How do you review all old and new patients in 30 minutes? That's awesome. I'm a second year resident and it still takes me an hour to 1.5 hours to review 9 patients. I'd really love it if you addressed this in a future video :) thanks for all the info

    • @TheMDJourney

      Happy to.
      1. It easier when you're an attending cause I'm looking for the info I care about (ie. If a pt is there for diuresis - then I'm looking at oxygen needs, ins and ours, and electrolytes. If a pt is there for INR monitoring before DC then that's where I'll go to first).
      2. Have a flow you take for each pt quickly (vitals, labs, studies, and overnight notes/consults) helps move things a long
      3. Write down what matters vs. everything. Obviously your attending may care for certain things. As an upper level you need even less as your interns you may be covering should have the smaller data. If the vital is the same or stable - then I either don't write it down or notate a check next to BMP for example.
      4. Don't let pages or requests stop you from finishing your current task. If you get a page - take a quick look to ensure it's not an emergency and then finish your current chart review. Then answer. A nurse is likely not going to notice you not responding for 2-3 mins.

    Dr
    Good day
I really want to ask for a favor as an internal medicine resident who works out of the US
Would you be interested to do some shorts talking about some cases you have
    Including
    Managment plan as in medication
Or if the patient needs any consultation
The note you write about the patient (what are the most important things to write down ) and so on
    I’ll be grateful forever 😊😊

15-19. This counts new admits and discharged pts for the day. So likely an even turnover of 2-3 DC (2-3+ admits). Sometimes more DC than admits and sometimes the other way around.
