Fallout 4, and the wider Fallout Universe, still leaves a lot of mysteries and secrets. One of these takes the form of a question: Why do Ghouls go Feral?
Feral Ghouls:
One of the msot savage creatures we encounter in Fallout 4, and the wider Fallout universe. However, many, indeed most of them, didn't start out this way. Their skin may bear the marks of radiation, and many of them are ancient, but we encounter many Ghouls who are perfectly sane. Why then, do some of them go feral? What acts and the catalyst to this process? Why do some ghouls, like Zao and Oswald, last for so long? Is it because of Ferocious Post-Necrotic Dystrophy, as Barrows in Fallout 3 hypothesizes? Or is it down to genes, like Billy Peabody's parents seem to think. The exact process may also be a contributing factor, as ghouls like Eddie Winters and Desmond Lockheart intentionally underwent the process of ghoulification, and after centuries they still seem fine. We will explore these and other questions in this episode.

@@damenwhelan3236 Doctors at hospitals will put patients to sleep, even induce comas in some in order to speed healing. But EMTs at the scene want an alert patient because they are invaluable in helping to assess the extent of an injury as well as the method of injury. Even if their alertness might risk shock setting in, shock is treatable, but if someone is unconscious, lying at the bottom of a staircase, they will take time assessing broken bones or spinal and head injuries, maybe even electrical shock, and not that the person might have had a heart attack or stroke without even putting one foot on a step.

I think there's a point that could help explain part of the feralization: trauma. For this, look to New Vegas and the Bright Brotherhood: it's very heavily hinted that the feral ghouls you find at the REPCONN site were those who went feral after the attack by the nightkin. Harland, the single ghoul survivor of the attack not with Jason, is a survivalist who likely has skills to deal with the stresses of attack as well as possibly the same genes the Peabodies have.
This ties right to Billy in another way, because it's a well documented that children can sometimes be very good at recovering from trauma in ways adults can't. Perhaps the very isolation that drives other ghouls feral acted as a cocoon of sensory deprivation that relieved some of the impact the Great War had on a ghoulifying Billy.
Just a thought.

For that, I have a great large heaping of "Fuckifiknow".
There’s also Nuka World Kiddie Kingdom, where Oswald The Outrageous is Rachel Watkins is his girlfriend. From reading his wiki Oswald is one of the original survivors from the band of employees that took shelter in Kiddie Kingdom during the bombing and immediate fallout from the Great War. After time had passed, the entire group began changing into ghouls due to radiation leaking in. One by one, the survivors began turning feral, leaving Oswald and Rachel as the last non feral ghouls in Kiddie Kingdom. He took on the role as a protector of his former friends, strongly believing that feral ghoul is a sickness (that he and the other survivors via terminal entries call “the Affliction”) that can be cured. He is in wait of one of the original survivors Rachel, whom he believes will return with the cure someday.
Just letting you know that was Oswald’s wiki. He’s just dangerously deluded. Now onto Rachel’s wiki which was touched upon in this video.
Among them was Oswald Oppenheimer, a stage magician. The two became fast friends and then lovers. Last part on the wiki, However, even with Oswald’s abilities, the affliction continued to spread. By 2286, only a handful of sane ghouls were left and Rachel exhausted every avenue of research. In desperation, she decided to head outside the protection of Kiddie Kingdom to seek a cure. Her first stop was Bradberton. From there, she was headed deeper into the wastes. Her search was for naught, as she found out time and time again from countless towns she visited that no cure existed and ghouls simply go feral eventually. Distraught, she returned to the valley, only to find out herself was deteriorating. Unwilling to become danger, she holed up in one of the buildings in Bradberton. After recording a final message to Oswald, she took her own life, ending it on her own life.
So even if you do become a non feral ghoul overtime you still start to deteriorate and become a feral ghoul.

As to Billy, it seems that gouls can go into a hibernation state. He probably had some food in the fridge, but he most likely spent most of that time in hibernation only waking up periodically when he heard sounds.

I'm of the mind, it's a combination of things
1. Isolation can increase the chance. Ghouls isolated, for the most part, will have less mental stimulation, leading long term to those higher brain functions to essentially shut off
2. A mental anchor can stave it off. Rachel played a good example. She and Oswald had remained hopeful of a cure, of something to save their friends. The others in their group slowly lost their hope and went feral. Rachel did as well upon hearing what she was told, while isolated from Oswald. There's also the scientist Cito met in Safari World. He had his work to keep him occupied.
3. actual food can also stave it off. Ghouls with a regular source of food maintain themselves on it and don't need their own biology eating away at them. The body possibly is shutting off higher brain function to reduce the caloric needs if they don't eat often enough. Note how a lot of ferals in 4 are sleeping until you disturb them, or hunched over something eating voraciously.

What about the Vault Tech Salesmen? He seemed to be full of despair when you first meet up with him again in Goodneighbor. But he wasnt showing any signs of turning feral. Would that be more of the genes kicking in to keep him going?
This is just my own personal take on it, but I think the Vault-Tec Salesman probably kept from going feral due to his own exaggerated sense of self-worth. Like, when the army guys refuse to let him into Vault 111, he goes full “do you know who I am?!” and he’s still arrogant enough to yell threats over his shoulder at the army guys as he flees. Between him, Eddie Winter, and Dean Domino from Fallout New Vegas, I’d say that an over-inflated ego might serve to keep someone from going feral. Like, they think too much of themselves to lose themselves, or something like that.
TL;DR: If you’re a ghoul and you want to avoid going feral, you might be able to substitute being a Narcissistic Son of a Bitch for having friends.
Perhaps he was on the road to it, and the genetic fortitude was still decently intact. Despair and other chemical imbalanced, probably play a role in how fast the mind breaks down, and the more terrible, the more the brain is releasing a certain type of chemical to cope--which then undermines the new fragile structure the mutations have done. Tough enough genetics could get you through that, but rust never sleeps.
I personally have a theory that sleep plays into it, believe it or not. Sleep is the body's way of repairing the brain and body- ghouls are naturally regenerative in the body, but that doesn't extend to the brain. most people who are traumatized struggle with sleep deprivation, some don't. Hell, some ghouls may not even know about these points and be like me, thinking sleep is a waste of time and choosing not to do so now that they're regenerative enough to be fine without it, as far as they know- ever notice how non-feral ghouls can be found sleeping but the feral ones are always awake regardless of the hour?

The psychological theory still holds up for places like camp searchlight. The NCR troops stationed there were already pessimistic about the outcome of the war, so when they became ghoulified they gave up and let their minds be overcome. Private Edwards did not become feral because despite his insistence he has no place left in the world, he still wants to live, as evidenced by his fear of the radscorpions and convincing him to be stationed at Echo. Also, people like Eddie, Zao and Edward Deegan had ultimate purposes through their ghoulification: Eddie wanted to lord over Boston, Edward served the Cabbots and Zao wanted to bring his men home, while someone like Watkins was convinced she had nothing left to live for. The only ghoul I can think of that ruins the theory is the Vault Tec Rep, who is completely depressed and rejected but is obviously not feral.
Tl;DR, willpower is a good theory and there are too many people named Edward or something close to it in Fallout.
It could be that the Vault Tech tech rep simply had a sub conscious want to continue to survive.
Gopher this seems likely given that despite his apparent despair in the two hundred years of life he experienced, he never went to the point of actually committing suicide, and as well, there could be a drive to be productive, as offering him productive work in one of your settlements will help him get over it.

Moira browns transformation is a pretty good indicator of radiation being the cause. She hadn't had FEV exposure but blowing up megaton changes her.
I think the trigger for going feral is just normal everyday dementia. We're already living past our natural lifespans and started seeing dementia more and more because of it. Guouls are long lived so it could just be that simple.
I was starting to feel hopeless when the 2 year mark rolled around but thankfully you are back I hope to see many more in the future
