    I had the same problem with the Mcmurrys test. I performed the test after my brother came home after soccer practice believing that he "hurt his knee" I then performed both Aplys and Mcmurrys with no discomfort during the Aplys and only one click during the Mcmurry. I told him to go see a specialist after he was unable to fully flex the knee without extreme pain, which was another test that I learned in school. It turned out that he did have a posterior lateral meniscal tear after the MRI. According to my brother the PA was much more aggressive then I was during the mcmurrys test, so it seems everyone has different way they perform it. He said he felt like she was going to "tear my knee off."

      We think that physios (especially students) are often too afraid to provoke pain in patients, although most tests are designed to provoke pain.
      On top of that, the fact that pressure and performance of these tests already tells a lot about the weak reliability of all meniscus test.
      At last, their validity is bad and even if a meniscus lesion is detected, the question is if this changes anything in our management --> only in case of severe locking of the knee it could make sense to get surgery, for the rest conservative management should be advised.

    Hi, First of all thanks for sharing this content,
    I had meniscal suture surgery for complex radial rupture of the join of the body with the anterior horn of the lateral meniscus 5 months ago. I am a long-distance runner and 10 days ago I had a new MRI scan to see the healing and a hyperintense stroke emerged at the junction of the body with the anterior horn. Since surgery, I have a medium sized parameniscal cyst that presents a little pain during the test, however, every week I have been stronger, more flexible, and recently I have been trying plyometrics at therapy sessions but I am still a little concerned about the cyst and the slight pain pricks during the test.

    Could you have a meniscus tear without any pain and swelling? I have clicking and popping and really minor pain bm that resolved with exercising the surrounding muscles. However knee is still clicking /catching when I walk

    if the patient feels pain in one direction and not on the other direction, would it pertain to a specific meniscus or ligament?

      I think the direction In which the pain is present indicates whether it is the medial or lateral meniscus.

    Hi,
    I twisted my knee 7 weeks ago and I still have a pain only during Tesaaly test (internal rotation tibia - pain on medial size od knee). MRI shows nothing. Have you some idea if not meniscus?

    Can I please get some clarification on this test’s specificity and sensitivity? Many resources and my university still claim it is very sensitive and specific for Meniscal tears. Thank you

      @@Physiotutors I believe my university still uses the original study by Karachalios et al., 2005. I have brought up this issue with my teacher. Hopefully by next week, I will have an answer as to why they still use the original study.

    Pain during internal rotation in Thessaly test caused due to medial or lateral meniscus??...is there a way to differentiate individual ligament....

      Internal rotation will stress the medial meniscus posterioly and the lateral meniscus anteriorly.
      The ligaments can be tested with several tests that can be found on our channel.

    I have got grade 1 Meniscus around 5 months ago... and over time my pain is healing drastically due to my intensive care, and unusual stress on the knee. Does this indicates my torn meniscus is getting repaired?
    with this can i also do cardio activity? just the elliptical/airwalk? - Please respond! Thanks

      Hey Waleed, it depends on where your meniscus was torn. If it was on the outside (red zone) then it has a good chance to heal, if it was on the inside (white zone) it will not regenerate. Even if it doesn't regenerate, most people can live with it without any problems if the knee is not locking or instable.
      The best advice we can give is to see a local physiotherapist as therapeutic exercises usually have good outcomes in case of smaller tears like yours. You should be able to do cardio without any problems, but this would also be something you can gradually start under the supervision and advice of a physio.

      Hi! Thank you very much for the reponse, i highly regard it!
      this is what the MRI impressions were:
      "Small hyperintense signal intensity area seen in the posterior horn of the medial meniscus not reaching the articular surface, suggestive of Grade I meniscal tear.
      2. Anterior Crucuate ligament appears bulky, suggestive of sprain.
      3. Minimal joint effusion noted.
      This was 4 months ago, at that time my pain was sharp and my knee had visible swelling and i couldnt move/lean my knee forward due to the pain.
      Now, The pain has drastically disappeared as explained and all the swelling has gone, my knee doesnt locked and never locked (thankfully) and exactly appears the same as my left knee. Can you enlighten me with your best advice if i can resume cardio only with elliptical/airwalk? I will highly appreciate your advice!

      Like mentioned already, our best advice we can give is to see a local physiotherapist in order to start supervised exercise therapy. You should also be able to resume cardio in any form (running, elliptical, airwalker) given that you start gradually.

      Could be, as we know that concurrent ligament injuries render tests for the menisci less valid and the other way around.

    I had many tests by physios, but until I got the MRI done there was no confirmed diagnosis. After that I had to have key-hole surgery to clean up the damaged parts of the miniscus.
    My biggest mistake was not going to a doctor early to get the confirmed diagnosis and then referral for the operation. This meant that the damage was much more extensive, as a result of almost a year delay after symptoms first presented.
