Understanding drug induced ILD / pulmonary fibrosis

  • An overview of drug induced interstitial lung disease / pulmonary fibrosis. This video explains how the connection between a certain medication and pulmonary fibrosis is made.
    Several drugs can induce lung reactions and fibrosis, but unless we have a clear timeline it can be very hard to confidently say it was a specific drug that caused the fibrosis.
    Remember there may be other causes for pulmonary fibrosis that may need to be investigated.

    My friend likely has drug induced ILD, starting with an ICI drug used in her case to treat NSCLC. The drug was ineffective against the NSCLC so she was given several kinds of chemo subsequently, all of which are associated with drug induced ILD but at a lower frequency than the ICI. We suspect that the subsequent chemos made the ILD worse as though the first ILD sensitized the lungs to the further insults. This drug induction seems to take place over a long time since the use of the drug. Is there a way to detoxify the lungs against further insult (maybe on a drug by drug basis) so that treatment may be more effective? Are any treatments especially effective against drug induced ILD? If long term high dose (tapered) steroids have been ineffective might short term super high dose steroids work? Are there any steroid saving drugs that are effective against drug induced ILD?
