ACL tear management: surgical vs conservative & graft options
Hi everyone! I wanted to make an informational video explaining the options for management of an ACL tear. Technically you can manage this injury surgically or conservatively (non-surgical), depending on severity of injury and general activity level. If you choose to have a surgical reconstruction, there are several different graft options that I discuss in the video, along the the pro's and con's of each. As I state in the video, I am not a doctor yet and none of this is meant to be medical advice--it is important that you discuss your individual situation with your physician and come up with a plan with them. I hope this video will equip you with the background knowledge to have an informed discussion with your physician.     Hi Megan! Thanks for another informative video. I'm also recovering from the same surgery and facing a scary situation atm. At 5 months post-op, I have only about 100 degrees of bending and might need another arthroscopic intervention (or manipulation under anesthesia) for scar tissue which seems to be what's blocking me. In one of your videos, you mentioned you faced a similar situation but you ended up not having either intervention. Do you have any advice? Did you increase your bending exercises, push through the unbearable pain, or use a lot of other techniques like acupuncture? Thank you so much again !

      @@goharsharoyan2173 hi! Sorry to hear you’re going through that too, it’s definitely a mental and physical struggle. To be honest, I don’t know if there was one specific thing that helped but I did try acupuncture and would push through a lot of pain at PT-however if I pushed too much I noticed that it ended up limiting my bending more so it was a fine line. I think those surgeries are very helpful if your bending is truly not improving and your doctor thinks they will help. I got to the point where I was willing to do it and then right at that same time I started to randomly have some improvements in my bending. It’s so different for each person but I’ve talked to people who had MUAs and were very happy with the results. It comes down to you and your surgeon deciding what would be best for your individual situation. Best of luck, I’m sure it will all work out!

    Had my surgery 2 days ago, as soon as i woke up in the post operation room i was in so much pain i almost cried and there was a guy beside me that had the same surgery and we woke up around the same time and he said that its not even that painful and that i was over exaggerating, we came to find out that the spinal anesthesia (the nerve blocker) worked for him but didn't work for me.

    To everyone in this comment section who may be worried that they are behind on recovery, and comparing to other videos such as this one on youtube and other platforms, do not worry, everybody recovers at their own rate. I had an ACL reconstruction with ITB tenodesis, and medial mensicus repair 12 days ago and i am at 110° bending at ease. I was off crutches after 5 days, however i have a close friend who only got off crutches after a month and only managed 90° flexion after 3 weeks. He is also now fully recovered and playing sports again. Take this as a note to yourself to not rush and to recover at a rate that u feel comfortable with, and that everybody has their own pace of doing things.

    Sorry I have a question, why did you say you’re not supposed to bend past 125 degrees for atleast a year? Sorry was just curious as I’m almost 3 months out and only at 120, and have a goal of about 135 that I really want to get to asap. P.S. you are awesome and your videos have been keeping me afloat for since my surgery a couple of months ago. Thank you you’re content I’m sure has touched and helped a lot of people like me I’m sure

      Thank you so much for your kind words, I'm so glad the videos have been helpful! As far as bending, I was told to try to avoid bending my knee past a certain point due to my meniscus repair, because when you bend your knee past 125 ish it puts a lot of strain on the meniscus that was just repaired. With that being said, I have definitely bent my knee past that point over the past several months so I am mostly trying to avoid load bearing exercises with my knees super bent.     thank you for giving me hope, been in tears for half a year since both my bend and straighten is catastrophic. after my OP the extension is way worse then before (it wasn’t good before either) and my bend is also way out of hand. feeling so down and question my life every day, your videos give me little more hope maybe i will get my life back. i hope…

    It’s day 2 of acl and meniscus surgery and I can’t bend for nothing😢 I can straighten it out pretty good but the pain is so bad I can only sleep for 2 hours then I wake up

    I’m on week 5 post-ACL and meniscus repair and I can’t wait to start walking without crutches again. I admire your perseverance! I’m still having trouble walking with one crutch but very comfortable partial weight bearing with 2.

      crutches are the worst!! I was so happy when I could finally ditch them, but I'm glad I waited the full amount of time before walking again to give my meniscus sutures, graft, etc enough time to heal. Best of luck in the rest of your recovery!

      I haven't really tried yet, I think I'm still a bit nervous to try any sort of contact or cutting sport. I have been golfing but that's not a super demanding sport on the knees!

      It's so different for everyone and I found that sometimes when I pushed it harder it would bend less because it would get aggravated and inflamed. So, I would just follow the advice of your PT and surgeon and it should work itself out eventually! Good luck!

    Thank you Meg! I had a patellar tendon graft for my torn ACL and also a meniscus repair about 7 weeks ago. I went to an urgent care after a rock climbing accident and they had me in an immobilizer for the 2 months before my surgery and non weight bearing, so I had really poor movement going in to surgery. Currently I only have about 50 degrees of flexion on a good day. I changed physical therapists about a week ago, and the new one has me on a more aggressive plan to work on the bending. I had a hard time finding anyone who struggled with the flexion, so thank you so much for posting about your journey. I think I just need to keep plugging away and bending as often as I can. You’ve given me hope for the future!

    so how long's your total recovery until you go back to playing sport? How's your quad feeling usually?

      Usually they say around 9-12 months before returning to sport. I haven't gone back to sports that involve cutting or jumping yet, as I haven't felt totally ready (I'm at about 10 months post op). But, I also wasn't able to go as hard with my PT in the past few months since I have been very busy with school, so I think my recovery has taken a little longer due to that. My quad feels pretty good generally--I have to make sure I am always stretching/foam rolling it to keep it loose (otherwise I can get some knee pain), and after I run/go to the gym I generally have some knee pain as well. Still trying to strengthen my quad to hopefully minimize this pain.

      I'm not quite sure what this is so I'm guessing I did not experience it. I would definitely check with your doctor or PT if you're concerned!

    Scapholunate ligament tear here. I guess it’s easier since we don’t walk on our hands, but yeah it sucks too. I’m having a surgery in a few weeks and with sl tears and after the surgery, there is no chance for full recovery…and that’s what I guess makes me feel down, knowing that my wrist will be changed forever soon

      It's definitely not the same as pre-injury still, if I run or do a lot of walking it will get sore/stiff and I do still notice myself favoring it. I'm hoping this will continue to improve over time

    I am going thru my first week taking it day by day my pain in my knee kicked in after i took off my catheter i really felt the difference of level of pain but after taking my pain meds and also taking them before my pain kicked in i was great . Another thing i really got scared was when it took me 4 days after the surgery to be able to use the bathroom #2 kinda embarrassed but it is something i got to say because some of us felt this way well i did 😂 other than that i still haven't went to rehab yet i go in two days but ive done some movements and exercises just taking this journey day by day. Your videos are really helpful also your Beautiful 😍.

    Iam on day 4 post meniscus repair, watching ur really interesting videos, just asking when did u start going to PT as my doctor postoponed it till i get the sutures out, thanks for sharing ur experience

      I believe I started PT about a week after my surgery. My doctor said I could've theoretically started the day or two after surgery but I was in too much pain. I think it's different depending on the type of surgery/doctor preference!

      @@megmed2860 thanks I've have waited six years for this I was playing soccer when I tore my ACL I was running then I ran into a hole in the grass and all I heard was a loud crush sound and a pop

    Damn still limping after 3 months. Is that normal or was your recovery slower than usual

      I was definitely not walking totally normal at 3 months, I'm pretty sure I still had a limp (especially starting to walk after sitting down). Best of luck with the rest of your recovery, just take it at your own pace!

    Hey hun, I wanted to thank you for all your videos, they have really helped. Had my surgery 18 days ago, these have helped me to try to stay positive. I had an allograft done (I’m 37), never really felt much pain, today I’m in a little pain (my first PT session ☹️). I know it’s a long recovery but if I want to get back to snowboarding and weightlifting I have to push myself. I’m glad to know it does get better and I should be able to get back to normal I just have to give it time. Thanks again ❤

    Man kudos to you for going through this and at the same time going to school. I am just past 6 weeks po and also have to study for my job and I haven’t Lmaoo my flexion also wasn’t too good and the PT cranked it and that pain was EXCRUCIATING. I’ll take that over getting the second surgery tho !

      I was non-weight bearing on crutches for 6 weeks!

    I only got my ACL reconstruction with a quad graft and I’m unbelievably grateful (and surprised) that even today being exactly a week after surgery, I have felt zero pain from the surgery from the day of till the time I’m typing this. The only pain I have is the brace rubbing against the bruising from my thigh causing a painful pressure point. I also was on top of general pain meds from Motrin, Tylenol and baby aspirin for blood clots. But even my dad tore his ACL multiple times and also got a full knee replacement and he told me he’s shocked and jealous that I have no pain (incision wise). Decided to stay at my dads while recovering considering he’s been through this before and he knows how to help me so I’m very grateful for him

    i am about 2 months into physical therapy and my range of motion is still about 90° when lie down and around 100° on a chair . Is it okay or my recovery is going slow?

      The timeline is really different for each person, so it's hard to compare. It sounds like you're doing pretty well though! If you have concerns about your progress I would recommend talking to your surgeon or physical therapist. Best of luck in your recovery!
