Awww thanks Shauna! Yes, we're not into taking the fast and incomplete approach - surgery. Of course, it is needed at times, but not always necessary. Our culture is so quick to go there and think of it as a final fix, but that's just not the case all the time!
Thank you for this information. You described my situation to a “T.” I had left ACL reconstruction some years back (complete tear after a jump kick - I’m double jointed). Then a subsequent surgery due to becoming septic in the joint. I also slightly injured my right knee but the concentration of course was on the left. Fast forward 20+ years, lots of physical therapy, and weight gain, I have osteoarthritis in both knees. Left knee is bone on bone. So as you can imagine, over the years, the right knee has been over compensating. Recently, the right knee gave out when standing. I heard a pop and felt a slight burn. My orthopedic doctor took X-Rays, did a physical exam, and gave me my second ever corticosteroid injection. I also had an injection in the left knee recently but declined additional injections due to the results of my own research. But because of my symptoms and no range of motion in the right knee, I agreed to the injection. I have a high threshold for pain and don’t usually present with textbook symptoms. However the doctor declined to order an MRI. Now I am experiencing all the symptoms you described in this video. Thanks to you, I will be able to have a more well informed discussion with my doctor, regarding my current situation. You are certainly living in God’s purpose for your life. Continued blessings!

The best explanation that I hv heard in distinguishing various knee pain symptoms. I thought my popping sound was arthritis related but nw know it is meniscus related. I fell from my bike and fractured my femur. Hence, I underwent hip (bipolar) surgery and was bedridden for over a month. On Dr advice, I began walking with aid. But, since I was limping on the operated leg I may hv injured the meniscus. I saw your video where you adviced to walk with your foot outwards and it really helped. What other exercise can I do to naturally heal the meniscus?

I had a meniscus tear when I was 65 (it did heal) but I’ve often wondered how or why it tore in the first place. Please advise I am 55 years old lady 2019 right knee meniscus tear also due to hypothyroidism I have weight gain issues. So, I recently got my knees examined by my Primary Care Physician. Long story short, I've played basketball all my life through college and even played some soccer. However, I didn't have any injuries throughout that time period. It wasn't until Winter of 2019 when I smoked my knee on the edge of the bed, in the middle of the night, that I noticed I had pain. Nothing really substantial until last year when I began playing some golf that I started having to wear knee braces. After I play I'll have a dull ache for at least a week until I play again and recently I've started to notice it more -- which leads me to believe it's probably cartilage damage. Is there anything you would suggest I do to alleviate the pain? I've just started incorporating tailgate swings with weights. Can I still golf some with the proper knee brace? I'm only 33 and consider myself to be in pretty good physical condition. Sorry for the long post. Thanks, Seth
Hey Seth! So sorry to hear about your knee situation. It's frustrating when it seems like the injury came on with such a trivial thing like moving around your bed. You likely have a muscle imbalance that is catching up with you now that you're out of your 20s. Golfing should be ok as long as you start to get your glute strength up. When I had a 4th 5th lumbar disc issue, I had sciatica, and pain all around the area. As the injury healed the pain got confined to the exact site of the injury. That is what my meniscus injury is doing now. The injury was not a tear, but possibly the start of one, was what the surgeon said, but how can he know that, if there was NO MRI? I got a shot. It lasted many months, then the pain returned, and was exacerbated by a HBP drug that made both legs swell. NOW...I HAVE BEEN doing your exercises, mostly the tailgates, because I wanted to start out gently. But, now that the pain is less, it is just like the back pain was. Instead of all over, it is SO concentrated in ONE spot . If my knee were a square block, and I were facing you, the knifelike pain would be on the very bottom left corner, just below the kneecap. I was going to ask if there were any ligaments that may have a pain that was very similar to meniscus. I never have locking, and don't hear recurring pops, but if I put my finger on my kneecap, while bending it, I feel a slighting grating sound. I have scrubbed many floors, and I am constantly crawling under shrubs, in the garden. That is my therapy. I did walk a lot two days ago, and had some leg swelling, but that ankle was broken, a real bad break, both bones. So I have four plates and fourteen screws. Could that interfere with healing, as that ankle will swell at times..But it rarely bothers me. I am so eager to increase my exercises now that the pain has lessened. Thank you for the site. I will put off surgery if I can, and I am pretty disciplined, and PT has helped me with my back, and Fascitis, so I would prefer it to heal my poor abused body. LOL THE TIMES THIS PAIN IS THE MOST INTENSE IS IN THE CAR, AND GETTING INTO A COMFY SLEEP POSITION...so frustrating. But, we will prevail.
Hey Jean, thanks for the update on your knee situation. There are lots of ligaments in the knee joint in that area you're describing. It is possible to know if a knee meniscus is starting to tear based on the signs and symptoms leading up to diagnosing a tear through MRI. We do it here all the time. Fully bending the knees when your kneeling can worsen an already irritated meniscus. If you're feeling better, then it's probably time to work on strengthening. @@epmanualphysicaltherapy I have always done glutes, and abs to protect my back. The reason I am so torn...that was a pun, lol...is because I have been hearing that the periformis muscle, pain is very similar to the Menicus pain. My knee is so much better, yet if I get into the passenger seat in our car, the pain hits, unless I remember to pivot in. And yesterday, the pain was on the inside of the thigh, radiating upward. Lying down to sleep, and the car, are the only times when I have the pronounced pain. Ouch. It is So much better than before, and I am so grateful fot your exercises, because I know PT works. I have recovered from a very bad broken ankle, which, required waiting before they could perform surgery, as the bleeding was bad. I wasn't required to wear a cast, because the surgeon said, if I did, it would take up to a year of PT, before I could walk again, so into the wheelchair, I went. But all that time I was doing exercises with my foot, stretching toes upward. Then I progressed to a walker, and a cane, which I tossed. PHYSICAL THERAPY is what gets the real healing going. Thank you again and again!
Hey great question Halkawt Muhamad! It's questionable if a complete ACL tear can heal on it's own. Most healthcare professionals would likely say "no it can't heal on it's own and surgery is needed." We've seen people with MRI diagnosed full ACL tears get back to being active again. It is possible that the MRI was wrong and it actually wasn't a complete tear which gave the patient the chance of a full recovery. But there are people that get back to being fully active even without an attached ACL. Normal healing times (without flare ups and proper mechanical stimulation and nutrition) is about 6-12 months for ACL injuries. Usually a brace is needed for ACL injuries. I’ve watched this video twice looking for the “four causes of a meniscus tear”like the title says. You spoke about “four symptoms” of a meniscus tear but, the only cause I am hearing you mention is prolong standing on your leg. Am I missing something? Can you clarify what the other three causes of a meniscus tear are? Thank you.
Hey Merry Red, great question! Bucket handle meniscus tears do have a higher chance of not healing well just because of the nature of the tear, but we do see people in our clinic improve enough to get back to exercising vigorously even though they have a bucket handle meniscus tear. As far as time, the best answer for you is - it takes as long as it takes! That means that as long as you are improving and actively fixing the underlying root problem, then you shouldn't put yourself on a deadline that if you don't meet, you will have surgery. Sometimes surgeons do that to you. And surgery can be a good idea if you're not making progress even though you are doing the right things. Thanks a lot for the videos. I have a couple of questions regarding this video which could be very encouraging to hear answers for right now. Especially after hearing about surgery and lack of ability for these areas to actually heal without it. By heal, I mean heal, not just feel better than after injury or become asymptomatic. I was wondering about the statement in this video about seeing a lot of patients with meniscus tears that heal without surgery... In those cases, were MRI's performed before and after the physical therapy to confirm there was a tear and then that it healed, or was this statement based on symptoms that were present and then disappeared in time? Secondly, the statement about cartilage in the knee being damaged that is not the meniscus... if that's the case, can other areas of knee cartilage actually heal/repair without surgery as well? If someone has rough cartilage that causes crackling, I understand that may be different, but I mean if the cartilage can be damaged like the meniscus can, such as a tear, have you seen imaging proving healing without surgery? Hello, I was wondering if you could point me into the right direction with one of your videos. About a month ago I was walking in the woods and and hurt my knee. Not sure at what point I hurt it. But I noticed my knee starting to tighten up like a dehydration cramp. The pain is in the inner part of my left knee. No piping or swelling. But I do feel pain into the calf muscle. Light pressure is tender and with ankle flexion up and down is painful. Also walking up steps when I bring the leg up to the next step it’s painful and feels like a tight old rubber band. It was feeling better and yesterday I re-aggravated it and now I’m back to square one. Any help would be appreciated. Thank you!

Hey Felcy, sorry to hear that you couldn't get a clear MRI :-( You should try looking for a manual therapist in your area. They can usually diagnose a problem without requiring an MRI. And if they cannot tell you with certainty, they should be able to tell you so you can go try for another MRI. There are also open MRI machines out there that might be easier for you. You'll have to ask or check online so you can see if there is a place that has one nearby.
Hello dr david, today I received my diagnosis from the MRI scan. Unfortunately the result was not positive, I have a bucket handle rupture lateral meniscus where the posterior horn is displaced medially. The orthopedist said he could do surgery, but I would like to avoid that. I cannot extend my leg 100 percent, it is locked and I am still missing about 15 to 10 percent. I wanted to ask you for help, what is the best way to deal with this? because I don't know what to do and unfortunately I also live all the way in the netherlands and can't come to america? I can still walk nicely and just stand only I notice that I shouldn't turn too fast. I already started with the butt exercises, but are there more effective exercises I can do? What are things i should especially not do? and do you think this can be done without surgery? i thank you in advance for the effort on answering my question.

I just have some questions and if you can answer them below I would like to start with the program. 1. my knee is still locked as I mentioned before and I am still missing about 5-10% to extend it completely. with such a type of problem it is just possible to follow such a program still right? (of course we don't know what the end result will be but at least there is a chance) 2. I had a meniscus keyhole surgery in 2019 but pain and catching returned and last year I had an MRI which showed 2 pieces of broken meniscus in the knee , one of which was a 10mm chunk . If I straighten the knee out without gauging carefully it can catch and its agony so much that I will yell. If the loose bits are the problem then does it make sense to have them removed or will that help degeneration? Thank you for the video , by the way.

Hlo sir I am from India...i have only little popping in my knee when I strech my leg or bend my knee.. and mild pain inner side of my knee...no swelling no difficulty in walking...I had a mri a year ago it shows second grade meniscus tear and doctor suggest surgery...but than other doctor said it is nothing serious only weakness... what's the actual problem????

I had surgery for meniscus tear. Now I have osteoarthritis in that knee. And I won’t have surgery. Shots don’t work for me anymore. I wish I knew this before surgery.

This bothers me too but elsewhere he mentioned that liquid in the knee helps healing. It takes time and requires simple excercises. I am going to give it a try as I do not want to try surgery, my case is not dramatic but rather limiting
@@Jan-l8p8o I am in the same situation as you. I did my rersearch about the capability of white zone meniscus tears to heal. It all comes from the mesnchymal stem cells present in the synovial fluid aparently. Thing is that it can heal, but it takes much time and not all tears can heal. Imagine you have a pretty large complex tear. That won't likely heal.

I had a meniscus tear when I was 65 (it did heal) but I’ve often wondered how or why it tore in the first place. I don’t believe I did anything to cause it (?), but going forward, I’d sure like to do what I can to avoid it happening again. Any suggestions?

Please advise I am 55 years old lady 2019 right knee meniscus tear also due to hypothyroidism I have weight gain issues. Till date I do have pain in my knee while climbing stairs and morning stiffness the pain comes and goes. Your knee exercises are very helpful and the RICE method helped me. Recently my Orthopaedic surgeon is suggesting me to go for meniscus root key hole surgery, please give me your honest advise. So, I recently got my knees examined by my Primary Care Physician. Long story short, I've played basketball all my life through college and even played some soccer. However, I didn't have any injuries throughout that time period. It wasn't until Winter of 2019 when I smoked my knee on the edge of the bed, in the middle of the night, that I noticed I had pain. Nothing really substantial until last year when I began playing some golf that I started having to wear knee braces. After I play I'll have a dull ache for at least a week until I play again and recently I've started to notice it more -- which leads me to believe it's probably cartilage damage. Is there anything you would suggest I do to alleviate the pain? I've just started incorporating tailgate swings with weights. Can I still golf some with the proper knee brace? I'm only 33 and consider myself to be in pretty good physical condition. Sorry for the long post. Thanks, Seth
Hey Seth! So sorry to hear about your knee situation. It's frustrating when it seems like the injury came on with such a trivial thing like moving around your bed. You likely have a muscle imbalance that is catching up with you now that you're out of your 20s. Golfing should be ok as long as you start to get your glute strength up. Doing what we talk about in these 2 videos would likely set you up nicely.

When I had a 4th 5th lumbar disc issue, I had sciatica, and pain all around the area. As the injury healed the pain got confined to the exact site of the injury. That is what my meniscus injury is doing now. The injury was not a tear, but possibly the start of one, was what the surgeon said, but how can he know that, if there was NO MRI? I got a shot. It lasted many months, then the pain returned, and was exacerbated by a HBP drug that made both legs swell. NOW...I HAVE BEEN doing your exercises, mostly the tailgates, because I wanted to start out gently. But, now that the pain is less, it is just like the back pain was. Instead of all over, it is SO concentrated in ONE spot . If my knee were a square block, and I were facing you, the knifelike pain would be on the very bottom left corner, just below the kneecap. I was going to ask if there were any ligaments that may have a pain that was very similar to meniscus. I never have locking, and don't hear recurring pops, but if I put my finger on my kneecap, while bending it, I feel a slighting grating sound. I have scrubbed many floors, and I am constantly crawling under shrubs, in the garden. That is my therapy. I did walk a lot two days ago, and had some leg swelling, but that ankle was broken, a real bad break, both bones. So I have four plates and fourteen screws. Could that interfere with healing, as that ankle will swell at times..But it rarely bothers me. I am so eager to increase my exercises now that the pain has lessened. Thank you for the site. I will put off surgery if I can, and I am pretty disciplined, and PT has helped me with my back, and Fascitis, so I would prefer it to heal my poor abused body. LOL THE TIMES THIS PAIN IS THE MOST INTENSE IS IN THE CAR, AND GETTING INTO A COMFY SLEEP POSITION...so frustrating. But, we will prevail.
Hey Jean, thanks for the update on your knee situation. There are lots of ligaments in the knee joint in that area you're describing. It is possible to know if a knee meniscus is starting to tear based on the signs and symptoms leading up to diagnosing a tear through MRI. We do it here all the time. Fully bending the knees when your kneeling can worsen an already irritated meniscus. If you're feeling better, then it's probably time to work on strengthening. Have you tried these exercises?

@@epmanualphysicaltherapy I have always done glutes, and abs to protect my back. The reason I am so torn...that was a pun, lol...is because I have been hearing that the periformis muscle, pain is very similar to the Menicus pain. My knee is so much better, yet if I get into the passenger seat in our car, the pain hits, unless I remember to pivot in. And yesterday, the pain was on the inside of the thigh, radiating upward. Lying down to sleep, and the car, are the only times when I have the pronounced pain. Ouch. It is So much better than before, and I am so grateful fot your exercises, because I know PT works. I have recovered from a very bad broken ankle, which, required waiting before they could perform surgery, as the bleeding was bad. I wasn't required to wear a cast, because the surgeon said, if I did, it would take up to a year of PT, before I could walk again, so into the wheelchair, I went. But all that time I was doing exercises with my foot, stretching toes upward. Then I progressed to a walker, and a cane, which I tossed. PHYSICAL THERAPY is what gets the real healing going. Thank you again and again!

Hey great question Halkawt Muhamad! It's questionable if a complete ACL tear can heal on it's own. Most healthcare professionals would likely say "no it can't heal on it's own and surgery is needed." We've seen people with MRI diagnosed full ACL tears get back to being active again. It is possible that the MRI was wrong and it actually wasn't a complete tear which gave the patient the chance of a full recovery. But there are people that get back to being fully active even without an attached ACL. Normal healing times (without flare ups and proper mechanical stimulation and nutrition) is about 6-12 months for ACL injuries. Usually a brace is needed for ACL injuries.

I’ve watched this video twice looking for the “four causes of a meniscus tear”like the title says. You spoke about “four symptoms” of a meniscus tear but, the only cause I am hearing you mention is prolong standing on your leg. Am I missing something? Can you clarify what the other three causes of a meniscus tear are? Thank you.
Does a bucket handle meniscus tear heal the same as other meniscus injury? Is there a different healing time for bucket handle tears???
Hey Merry Red, great question! Bucket handle meniscus tears do have a higher chance of not healing well just because of the nature of the tear, but we do see people in our clinic improve enough to get back to exercising vigorously even though they have a bucket handle meniscus tear.
As far as time, the best answer for you is - it takes as long as it takes!
That means that as long as you are improving and actively fixing the underlying root problem, then you shouldn't put yourself on a deadline that if you don't meet, you will have surgery. Sometimes surgeons do that to you. And surgery can be a good idea if you're not making progress even though you are doing the right things. Thanks a lot for the videos. I have a couple of questions regarding this video which could be very encouraging to hear answers for right now. Especially after hearing about surgery and lack of ability for these areas to actually heal without it. By heal, I mean heal, not just feel better than after injury or become asymptomatic.
I was wondering about the statement in this video about seeing a lot of patients with meniscus tears that heal without surgery... In those cases, were MRI's performed before and after the physical therapy to confirm there was a tear and then that it healed, or was this statement based on symptoms that were present and then disappeared in time?
Secondly, the statement about cartilage in the knee being damaged that is not the meniscus... if that's the case, can other areas of knee cartilage actually heal/repair without surgery as well? If someone has rough cartilage that causes crackling, I understand that may be different, but I mean if the cartilage can be damaged like the meniscus can, such as a tear, have you seen imaging proving healing without surgery? Hello, I was wondering if you could point me into the right direction with one of your videos. About a month ago I was walking in the woods and and hurt my knee. Not sure at what point I hurt it. But I noticed my knee starting to tighten up like a dehydration cramp. The pain is in the inner part of my left knee. No piping or swelling. But I do feel pain into the calf muscle. Light pressure is tender and with ankle flexion up and down is painful. Also walking up steps when I bring the leg up to the next step it’s painful and feels like a tight old rubber band. It was feeling better and yesterday I re-aggravated it and now I’m back to square one. Any help would be appreciated. Thank you!

Hey Felcy, sorry to hear that you couldn't get a clear MRI :-(
You should try looking for a manual therapist in your area. They can usually diagnose a problem without requiring an MRI. And if they cannot tell you with certainty, they should be able to tell you so you can go try for another MRI. There are also open MRI machines out there that might be easier for you. You'll have to ask or check online so you can see if there is a place that has one nearby.
Hello dr david, today I received my diagnosis from the MRI scan. Unfortunately the result was not positive, I have a bucket handle rupture lateral meniscus where the posterior horn is displaced medially. The orthopedist said he could do surgery, but I would like to avoid that. I cannot extend my leg 100 percent, it is locked and I am still missing about 15 to 10 percent. I wanted to ask you for help, what is the best way to deal with this? because I don't know what to do and unfortunately I also live all the way in the netherlands and can't come to america? I can still walk nicely and just stand only I notice that I shouldn't turn too fast. I already started with the butt exercises, but are there more effective exercises I can do? What are things i should especially not do? and do you think this can be done without surgery? i thank you in advance for the effort on answering my question.

I tried first some physical therapy in the Netherlands, but unfortunately it didn't work out. I still want to at least try your program and give it a try. I just have some questions and if you can answer them below I would like to start with the program.
1. my knee is still locked as I mentioned before and I am still missing about 5-10% to extend it completely. with such a type of problem it is just possible to follow such a program still right? (of course we don't know what the end result will be but at least there is a chance)
2. Do i have to take the meniscus recovery course, where there is a video of 8 minutes and 35 seconds at the very top and below i can then click on the button where you can start today. This is the course most suitable for me right? I ask this just to be sure because I actually saw other courses on the website and I don't want to click on the wrong one.

I had a meniscus keyhole surgery in 2019 but pain and catching returned and last year I had an MRI which showed 2 pieces of broken meniscus in the knee , one of which was a 10mm chunk . If I straighten the knee out without gauging carefully it can catch and its agony so much that I will yell.
If the loose bits are the problem then does it make sense to have them removed or will that help degeneration?

I'm from 36 yr old female from India ...ur videos r so informative...while traveling in train i used trains indian toilet..after I got up from deep squat I got a severe pain in inner knee n locked knee..MRI scan showed bucket handle tear..went for a surgery with complete meniscus repair...it's been 3 weeks since surgery,I'm recovering well with physiotherapy..i want to know the root cause of this tear..a Lil past abt me i had endometrial cyst 3 years back which resulted in removal of left ovary was under medication (dinogest)for 3 years post surgery (which means no periods for 3 Years)read somewhere that there is a link between estrogen n laxity of knee....is it true...??how can I be careful in future to avoid this?

Hlo sir I am from India...i have only little popping in my knee when I strech my leg or bend my knee.. and mild pain inner side of my knee...no swelling no difficulty in walking...I had a mri a year ago it shows second grade meniscus tear and doctor suggest surgery...but than other doctor said it is nothing serious only weakness... what's the actual problem????

It is possible to have knee osteoarthritis and feel better in your knee depending on how severe the osteoarthritis is. So avoiding surgery might be very possible for you.
Strengthening the right muscles is key for helping a knee with a meniscus injury and osteoarthritis. What are you doing to work on that?

This bothers me too but elsewhere he mentioned that liquid in the knee helps healing. It takes time and requires simple excercises. I am going to give it a try as I do not want to try surgery, my case is not dramatic but rather limiting
@@Jan-l8p8o I am in the same situation as you. I did my rersearch about the capability of white zone meniscus tears to heal. It all comes from the mesnchymal stem cells present in the synovial fluid aparently. Thing is that it can heal, but it takes much time and not all tears can heal. Imagine you have a pretty large complex tear. That won't likely heal.
