    Melasma is an ongoing issue that both young and old struggle with, and it oftentimes gets confused with hyperpigmentation. While hyperpigmentation can usually be traced back to a specific cause, Dr. Karam explains that melasma is a different story. The cause of melasma is not clear, but there are certain circumstances that experts say can attribute to melasma's occurrence, such genetics, birth control pills, pregnancy, and sun exposure. While the cause may not currently be 100% understood, there are certainly things we can be doing to help prevent melasma from happening. It's most important to understand that living a healthy skincare lifestyle, and consistently using quality active skincare ingredients along with sun protection, will always help you to prevent skin conditions like melasma from happening. Making good decisions for your skin and overall health and wellbeing will always be the key to maintaining a beautiful, youthful complexion.

      I do fairly heavy cardio five times a week (as in, drenched in sweat) - lately I’ve been wondering if I should back off a bit from doing that. 🤔

    Dr. Amir I am an italian follower of yours and I just want to say that your work online is incredibly good and really saved my skin. been almost 3 years following you and this is the best channel I follow. I am dealing with a melasma and the first thing my dermatologist gave me was hydroquinone, but now I more and I get your point. I still havent used any of the product she prescribed me and this video is really useful and gonna 100% follow your tips. ❤

    New subscriber here ❤ and best video on melasma I’ve seen so far! I’m 51 and barely have a wrinkle on my face but have had melasma on my cheeks for the past 6 years (birth control pills, genetics and maybe rebound from using hydroquinone for 10yrs without break for maintenance after laser for sun-induced freckles). I’ve tried everything and finally gave up last year, just using spf50+ daily and full coverage foundation. A few months ago, l saw a new derm who put me on a 20% azelaic acid+4%hydroquinone compound cream, then 20% azelaic acid+6% arbutase, and now Triluma. I’ve also been using Vit C and obsessed with sun protection. And in just 2 months the melasma had cleared up to the point I am comfortable wearing no foundation in public. So happy! Just last week though, I over-exfoliated and my skin became irritated. Now parts of my melasma have darkened again… doctor is right, patience and consistency are key 😥

      Hey just a heads up. I was given exact same treatment for my melasma as you commented. Triluma cream worked wonders on my skin for one month. One day I woke up and my dark patch was back. More darker than before. I’ve got to know later that there is some timeline for using triluma. Please talk with your dermo.

    I appreciate the doctor telling his audience the honest truth about in office treatments. I spent a lot of money for laser treatments for it to come back. Money went down the drain. Melasma will never go away just have to control it.

    I was neither on HRT nor Birth Control I was however perimenopausal! I struggled with a bad case of melasma for a couple of years and finally found relief with prescription Tri-luma which was slow but worked for a couple of years, but for a better result after that initial effort with Tri-luma I used Treitinoin as well as vitamin c. My skin in not perfect especially during the summer because not only does the sun darken the places on my face but the heat enflames my skin as well. I am actually learning to live with it and my husband makes me feel beautiful no matter how terrible I think I look! Thank you for talking about this issue! Maybe someday melasma will be easier to deal with.

    Oh em ghee! Ty Dr A, I’ve been going to derms for so long and they didn’t tell me all of this. I noticed the melasma on skin in my early 40’s and I feel like it gets worst everyday. I will finish up with the current treatment that I’m using and I will look forward to using your Trifecta in the future. I’m really glad I came across your channel. 🙏🏻

    Very helpful video. I have been managing Melasma and sun damage. I have had in office treatments results of which were temporary. I now use alpha arbutin twice per day, vitamin C & SPF 50 morning, Red light therapy daily, Retinal alternate nights, wearing hat, face covering & gloves. I use a UVP umbrella - I too don’t care what others think. 😂😂

      Hi! Is Red light therapy helps your melasma? I thought that makes it worse, I used de infrared and my melasma got deeper 🥺 I saw videos saying the green light therapy helps melasma a lot, I will try

    Your wife is absolutely stunning. You are a beautiful couple. Thank you for this video. I think I learned that I am dealing with hyperpigmentation vs. melasma. I have lupus & when I was in my 30’s (I am now 59), I had severe flares that resulted in sores on my face. It was terrible. I now have white spots where some of the sores were & also darker age spots. It is the worst on my lower cheeks & in my hairline. I have also lost a lot of hair where I had these sores. I have had quite a bit of improvement with adding tretinoin, salicylic acid, lactic acid, & mandelic acid (all in moderation, of course. I am always trying to learn as much about my skin as possible. I appreciate videos like this one, Dr. Amir. Thank you so much!

    I have upper lip Melasma, I never had it during my 4 pregnancies! I am not on birth control or hormone replacement!
    I use everyday vitamin C, retinol and Alpha Arbutin! Plus a mineral sunscreen 50. It really helps me.

    Great video! I’ve been on the pill since I was 19 and I am now 50. I’ve had in office treatments, chemical peels, IPl and I never knew I even had Melasma until the end of my last IPL appt and the nurse told me I had Melasma and that it would probably come back!
    That was an expensive and very painful lesson!

      I got melasma from the pill. I was on it from 16 to 30. Nothing has gotten rid of it

    Thank you for your knowledge!! Saying avoid the sun is not always possible , but being specific about “how “ hats , glasses, umbrellas, seek shade staying cool is important that way us melasma sufferers can know what to do ! Sunscreen is not enough 😢

    Wonderful video of skin School Dr. Karam. It is very educational and informative with information we all need to know. I never knew birth control pills had anything to do with Melasma or hyperpigmentation. I have always been very careful of the sun but not knowing of other things we need to know.     You're very skilled! Your wife will always be grateful to you. When looking up about dark patches hyperpigmentation will almost always be the info, but the advice of Tretinoin can also be applied to melasma. Not many dermatologists understand it and will have their own theories, adding to the confusion. Reaching for the Adapalene will be a less irritating alternative.

    I have had malasma for 15 years, my mother has it too. I wear 50+ sunscreen in the summer which helps but malasma is not curable. It gets lighter in the winter. It’s just something I’ve learned to deal with. Wear a very large hat. Yep, do it.

    I have heavy melasma on my face but have to use a lot of makeup to cover it and cannot reapply sun block all the time. I feel so embarrassed for this condition that I have never been out without makeup

      Try sunblock stick made fr japan.. i see in the instagram people use that uv stick several times a day without ruining their make up ..but i dont know the brand's name of it exactly.. maybe u can browsing

    Hydroquinone has always been a temporary fix for me. Now I use all natural melanin inhibitors, vitamin C, niacnamide, and retinol, mineral based sunscreen, hats, and cleaning up my diet have made it barely noticeable. Mine was really dark btw and hereditary . Have hope ladies.

      Hi. I use a hybrid spf does it works? I mean I am obsessive wit it . One thing that helps me is mixin the cerave resurfacing retinol ( green package with the Mar love green hyperpigmentation treatment. Fantastic combo. I use vit C in the am with a lot of spf and reapply durin the day several times

    Very informative, best video I’ve seen, thank you. The same thing happened to me, after a year of hard work, two days in the sun of Los Angeles visiting home, I lost all my progress and made my skin worse. Now i have learned to always wear hats (if exposing to sun for more than one hour), good sunscreen (I used ILIA brand and their spf ruined and aggravated my melasma by not providing adequate sun protection, never again, I’d avoid ilia products for melasma patients), I also avoid prolonged exposure to sun at certain times of year and geographical locations and most importantly reapplying sunscreen every 2 hours religiously. These things greatly improved my melasma, it has been 2 years since I started on this melasma healing journey and I finally feel I’ve made progress. I had better results using long term 15% azaelic acid and using less products on my face and going more clean and green. Dr, Bronners is great, anytime I use anything else to cleanse my skin it makes my skin worse. Also addressing my internal health almost helped more or just as much as the external creams etc. like thyroid, hormone, etc.

I have been dealing with it for years! I am currently using most products you mentioned and spf like craaaaaazy but see no results even after a rigorous and religious commitment for 1.5 year. :( do you think there is something internally that could be done?

    Thank you doc! You have given Some clarity on the differences in melasma and hyperpigmentation. I just can’t help but think that there is nutritional knowledge that could be implemented to help from the inside…we were not meant to be covered up and lathering chemicals( including some cancer causing sunscreens) all over our bodies! Also, I like having color to my skin( nothing crazy but some color!)🤷🏻‍♀️ I just think supporting the bodies natural functions and productions( including melatonin production ) is the way to go! I make my own lotion and use natural products on my skin! The expense of some of the other skin care items on the market is ridiculous! Just sayin’….thanks again! You seem to be fairly balanced and reasonable in your knowledge…I am going to continue to watch and filter what works for me! Thanks, Jen

    Great video. I agree with you. I've also noticed that physical heat, be it from the weather.....AND/OR from working out can bring out my melasma. My work around is working out while trying to keep my facial skin cooler. Swimming before the sun comes out, helps me work out without my face getting too hot and i use a cool towel or iced face mask to keep my face cool on my spin bike at home. Have you noticed this problem in your practice? Do you have recommendations for keeping the face cool in the heat and while working out?

      Yes, steam, light from lamp( i get my lashes done and I wear a mask so the light doesn't affect my skin.)

      I heard anything with heat like a sauna or hot yoga workouts will flare up melasma. So your cooling exposure makes sense.

      I experienced this when I had a patch of hyperpigmentation on my forehead after blowdrying my bangs and then getting out (it was just a little bit of sun in March, not some crazy summer weather), and I think the heat from the dryer is what triggered the hyperpigmentation. Fortunately it went away after a year or so, but it made me aware heat is definitely a trigger.

    I can relate to your friend, after a trip to Mexico, I was doing great, up till the day before we came home we went to the pyramids and the sun was so strong, I could feel my face burning, the next morning, all the progress I had made was gone just like that, I had applied my sunscreen like normal in the morning, but didn't consider the humidity, or how I sweated off my makeup and sunscreen. This was in early November and I think I have only recovered 50%. I know someone who did laser and her skin looks great, but I'm homely terrified about putting a laser to my face. I'm happy I came across your video. And yes mine was brought on when I switched birth control and now I have Hashimotos as well 🥴

    Thank you Dr. Amir this videos has helped me identify the difference between hyperpigmentation and melasma. I thought i had melasma all over my face heh but I've actually got a combo of mostly hyperpigmentation on the cheeks and little dots of melasma starting near the eye/cheekbone area.     Very helpful and honest video. Thank you so much! I know I have melasma instead of hyperpigmention after watching. I have a blood disorder called Pai-1 that causes blood clots. This is the reason my face is discolored severely.

    I rotate vitamin C with retinol and always use a balm and argan oil.
    I wear Tilley hat's designed for military for sun abd heat protection yet most are stylish. I have reaction to sun due to damage and sun poisoning much younger. It's miserably discomforting if a flare up occurs.
    I also use a collagen powder daily.
    My skin looks much younger and so soft.
    Appreciate the tip about inflammation. Will monitor closely.
    I just try to give what my skin needs.

    100% correct, my lifestyle is not conducive to having office treatments as I'm always out on the sun and go to the gym everyday. My melasma has gotten progressively worse. I'm just going to live with it as it's rather live and enjoy my life outdoors.

    Yes I heard about pigmentation issues with HRT and that's the negative part of HRT when it actually does so much good for us. I am very prone to pigmentation at my age and just begun HRT. Hopefully melasma stays clear from me! I don't agree that melasma induced by pregnancy will clear. Susan Yara, a skincare TH-cam content creator shared that she had that but from what I understand, hers did not go away and was quite a problem to get rid of.

      I will never understand this. We had plenty of hormones when we are younger but all of the sudden it's re-introduced after menopause it has the potential to trigger melasma. makes no sense to me.

    Thank you for looking out for us melasma people. It's reassuring to see that I am doing pretty much all the things you recommend. I did not yet manage to get it improve a lot but as it looks I stopped the progress (darkening), which is already good.
    To everyone with melasma who doesn't like to wear much make up: a colour corrector in peach mixed in with a bit concealer to get the perfect tone used as a spot concealer on the melasma patches gives more coverage and correction than thick layers of foundations. A light dust of powder on top and the skin looks a lot more even in tone.

    I have melasma, started in my mid 30s. I've tried everything. After years of tretinoin and hydroquinone (12%), peels, laser etc. Leaving it alone and just adding niacinamide with my normal routine, and occasional pen microneedling has made it nearly go away. Sunscreen daily. I've taken a break on tretinoin, but will start back up soon.

      @@georgia_rain Lasers didn't help. So I just use a microneedling pen occasionally (about once a month at 1mm) and use Niacinamide, vit C, tretinoin and arbutin. I"ve seen more results than when I was doing a lot more to my face. I only use hyaluronic acid the day of needling and the day after. Don't re-introduce topicals back into my routine until day 3. For some lasers do work, so just monitor your results, and then decide if it's right for you.

    My doctor prescribed Triluma! I use it at night onnthe affected area. Its been helping alot. Takes weeks to get alot of the darkness off,but it works well for me! I pray it stays this way!

    I didn’t have melasma until I had laser treatments for a few blemish spots 15 years ago. I applied sunblock, but the mistake I made was not apply it 2 or 3 times per day

      There’s different types of lasers. The dermatologist needs to use the laser that’s right for your skin type and at the appropriate strength. The laser that caused my melasma happened 15 years ago. So, it could’ve been the wrong laser at the wrong strength used on my skin, or that lasers should not have been used at all.. I don’t know. There may be better lasers out there these days, but personally I would never have another laser touch my skin again.

    What is your take on Tranexamic Acid? I take the tablet form and it works! (along with aggressive sunblock and hat, no exceptions). I stopped Hydroquinone as it made my skin red and inflammed. I use retinol a few nights a week as well as glycolic acid some nights. So far it's been working.

    I recently made a push to get on top of my melasma. I have to say identifying the type of melasma you have is really key. I have a history of rosacea that I primarily treated with sunscreen. This has calmed down with aging. I recently added Azelaic Acid topically and ramped up licorice root, AHA/BHA and mandelic acid treatment at night and this has really improved things. Good improvement in 2 weeks!! So really hopeful about this at last. Just encouraging everyone to think about what is your skin inflammation caused by and focus on fixing that.

      It has a lot to it because of adding things over years. AM: wash with CeraVe SA cleanser, moisturize wet skin with Curel Hydratherapy, add vegan Squalane or It's Skin's hyaluronic acid serum. Apply OneSkin- O1, apply Innisfree green tea seed serum. Apply Power of 10 Li effector serum and The Ordinary's Azelaic Acid 10%, focusing on melasma problem areas. Apply Alpha Skincare AHA/BHA serum, The inkey list collagen peptide serum, The ordinary 's vitamin C 23% serum. Apply SPF 50 mineral sunscreen. Then after this sets up, apply make up. PM: Remove make up with Yadah cleansing balm, hydrate with Curel Hydratherapy on wet skin, add vegan Squalane or It's Skin's hyaluronic acid serum, then rotate various night time treatments: The inkey list mandelic acid treatment serum to problem areas, prescription Tretinoin 0.1%, or the Ordinary's 30% Vit c in silicone.
      It's a lot of acids topically but I don't have sensitive skin and adding the Azelaic Acid has been working great. Good luck on your treating melasma at home journey. Hope this helps!

    After my hysterectomy in 2004 horrible melasma on arms and cheeks, my doctor at the time thought estradiol topical was the culprit. I stopped it and after 4 years of weekly microdermabrasion it subsided. Then a decade later I started to develop skin cancer that I was told was estrogen driven. Now that I’m in my 60s, the melasma has returned to my left cheek 😢

    My derm prescribed me hydroquinone but didn't tell me that it should only be taken temporarily, so I used it for 3 years straight. It wasn't until 8 months ago I read somewhere that it should only be used for 3-6 months max, at a time. So I slowly weaned myself off of it (& moved my records to a new dermatologist's office...). Luckily I had no rebound melasma. Not yet anyway. But I can say 3 yrs on hydroquinone wasn't any better than 3 months on it. At the very least, it just helped prevent it from getting worse.

    Everything he said is correct because I had malasma except about the sun. I always wear my cc cream with spf 50. I go in the sun all day, carribean vacation, boating, etc. My malasma doesn't come back. Got rid of it from hydroquinone, picosure laser, and retin A. Maybe because my malasma is from pregnancy and IVF injections and not from sun. Because the malasma i got from the sun on my upper lip is still there😩. Not as big since I got a lip lift, took most of it out. I am confident to go without makeup. I had bad skin when I was younger, breakouts, big pores, but rentin A has smooth out my skin.
    IT CC cream has spf 50 and it has coverage. I love it!!

    Dr. Amir, I cannot agree with you more about birth control causing Melasma. I’ve never in my life had a problem with this until a few years after inserting the Mirena IUD. Melasma started appearing on my face and just progressively got worse. Upon the 5 year expiration, I replaced it and a few short months later, my Melasma was darker then ever before. Once, I found out it could be from the birth control, I made an appointment and took it out after having it for 7 years.
    It’s been out for 6 months now and is getting lighter but I still can’t seem to get rid of it. I apply sunscreen everyday and I live life afraid to let the sun hit my face for too long. I can’t enjoy the beach or a pool or tan like I used to. This has severely impacted my life and I don’t know what else to do. I’ve heard of specific laser treatments to even the skin tone back out. Will that work? I know you say it may come back with that but I’m 41 and still have my whole life ahead of me and I certainly don’t want to live the rest of my life hiding my face with pounds of makeup and color corrector. Any other suggestions?

    You are absolutely right. I tried everything in the book, including going to a doctor who used a laser on me and made it ten times worst. For years I was so depressed about my face and one day, I just left it alone and gave up. The only thing I do now is wear SPF every day, keep away from the sun, and avoid sugar. It's still there but it's much lighter than before.

    I've noticed that making sure my skin is well - moisturised really helps, dry skin seems to worsen melasma and stress is a trigger for me too. Strangely sunlight doesn't affect me, but I think that's because I don't use actives on my skin - my feeling is they sensitize the skin, making it more susceptible to damage from the sun.

    I have melasma "mustache" and sun spots on high cheeks. Ive tried every active product with no improvement (i wont use HQ). Because I have tan Asian skin, i am apprehensive about in office procedures because I dont what to experience the side effect of rebound hyperpigmentation . There needs to be more providers who are experienced treating patients of color.

      Im southeast asian, very olive skin. I had malasma too, used to, was scared of using HQ like you. But my doctor suggested HQ and rentin A cream. Love it!! Malasma is gone. Be patient, dont use it daily. Every 3 days or so. Now I use it once a week but use just rentin A (alone)every other day & vitamin C.
      My malasma was from hormones(& pregnancy) so I think it depends on how it developed because the one from the sun on my upper lip isn't going anywhere and HQ doesn't work.
      I did get a lip lift and some were cut off during that surgery.
