My experience with fifth metatarsal fracture - recovery timeline, UK NHS, what you should expect

  • (My 6-8 months post break experiences) In July 2021 I had a silly accident (video included!) which resulted in a displaced avulsion fracture of my fifth (5th) metatarsal. I did a lot of research throughout and wanted to share my own learnings to help others who may be unfortunately in the same position.

Well done. You are a lucky man to work for such a wonderful firm which gives you support. I myself, am self employed and had a Jones fracture on 24th March this year. I had three weeks off, because it tipped down with rain( you may remember). I had to go back to work, with no surgery. The second day of being back, I walked 4.25 miles behind a lawn mower. The third, 5.60miles.
It’s half way through May now, and still hurts, but I have no choice. I can’t take painkillers, as they have me gout.
I’ve just got home, so I am going to rest up.
    All the best👍🏾😉🇬🇧

Thank you for taking the time to do this presentation. I broke my 5th metatarsal mad badly sprained/ tore ligaments in my left ankle, 7 weeks ago. Im 66 and worry that I’ll never get back to my previous baseline. Like you, I didn’t have surgery. I was given a crutch and a boot very similar to yours- also by the NHS. the effect on my confidence and mental well being is just as traumatic. I’m even wary walking around my own house. So I truly hope that will improve in time also. Your video has given me encouragement in that even someone as young as you are, can be affected mentally by such an injury. Thank you again.

You were lucky to be put in that boot from the beginning, I think it helped with faster recovery. I was stuck in a very uncomfortable cast that created pain unnecessary pain around the patella and toes.... My foot was hard as a stick for weeks after 7 weeks of immobilization. Almost 3 months after removing the cast my ankle mobility is almost back to normal, even though my calf still feels stiffer than the other. Good luck to everyone, the recovery and pain from the cast side effects was worst than the fracture itself.

I teared up when you got to the first run. I am going into week 3 of no walking, no driving, haivng to move into an Airbnb because living alone off-grid wasn't doable with a Jones fracture. Being so isoalted alone away from my home and unable to go anywhere, I've had some hard moments. Your presentation gives me great hope. Thank you!

Might be worth anticipating that recovery is not linear. My first 2 weeks were sloooow. Then suddenly your off crutches, and walking in the moon-boot. Then a fortnight or so later, it seemed to plateau. Then a few days of "Will this ever get better?!" and you realise you're improving again! You will get there!

@@PixelVibe42 You make an excellent point! To be reminded that, as with all progress and growth, linear is a fantasy is very helpful. I’m facing up to that truth on many fronts right now, as I’ve had to reverse, slow down, and put on hold various aspects of my life because of this injury. I have not been graceful in my acceptance of this 😆. To reacknowledge that it is the nature of human growth and change is a comfort. Thank you.

I’m wondering if you experienced any coldness, pins and needles, swelling and bruising along the way? I have all of those things at 6 weeks and just trying to walk in my boot without crutches. At what point in the timeline of your fracture did you get back into a regular shoe?

Thank you so much for this video! I broke mine one month ago and the brake is very similar to yours and how you broke yours. I have been so overwhelmed by the thought of needing surgery and the lack of mobility has me kind of in a dark place. I’m going to see a specialist in a couple days and I’m hoping my healing journey turns out like yours. The few drs i have already spoke with are suggesting a screw or a plate and I think some doctors seem too quick to make that determination. It isn’t the ideal option in my opinion so I hope I can go without. Thanks again for your video and sharing your experience.

I had a jones fracture at home after missing a step and landed on a hard plastic toy with my 5th metatarsal in October 2023 and today is the 22nd of January 2024. I never went through surgery and I never stopped using the foot because I’m self employed. I’ve started physiotherapy at home through TH-cam. It’s showing progress because I can now walk properly and jog a little. I feel pain here and there especially when I’m resting at night after work. Eating healthy organic foods, cutting out processed foods, processed sugar and almost all unhealthy junk & drink loads of water will speed up the recovery.

@@jameelhilal220 I’m not sure if you were asking me for a update, but incase you were- I am now 7 weeks post op, I’m still non weight bearing and waiting for a bone stimulator to be sent from workers comp to help the bone heal with the screw. The pain now is tolerable, at night especially my foot throbs and if I try to put any weight on it I scream because it hurts so bad. I know everyone is different though. My Jones fracture didn’t heal on it own and it’s now been almost 7 months since I fractured it. I’m praying that with the bone stimulator it will heal because if it doesn’t then I have to have another surgery to have the entire 5th metatarsal bone replaced with a cadaver bone. My understanding is that where Jones fractures occur their is extremely little blood supply which is what makes them extremely difficult to heal. I’m praying for everyone that has to go through this- the pain after surgery was the worst pain I’ve ever experienced (and I had 2 kids naturally) lol

Hi Adam... Wow, the way you tripped is almost identical to my misstep on a rocky beach here in Malta. 10 days ago while I was walking a dog and chatting away to someone without paying attention. I kept walking the dog for about 30 minutes when I thought 'Yeah, something is really not right'. Went to the hospital the next day and had an Xray, was told it was the fifth metatarsal, was given a boot and crutches, told to make a physio appt in a week and sent on my way. Doc said no surgery required.
The first 48 hrs I'd say were really hard and painful, but I only ever took two painkillers and used some CBD cream. I love the boot! Got the hang of it right away and made sure to use crutches to keep my gait even. I was really tired the first week - going to bed super early, needing to nap- but I noticed yesterday I am back on sched with waking up at my usual time. I have dropped all dog walking apart from one 'easy' dog, as well as my own older one who I walk in the morning but a friend walks in the evening, and wear the boot all day. I had my first physio appt two days ago and another today. They are doing heat therapy, some massage, electrical and magnet therapy for now. Today I was told it's looking good- swelling down and bruising almost gone, and I have started 'toe wiggles' and can move my foot forward and back at the ankle no problem. I haven't yet tried to put weight on it but when the boot is off I can walk on my heel (with a crutch).
Loading up on vitamins and protein. Before my injury I walked on average 2-3 hours a day and did yoga and Pilates so I'm hoping this speeds my recovery but I know I have to be patient.
    So happy I found this!

Thanks for sharing your experience. I rolled my ankle in mid Jan, avulsion fracture to base of 5th MT. My 5th fracture in 12 months (track cyclist!). Others were wrists and hands from high velocity crashes (lost a LOT of skin!). The MT has been a nightmare, as it was an instant stop to training. But - decided to just keep positive and rehab to the max. Have been meticulous in planning time every morning for a full hours rehab or gym work. Lucky that I have a garage gym tho.
Currently ending week 6 today. In Aircast Short Boot, but transitioning out and walking in the house. Was off crutches at the end of week 2. Rehab is all physio approved "... as long as in the boot ...": Bench Press, Shoulder Press, Abs, Leg Curls, Squats (around 20kgs only), Deadlifts (up to 55kgs), Hip Thrusts, Landmines etc. Managing 45 minutes on the Wattbike, made up of 10 mins warm-up, then alternating single-leg drills, then warm down. Can do just over 500w on the fractured foot side at the moment (in the Aircast of course!).
Supplementing daily: Vit C, Vit D, Q10, NMN, Hydrolyzed Collagen, Clear Protein, Iron. Clean, balanced diet. Tee-total already.
Cons review and imaging on Monday, so fingers crossed! I've seen videos from people who took months to recover, and some who claimed to be back to normal at 6 weeks. Just be you! Work inside of pain, play it safe at all times, do your rehab. You will heal.

Nondisplaced fracture 5th metatarsal last week. I was working at my desk at home, and had no idea my foot or leg had fallen asleep. Stood up and took a step and fell immediately. I looked down at my foot as I fell and it was completely sideways with all my weight on it. Went to urgent care for X-rays.
I have an appointment tomorrow and it’s healing very well already. I do not want a cast. Anxious to see what they say tomorrow. I’m very encouraged watching your progress that was done without months in a cast. I’m guessing that using your leg and ankle as it healed seemed to speed your recovery. Hi Adam! I did the same thing a little over a week ago. I went to the ER who gave me a boot and scheduled an appointment with a orthopedic doctor, coming up on the 5th of April. I can't put much weight on it currently without pain so since I know it's a fracture, I'm totally not going to push it until I see the specialist. The grinding feeling makes me sicker than the pain when that squishy grinding stuff happens Lol
Walking/hobbling to get around my house feels much better without the boot which squeezes and puts too much pressure on the area, so I've only gone out once with the boot outside to walk at the grocery store for 20 minutes with my husband. After coming back home, I took the boot off which was almost as painful as when I broke it, so I've just decided to stay off of it 99% of the time with short trips to the bathroom. It's funny, sitting around is driving me nuts, but I can't forget the squishing and grinding feeling. So sit, I shall, and happy I will try and be.
    Glad you're recovered now.

Broke my 5th metatarsal about the same time as you. Get yourself a knee scooter, It will help you to get around. I'm coming up 4 weeks and I am able to walk in my boot for a short period of time. Secret to quick healing is Vitamin D, increase your intake on calcium type of foods. Ice it to decrease swelling. Massage the bottom of your foot especially near the plantar area to increase blood flow. Wiggling your toes helps as well

Then went back to work yesterday after my week off. After 3/4 hours on kitchen floor I was in so much pain. So decided to go A&E and yes find out with X-ray, so now got a big boot for next 6 weeks. Your video is really helpful, Adam. Thank you! My accident 9 days ago was really silly. I literally slipped off the edge of a doormat and heard the crack! I've received conflicting advice from the docs. The main fracture is a pseudo-Jones (Zone 1 near the ankle) but there's a sort of "branch" or secondary thin fracture on the edge of the Jones area (Zone 2). So, doc #1 (a podiatrist) called it a Jones and therefore said I should be NWB. Doc #2 (an orthopedic surgeon) calls it a pseudo-Jones and WB in a supportive shoe is fine as pain allows. I was confused and have decided to do my own "hybrid" treatment: wear the boot (it hurts to put my foot into a shoe) but put a little weight on it while using crutches. To be continued! Your video is encouraging and a reminder of the power of the body to heal itself when we are patient. All the best to you!

Thank you Adam for this video. In my opinion, your video was more helpful than anything else I could find. Like you I was told 3 mos before I could return to running, but I think that won’t be the case for me (It’s been 9 weeks since my 5th MT Avulsion fracture and I don’t think I’ll be ready to run at 12weeks). My question for you - did you find you were still limping even after the pain had receded? I’m finding it difficulty returning to a natural gait despite very little pain at this point. I think my brain is telling my body to protect my foot and thus the limp remains… it is getting better, but I have to consciously tell myself to walk normally! Thx again for your help

    • @AdamStuart1
      @AdamStuart1  ปีที่แล้ว +4

Yes I had a "mental limp" for quite some time, wanting to protect that foot. I can tell you now, at nearly 24 month post break, that is completely gone. But even after 12 months I did not feel 100% doing full effort leaps etc, changing direction fast playing football was still playing on my mind.... those fast twitch movements are very different to repeated predictable movements like running, at least that was my experience.

Hi Adam, thank you so much for creating this video of your fracture experience. My dr has diagnosed me with a right fifth metatarsal fracture, minimally displaced, in Zone 1. Zone 1 is the area where your fracture is, the Jones fracture is in Zone 2, and stress fractures occur in Zone 3. My dr does not refer to my break as an avulsion. We are treating it non surgically, although I had another dr want to do surgery immediately. Both drs have told me I’m predisposed to this fracture as a side effect of a cavovarus foot deformity or subtle cavus foot. Do you have this as well? I’ve been wearing a boot for Five weeks now. Just had a follow up appt with X-rays three days ago. Dr said appropriate, moderate healing at this point and instructed me to begin weaning myself from the boot into normal stiff souled sneakers. He said take two weeks to do the transition, then I can go to just wearing the normal shoes. I go back for a follow up in six weeks. I’m excited for the progress and still very nervous about proceeding all at the same time. My dr did tell me at my last visit that complete healing will take months, just like you are saying. I’m presuming that by moving into normal shoes, this encourages normal foot movement and promotes healing. Is this your experience? Also, can you tell me if feeling some pain when transitioning is normal? Im just beginning the transition/weaning period and feel a bit of achiness but nothing dreadful. Just wondering if what I’m feeling is normal and good. I’m not a runner, but am an avid walker with my dog. I’m so looking forward to getting back to my walking routine of 2-3 miles per day. Thanks for any feedback and hope you are still continuing healthily on your journey with your foot!

I did not have "cavovarus foot deformity or subtle cavus foot". All I can say, is that time is a great healer. As per the video I was jogging at 6 months, and after 12 months I am running just as fast as pre-break. The first 6 months are really tough, and I certainly experienced some discomfort when starting to move again. It does get better though, you just cant rush it.

Thank you so much for this video! It really brings me hope of my injury healing without surgery. Almost 5 weeks ago I was rushing down the stairs and I missteped and had the exact same injury as you. First it was really painful and I could see a similar bone/ swelling sticking out but I assumed it was a normal sprain similar to what I had before playing football so I treated it with icing and wrapping it tight. After 3 days pain went away except for intense movement and I resumed partially my normal activities. One week later and upon my foot not doing better I decided to go check at the hospital after doing my research and self diagnosing a possible 5th metatarsal fracture. Doctor saw it and I explained everything and told about my speculation and he confirmed the injury but said that it is not a big deal since I can normally walk without severe pain and that it will heal by itself if I don't push it, but me reading all those papers on the Internet insisted on an x-ray. The doctor agreed and after seeing the x ray it was actually fractured zone 1 and the doctor was surprised because he said usually people couldn't walk on it and move. He put me into a cast and had me come every week to x ray to make sure that there is no displacement. The problem is, every week a different doctor see me, and tell me a new story, and one of them said I might need surgery since there is no progress after 3 weeks. I live in Japan and my Japanese not at the level to discuss difficult medical stuff and not all doctors here speak English so I was really confused, but then the initial doctor confirmed no displacement and some progress in the bone healing. Now the possibility of a surgery has decreased and he change me to half cast that I can take off from time to time but still no weight bearing. It has been 1 month now and I am supposed to remove the cast completely in 2 weeks since the doctors said it is ok, but the bone shape is still weird and it seems to be sticking out a bit still and I can't tell if it's the bone or a swelling around it (Has the bone that was sticking out completely healed and went back to normal shape for you after a certain time or is it still like you showed in the early injury picture?). Well hopefully I can go back to normal soon and that it heals without complications and without need for a surgery, I really don't want to undertake a surgery while abroad :/
Again thanks for the great video and feedback, and to all of you going through a similar experience I wish you good health !

My experiences: I had a nasty avulsion fracture which was more than 2mm wide. Hospital recommended surgery, i had it a week later. It was compression screwed tightly and looked well aligned on the x ray. Foot was looking and feeling horrible for 2-3 weeks. After that recovery went surprisingly quickly. After 4.5 weeks i started wearing normal shoes again around the house, and now at 7 weeks I do 4 KM threadmill walks at 4 KMH without pain or discomfort (still taking it easy though, there's still a little bit of tenderness). I will likely leave the screw in; as it does not seem to cause any issues. In my case I am pretty confident the surgery sped things up by closely aligning the bone. The only tip i can give others: Listen to your body and take it easy. Be gentle but also don't immobilise yourself if it feels right to move. The first 4 weeks i got clear signals from the foot that i was pushing it a bit too much (walking with the boot, etc) - then i took a bit more rest right away. I am an otherwise healthy early forties male. Those who are looking this up after their freak accident fracture, i wish you a good recovery - the first weeks are hell, but after that you will feel it get better every day!

Hi - thanks for your comment. I think the UK certainly takes a conservative approach to surgery on foot bone breaks, arguably too conservative in an effort to reduce expensive surgeries. The foot is doing good, I am now at t+9 months and back running 100+ miles per month. Steady exercise such as that is no problem at all. Movement that involves sudden change of direction (basketball, football etc) are still a no go. No pain in the foot, just does not feel strong enough yet for high impact, could be more mental that physical.

Thank you so much for taking the time for this video recap. This helps so much to all of those that experienced similar injury. I have an almost similar injury like yours where the tip of the 5thM bone got pulled off last few days and not sure what to do ... with your video, it is encouraging and full recovery is promissing. Want to ask you regarding your injury. You mentioned that the injured bone feel a bit off compared to your non-injured one. How so? and did you ever re-xray it to confirm that it is completely healed? are you fully back to your normal activities and sports, etc.? walking, running, biking, tennis, lifting, etc.? appreciate your comments to all. - Steve

Thanks and good luck with your healing. I never had an X-ray to confirm recovery . Consultant said to go by feel, and only X-ray if it’s symptomatic. I am nearly 3 years post break now. After 2 years I still had the odd ache, but now there is nothing. I am back running 1000 miles per year. No issues jumping or playing football etc. still have a mental tick when doing torsion style movements on the planted foot, turning on a basketball court for example, always reminds me of the break. The bone still feels strange to touch, points out more, and the tendon is embedded further in to the flesh than the other foot.

Thank you so much for your update. Since yours is a major fracture and now that you’re back to 100% full recovery and activity, does the bone fused back or just hard cartilage that bridging between them? Again, much appreciated your times and good luck to a successful recovery road ahead. St

@@stevethai2836 I’m not sure because I never had the X-ray post healing. The consultant told me it would be new bone and it feels solid so I assume that. The area is definitely larger to touch, representative of a larger mass of bone.

    Hello!
I’m in day 16 from 5th metatarsal avulsion fracture and I have a walking boot and the same shoe you have. My two questions for you are did your foot tingle at times and if so, how long did you experience this?
The strap on the boot & shoe crosses over my fracture and hurts to walk with either unless I use crutches and barely put weight on the foot. I gathered from your video that you did not use crutches just straight to walking in the shoe?

      Hi DJ,
I went back to the surgeon for additional X-rays and they showed zero healing to the bone! I’m starting physical therapy this week for at least 4 more weeks and will have X-rays again after. So depressing! My surgeon told me to get out of the boot but it’s impossible to “walk.” He also thinks I’m getting complex regional pain syndrome because I still have bruising at 5 weeks, major swelling and my foot is cold.
      How are you healing?

I’m so happy for you (and jealous). I went to PT today for the first time and the therapist seemed concerned that I was starting therapy without any bone regrowth. She thinks the regional pain syndrome is playing a big factor in my non healing. Ill go 3 days per week and see if it helps over the next 4 weeks.

My avulsion fracture has been pretty painful for 16 weeks now due to Chronic Regional Pain Syndrome which I attribute to the weight of the CAM boot which was so heavy on the injury site. It has healed, but am left with chronic pain due to vasoconstriction and deep bone aches in the ankle and shin. I am coming out of the critical phase but my hips and knee are stuffed. My gait is wonky needing expert and slowly, measured physio so as not to cause flare ups. If your broken foot is still very red or darker than your good foot after the boot comes off, or you find you cannot stand anything touching your foot, ie socks, sheets, and you have severe pain after 10 weeks, go see a specialist who knows CRPS. 12 months is my goal for normal walking.

Thanks for sharing. I am into about 3rd week of the same injury (5th Metatarsal based fracture). I twisted my left ankle while walking down stair. Fell and landed on my foot in twisted position. I were two steps from garage floor. A stupid fall too. I had 2.5mm displacement. ER put me in long air boot, NWB till I saw orthopaedic surgeon on about day 10th. Confirm no surgery. Doctor said I can continue airboot or regular shoes (NWB or WB , doesn’t matter). Let my comfort level decided when to start walking. Walk with pain is normal just not too painful. I am a little reluctant to walk. The swelling is still bad, especially evening. It won’t fit in my regular shoes.
I talked to my physio… she suggested 4 weeks in air boot, NWB. I use mobility device to help me around now. I have arthritis on my back and hip. NWB is harsh on back and hips. Then walk with support. Doc said 2-4 months to heal. I am relieved seeing you walk early on. I am prepared for 6 months as needed. I wish I can get back swimming sooner. It’s my cardio and back conditioning exercise.
I want to know did you have pain when you start walk with your boot ? Did you just walk with any support like crutch or walker ? 10 days after your injury, did you have swelling to your foot ? or cramp in your calf ?

    Hello Adam.
My name is Marjo. I just watched your video because I am in your stuation now😁
I am from Croatia and our way of dealing with that type of injury is little bit different. I have the same injury as you just little less serious. But my doctor didnt gave mi stiff shoe. He immediately put my foot in the cast. I would be very greatfull if you explain me why didnt you doctor recommended you to put your foot in the cast?
    What was his explation.
Beacause your way of dealing with the injury, with stiff shoe, is much better for my line of work.

    • @AdamStuart1
      @AdamStuart1  2 ปีที่แล้ว +2

Not sure, apparently in the UK metatarsal fractures are very rarely cast, unless its a Jones Fracture. I think the underlying science is that simply using a stiff shoe, and therefore immobilizing the metatarsal bone, is sufficient. But I did always find it disconcerting not wearing the shoe at night.... it felt "wrong" having a broken bone and going to bed with the foot naked, was already worried about banging it, dislodging the bone etc. In that respect I think a cast is more reassuring.
In additional point would be that whilst reviewing the whitepapers on the subject, you realise that its not just about bone healing, but also maintaining strength and muscle flexibility etc in the foot. From that angle, a cast is much worse. Using a stiff shoe still makes you do some work, and you do not lose much mobility.
Either way, I hope you get better soon! I was worried with all these questions, second guessing etc, but it works out in the end. In 6 months time you will be mostly fixed :)

Adam, first of all thank you for answering so fast.
I was thinking about this and my logic was going toward your way of dealing with this problem.
Is there any diffrence between stiff shoes so I know wich I am gona buy or only what is important that heel take all the weight?

    • @AdamStuart1
      @AdamStuart1  2 ปีที่แล้ว +2

@@marjosaric4710 Yes the stiff shoe type is important. It has to place all your weight on the heel. (So its not only stiff, with velcro, but also only your heel touches this ground, this disengages the metatarsal). Feels strange walking at first but you get used to it.
