This one looks pretty cool. Maybe when I've completed my 7th run of Baldur's Gate 3 I'll give it a go. Adding it to the wishlist for sure though. Favorite pasta is absolutely any form of pasta with Alfredo on it. Excellent work as always sir.
I found all of them in the first area I encountered after starting a new run. So when all requirements are met (which ever they are) you'll find them right away when you start a new level.
I got the first new knight after already beating the void giant a couple of times. You'll find an item in the first room of a new run which is the item of the knight. No clue if it's random or after achieving something specific, sorry... But yeah it is quite frustrating...

They've had an update including an earlier arrival of the knights, you might find them now 💪
