally a reverse replacement on my dominant arm. The no -dom arm suffered a dog pull about four years ago and the second round of PRP injections have done little. I had several second opinions and a couple of them indicate that surgery for AC joint arthritis, bone spur under the acromion, and biceps tendinitis would be the next step. Needless to say, I am reluctant to jump off that high dive, but preventing a rotator cuff tear seems to be a good idea. Good food for thought here!
Elizabeth, thanks for the comment. And thank you for subscribing! I understand your reluctancy considering the difficulties you had with your other shoulder.
Was watching a capsular release surgery video and was a bit confused. It seems they cut the capsular ligaments SGHL MGHL, IGHL, CHL. But are they like divided in half in the middle or are they all resected from their insertion sites and just kind of like dangle off not functioning anymore? And also mentioned was intraarticular subscapularis release so is that like removing deep layer of the subscapularis tendon?
@heyintegrity, Typically it is divided in half. The subscap release is usually the capsule being released
7 months post rotator cuff repair. Repair is good but now have frozen shoulder. In your experience what treatment methods are most effective to regain ROM.
Hey @huskercrue, thanks for subscribing. So sorry to hear that you have a frozen shoulder. The options typically are PT and or Manipulation. Talk with your doc about your options.
