    Wow, I remember growing up in Swaziland and my then 11th grade English teacher talked exactly about this in the context of us playing soccer and basically how it's a lottery when you get hit in a certain spot at a specific time. Prayers to him and hope he makes a speedy and full recovery

    When I was a teacher in the early 2000s, the boys at the middle school “played” a game called “OpenChest”. They would calmly start a conversation with a peer and then one of the kids would slam his fist into the chest of the other kid. Most of the time, the boys would be ‘fine’, but on at least one occasion, a male student at a different school fell down dead. We, as teachers, had already warned the students about the danger of this game. Not knowing the proper reason, I often said they were pushing the sternum (I think that’s what I said!) into the heart. Whatever, we were correct in warning the children about the dangers and by the time I retired in 2018 I had not heard of that old problem rearing its head anymore. I wonder if it was the same thing that hurt this young athlete.

      If true, most likely covid infection, which can cause heart damage. A few billion people had covid before the vaccines became available. BTW, where did you get this 1600 number from? The only study I could find with a 1600 number was one showing cases of myocarditis from covid infection.

    Commotio Cordis is from a small object at hi speed (baseball, lacrosse ball, hockey puck), not from a low speed wide-base shoulder pad/helmet that impacts another chest protector on a shoulder pad. In medical school we as well were taught in diagnostic medicine, “When you hear hoofbeats, don't look for zebras,” meaning that diagnosis begins with the simplest, most likely answer and proceeds in a methodical way. Only after eliminating the obvious should one probe for something more rare or unlikely. We live in an age of COVID, so what is most likely? Hi on my diagnosis would be undiagnosed S- Antigen induced myocarditis. We are seeing this in young men. Myocarditis affects your heart's electrical system and muscle cells, leading to irregular heart rhythms. Cardiac MRI would be best screening tool but way to costly for the general population. The tackle led to sinus tachycardia which may have set off an arrhythmia in a diseased heart.

      If it were cordis, does survival mean there is essentially 100% recovery of healthy heart function? Obviously, if any sort of myocarditis is involved then the heart would remain compromised after, and the patient might continue to experience things like sudden rapid heartbeat or symptoms that could be observed with continuous heart monitoring?

    Not questing your explanation at all, but I do wonder how we know it can only happen in those 20-40ms? I would assume there is no way to actually test this and no way to prove the impact did in fact occur during that time frame of the cycle.

    What a fascinating talk! Thanks Dr. Hansen. I clearly recall one incident when I was about 10. This is close to 50 years ago. I was running in the classroom and ran into a broom stick straight on. It hit me right at that spot! I fell on the floor and gasping for air for about 20 sec (felt a lot longer) and other kids were just laughing at me. I have been wondering what that was! I guess that was a close call for me!?

      You probably just got the wind knocked out of yourself. It's fine nothing to worry about it happens to me sometimes. Taking deep breathes while it happens helps.

    I noticed as he fell, he hit the back of his head really hard against the turf. It's difficult for me to imagine how that wouldn't result in a concussion or some form of head injury. My question is, if he did suffer a concussion along with the cardiac arrest, how much would that affect his chances of survival and recovery?

    Try releasing his chest blockage from underneath his armpits. when Demar's was hit, his body bent forward and chest/ribcage contract, the teres major carry the clavicle and arms downward, the ribcage press inward, the intercortal muscles contracted dramatically so the nerves were inert under great muscle tensions.
    Therefore try to restore the tergidity of the ribcage
    1.gently rub and release the tensions round his upper arms, restore the his deltoid to anatomy position: that is the clavicular, arcomial and the scapular parts, especially the clavicular front part, the insertion of the teres major and the biceps,
    2. gently rub, release the intercortal muscles under both armpits, place the fingers between the intercorsal muscles of 3 to 7 ribs, (especilly the left 3 to 5 rid gaps on the left where affecting the heart bit rhythm) follow the breath-in motion, press both lateral ribcage inward, hold a little until his out breath widen, keep pressing, observe the filling and tergidity of the chest,
    3.release the tensions between the floating ribs, correct the balance between both ribs, release the tensions at the tip of the xiphoid process, gently grap the muscles up at the tip, this can release tension of the diaphragm and restore his chest breathing,
    4.then hold two arms, bring it close to the armpits and apply a little bit pressure onto upper ribs cage, raise the arms, clavicles and upper chest up by holding the upper arms up.
    Hope this can help him, maybe we need to release and correct his throat, widen the supraclavicular fossas and hyroid bone, but too complicated for you guys maybe.
    This can help many breath blockages due to post COVID-19. post chest operations etc.

      Commotio Cordia is actually 20 cases in the last 20-30 years! Not 20 cases per year, so it is even more rare, also found to occur in youth.

      Yours is a great question.
      In my opinion, he could not have been in V-Fib when he got up and was adjusting his helmet.
      IF…..this was a case of commotio cordis, as the doctor suggests. Hamlin would have gone into V-Fib the exact instant his chest was impacted- not 5 or 6 seconds later.
      Also, and only my opinion, his chest was impacted by the shoulder pad of the other guy- not a small hard object as this doctor states would be most likely to cause a commotio cordis event.

      I see what you’re saying but without knowing a number of things about his heart, structural abnormalities or a conduction disorder etc., we don’t have enough information to confirm or reject commotio cordis. Years ago I was taught an individual might have about 8 seconds of consciousness until collapse while in a ventricular arrhythmia. Just wondering if a supremely fit athlete would be able to initially jump to a standing position at the initiation of v-fib just prior to complete collapse. It’s a crazy question I know🤷🏼‍♀️Thanks for your input.

    So back in college as a soccer goalkeeper, when I had a collision on a breakaway where I took an opponents knee to my sternum which partially tore my right SC could have killed me if the opponents knee impacted more to the left tearing my left SC joint.

Especially in the climate we live in. A young, healthy, fully Vax male suffers heart arrhythmia/heart attack after serious blow (that condition has been defined for so many others MINUS, MINUS the blow)...during this ongoing pandemic.       I do not believe so. It usually happens to young kids. I knew a 7 year old hit in the chest
      with a baseball and died. These NFL players have physicals all the time, do they not? I
      would think a preexisting condition would have been found.

I’ve just witnessed a vagal response in many patients and it’s a known contributor to MI’s while using the bathroom. That’s one reason we find them next to the toilet so often.

Takes about 20 seconds to pass out after your heart stops from lack of oxygen which leads to the nerves not being able to function since the brain can’t send electrical signals, as you suggested. That’s a resulting factor not a deciding factor

      Players get injured all of the time and there's a move in pro wrestling called the heart punch and people have died from that too.

    You said it, doc -- there is a very small chance that a traumatic chest injury could cause this type of arrest. Much more likely is a preexisting condition due to hereditary or acute disease. NFL players are extensively screened for both. That leaves heart dysfunction arising more recently than his last screening. Now, do we know of any such common or uncommon adverse cardiac pathologies that have surfaced in recent months or years in young men?

      its known to be very rare and known in baseball and hockey where there is a small concentrated area versus even a helmet to chest contact. its almost never happened in football and is mostly known in other sports where there is a small ball or hockey puck.
