Ultra Mode is a patch that you can apply to the game. What it basically does is double the amount of bullets, double their speed, both, or something else. This makes for a fun challenge most of the time and sort of gives you a difficulty above lunatic.
There are patches for almost all the Windows games and they can be downloaded here:

(Unzip the thxxu.zip in the game folder and patch the Japanese thxx.exe).
All the Ultras give the player autobombs but in IN it's almost pointless as you have to keep deathbombing to a minimum duo to the two bomb deal.
As far as Ultras go this is probably the second easiest one after PCB.

I think I get it now, the dialog bits are when your suppose to blink and re-hydrate your eyes because otherwise you would be unable to blink for 30 minutes straight
If you're above 80% on the blue side (human side), your attacks will net you extra time points and you can target familiars (those white balls). If it's below -80% (red, youkai side), then grazing bullets will give you extra time points and you'll be able to pass through familiars, unable to target them.

Marisa becomes so much easier on Hard/Lunatic. Non-directional Laser is replaced by a joke, and Shoot The Moon becomes graze heaven. The other bosses make up for this though...
yeaaah dealing NDL is a pain even on easy (fucking Patchy Lazers plus not-aimed bullets)
also, why did zun make ELR be kinda hellish while STM is just streming with lazors
ofcourse ALL of Marisa's nonspells make up for it. and Even Horizon (i can't even figureout startdust reverie)
i'm saying this and i haven't even played hard/lunatic yet due to being kinda bad at the games themselves

Anyway, joke aside...the game is hard, but this is an "Ultra" difficulty hack.
Most Touhou gameplays here on youtube are on Lunatic (the highest difficulty), Extra (a difficulty higher than Lunatic, but only exclusive to a "side path" on every game) or Phantasm (even higher than Extra, but only appears in one game)
Seems that no one likes to show off the games on Easy difficulty, there's still plenty of bullets, but it's not this crazy, so don't be so scared with this xP
Extra isn't a difficulty, well sort of, it's just a whole different stage, and it's not harder than lunatic the difficulty is about that of a hard, also the Phantasm is just there because the extra was made to be just an easy version on phantasm which is at the level of a classic extra

@megaboundz In Ultra you can autocollect just by going to the top, but in the regular game you have to go to the top and focus unless you're using Magic Team. Dunno if you knew this already.
I just realized graze and time item collection sounds resemble a Geiger counter.

The Ultra Mode works with English? Really? Oh. By the way. You think Lunatic is easy? You passed Lunatic Ultra with only 3 lives? You, sir, are a god of Touhou.
Pretty interesting to see how his skill level on this matches my skill level on normal mode and how there seems to be issues where I have strugged on the respected difficulty :P

You SHOULD be able to just delete the english patch files, if you look through the game files. Either that or buy the disc, I guess.

Eirin's midboss nonspell's round bullets are sometimes aimed at you, sometimes not, and that's not depending at all of what stage 6A or 6B it is (cauz I was on the stage 6A and the bullets were aimed, but in this vid, in the stage 6B, the bullets are also aimed), it's either random, or it depends of the character(s) you choosed.
Braviaggron Human: Not aimed.
Youkai: Aimed.
The idea is to switch from Youkai to human for a few seconds to turn it.

also mystia is stupid easy once you realize that if you unfocus whenever you hear the sound of Familiars being shot, nothing will ever hit you on Ill-stared dive and there sn't much on her other spell
marisa can go to hell though. worst part his 2 of her hardest spells (Non-directional laser and earthlight ray) become complete jokes on hard/lunatic
this kind of dodging is so bad ass that Chuck Norris can't do it.

I was ding three treasures - mirror (I play easy, that was lunatic) and I, at one point, just ran to the right without trying to dodge bullets. I dodged EVERY SINGLE ONE.
heck, sometimes I walk past bullets then retreat because I thought I wouldn't pass. I dodge better when I'm not trying, apparently.
Keep playing, when I played Touhou first time, I use all continues and didn't complete the game. A year after, I can 1cc almost all of the games (without SA and UFO... They are horrible. ) :3
