    Thanks, Doc! My total cholesterol recently was 428, HDL 104, Trig 76 (higher than usual for me bc I've been eating too many carbs!). Fasting insulin was low as was LpLpa2 & CRP. Went for a CAC scan and, at age 74, my score is ZERO! My doc has pushed statins for years and I refused. So thankful for the education you provide us! Glad you had a video about CAC score because my doc never mentioned it. I think now he'll back off about cholesterol and statins. He knows he's not going to change this old lady's mind and with a perfect CAC score, what can he say?!

      My cardiologist recommended a low dose of statin even with a CAC of zero (I'm 65). This is because, even though my trig/HDL ratio is good, my LDL is 250. I had been on low carb for 1 year when that was done. I said no! He said, if it was his mother he would put her on it. I didn't say anything but thought I'm glad I'm not your mother.

      I wasn't bored at all! I'm actually going to watch this again! Last blood work my HDL went from 37 to 33. However, my triglycerides went from 293 to 209. I've lost 30 pounds since starting Carnivor the end of March. My A1C went from 6.1 to 5.9. My next blood work is due in July. I'm going to go get baby asprin after work today! I'm so glad I found you!! Thank you!!

      I had a stroke in 2020, is anyone in the carnivore community who is advising this population? I’ve listened to all the OG’s I know, my doc insists that he sees patients do not return with stroke if on a statin or PSK9 and recommends it.

    My Western Medicine trained cardiologist advised that I go on a STATIN. Cholesterol - 250, LDL 129, HDL 107, Triglycerides 44, VLDL 9, LDL Pattern A. I got a second opinion from best-selling author and famous cardiologist, Dr. Steven Gundry ... and he said I was very low risk for CVD, my high LDL was fine, and I did not need a STATIN! My CAC score two years ago at age 65 was an 85 .... I had mild calcium plaque in one of my heart arteries. Dr. Gundry wasn't concerned now that I am following his diet recommendations. All of my inflammatory markers are in the green. I'm so happy doctors like you - Dr. Cywes - are finally getting the truth out about STATINs and the real cause of CVD. Thank-you!
    By the way, I would love to see you talk about the fact that infections like COVID can raise cholesterol levels. Which makes sense because LDL works with WBC to eliminate viruses and bacteria....and precancerous cells as well. Why would you want to lower LDL???

      Thank you for sharing that information. My LDL went up to 250 after I started low carb, and with a CAC of zero at 65 years old I said no to the cardiologist who recommended low dose statin. My trigs have gone down and my HDL went up, and it's that ratio that I'm keeping an eye on.

    I'm a 68 years old female, 5'4", 135 lbs with heart disease in my family. Been on Keto for 6 weeks and lost 15 lbs. My Trig are 188, HDL is 60. I tried Statins several months ago and became so weak I could hardly walk. My Doc said it had nothing to do with the Statins; however, when I stopped taking them the weakness went away within a month. I have made the decision to never take Statins again. I will; however, take an aspirin a day and continue with Keto and exercise. I don't know how much time I have left but I can assure you I'm going to enjoy it as much as possible. My quality of life means way more that quantity.

      I wish that I would have read your comment when you posted it. As a 69 year old male with congestive heart failure, I blamed my muscle weakness on age. After five years of statin use, I stopped it this past weekend. I have already noticed an improvement in mobility. There is still a long way to go, but I am hopeful. The heart is a muscle already in a weakened state, so the cardiologist puts me on a statin that can potentially do muscle damage. It boogles the mind and just goers to show you how much or medical systm is messed up.

      @@JeffSmith-xt9wt That reminds me of the "Fremantle Diabetes Study", where 33% of people on Metformin for diabetes were found to have one or more contra-indications to the med. After looking at those contra-indications, I found that I've got at least 3 of them, so I have stopped taking it after being on it for nearly 10 years. It boggles the mind, especially as I've raised the issue with my GP, but he just ignored my concerns.

      Wow, we're same age, same height and weight. non-smoker. Hashi positive. Went gluten, dairy, sugar, lectin, grain (corn) free in June and lost 15 pounds. Stopped my statin in July. Unfortunately I'm still hypertensive and test as of last week my LDL shot up to 450 (from 150). Both parents had heart attacks at 70, dad's fatal. I'm having a cardiac calcium scan tomorrow.

    Thank you Doc. My Doc in the UK freaking out because my cholesterol went up. But he would not share the results of any other markers. Immediately threw a statin at me and said I am at risk of a heart attack and or stroke. He has requested another blood test.
    I am a 65 year old women who suffered a car accident in 2007. Suffered chronic pain in my neck lower back, right shoulder, right wrist and both knees. In 2009 I had a shoulder op and right wrist fixed. Then in 2010 had my left knee replaced. Because of in activity high carb diet, became hypertension followed by type 2 diabetic. The NHS guidelines on diet are still very high carb whole grain diet. It made me worse. This January I started a Keto diet my chronic pain diminished. So I with help from my chemist came off all the pain medication, completed that in February. My weight is coming down dramatically 75lbs in six months. I am staying off carbs for good and now know to look for hidden sugar's in all food I consume. So had to clear my food cupboard and freezer out. I have turned my back garden into a vegetable patch so I know how my vegetables are grown. Still looking for a butcher that supplies grass feed and grass finished meat in my area.
    Since May I have added the intermittent fasting and can do 14 hours.

    Thank you Dr . This is the best explanation for why I do need to be on statin plus a baby aspirin. None of my doctors have told me THE WHY so I kept taking myself off of them ! Thanks to you I will restart them all nt with low carb ! Grateful !

    I've been low carb / keto for 24 years. (I'm 67.) But when I added Intermittent fasting 6 years ago, my average fasting HDL went up to 77. For the 10 years prior to I.F., average HDL had been 63. That's consistent with what Dr. Cywes said. I eat keto, with IF, now. Last Physical at age 67, HDL 79, Triglycerides 41, LDL 173 (and it IS large particle! -- been measured twice), Fasting insulin is 1.9, Fasting Blood Glucose is 83, but my Morning fasting BG is about 98 due to a heightened dawn phenomena that keto dieters have.

    Thank you for a very informative video. I am not sure whether the SA labs measure VLDL, but will check on my next tests.
    am quite surprised that you have a case for giving statins the OK. In 2017, I was on statins, total cholesterol super-low but triglycerides high. My doctors were happy because my total chol and LDL-C numbers were nice and low. Never mind the high trigs!
    Soon after that, I trashed statins, went on LCHF, and on the next blood test a year later my cholesterol numbers were higher, but my HDL-C was around 60 mg/dl. and my trigs below 100 mg/dl. Currently my trigs hover around 75 mg/dl.
    Statins interfere with the operation of the liver, and slurps up LDL particles because it makes the liver grow more LDL-C receptors. But it interferes with the production of other important hormones like CoQ10. I won't even go into the host of other potential side effects of statins. I personally would never use them again.

    I went on Keto and my cholesterol went through the roof. TG dropped huge to 50 from 225 and HDL-C doubled from 40 to 75. LDL-C sky rocketed from 105 to 360
    I removed added fats and cut way back on red meat. Instead, I went for chicken and seafood. My LDL-C dropped hugely to 140. TG dropped a few more points and HDL-C dropped to 65.

    Thanks Dr. Excellent presentation. I learned a lot and you have helped me. I am type 2, 70 years old ,strictly low carbs for the past 8 years. I only take metformin one g a day. I had 3 stents 10 years ago. I just got out of the hospital after chest pain. They installed an other stent in the last coronary not stented. It was plugged 95%. I am on blood thinners and beta blockers, due back in a month to repair a stent. Only 2 arteries are good so far. Looks like my keto is not working. I am perfect weight blood pressure,my HDL is 34 triglyceride 138 LDL183 . I refuse to take statins. Either I lower my protein intake which in retrospect was too high, or go on statins. I think I will adjust the keto first. I am very disciplined, never deviated from the keto in 8 years, no bread pasta soda sugar rice etc...My A1c is 6.2% . Thanks from France.

    I found that extended water fasting is even more effective at increasing insulin sensitivity. Combine that with keto or carnivore and it will blow insulin resistance out of the water without need for any meds

    Doc - great stuff. I have CAC 332 & CVD with no symptoms. Question - my doctor wants me on statin to lower cholesterol. So it lowers, so what, if the plaquing process still continues because of other inflammatory factors. What am I missing logically ?

    If all your levels are horrible for the first time, would losing weight on Keto and Phentermine be a good idea? I lost about 20 lbs so far doing that and have about 30-40 to go. I’m scared. My numbers are so, so bad and yes, my doctor saw them. She is giving me a few months to drop some weight and see what happens. She doesn’t believe in keto, but emphasized protein at least. Great video.

    I would like to know you take on GENETICS in all of this. I am a Registered Medical Technologist, MT(ASCP) and have over 20 years of Clinical Laboraotry Experience. During most of my career as a Med. Tech. I taught DOCTORS how to interpret LAb results. Where would the doctors be without all us Allied Healthcare Professionals getting their tests results for them? :) During my Internship days in the lab (1971) I did a LipoProtein Electrophoresis on myself and found genetically, I had the pattern that showed I would have a Normal CHOL, Low HDL, and High TRG. This was back in the day before statins and a Normal Chol was 300. And just as my Genetics dictated I have always had a NORMAL Chol, Low HDL and somewhat elevated TRG. (I take Vascepa to lower my TRG.) Right now my T. CHOL is around 160, HDL about 35-40, and TRG right at 150 and LDL 100. My HDL has ALWAYS been around 40. From what I can remember my TRGs. became elevated only after being put on Prevacid for GERD. I weened myself off Prevacid years ago and never looked back... I need that stomach acid! So, I guess the bottom line question is... Can we fight GENETICS?

    Thanks for the info. I have known for a few years I have some severe blockages in the arteries of my heart due to a nuclear stress test. Asymptomatic, fortunately. Recently had a CAC done for info. Came back with being in the 85th percentile, so not good. Still no symptoms and have lost 60 pounds. I was reluctant to take any statins as I was hoping my clean diet was protective. This gives me the info that I still have potentially dangerous levels of damaged LDL and that statins could be protective until I get my blood markers in better shape. I am no longer prediabetic, fatty liver is healed, kidney function good. Triglycerides down but still 150. HDL just creeping up, VLDL slowly coming down. So, planning to take a low dose of statins and Zetia, as well as continuing to exercise and eat very low carb, until then.

    I am looking forward to seeing you in November. I’m going to have to do a re-watching relisten as well as have my latest bloodwork at the time however, I . do think my HDL is lower than what you say. I’m not sure I have access to the VLDL it was via quest. I will check maybe it’s time to pick up that Statin or at very least a baby aspirin.

    This is so true I went carnivore and Joined the BBE challenge Hoping I was doing the right thing eating more red meat well.. my HDL Dropped way down... It was already lower than I wanted at 67 and then it dropped down to 38!! I was Doing resistance training at the same time lifting heavy weights and all that stuff All these experts told me to do eat lots of red meat and lift weights and your HDL will go up well that's not necessarily true it didn't work for me. I definitely did not stick with that dietary plan. I stayed low carb but I am not ever doing Eat a pound and a 1/2 a meat A-day carnivore again. Not for me anyway that's a really good way to just have no HDL left at all. My Try glycerides Are low Thank God and Low carb for that

    Hi Dr Cywes great presentation as always. I am from the UK so we work on a different lipid scale. I had my lipid screen earlier this month and was disappointed that they didn't do my triglycerides. The reason given was that if the cholesterol figures are good there is no value to the triglycerides because they would be low. It was explained to me that there are trigger numbers which if met would have advised Triglycerides to be measured. As it is with a total cholesterol of 4 it didn't meet the criteria for testing. Bytheway all my figures were good and they have stopped my diabetic medication with a repeat blood test in 3 months.

      It was an 124lbs lighter and low carb dieting. My GP is delighted would like me to talk to other patients who they are struggling to get the message across.

    Bravo! Finally, the truth is coming to light. I would love to see a video of you debating this subject with my Dr. He is a teaching Dr.and there are always young students in the examination room. Who needs this information more than our future doctors.

    I found the referenced article, but it doesn't seem to correspond to what is said in the videao. From the article
    Findings: Pravastatin lowered LDL cholesterol concentrations by 34% and reduced the incidence of the primary endpoint to 408 events compared with 473 on placebo (hazard ratio 0.85, 95% CI 0.74-0.97, p=0.014). Coronary heart disease death and non-fatal myocardial infarction risk was also reduced (0.81, 0.69-0.94, p=0.006). Stroke risk was unaffected (1.03, 0.81-1.31, p=0.8), but the hazard ratio for transient ischaemic attack was 0.75 (0.55-1.00, p=0.051).
    Interpretation: Pravastatin given for 3 years reduced the risk of coronary disease in elderly individuals. PROSPER therefore extends to elderly individuals the treatment strategy currently used in middle aged people.
    I would really like Dr Cywes to clarify this.

    Thank you for all the excellent advice! I’m a 74 yr old woman with a genetic marker Apolipoprotein E - 3/4. My father’s side all died of heart disease. My recent Calcium score was 69 . My inflammation test was 3. I exercise daily cardio and strength training and I have no joint pain. I’m on Rosuvastatin 5 mg. and also 1500 mg Niaspan to bring down the high LPa which is 115 now . Do I need the statin??? My cardiologist tells me “ don’t stop taking it’ but my LDL is 85 mg and I think that’s too low. My concern is that because of my genetic marker I do not know whether I should or should not be on a statin. Thank you for all your good advice.

    so far nobody talks about by what means the liver knows the body needs more cholesterol, maybe high LDL is normal and not a problem since our body requests our liver to generate more and thus more LDL too. Unless there is something wrong in our body that not working normally then the liver will generate extra cholesterol and thus LDL. In that case we should find out what is abnormal in our body instead blindly lower the LDL. Blindly lower LDL will cause shortage of LDL to carry cholesterol to our body. in that case our body will be lack of cholesterol to fix our body, we all need cholesterol to survive, right?

    Was on high sugar carb fruit diet with low red meat for decades
    Been possibly Insulin resistant: fast glucose usually in upper 90's
    10 months ago started IF 16/8 to 19/5 and still on same diet with relatively lower table sugar intake
    After 7 mo. (3 mo. ago) started low carb diet (50-70g /day) w/ IF.
    2 months after that I did my first triglyceride blood test ever
    Fasting glucose 84
    Total chol 202
    HDL 55
    LDL 16O
    TG 302
    Freaked out and stopped eating red meat and meat fat
    After 10 days repeat work but I was 45 hours into a 48 hour fast (coincidence)
    Fasting glucose 64
    Total chol 184
    HDL 63
    LDL 102
    TG 95
    1. Is the low carb diet for 2 mo. Caused TG 302 or it must hv been 400 or 500 and low carb/keto brought down to 302?
    2. What brought TG down from 302 TO 96 in 10 days is it from obstaining from red/fat meat or caused by prolonged fast towards the end of which they draw the blood sample.
    Is 302 to 95 good sign or just the product of not eating for 45 hours.
    Please provide your opinion thoughts about this.

    Mendelian randomization studies show a direct correlation of LDL level and duration of exposure with cardiovascular disease. People with homozygous FH have an extraordinarily higher incidence of early onset ASCVD versus people with heterozygous and those without FH. Furthermore, people with genetic mutations that cause lower levels of LDL have lower incidence of ASCVD than the general population. Because these are genetic, they are naturally independent of any of the confounding factors that might impact epidemiological, observational or RCT studies. And the relationship is linear: chronically lower LDL equals lower incidence of ASCVD and chronically higher LDL equals higher incidence of ASCVD. This is across nationality, economic status, diet, lifestyle, etc. To say that higher LDL is not a causal risk factor for ASCVD is simply to deny the overwhelming evidence and the science. This doesn’t mean LDL is the only risk factor or that high LDL means I’m going to have CAD. It doesn’t mean insulin resistance is not a significant risk factor or that IR does not exacerbate and accelerate atherosclerosis because the evidence clearly suggests that it does. However, studies have demonstrated that non-HDL is atherogenic independent of inflammation or diabetes.
    Finally, RCTs have demonstrated significant benefits of statins for secondary prevention in high risk patients. So that is another group where the benefits outweigh the risks. If I had already suffered an MI and had high LDL I would take a statin without a second thought. I would not take one for primary prevention without other significant risk factors beyond high LDL.

    Most of what you say is correct and i agree with you however you are incorrect with regards to the fact that other organs and tissues and NOT just the liver contribute to HDL production, namely the intestine and peripheral cells which you never mentioned. Your statement " HDL cholesterol is produced by the liver under the influence of glucagon is inaccurate" The production of HDL cholesterol is influenced by genetic factors, hormones and lifestyle choices. Also while glucagon indirectly influences HDL metabolism by affecting various metabolic pathways, it is not the SOLE regulator of HDL production nor is it the primary hormone responsible for its synthesis. Adipose tissue and muscle also contribute to HDL metabolism. They use the reverse cholesterol transport mechanism. Another is LPL and CETP.

    How long dose it take to lower triglycerides if I didn’t eat sugar or carbohydrates and raise my HDL . One week , two weeks --------- 8weeks . Or just a guess this may not be easy to answer. But very good video. Thanks a lot

      Suggest you stick your low carb/sugar regime, for two months, request a blood-work, then compare lipid levels with your last bloodwork, hopefully your GP will interpret, if the differences are good or bad. Best of luck.

    My friend's A1C is 5.0
    His Triglycerides is 265. He cut way back on carbs. I mean way back. In fact, he only consumes some vegetables, hardly any more than 30 carbs daily maximum and the rest meat. His A1C went from 5.2 to 5.0 and his triglycerides went from 250 to 265. His HDL is around 30 and LDL around 65. So according to this video he is insulin resistant and he should take statin. ODD IF YOU ASK ME.

    Thank you for the great teaching. I do have a question though. I am a 60 y.o. woman, 5 foot 6 inch tall, 123 lbs. have been on keto for past 2 month and noticed that my LDL went from 215 to 240, HDL from 85 to 130, Triglycerides from 79 to 45. One thing interesting is that my glucose level is pretty much the same. - 98. I am a little confused about the numbers. It appears I have some insulin resistance but my hdl and triglycerides are excellent. Any insights? Should I remeasure lipid panel and a1c in another 2 months? Thanks,

    Why don't more doctors know/order carotid intima thickness test (CIMT)? It's non invasive, unlike CAC.

    Interesting. My Cardiologist put me on 10mg rosuvastatin. My LDL is higher than HDL. Triglycerides are at 100 or so.

      My LDL went up to 250 after I started low carb. But my trigs went down and my HDL went up - that ratio is what I'm going by. Dr Ken Berry's observation is that after cutting the carbs for about 1/3 of people LDL goes up, 1/3 stays the same and 1/3 it goes down.

    Can anyone help give me some insight? I feel like there's nobody out there like me!? I have EXTREMELY high HDL. I know it's supposed to be the "good cholesterol" but a level of 190 is crazy. It's always been high (over 100) but it keeps climbing. I have all other good markers (LDL 103, Trig 49, a1C 5.2) Im a bit underweight and have a lot of muscle (5'5" 117 lb female) and eat a keto/ketovore diet. I do exercise a lot and I 'm wondering if it's too much!? I am scared I may have a heart attack due to too high HDL and what in the world am I supposed to do about this?? Thoughts? Dr Cywes is too busy to answer im sure but just some insight would be great. Ive read about hyperalphalipoproteinemia but I don't think anyone in my family has had this.

    What HDL/Triglyceride ratio are you looking for? You mention this but did not give an ideal range. My HDL is 66 and Triglycerides 46.

    Is Lp(a) causative or is it just like LDL and a "firefighter?" Should people worry about high Lp(a) concurrent with a low high sensitive C reactive protein test result of less than 0.5 mg/l.
