"Ultra mode" is a patch that doubles the amount of bullets fired by enemies. "Lunatic" is the highest difficulty of the game. Combining these two makes the game a complete nightmare; with EoSD's random bullet patterns and other sweet stuff the game can get nigh impossible when the pew-pew count is doubled. The patch also gives you autobombs - I'm actually not this handy with them.
Guide to installing Ultra patches, props to the person who wrote this:

-The patch needs to applied on the Japanese .exe (東方紅魔郷.exe)
-System locale might need to be changed into Japanese
-After patching, rename 東方紅魔郷 ultra.exe (or whatever it created) into 東方紅魔郷.exe if you want vpatch to work!

I don't think the goal was to make a balanced game mode in the first place haha
These aren't even custom patterns anyway, it's all algorithmic. It searches for all instances of bullets being summoned in the code and adds more bullets whenever it happens without regard for the actual design of the attack.

It's actually not that bad considering it's possible to beat it with one bomb fairly consistently. While dodging the pattern is out of question, that goes for a lot of things in Ultra.

The difficulty is so uneven in Ultra haha. Would be cool to see a more thorough redesign of the bullet patterns rather than just adding more bullets, especially seeing how most of the directed shots are no more difficult than before :P

Reminds me of Story Of Eastern Wonderland's bullet doubling in its higher difficulties (Including lunatic), where if something were to shoot, they'd fire a second bullet that's a little slower than the first one shot
Yes, I noticed this behavior too while playing. It is quite apparent in attacks where the large orb bullets are fired.

Is this game have hitbox? Because in this video, I can't see the hitbox?
EoSD does not show the character's hitbox when holding focus button. It became a feature from PCB (Touhou 7) onwards. However, there is a third-party patch for this game that points out the hitbox in the character's sprite. Using this patch, however, is considered cheating by some since it technically makes the game easier. Although a knowledgeable player knows the character's hitbox even without a visual guide, but I digress.
The hitbox in this game is, if I recall correctly, 5x5 pixels for all characters. For Reimu, it is at her waist. More specifically the white round-ish part that you can see if you don't move.

Why everyone is using Reimu B?
EoSD Reimu B is one of the best shot types in the entire francise. I gave a more in-depth reply to an earlier comment.

Marisa A is great because you get full damage in from full screen with her narrow attack. IMO the 2nd best shot type in the game. Marisa B has less damage but more range, in which case you might as well use Reimu B. Her lasers have inconsistent damage because they only fire momentarily which makes her shot annoying to use. I think Marisa B's bomb is pretty terrible. While it deals excellent damage, damage rarely matters because you can skip boss attacks and tough parts even with weaker bombs. Secondly, you move absurdly slowly while firing the bomb which makes you unable to collect items the enemies drop or reposition yourself in the screen.
Nickyan for me still better than reimu A The damage is hight and the bomb have a long duration that's combined with the speed of Marisa.
