FINASTERIDE: Can one get away without taking it after hair transplant ?

  • Finasteride is required after a hair transplant. However all patients with androgenetic alopecia do not need Finasteride. There are certain indications where Finasteride is not required for a hair transplant. Finasteride is not needed in the following indications: 1. When the baldness is stable 2. For grade 2 baldness where there is no evidence of baldness but the person wants the hairline corrected. 3. Sikh pattern traction alopecia & tractio alopecia in African origin women 4. In those who have baldness but know limitations of a hair transplant without Finasteride. These people need to be kept under close supervision 5.Those who need body hair transplant and those getting facial hair restoration using beard grafts.

    It depends on if your hairloss has stabilized or not.
    For example,my hair loss stabilized about 3 years ago,
    And the hair i have left grows fast and strong.
    Im not completely bald,just thin on top,not the crown,just the area behind my hairline to the front of my crown area looks like a balding tyre,but everywhere else on my head has plenty of hair.
    Im 44 now,so i believe a hair transplant will be good for me now even without using any prescription medications afterwards.

      Thanks for sharing. However for 6 months after the procedure Finasteride is ideally needed for better growth, better results.

    Does natural DHT blocking capsules consisting of natural things like grape seed extract, pumpkin seeds extract etc good and effective against DHT? Have heard the many clinics prescribe that….what are your thoughts ?

      These are not in any way equivalent (even remotely) to the benefits you will get with pharmacologic grade Finasteride.

      There are so many respectable brands in India. You can buy generic Finasteride as well. But whatever you choose, do not switch brands.

    One more thing a Topical Finesteride+Minoxydil solution is available which has less side effects. It's equally effective and negligible side effects. DHT in the scalp is the issue. Topical is still absorbed to blood. DHT is an important hormone in our body. Sexual side effects are over hyped but not Anxiety/Depression. A low mood and mental health can happen with blocking DHT although customised does of 3 times a week and starting/stopping may help

    Hello dr I had my ht and it has been like 5 months now. I'm on finasteride n minoxidil and have decent growth by now.. my concerns are as follows
    1) hair shedding began recently most probably because I moved to another country n change in lifestyle though I'm taking finasteride
    2) my left temple is filled up with haiir while the right temple still lacks density meaning its uneven growth
    3) under normal light conditions one cannot say I'm bald but when a bright light falls on my scalp, one can easily see bald patches meaning I have a combination of transplanted hair which some of them.have grown while others are still small like beard though lot of hairs have popped out by now but still aren't enough to give my density yet n makes it look thin

    It's the first time I hear that finasteride is not as important for people older than 35-40. That's good news to hear but doesn't balding become much worse at older age? If androgens are much lower then how can this be?

      "Finasteride is often times not required in patients who are above 35-40 years" is what has been said!
      There are no generalisations in medicine. However, I am one of the strongest advocates of Finasteride.

    To implant 4000 FUE grafts how many grafts is extracted from the donor area ?
    I mean i want to know the ration between grafts harvesting and grafts usually implanted, yes especially at your hospital. What is the damage rate before implantation ?

      What you mean to say is 'transection rate'. In FUE there is no damage since the root that is cut (transected) remains within the scalp and lives to grow another day. Clinics like ours who have been doing FUE since eternity have a TR of 2-5%. It varies from person to person. People with thick stratight hairs have the least TR- sometimes 0%.

    Sir if I don't get sides from using finasteride in the 1st year,,, does it mean my body is tolerating the drug well and I will never get sides and I can safely consume the drug free next 10 yrs.... Cuz sir I have read a couple of studies that say that most ppl get sides upto 8 months after consuming this drug.

      Not a bad protocol. Take Dutasteride on Saturdays. Will cover weekend DHT spike.

      Marginally, yes, by improving the quality of hair.
      However the progression of baldness due to DHT will contiinue unhindered since these drugs are not DHT blockers.

    Hello Doctor, I am 38 years old and started taking finasteride for 5 weeks post my hair transplant, but lately I’ve been experiencing a lot of brain fog and other cognitive issues such as memory loss and inability to focus/express myself with words.. and now I’m considering quitting finasteride.     What would one do if they absolutely cannot tolerate finasteride and has aggressive balding that started at 19. Finasteride at 1mg was causing anxiety, mental fog, and depression +suicuide ideation, and at lower doses of 0.3-0.5 there was still some ED and loss of libido
    Is there any hope for patient like this? Without finasteride?
    What about a transplant in the shape of a Mohawk and then scalp micro pigmentation on sides, and keeping hair short could that be achievable?

I decided this time to take Finasteride as my boldness has progress while the first time I had HT I did not take anything at all.       I did not use the word'NOT'. There are no absolutes in hair loss and hair transplant. In some patients who are evaluated and assessed to have stable baldness and who are not candidates for Finasteride or unwiling to take it, we can do away with Finasteride but keep them under watch.

      I would wait for 2 years and see how my patients respond before making it a routine in my practice.

      @@rickr2029 If the patent is happy, I will never ask him to change medication. I do not sell medications. I only give honest advice. Whatever seems accurate to me given my clinical experience in the field. Dr Alan Bauman uses it with good results.

      Beard growth is DHT dependant till the growing phase which lasts till the age of 21. After this DHT blockage will not do harm. Very infrequently we hear that beard hair got thin with Finasteride even later in life. But this is uncommon.

    Sir i am 18 , i noticed few initial hair in my left side of hair line are looking too short and thin , which make scalp visible in certain hairstyles while the right side dont have this issue. I went to a dermatologist he said its male pattern baldness and recommended me minoxidil once a day . I want to ask can male pattern baldness occur on just one side ?? And just minoxidil which i got recommended will help

      Yes MPB can start on one side. It is not 'mirror-image' symmetrical.

Causes hormonal acne, hair loss and later in life prostate cancer. I'd rather just take a simple pill every day and have my mind at ease and enjoy my time.

    What about the multiple shedding phase
    It has been observed that person who have gone under hair transplant faces two or three shedding phase.
    Or even after taking regular finastx his hairs seems a bit miniaturised for few months.
    What is this doctor ?
    What can be the reason behind this if true ?

      If you were in the growing stage under 21, it would definitely do so. For others it is variable. Most patients do not report thinning of beard. But in some it is clearly evident.

    Doctor do u have any any patient who was taking finastride for more than 5 years with no side affects? As I talked to about 50 members who has taken finastride but everyone has some side affect but u r saying chances of side effects is only 1 percent

      I have patients over 200 who have been on Finasteride for over 10 years without any problem. But then we have a strict process of weeding out patients who should not take Finasteride- those who will get side effects. That has been made amply clear in my earlier videos.     Doctor I m 18 i have receding in hair line ....i just wanted to ask ..have can i use my arm pit hair for hair transplant at 19 - 20 with those hairs for now....just to make my hairline defined NOT pushing it forward even by a bit and just Defining the receded hairline there only cause its really harming my confidence .....I this way my donor will not even be touched?....because the Hair in my arm pits Match the diameter of my scalp hair...

      Firstly armpit hair is thin and we do not get too many grafts from here. Secondly it has a long cycle and results may take upto 2 years to appear. The reason we do not do a hair transplant at 20 is not that hair ave not matured, but to determine if your grafts at the back of the scalp are permanent in nature. If at 24 we determine that you are not a candidate for a hair transplant since the scalp donor hair are impernanent, armpit hairs along the hairline will not only cause you great embarrassment but will look out of place and be worse for your self confidence and self image. Best is to exel in whatever you do- profession or college, or get an SMP done or even wear a wig till the day we can make an assessment for fitness for a hair transplant procedure.

      @@HairlossBUSTER It will cause intense scarring, is not permanent (lasts 2 years maximum), is very expensive and scarring will not make you a candidate for a hair transplant in future. This is the worst way forward.

      The Minoxidil only in this combination is effective. For Finasteride, you need to take it in its oral form.

      Probably either way. The Finasteride doesn't have a brain, soul or conscience. It will "never" know that it was being consumed. So don't think that you will upset the Finasteride if you change from evening to morning. It's not like you broke a date with the Finasteride.
      I would say though and get the fine doctor's input but take it regular and about at the same time everyday. And remember unless you have a flux capacitor and can go back and forth into time and back then a day is a 24 hour cycle. Thank you!

    Hi doctor, I just have a question, will finastride cause bad beard growth? Since dht is needed by beard so suppressing it will cause bad beard ? My family has a good beard gene but all have bald heads 😂, so I don't wanna loose one feature to another..

      If you are above 21 on Finasteride it most likely will not.

      @@vysakhm7835 Not if you do not have evidence of thinning. When thinning starts (if it does at all) there is enought time to get hair thicknes back with Finasteride. Do not worry!

    Doctor, I am taking finasteride, 1.25mg daily, but I had 10 days off due to certain reasons. I noticed an increase in hair loss some small baby hairs dropping along with large strands. Now I am back on finasteride for the last few weeks. The hair fall has gotten better but I am still dropping the occasional large hair and baby hair everyday. I understand the old hairs falling, but small new hairs shouldnt be dropping. Should I keep my dosage the same? And just not worry about it. Thank you

      Hair will continue to fall and regrow and thinning hair will fall off permanenetly even with Finasteride. This is since the drug does not block 100% DHT. However it gives a lot of protection still.

What I have noticed is major improvement in my hair loss. before my pillow would be surrounded with small and large strands. Now it's is just a fraction of before. And my day to day hair loss also follows. I'll take it as to remain with my continued dose of 1.25mg. thank you doctor

    Sir I am using it for 3month in every alternative day..In past 1 month I found my hair fall reduced to 80%..I have not any issue like erectile dysfunction but sir in last 2 week I have issue with nocturnal emission this is not a placebo effect sir in which day I take in that night it happens I left it for 1week in that 1 week there is nothing such kind of emission..Sir what will i do...Is this a big concern...Pls reply sir

      @@kerunsuresh7131 After the crusts have come off start with 2% and if there is no problem, start 5% Minoxidil. Yes yu can apply to other thinning hairs. Caution: though transplanted hairs will remain in healthy condition when you stop it after 6 months, the native thinning hair which benefit by Minoxidil will go back to their previous stage when you stop.

      It is not likely that all members in the family will get affected by AA. But Finasteride helps if you have male pattern baldness. Preserving miniaturising hairs with Finasteride is all good, but then you cannot stop using the drug after the rocdure is over. The hair are Finasteride dependant now.

      No doc can answer that question I think. But I've seen some guys getting HT done who went bald after 1-2years of the transplant because
      A. They went to a shitty doctor
      B. Didnt take fin.
      If you get it done from India please only go to tajinder bhatti. Indian HT doctors are the biggest scams. They don't perform anything and even if they do, most of them are gonna over harvest or give you a horrible hairline or give you stupid advice in general or don't perform the surgery themselves but a 10th pass guy would be doing it. So please, go to the best doc of India and that is tajinder bhatti

      @@shaswatachatterjee5976 I will start taking finasteride really soon but I am 23 and been balding since 16 and still can pull of certain hairstyles so I’m pretty lucky I didn’t have the most aggressive baldness but you never know how much balding can accelerate in the future. But there r people who started balding at 18 and are fully bald by 23 so I’m grateful even though I want a hair transplant to fix my hairline because most people my age have perfect Norwood 0 hairlines.

I could’ve been a nw 5 by now but I guess my genetics let me still be a Norwood 2 or 2.5. But I wish I started fin when I was around 18-20 cuz that was when my hair loss was so minor that I could’ve styled my hair almost anyway I wanted. Now my hairstyles r very limited so I’m getting on fin to prevent further balding and getting a hair transplant sometime whether it’s this year or within the next two years for sure.

    Good morning doctor, I have been on finestride for 91 days now.. I am shedding since day 14 and till date, I started minoxidil 10days a go and noticed 150hair in my shower today... And after bathing I noticed 60-70hairs on my towel as well is it normal.. or should I start dutastride once a week... Btw I use follihair (multi vitamin) and ketoconazole shampoo as well

      use a hairdryer for your hair,stop using towel,apply ketoconazole 2% shampoo for about 10 mins,get minoxidil foam,finasteride 1mg per day should be enough.

    42 post TP 20 days now. I feel my baldness has ended or considerably slowed down. I don't have the fear that a lot of people have of Fin, I want to give myself the best shot of success. Would you recommend atleast start fin for 6months and then reevaluate at that point whether I need to continue? Also immediately start Rogaine foam?

      Start with a low dose like 0.5 mg on alternate days and scale up gradually to get to the sweet spot that works for you. Generally1 mg on alternate days works for the vast majority. Rogaine has a different mechanism of action and has additional benefits.

      Cannot predict anything in male pattern baldness since the gene expresses itself according to the level of penetration/severity. However, if you are on Finasteride, the recession will be significantly slowed down. Also, grade 2 is considered normal in some and does not progress further since it may just be a family trait of broad forehead, and not necessarily androgenetic alopecia.

Sir i'm using finasteride 1mg per day from last 9 month..at the time of beginning i was using 1.25 mg per day then i started 1 mg per day.. It give's me good result but . Sir from last 2/3 month i have noticed that my sperms looks like more thickened & some gelly like Cluster within it... Sir shoud i continue the drug? Is this side effect is more dangerous?       The side effect of Finasteride (though rare) is that it causes watery semen. Your problem cannot be ascribed to Finasteride. But do get a semen analysis done.

    I'll go to you for my HT. But you apparently don't perform the procedure on anyone below the age of 25. I don't trust one doctor here in India except you. Is there a way our? Can I take steroids for a month to see which hair follicles are sensitive to DHT? So that you get to know which hair follicles can be transplanted and which ones can't be.
