Loss of Pronation and Neck Breathing: The Connection

  • When you can't pronate, you can't "ground". When you lose "grounding", your tension goes up, your lower back goes into extension, and your neck becomes used for breathing in a dysfunctional way.
    To decrease bodily tension, you need to "ground", which means you need to pronate effectively and stop using your neck to breathe. In the videos you'll see the women's sternocleidomastoids and scalenes as they are used to pull the ribcage up to try and facilitate air flow to compensate for lack of diaphragmatic breathing.
    People who have had repeated ankle sprains, breaks, or other injuries to their lower extremities may need to consider a custom made orthotic to guide their lower extremity into proper pronation. Without it, they may not be able to stop using their neck for compensatory breathing nor get themselves out of extension.
    Grounding and proper breathing go hand-in-hand, which means pronation and breathing go hand-in-hand.

    Injured my left foot seriously when I was 13 and have suffered so much neck pain and headache since then, now I’m 42, tried so many things and useless. I believe this is it! Finally found it!

    That makes sense why I went from ballet to salsa, because I already loved living in an extended state. It also explains why I had trouble getting on my left leg. Once I took some PRI courses I started to understand why my injuries and tightness was all on my left side.

    I tried this left heel thing, and you're right about it giving a reference for symmetry, but you can also get it without the heel touching down by proprioception from the front ankle, which is enhanced in a dorsiflexed and supinated position...

    In November, I had a coronavirus. I did a CT scan. After the illness, the pain in my neck increased. And thank God that I came to this channel. I decided to look at the result of the CT scan. On the right side, 1st and second ribs are lower than on the left about 1 cm. The lower ribs on the left are much wider. Thanks to you, Neal, I feel much better now. I think that covid does something to increase the pain, maybe with a diaphragm.

    What about bow legged (varus knees) people? Can orthotics be tilted enough to correct this? Knees already outside hips and ankles so lining them up would be impossible without a knee replacement or tibial osteotomy - kinda extreme.

    So to sum up, if I lose my sense of grounding, I will begin chest/neck breathing? My issues with keeping my heels on the ground started before my TMJ/neck tightness, and I've had a lot of trouble performing the PRI breathing techniques without compensating (neck breathing). Do you think orthotics could help me lose my neck breathing?

    loss of pronation on either side could be fixed , now you just have to know on which foot is the issue, so yould be in: for example in a early stage of propulsion on the right leg and a late on left , so basically you just need a weight on the right hand to delay the early propulsion on the right foot so yould have an effective push / early left foot .. no big fuss about it and to avoid posterior compression you need that medial heel

    For athletes is it really smart going to orthotics? (Thinking about the soles weigh alot, and most importantly, fitting in soccer shoes is difficult...

      Orthotics might be tough during sporting events due to cleats and other "required" footwear. However, they could wear them as they go about their daily life.

    we know when you go into pronation that activate the external rotator hip muscles on the same side and the IO/TA on the other side; now if we make that this happens on both side i.e. you get ER hip musculature on both side which causes IO/TA to activate on both sides what position is the scapula in on both sides?

    If the input throught the feet is faulty and orthotics gets it right, are the technics still needed to get neutral, or are they just there to do it faster?
    Are Chiropracticer safe or should i avoid them as a Left Aic right bc patterned?

      Depends on the person, but regardless of neutrality, they won't be stable. They need the techniques to stabilize and re-learn how to walk without compensation.

    Hey Neal, is it possible for a bad pair of shoes to force someone back into compensation, even after they are neutral and stable? I notice that when I wear a pair of chacos or dress shoes I have (both are very stiff through the toe = induced loss of pronation ?) I tend to repattern very quickly.

      Great question. If its due to intrapelvic torsion, it can change. If its a true discrepancy, might need an orthotic

    hello Neal, I have fallen arches, wouldn’t this mean I am overly pronated and the issue is I don’t know how to supinate?

      I suppose you could state it that way. You might actually need "support" in the traditional sense.

    Is there a way to get PRI orthotics direct to consumer or do you have to go through a health care professional who offers them to their patients, like it says on Dr. Coffin's website?

      I contacted Dr.Coffin last year...he would send the orth.overseas, but only through h.c prof i your area..he said😣 i did look for a hcp in my country (Austria) but NO ONE ever heard about PRI ...so i doubt that it works out that way. I feel lost 😑

      There are PRI Orthotic affiliates. I got mine at ProActive PT in Fort Collins, CO

      Hi Bianca, regardless of whether they have heard of PRI, perhaps you could find a HCP who has a similar philosophy of orthotics: the orthotic restores proper biomechanical movement of the foot/ankle so you don't need to compensate. That's Dr. Coffin's philosophy (as opposed to simply "supporting" the foot, or preventing over-pronation). I don't think Dr. Coffin's philosophy is unique to him, so maybe you can find someone in Austria who sees things the same way.

    Can an issue at the neck/cranium actually be causing the supination on their right foot though? Right ribcage is more forward as well.

      To be as precise as possible: the entire "system" including our body/brain, respiration, ground forces, and gravity are causing it, but absolutely the neck/cranium will "lock" you into that supinated state. No doubt. My eyes and teeth locked me into that state.
