And "kiwen" is any "hard object, rock, metal (not in a musical sense though for these 2)" and since there are no words in toki pona for "pills", it's an appropriate way to describe them. There are also no grammatical tenses in toki pona. So the sentence "oko mi li pona" can mean "my eyes are fine", "...were fine", "...will be fine", "...will be fixed", or even "...would have been simple". Context should help the listener/reader getting all the nuances, but a song is a short form and different people may understand different (conflicting) meanings. The whole "message" of the song I guess is that mental health is important and taking your prescribed treatment is helpful even though it may seem "unnatural".
