  • L4 L5 - L5 S1 disc herniation recovered pain free without surgery
    Reny is an online client who lives in Dubai and has been suffering from lumbar disc herniation in L3 L4 / L4 L5 / L5 S1.
    He has been told to go for surgery from his orthopedic and neurosurgeon and has been advised to not lift any weight otherwise he will end up paralised.

    I am also suffering from L5 S1 disc buldge for an year and I believe it's more depressive than painful so I am trying to heal it with some spiritual as well as physical exercises and I am hopeful that I will get better one day InshaAllah.     Hi bob, nice to stumble upon ur channel while researching on my L4/L5 disc bulge and pain associated with it. I have posterior bulge. My question is are the rehab exercises for posterior and anterior disc bulge same? Like if I am doing back extension ( like cobra pose) will I be pressing the posterior bulged disc even more?     Very positive video, im happy to see someone is getting better without surgery. Been in very bad pain for the past three month L5 S1 huge disk bulge, all docs suggesting surgery and im very close to be convinced but i see some people can do it without it.
    Im in Dubai and i tried many things and exercises but its getting worse especially with my leg pain. Somedays are worse than the others but still trying to avoid the surgery.

    Hi bob I stumbled across your channel can you help me please.
    I had an MRI and the neuroradiologist told me that there was a posterior disc bulge which was going off to the right-hand side at the level of L5/S1. This would be compressing the S1 nerve root as it left, causing the type of sciatica that I am getting.
    The neuroradiologist noted that there was a tiny focus of T2 hyperintensity within the outer border of the disc consistent with a small annular tear. Usually, in that situation, there is both lower back and sensitivity to movement.
    He told me not to do any weights at the gym or dumbbells and refocus my excerise on other things. I am walking everyday now but will I be able to go back to weights in the gym if I feel better ? also please can you advise which excerises I can do for recovery and how long should this take? Been suffering this issue for last few weeks . Thanks

    Hi Bob! I hope your doing well. My report of MRI describes the findings of the examination, which include:
    • Normal lumbar lordosis
    • Normal spinal cord termination
    • Normal lumbar vertebra corpus heights and signal intensities
    • Normal intervertebral disc heights and signal intensities
    • No significant neural compression findings at the L1-L2 or L2-L3 levels
    • L3-L4 intervertebral disc slightly inferiorly migrated causing spinal canal and nerve root compression
    • L4-L5 intervertebral disc diffuse symmetrical disc bulging, more on the left, causing thecal sac and neural foraminal narrowing
    • L5-S1 intervertebral disc diffuse symmetrical disc bulging, causing mild thecal sac compression and obliteration of fat between the disc and nerve root
    The conclusion of the report is that the patient has disc herniation at the L3-L4, L4-L5, and L5-S1 levels. These herniations are causing nerve compression.

    Sir I have back pain issues for the past two decades.,somehow I have managed it but now a days it affects my regular life. As I am going cycling for one hour with normal speed but could not have walks as I told you cycling is more easier than walking. But the sever problem is that I couldn't stand not more than one or two minutes and feeling sharp pain in the back and numbness on my both fore thighs and it forced me to sit on a chair.     Hi, I had emergency spinal decompression surgery 3 weeks ago now. A large fragment had snapped off into my spinal canal. First off I was pain free, my right leg was the issue, that has stayed pain free. But now I have developed very painful sciatica in my left leg and pain in the front of my hip and my buttocks. What would cause this as I didn't have pain in the left side before. I can only walk for up to 5minutes at a time before I have to lie down again. I'm just so gutted I feel like I'm back at the start.

      How are you now? It's possible that the bulge is now happening on the other side or the middle of your spine. Most of my pain is on the right with no numbness or tingling. However, on my left side, I have numbness at the heal of my foot. No pain or other symptoms.

      @@alexmighty9004 I had another surgery on my left side 3months later. I'm a world better now. But I do still have lower back stiffness and pain but nothing I need medication for. I ended up with nerve damage in my right foot/leg which causes its issues, but I can live a some what normal life now.
      I've also changed careers to help the situation.
      You need to look after your spine honestly.

    aAt l3-l4,l4-l5&l5-s1 levels:central and both paracentral disc protrusion with postero-lateral osteophytosis, facet and flaval hypertrophy are causing thecal sac indentation, both lateral recess obliteration, moderate spinal canal stenosis, bilateral neural foraminal narrowing and corresponding existing nerve root compression.

    I recently got an MRI with impressions: 2mm posterior broad based disk protusion with left posterolateral annular feassure. Very single time I swing my left leg my right one gets a shocking numb sensation. Also when I stretch back my legs feels the same numbing sensation down to my calf and foot. I’m really stressing about it. I also have sciatica pain. What exactly is happening and can it be reversed? Pls help

    Hello Bob,
    My name is Dipak Thapa. I'm from Nepal. I'm suffering from severe back pain due to the disk protusion in L3-L4, L4-L5 level, disk extrusion with caudel migration with moderate stenosis of right lateral recess that has caused compression of right L5 exiting nerve root.     I got injured 5 days ago.. MRI scan shows 2 disc bulges L4/L5 L5/S1.
    At this stage Im pretty confused about what I’m allowed to do, what to eat and which exercises and stretches I should do, when the injury is so new. (Also further into healing process)
    So I really need a plan to get started!

      hey how are you doing??? I just injured myself and got a MRI with a mild bulge at L4/L5 and a 8mm disc extrusion at L5/S1. I’m in severe pain 24/7 and nothing helps i can’t even walk :(

      @@TheBeast-go2mm I’m doing a lot better thank you for asking! I’m still in 24/7 chronic pain just waiting to get my epidural shot scheduled next weekend. I still can’t be on my feet for more than 10 minutes without pacing around in pain.
      Unfortunately, medications or physical therapy doesn’t help or react good with my sciatic nerve pain so i’m limited on finding relief when it’s at its worse. I’m really hoping that the epidural gives me relief, but most likely surgery is the only answer for my current situation. The sciatic nerve pain has also developed in the opposite leg since my last comment.
      I’m only 18 years old and this was/is the worst pain i felt in my life. At its worst I thought I was going to die and thought about suicide often. I’m not sure if your personally suffering from this injury also, but don’t let my experience scare you.

      ​​@@RomvnSossaSame here,I am 18 too and I was in the gym,I was pushing on the Fly machine,I did a wrong move in the machine,I tried to adjust my sitting position when I pushed the hand of machine,and I noticed something was wrong, finished the day and arrived on to the house,and massacre started.Its been a almost 3 months and I think it's getting more better,at the early stages, I couldn't be able to move my legs, I can't be able to take off my pants,can't be able to crouch without any problems.The pain was hitting almost every time when I am not in the lying in the bed.I have L4L5(it's pressing into the sciatica nerve).The good thing is(I think it's getting better,I pray to God to hope getting better 😭)the pain getting smaller in my body,it was hitting on the left leg and sometimes its hitting on to the right leg but I am still making sciatica exercises and still going to the gym,no any leg exercises,the pain will be supposed to go away in the almost 5-7 month timescale(it depends on your condition, weight,and size of your body), I heard this from a physician from TH-cam.

    I subscribed and Im following you for the quality content- I have a question: is it possible to get MRI that shows that bulged disc is not bulging anymore, just annulus remains degenerated. But sciatica symptoms continue ? I though that its bulging disc that triggers sciatica and once its not bulging anymore we should not have persisting symptoms? Many thanks

      That’s a great question. Bedside disc bulge & annulus, other structure can be involve and create those symptoms. To give you an example, you could follow some rehab that’s consistently aggravating your symptom or actually taking complete rest and that’s not idea either !! I need to read your MRI scan and know your injury and understand your rehab plan you have been following to answer your questions. I go through and hour of consultation with my online clients initially to cover many questions and test.     Hi Bob, I am very interested in your therapy. I am a gym owner, personal trainer, fitness coach. Over the last few months, I have experienced back pain that I have never had before. After going to many different doctors/ physical therapists I recently had an MRI. It shows severe bulging disc of my L5 and more bulging discs in my whole lumbar spine. I also have Spondylolisthesis. This news has really come as a shock to me. I am so physically fit, and actually feel best when exercising. I was told I need to see a spinal surgeon. My question is, so you think someone in my profession could rehab this without surgery? I have recently been following your videos and doing the therapy you suggest. I always feel better after. I have numbness in my left leg all the way to my pinky toe. My case seems very similar to Reny's.

    Sir I'm 25 years old, with L5-S1 grade 1 anterior listhesis with B/L spondylolysis of L5 with disc degeneration at L5-S1 level and B/L formainal disc herniation at L5-S1 level causing narrowing of B/L neural foramen, compression of exiting nerve roots and lateral recess stenosis. I have morning stiffness in back mostly on bending forward, but no radiation of pain. Stiffness near dimples on prolonged sitting. How should I proceed?

    Thank you for this video. I have L5 and S1 disc herniation, Neuro spine surgeon after going through MRI, suggested immediate surgery besides replacing a bone with artificial with bolt and nut attachment. I am worried, Now I am taking Ayurviad treatment. but of little help. Please suggest.

    Based on the provided medical report, let me explain the findings in non-medical terms:
    The patient, Mr. Ravinder, underwent an MRI scan of his lumbo-sacral spine. The scan revealed several observations:
    1. The lumbar spine appears straightened, which could indicate a loss of normal curvature.
    2. Osteophytes, which are bony outgrowths, were found in the front and back of the spine.
    3. Schmorl's nodes, which are herniations of the endplate of the vertebrae, were found in the L1, L2, L3, and L4 vertebrae.
    4. The superior endplate of the L2 vertebra showed mild erosions.
    5. Modic type II endplate changes were seen in the L3 and L4 vertebrae, which indicate inflammation.
    6. The ligamentum flavum, a ligament in the spine, was thickened.
    7. At the L1-L2 level, there is an annular tear (tear in the disc), posterior disc extrusion (disc material pushed out), and diffuse disc bulge (disc protrusion) with disc desiccation (loss of disc moisture). This is causing severe compression on the anterior thecal sac (membrane surrounding the spinal cord) and spinal canal stenosis (narrowing of the canal) leading to compression of the nerve roots of the cauda equina (nerves at the lower end of the spinal cord) and narrowing of the neural foramina (spaces where nerves exit the spine) on both sides.
    8. At the L2-L3 level, there is a diffuse disc bulge causing mild compression on the anterior thecal sac and the nerve roots.
    9. At the L3-L4 level, there is a diffuse disc bulge with partial disc desiccation causing mild compression on the anterior thecal sac.
    10. At the L4-L5 level, there is a diffuse disc bulge with partial disc desiccation causing mild compression on the anterior thecal sac and narrowing of the neural foramina with compression on the nerve roots.
    11. At the L5-S1 level, there is a reduced space between the intervertebral discs, an annular tear, posterior disc extrusion, diffuse disc bulge with disc desiccation causing mild compression on the anterior thecal sac, and compression on the nerve roots.
    12. No evidence of spondylolisthesis (slippage of the vertebral column) was found.
    13. The facet joints (joints between the vertebrae) appear normal.
    14. The rest of the lumbar vertebrae show normal morphology (structure).
    In addition to the lumbar spine, the report also mentions the following:
    1. The lumbar canal (space within the spine) has normal dimensions at different levels.
    2. The spinal cord, conus medullaris (end of the spinal cord), and the subarachnoid space (space around the spinal cord) are normal.
    3. The sacroiliac joints (joints between the sacrum and ilium bones) appear normal without any erosions or edema (swelling).
    4. The paraspinal soft tissues (tissues surrounding the spine) appear normal.
    In summary, the report indicates degenerative changes in the lumbar spine, including disc bulges, disc desiccation, annular tears, and spinal canal stenosis at multiple levels. The report suggests that these findings should be correlated with the patient's clinical symptoms for further evaluation and diagnosis.

    Hi Bob. I'm confused by the notes from doctor. I'm hoping you see this and can shed some light. There are different measurements and I'm not sure how big the herniation is. It states: 3 to 4mm central to paracentral inferiorly directed disc extrusion. Extruded disc component measures 9mm in craniocaudal dimension. There is a 2mm undulating disc bulge. I have been suffering with back pain since 2007 and have yet to find a doctor take it seriously. In 2007 at 27yo, I went 9 months without being able to care for myself. I needed help to do everything. I have more bad days than good with major pain episodes lasting 3-4 months. A month ago I could barely walk, I had to use a cane and walker.

    Hello bob I am a 23 year old with the same kind of disc bludge at l4l5, l5s1 and it’s really bad the pain and inability to move much.
    I would really like to follow a plan for workouts and nutrition
    I got suggested to do only a couple of excercises like calf camel and all but the pain isn’t reducing
    What can I do now? I’m a student and it’s really hard as I have to move form one place to another and am ready to get myself better

    i have disc herniation L4-L5 and L5-S1 and its causing sciatica pain down my left thigh, leg and foot. i honestly feel very confused. some say to do deadlift, some say to dont do squats...what should i do?

      Hi there, you can definitely do deadlift and any other movement or exercise as such. But, you can’t jump to deadlift and try it out of not doing any preparation !! Your rehab plan will progress gradually and based on each individual ability you will be doing deadlift in late stage of your rehab. My question is, what type of rehab are you doing at the movement ? And how long you’ve been doing it ? Bob

      @@bobthephysio my physio told me to do nerve flossing leg raise head up as well as a back stretch unfortunately 3 weeks of doing it made no difference, i have been with this sciatica pain for 5 months now..i finally got an MRI that showed a disc herniation. my doctor will advise some sort of an injection. what do you suggest?

    The presentation showed the mri scan before the treatment. The mri scan after the treatment may be shown. The saggital diameter from L1 to L5 before treatment and after treatment may be shown

      You can definitely recover Sai ,the first option is always rehab and physio. If you failed physio after a 6months you can think about other alternatives. Are you doing any physio. ? Bob

    Hi Bob,my senior neurologist and spinal special did Acdf surgery on my c6 July and said that it will be the most support on my whole spine as l suffer with osteoarthritis from head to to and even have a bone marrow oedema on my left leg which l have chronic pain and take 2700mg of gababentin every day for my back and leg.He took a mri with contrast because l had a revisional micro discectomy 10 years ago.He is semi retired but was the clinical lead for 15 years as a neurologist and spinal specialist including gamma Ray brain techniques. He says that l have a mass of epidural fibrosis and his opinion is that if he tried to operate l could lose loss of motor function. But my sciatica seems to be getting worse.
    What do you think please?

    Hi Bob, my wife has l4 l5 s1 disc bulge and had l5 s1 endoscopy discectomy 2 months back and still she is feeling the same pain and doctor suggested MRI and found same bulge again as before , Please help her to recover

    As per alopethic doctor disc doesn't goes back if disc bulge is old.
