For the Lachman you can relax the cruciate ligaments and increase ROM. Think officially the test is described in neutral so we might have to do some due dilligence here. For the Valgus Stress it should be in neutral.
Of course that is something we would like to do for every test, but this is just not feasible due to the lack of patients that we would have to film in our studio.

Because if the tibia sags posteriorly (due to a PCL rupture) you will get a lot of joint play and movement when you pull the tibia anteriorly which mimicks a positive test
The Lachman is the best test we have, especially in an acute situation. In a chronic situation, the anterior drawer scores as good as the Lachman. Generally, an orthopedic surgeon (and physio) will usually perform the Lachman, Anterior Drawer and Pivot-Shift altogether to increase the chance for an ACL tear if positive.
is it possible to have a negative lachman test with a complete ACL tear?? .. 4 orthopaedics performed this test on my knee but no results and the all of a sudden MRI says ACL tear.. i really am suspicious of getting trapped in a fraud😟😟

I have grade 3 pcl tear, and I’m terrified before these tests are performed because I feel like they will just tear all my ligaments completely. And second question - I’m trying to find the bestest exercises to strengthen all the muscles that need to be strengthened before total hip replacement surgery. The best way to remember is always to understand the biomechanical and anatomical background behind a test. Then it becomes logical and you know how to test/stress/provoke the different structures.
