  • If a hair transplant result without Finasteride is possible, why do doctors insist on Finasteride after a hair transplant?
    There are so many variables in hair transplant. I have seen patients with extensive progressive baldness who I predicted would continue to bald further since their baldness did not look stable to me. But they surprised with a great result without Finasteride.
    On the other hand, there were patients who had stable baldness at 45 and took Finasteride, but the result after their hair transplant was nothing to write home about!
    So what are we missing?
    The human body never fails to surprise. Science has yet to fully unravel the secrets, the working of the human body. And hair is one of the most sensitive organs of the body which responds almost immediately to minor changes in milieu interior- where there are so many hormones that can affect it’s growth and survival. Also, it responds to some unquantifiable stimuli like changed food habits, exercise, stress and even change of place of residence or place of work.
    Like other fields of medicine, the field of hair loss is not a perfect science.
    Though aberrations exist in how hair grows out after a hair transplant, it is the experience of many doctors in the field that the vast majority of patients do not respond well - they do not get a favorable outcome- without the cover, the protective umbrella of a DHT blocker. With the use of Finasteride, the most commonly used DHT blocker (in the dosage 1 mg a day after hair transplant and in half dosage for thinning hairs) you have the best shot at retaining native thinning hairs and maintaining transplanted hairs in the prime of health, postponing the relentless onslaught of progressive baldness and more procedures in the future as a corollary.
    Since we have so many other variables that affect transplanted hairs, and since use of postoperative medication Finasteride is one of the major variables proven to determine eventual outcomes, it is counter-intuitive to not take it.
    Navigating the rough high seas of hair loss without a compass and a seafarer’s map- without a plan- will surely take you no where. Relying only on the advice of passing boats without checking their antecedents.
    The time span in which balding progresses rapidly especially in the young does not allow experimentation with medications publicized for commercial gain. Precious time is often lost in this process, in this hesitancy to take Finasteride; and when you start using it- when all else has failed, it is much too often very late in the day. Your options are depleted, your bridges have been burnt.
    The USFDA approved DHT blocker Finasteride which is actually a 5AR blocker, and is very affordable in most countries, has been with us for over 40 years and we have not been able to do better. It has proven to be a relatively safe drug for the vast majority. For others caution has to be exercised.
    So do consider using Finasteride.
    Just because your friend or a colleague did not take it but had a good hair transplant result does not mean you will replicate his outcome. You are different- your body responds differently. By not taking the medication prescribed in your best interest, you are playing Russian Roullette with your hair loss and setting yourself up for disaster.

    Why did you defer from giving this patient finasteride? He may lose the existing scalp hairs right ?
    I have done my hair transplant and I'm 28. I'm very doubtful about taking finasteride because I felt some minor side effects earlier when I was 25. And i have always had a low libido.
    I don't have aggressive hair shedding or loss of hair.. How long can I give myself time / take a chance without taking finasteride to understand if i fall under AA / MPB or if it's just a maturing hair line?
    Probably grade 1.5 -
    I hardly had 0.5 - 1 cm receding in the sides.. no temporal or center or front lock receding.
    I humbly follow only you and look up to you.

      Tahnks for watching. This patient like all others was advised to take Finasteride. Whether a patient has taken it or not is hard to tell till baldness progresseses and they confess that they were not compliant.

    Hi Doctor! I am 39 (40 soon) and recently got a hair transplant for my receding temples. I was NW2 close to 3 (no vertex). My hair loss has always been VERY slow since my early 20's. My surgeon said I don't need to get into finasteride since I am 40, my hair loss pattern is already defined, and my hair loss has been slow and stable for many years. I have never been on any type of hair loss medication.
    Also, is there any negative effect on finasteride for patients with 1 testicle since birth?
    What are your thoughts?

    Doctor , it’s been 1 month since my hair transplant. I see a lot of hair falling off(implanted hair). Now the places where I did not have hair before , after the shedding phase ... now seems to have nothing. Meaning, it’s completely blank and I can’t see a single hair there. It’s just plain skin. Is it normal?

      I can only ask you to consult your treating doctor. Only he knows what has happened. Every clinic has different shedding and regrowth which is unique to the person, to the clinic and the technique used.

    Doctor I have some questions
    1.i'm on 2 month of finasteride and shedding a lot of hairs both terminal and thin hairs .so, I want to know why I'm loosing this hairs before starting finasteride I never noticed loosing thin hairs and this hairs are very small like colourless, invisible and are zigzag hairs
    2. Also I have thinning in only in tempals but now I'm loosing thin hairs from whole upper scalp.so,why this is happening ?
    3.You said that if sheeding doesn't stop till 3 months then you baldness is very aggressive but I know that this sheeding is induced by finasteride and not by normal MPB and after only starting this medication my baldness has become agressive ?

      Finasteride itself can cause initial shedding which lasts for 30-60 days. You cannot stop Finasteride now.

    Doctor,I am taking finasteride regularly since 9 months and applied minoxidil irregularly.Initially for 5 months my hair fall stopped completely and looked thick but since april i have heavy sheeding of hair and hairline has receeded a bit comparatively.
    I have similar situation of heavy shedding since two years from april to november when i was not taking any medicines.
    Do you have idea why this is happening?

      Finasteride does not block all of DHT and hence balding will continue, though at a lesser pace. There will be episodes of greater shedding which can be due to stress, or accelerated thinning.

    Sir l have started using minoxidil after one Month of my hair transplant on my existing and transplanted hair too. Now its four months completed of using minoxidil. But still l am seeing some hair fall (8 to 10 hairs) mostly in my existing hair while washing or putting minoxidil. Is it normal or minoxidil is not working? I am taking finastrid 1mg everyday too. Pls response to my question

      Hair will fall and grow back.
      If you are on Finasteride, you do not have anything to worry.
      Minoxidil is not that important.

      Both Minoxidil, with or without alcohol, are good. You can try Lasers and see if they work for you.

    Sir does masterbation cause hairloss,,, because whenever i did it i gets sudden itching on all over scalp n wants to rub on scalp due to harsh itching immediately after masterbation ,,,,it will be big help sir if u answer my this question please sir

    Is testosterone also has androgen receptivity to follicles

      It is not proven that testosterone attaches to the androgen receptors directly causing hair loss. Testosterone levels are not decreased even with Finasteride. In fact the levels are increased. Estrogens will decrease testosterone levels.

    Doctor , I have been recommended topical minoxidil + fine stride to be applied on head. Is topical fine stride almost close to taking finatride orally ? Is there a big difference or it doesn’t really matter ?

      Topical Finasteride is a commercial preparation for profit. Finasteride does not cross the skin barrier when applied to the skin.

      I normally agree with you but this is wrong information! topical fin has its place and is effective for some yes oral is better but that doesn’t take away from all these studies on it’s effectiveness
      Here are some studies on topical finasteride:

    I am following you for the last four months. I am taking 1mg Finasteride daily and multivitamins along with 600mg Omega 3 fish. I want to know if I don't take PRP sessions will it affect the results?

    Doc i am 30
    The corners of my hairline was receding.
    So 6 months ago i started taking 1mg on alternate days.
    I dont think i have any side effects. Labido is low but it was low sometimes even before starting finasteride.
    Anyway i feel like my hairline has gotten worse.. whats the best course of action.. shall i increase to 1mg daily? Or just stay persistent as 6months isnt long enough.
    Or shall i take a break? And then take 1mg a day??

    Dear Doctor, hi, I'd greatly appreciate if you could answer me. I was taking finasteride 1mg daily for 3 months and unfortunately I ended up with lots of acne on my face, I didn't have acne before I started. Other obvious (not a nocebo) sides were decreased ejaculate volume and force, watery ejaculate. I had to stop taking it because of strong acne in the first place, otherwise I'd harm my face. It's been 3 month since I stopped taking it and I'm still recovering from acne and post-acne. I'm currently taking Saw Palmetto. What would you recommend me? Seems like my body can't tolerate such an aggressive DHT blocker as finasteride :(

    Sir, i got hair transplant 2 months back. I am 27 years old with grade 5 baldness and my baldness is not stable. I got 5000 grafts implanted by a combination of FUT and DHR(FUE). After 1 month of transplant ie from the past 1 month i am only taking minoxidil and multivitamins. Am i doing seriously wrong by not taking Finasteride? Secondly, if i start it now ie 2 months after transplant will it recover the time lost in 2 months?

    I have a bald spot on left side forehead since childhood but no baldness. I want to get transplant on one side. Is it possible?? Also i dont want to take finasteride as i don't have hair loss? Do people like me also need finasteride?

      One side bald spot is not androgenetic alopeca. It can be a skin disorder and for this Finasteride is not required. I will need to see pictures.

    Hi doctor. I recently did a trichoscopy examination on my hair and found that despite thinning on my temples and hairline, I have next to no thinning on my vertex/crown. From your experience of seeing many patients' balding patterns over time, would you say that it is likely I will not develop into a norwood 7 since I'm only balding at the front, or not?

      If you are young, it is to early to predict anything. Predictions in MPB are not possible. How old are you?

    can we consume both finasteride and saw palmetto together?
    As I have known saw palmetto is pure natural substance.
    But taking both together ,will there be any side effects increased or any other disadvantages?

      Saw Palmetto is an extremely mild DHT blocker. Natural sources cannot compare with efficacy of pharmacological grade products. You can take it alongwith Finasteride, but then why would you need to do so!

      @@narendarrajaa6656 taking extra fin would be more effective. So yes palmetto and fin would be more effective. Its just more dht blocker going in you. But don't do it, fin is enough, more than enough. I'd actually try just palmetto, no fin, see if it works.

    Hello sir, if someone has lowered libido with the smallest finasteride dosage what can one do. Can someone have multiple hair transplants? What about topical liposomal finasteride?

      Yes if baldness is relatively stable and they have huge donor resource to tap into. Topical Finasteride does not work as per what I have seen in my practice.

    Please give the guidance on Shampooing after transplant
    I am 2 months in after surgery and currently using Johnson baby shampoo indirectly--as in making a foam of it in a mug first and then applying it on the scalp
    Is it the right time now to just shampoo my hair directly as it is done normally without worrying about the foam.
    Just shampoo my hair directly
    Also,
    Can I use a regular shampoo brand now as in Dove or Loreal

      Please continue a mild neutral pH shampoo till 6 months . You can shampoo directly at this stage.

      Not exactly dandruff
      But sticky type collecting

      A shampoo brand for my need

      Maybe baby shampoo would do good, I just need to clean more vigorously and thoroughly

    I had a FUE 1500 grafts 3 years ago. I've never taken Finasteride in my life (I only put on minoxidil topical once aday ) and my hair looks great (the transplanted hair and the original hair both)..
    But maybe because I don't have agressive hair loss (only slight receading which I covered )

      if you have good results with topical solution Minoxidil, just stick to it. All hair loss medications when used for androgenetic alopecia have to be taken long term.

Can I ask, have you found in your own clinical experience, that taking finasteride for a patient who is say Norwood VI can result in improving the hair quality/thickness, etc in the donor region to the point that it can help them to become a hair transplant candidate (all things being equal)?

      This occurs most commonly (in my experience) if we do not clean the scalp properly. Minoxidil accumulates and on top of it oil is applied. The caking eads to crusting and susts keep breaking off. However, dandruff may also pre-exist. If the flaking is too much see if you are applying too much medication. If not, switch to 2% Minoxidil and see. If still there is no respite, stop Minoxidil altogether. But then you need to be taking Finasteride orally or else all hair supported by Minoxidil will fall off in the blink of an eye!

      There are none even near to pharmacological grade Finasteride. Saw Palmetto has been used for many years but if your baldness is aggressive and you are young, please do not waste time on such things which you will repent later. Precious time will be lost and Finasteride cannot regrow lost roots.

      The oiliness of the scalp is likely normal for your scalp and existed even before the hair transplant. Suggest keep it clean with twice a day washing. Do you have flaking?

      There is no point going in for a hair transplant without Oral Finasteride.
      Second point: mechanism of action of Minoxidil and DHT blocker Finasteride is different, hence shedding will take place irrespective.

      I'm not a doctor so still verify what Im about to say. Finasteride on it's own addresses the root cause of hereditary hair loss and minoxidil is not. So finasteride on it's own is still very effective long term in keeping hair if the drug works on you. Minoxidil alone will do nothing to prevent hair loss and your natural balding progression will continue with minoxidil alone.
      So finasteride -> minoxidil.

      First see if you are otherwise in good health, do not have other cause of baldness and that you are physically and psychologically in a good condition to start Finasteride. You will need to rule out pre-existing hypogonadism or erectile dysfunction. A visit to local doctor will help. Also these are prescription drugs.

      You are welcome. Those on Finasteride also need it and have no problem. Problem happens when used wantonly without medical supervision.

      Finasteride is generic. I never mentioned brand medications. How can someone benefit by promoting generic medication? If theer is a way let me know so I can get some money on the side.😊
