I had extreme pain in my Patellar tendon area of both knees whenever I did something that would stretch the tendon from aged 15 to 18. I played basketball and volleyball. Bad enough pain that I had to take 4 ibuprofen before every sports practice or game just to be able to function.
Two visits to a doctor and they never gave me a diagnosis of anything, however. They just told me to "strengthen my legs". However, at the time it hurt way too much and I was scares to lift and put my legs under more of a load.
I then went to a chiropractor (yeah yeah, make fun of chiropractors), and in the imaging they took of my knees, they pointed out the apparent damage of that tendon area.
Through working with them, it was discovered that my hips were rotated a certain way that kinked the chain of energy up through my knees. My feet had always been at a 90 degree angle of one another my whole life when I walked, stood, or ran, but I never thought anything of it. The rotation of my legs literally caused exactly what you described in the video with the lateral shifting of the tendon when I bent my legs.
For about a year I went every two weeks to keep adjusting my hips as they literally straightened out when adjusted, but in a few days got crooked again. Over time, I had to go less and less frequently, and my hips stayed in line.
At that point my pain was almost gone, and any lingering pain went away once I got serious in the gym and strengthened my legs after all.
So that's my story. Maybe the doc helped, maybe the chiropractor helped. I dont care

A lot of chiros are quacks but if you find a good one you’re lucky. Mine fixed my hips and my back pain went away for good. People also sleep funny (and also have bad posture like you explained) and getting your hips adjusted clears up most common problems

Dude hearing your story, I have a feeling that your a point guard who moves side to side alot or you got flat fee? When you crossover attention to your knees, u can see that it's bending outwards. Or if your a dunker do u land on the heels? If yes to these then you have to relearn all those movements again that puts less stress and prevent knees bending.
If your a dunker try to land on your toes instead of heels

Thanks Doc I play ultimate disc an have suffered for this condition for at least a few years now, initially is was just under my knee caps an now the pain has moved lower to where the patellar tendon meet the shin bone. Thus far I’ve only applied the PRICE technique in terms of rehab. Anyway this vid was very informative learning about the q angle an the fact that weak quads could be a contributor was eye opening.
I will also be resting rather then pushing thru the pain for now on.
    Appreciate it!

Great video! I was a catcher in baseball and goalie in hockey. I’ve had issues with this for 10+ years. Started with a 8 month rehab and was back. Fast forward several years the pain got severe again. I ended up getting injections that didn’t help, in fact one knee got worse and ended up requiring surgery. What I’ve noticed is that the post surgery rehab is brutal. Definitely going to show this video to people asking what’s wrong with my knees.

I have jumpers knee. I hurt my knee last August in basketball, and it's now July. It's extremely painful when going down slopes and stairs. I can't even bend down on my knees without excruciating pain. I can't run or jump. You literally feel like an 80 year old man. I recently started focusing on quad exercises to increase quad strength, as well as taking naproxen. I also take a collagen supplements, as well as l glutamine, protein shakes and fish oil. I've noticed a huge improvement. Hopefully I'll be back to dunking soon.

Hi Doc, thanks for the video, unfortunaly I can really relate to this, I was diagnosed with this (i didnt know it was called "jumpers knee") and I havent played basketball for 6 months and I have been doing physiotheraoy. Interestingly, I havent done the exercise you mentioned, I have just done exercises to overall streghten the VMO, to get the kneecap back to place. I also found that a patellar tendon strap really helps out a lot during sports activity ( I still play from time to time). Again, thanks so much for the information (not only from this video), keeo up the good work! You really helped me out (and possibly many others) young players, that want to make it to professional, but at the level I am currently at I dont have an exclusive sports doctor. So, once again thank you so much!
P.s: could you make a video about ligament avulsion ( i had an ankle sprain in which this happened)

I understand this information is not a substitute for actually visiting a physician. But it does give us a gross idea of what may be occurring and how not to make it worse.

My man, explained it so much better than the doctors I've seen. I've been trying to figure out exactly how to rehab my knee, since the physical therapy was way off. Now I know I need to lower slowly and perhaps assist myself back up. I have quadriceps tendonopathy, but I can run full speed sprints with no pain. If I try to squat, the top of my kneecap kills! It's a very hard disease to deal with.

I had the same issue from my 4th year of college basketball, where I landed wrong after a lazy layup and my left knee started hurting. It never healed throughout the next two years while i continued to play, I'd use knee support, which would prevent the pain while playing but without it i was practically immobile. Cut to almost 1.5 years of not playing basketball through the pandemic, now when i go play basketball, I'm completely healed and i also continue the rehad to strengthen my hip flexures and abductor movements which caused the issue back then. So long break and rest is clearly the one of the ways to let the knee heal, so if you get an opportunity like that be sure to take it and make the most of it.

Another thing that exacerbates this pain for me is having weak/tight abductors. I’ve been strengthening my VMO for a while now and doing some sets of hip abductor exercises before my leg workouts basically eliminate the pain from working out. Still get soreness after the fact sometimes though.

Something that helped me get rid of or at least being able to keep the "jumpers knee" in control is using IMS (Intra muscular stimuli) needles for my tractors. The physio for my team gives me this on a regular basis like every third week depending on how much weightlifting and jumping I have been doing. Is this something that is commonly used for treating this condition?
I'm also working on making the VMO muscle stronger + the glutes in general to ease the load from the patella joint.
Thank you for the instructive video!

Thanks for the video, Brian. I learned a lot. I got PT from doing tree work and being out of shape. I want to get better so I can keep on climbing.
While using climbing gaffs, (spurs) sometimes one is in a static position for prolonged periods of time. When it's time to ascend to the next working position, the gaffs sometimes have become stuck too deep into the tree. it takes some effort to pull the spurs out. I'm thinking QT angle. "patience grasshopper", because I wish there was a quick fix.

    • @marcusporcius9842
      @marcusporcius9842 ปีที่แล้ว

Interesting you say that. I hope you are fully recovered by now. I came here to look for something for my daughter who is a runner but I can say that 20 years of tree climbing has left me with very tight quads on the outsides. I only realised it gave me knee trouble when I tried to start running again to slow my daughter's long runs down. I am now on the foam roller every day and stretching too. I think the Q angle is going to quite badly out on spikes and I think the quad may also be tight most of the time when in most work positions in a tree just to stabilise the leg. Touch your quads next time you are in a work position. I think it may be this stabilisation that has more of an effect than recovering your spikes from the tree. I doubt I get a gaff stuck more than once a year these days -just keep them sharp and walk up softly- but my quads are way too tight.

Ty for the great video! I am puzzled what to do - last week I did more HIIT workouts than usual (indoors and a lot of jumping squats). Previous couple weeks I’d done more skipping but before that did normal HIIT (jump squats etc..). Since this week of increased HIIT my knees feel a bit tender.. they don’t hurt but they’re very sensitive and when I crouch down they feel very tight - what do I do? Apart from avoid HIIT for a week and maybe wait a bit longer to do glute/leg day.... praying this is a one time thing and can heal quickly :( never had this before in my life

Honestly I’m currently at a loss, I’ve had this issue for a while now. 2 summers ago I went to a doctor for it and they just said to stay off of it, and so I did for the entire summer. Once I got back to playing volleyball in September, the exact same pain or even worse came back .
I now have a slant board to practice some eccentric squats, hopefully this helps me.

Prp is different from regenokine. I did a video on regenokine ( titled about the kobe treatment). Prp is mixed, we have the most experience with knee arthritis and prp, seems to be around 50-70% effective and if it works it's usually longer than steroid shot
